Abstract

Aim Anticoagulants are underused in older patients with atrial fibrillation (AF). Scoring systems, such as CHA$_{2}$DS$_{2}$-VASc and HAS-BLED, are recommended to guide clinicians in anticoagulation decisions, but patients’ frailty might be an underrecognized factor. We investigated the association between the Clinical Frailty Scale (CFS) and community anticoagulant prescribing habits in patients aged ≥75 years with AF admitted acutely to hospital. Methods Data were gathered retrospectively over 3 months on individuals admitted under a medical team to a tertiary teaching hospital in the UK. Demographics, AF history, CHA$_{2}$DS$_{2}$−VASc, HAS-BLED and CFS were collected. Bivariable analysis compared anticoagulated and non-anticoagulated groups. Each component of the CHA$_{2}$DS$_{2}$-VASc and HAS-BLED scores, as well as frailty, age and sex, were entered in a multivariable analysis. Results A total of 419 patients with known AF were included. Of these, 215 were not anticoagulated (51.3%) on admission. Non-anticoagulated individuals were older (median age 87 years, [interquartile range (IQR) 7] vs 83 years [IQR 6], P < 0.001), more likely to be frail (81.4% vs 52.5%, P < 0.001) and had lower CHA$_{2}$DS$_{2}$-VASc scores (median 4, [IQR 2] vs 5 [IQR 2], P = 0.01). In the multivariable analysis, frailty had the strongest effect against anticoagulant prescription (OR 0.77, 95% CI 0.70–0.85, P < 0.001) compared with other significant risk factors, such as age (OR 0.98, 95% CI 0.97–0.98, P < 0.001) and bleeding risk (OR 0.85, 95% CI 0.74–0.97, P = 0.02). Conclusions Frailty is associated with non-prescription of anticoagulants, independently of CHA$_{2}$DS$_{2}$-VASc and HAS-BLED. It could be an important unmeasured factor in anticoagulation decisions. The utility of explicit frailty measurements in anticoagulation decisions and patient outcomes requires further research.

Abstract

WHAT IS ALREADY KNOWN ABOUT THIS SUBJECT • The treatment options for heart failure with normal ejection fraction (HFNEF), which constitutes about 50% of the cases of heart failure, is varied and consists of β-adrenoceptor blockers, angiotensin converting enzyme inhibitors (ACE inhibitors), angiotensin receptor blockers (ARBs), calcium channel blockers and digoxin. However none of the treatment options have shown consistent results in improving the condition. WHAT THIS STUDY ADDS • The meta-analysis conducted by us on the pooled data from the clinical trials on the various treatment options has revealed no definite benefit with pharmacotherapy in HFNEF. A systematic review of these data reveals a possible benefit with β-adrenoceptor blockers in this condition. But to evaluate this larger studies with preplanned subgroup analysis, longer follow up periods and with echocardiographic parameters as outcomes to measure the LV diastolic dysfunction are required. AIM The pharmacotherapy for heart failure with normal ejection fraction (HFNEF) is not as well defined as that for the treatment for heart failure with reduced ejection fraction (HFREF). Studies of the various drugs given for HFNEF have revealed conflicting results. The aim of this systematic review was to determine whether there is any benefit with pharmacotherapy in HFNEF in terms of cardiac outcomes. METHODS Electronic and printed sources were searched until August 2010 for randomized controlled clinical trials (RCTs) comparing drug therapy with placebo in HFNEF. Weighted mean difference and pooled odds ratio (OR) with 95% confidence intervals were calculated. RESULTS A total of six RCTs including 8410 patients with a mean follow-up period of 21 months were included in the analysis. Although there were no significant differences in all cause mortality between the two groups (pooled OR 0.95, 95% CI 0.79, 1.13, P= 0.55), the subgroup analysis revealed a slight but non significant advantage with the β-adrenoceptor blocker group. There was no significant difference between the two groups in terms of cardiovascular mortality, hospitalization, worsening heart failure, ejection fraction, E : A ratio, deceleration time and E : E′ ratio. CONCLUSION There was no significant benefit of pharmacotherapy in HFNEF. This might have been because of a lack of stringent inclusion criteria for patients in the trials and lower power of the studies. Hence trials with well defined inclusion criteria, better power, longer follow-up periods and with echocardiographic parameters as endpoints are required to shed further light on this topic.

Abstract

The precise role of immune cells in beta cell killing and their manner of invasion of pancreatic islets in insulin-dependent diabetes mellitus (IDDM) are unclear. We have attempted to target pancreatic islets of severe-combined immunodeficient (SCID) mice with spleen cells from diabetic and non-diabetic female non-obese diabetic (NOD) mice given i.p. or i.v. Pancreatic, liver and kidney sections of SCID mice were assessed histologically for the presence of donor cells. The presence of raised levels of serum Ig was also used as an index of engraftment of donor cells in the periphery of SCID mice. All six SCID mice which received i.v. spleen cells from normal Swiss mice died within 2 weeks from graft versus host disease (GVHD) whereas five out of nine mice survived for 30 days after i.p. injection. No deaths were recorded after i.v. or i.p. injection of spleen cells from NOD mice. Pancreatic islets of four out of six SCID recipients of diabetic and three out of five recipients of non-diabetic spleen cells following i.p. injection showed lymphocytic infiltrates in the peri-islet and perivascular regions. All SCID mice which received i.v. spleen cells from diabetic (six SCID recipients) and non-diabetic NOD mice (seven SCID recipients) showed peri-islet and perivascular infiltrates in their pancreas. Immunohistochemical analysis showed that the islet engrafted cells were of CD4 and CD8 phenotype. Donor cells were also observed in the exocrine pancreas of some recipients. A majority of mice showed various degrees of lymphocytic aggregates in the perivascular regions of the liver but not in the kidney.(ABSTRACT TRUNCATED AT 250 WORDS)
