The purpose of this study was to examine baccalaureate dental hygiene faculty members’ attitudes and practices regarding student plagiarism. An email containing a link to a thirty-two-item survey was sent to fifty-two baccalaureate dental hygiene program directors in the United States; thirty of those agreed for their faculty members to participate. Of the 257 faculty members who received the survey link, 106 completed the survey, for a response rate of 41.2 percent. The responding faculty members reported thinking plagiarism is a rising concern in their dental hygiene programs (54.5 percent, 54/99). The majority said they check for plagiarism on student class assignment/projects (67.1 percent, 53/79). For those who did not check for plagiarism, 45.8 percent (11/24) stated it took “too much time to check” or it was “too hard to prove” (16.6 percent, 4/24). The most frequent form of student plagiarism observed by the respondents was “copying directly from a source electronically” (78.0 percent, 39/50). Most respondents reported checking for plagiarism through visual inspection (without technological assistance) (73.0 percent, 38/52). Of those who said they use plagiarism detection software/services, 44.4 percent (16/36) always recommended their students use plagiarism detection software/services to detect unintentional plagiarism. For those faculty members who caught students plagiarizing, 52.9 percent (27/51) reported they “always or often” handled the incident within their dental hygiene department, and 76.5 percent (39/51) said they had never reported the student’s violation to an academic review board.

Context We live in a world of ethnoracial conflict. This is confirmed every day by opening and reading the newspaper. This everyday world seems far away in the pages of a medical education journal, but is it? The goal of this paper is to suggest that one need not look very far in medical education to encounter ethnoracial issues, and further, that research methods that are not ethnoracially biased must be employed to study these topics. Discussion We will draw attention to the relevance of employing an ethical conceptual approach to research involving ‘race’ by demonstrating how one author researching internationally educated health professionals has put ‘race’ front and centre in his analysis. He does this by using a postcolonial method of analysis termed a ‘doubled-research’ technique that sets up categories such as ‘race’ but then decolonizes them to avoid essentialism or stereotyping. We compare this method to another mainstream method employed for the same topic of inquiry which has sidelined ‘race’ in the analysis, potentially hiding findings about ethnoracial relations involving health professionals in our ‘multicultural’ society. This demonstration leads to the important question of whether research methods can be epistemologically racist–a question that has been raised about conventional research on education in general. Our argument is not meant to be the last word on this topic, but the first in this journal. Conclusions We conclude that there is an internal ethics or axiology within research perspectives and methodologies that needs to be examined where ethnoracial issues are prominent. The use of mainstream approaches to undertake research can unintentionally ‘leave unsaid’ central aspects of what is researched while antiracist methods such as the one described in this article can open up the data to allow for a richer and deeper understanding of the problem.

A national survey of dental hygienists was conducted to explore ethical issues arising from the use of live patients for dental hygiene clinical licensure examinations. Data were collected regarding respondents' demographics, additional costs they incurred associated with their examination beyond the examination fees, delays in patients' treatment resulting from the examination, unethical candidate and/or patient behaviors they experienced, and provisions they made for patients' follow-up care related to the examination. Five hundred surveys were mailed to dental hygienists from two states in each of the five licensure examination regions. The response rate was 40.6 percent (n=203). Descriptive statistics were used to analyze the data. The results showed that the majority of the respondents spent additional money on examination-related expenses (69.2 percent). Sixty-one percent of the respondents reported paying their patients; however, only 50.5 percent felt such a practice was acceptable. More than half (53.1 percent) reported believing it was appropriate to delay treatment in order to have a patient participate in the examination, although only 16.4 percent reported actually delaying treatment. Informed consent was said to be obtained by 94.9 percent of the respondents. The majority (86.6 percent) said they referred patients for follow-up dental hygiene care. When asked if they felt the examination was an accurate assessment of their clinical skills, 78.7 percent of the respondents agreed that it was.

The purpose of this study was to determine the nonmedical use of prescription attention deficit disorder (ADD) stimulant medication among dental and dental hygiene students. A questionnaire was used to examine demographic information, student experiences, and perceptions of prescription stimulant medication and to determine if students used a prescription stimulant nonmedically. In 2008, 401 surveys were mailed to dental education institutions in the south-central region of the United States, and 243 surveys (61 percent) were returned. The survey found that 12.4 percent of these students used a prescription stimulant nonmedically and, of those, 70 percent took it to improve attention and/or concentration. The most commonly reported stimulant medication used nonmedically was Adderall (77 percent). The majority (87 percent) of the students obtained the medication through friends, and 90 percent began using the drug in college. Even though 74 percent of the students reported being stressed, chi-square analysis found no significant association between nonmedical use of ADD stimulant medication and stress level (p=0.585). Sixteen percent of the students surveyed felt it was easy to obtain stimulant medication for nonmedical use at their school, and 17 percent thought it was a problem within their institution. These results may help administrators and faculty members become aware of potential problems with the misuse of ADD stimulant medication.

La presente etude sur les pharmaciens en milieu hospitalier demon-tre que, depuis 20 ans, ce que l'on qualifie de «perspectives d'avenir» presentees par ce secteur en pleine expansion attire un nombre disproportionne de femmes, dont la plupart sont tenues a l'ecart lorsqu'il s'agit d'avancement professionnel. Meme si les detentrices de qualifications postbaccalaureats ont accedea de meilleurs postes de premier echelon, elles ont neanmoins perdu l'avantage pendant la premiere decennie de leur carriere, ce qui correspond aux resultats de recherches prealables realisees sur les professions non feminisees. Cette situation semble indiquer que ce sont des facteurs lies au sexe, et non a la productivite, qui influent de facon negative sur les per-sonnes qui sont en mesure d'accorder les avancements; de la meme maniere, au moment de l'embauche pour des postes de premier echelon, ce sont les qualifications plutot que la productivite qui influent sur leurs decisions. This study of hospital pharmacists shows that the “opportunity” presented by this rapidly-expanding field over the past 20 years differentially attracted women, most of whom became ghettoized in terms of career advancement. Those women who acquired post-baccalaureate educational credentials obtained better entry-level positions, but, consistent with previous research on less-feminized professions, lost this advantage over the first decade of their careers. The pattern of advancement suggests that gender-related factors serve as a “negative signal” to gatekeepers, irrespective of productivity, just as credentials serve as a signal or proxy for productivity in hiring for entry-level positions.

White and brown adipocytes are usually located in distinct depots; however, in response to cold, brown adipocytes appear in white fat. This response is mediated by β-adrenoceptors but there is a controversy about the subtype(s) involved. In the present study, we exposed to cold β3-adrenoceptor knockout mice (β3KO) on a C57BL/6J genetic background and measured in white adipose tissue the density of multilocular cells and the expression of the brown adipocyte marker uncoupling protein-1 (UCP1). In brown fat of β3KO mice, UCP1 expression levels were normal at 24 °C as well as after a 10-day cold exposure. Strikingly, under both conditions, in the white fat of β3KO mice the levels of UCP1 mRNA and protein as well as the density of multilocular cells were decreased. These results indicate that β3-adrenoceptors play a major role in the appearance of brown adipocytes in white fat and suggest that the brown adipocytes present in white fat differ from those in brown fat.

Smith-Lemli-Opitz syndrome (SLOS) is an autosomal recessive developmental disorder characterized by multiple congenital malformations, dysmorphic craniofacial features, and mental impairment. SLOS is caused by a deficiency of the enzyme 7-dehydrocholesterol delta7 reductase which converts 7-dehyrocholesterol to cholesterol. This error results in elevated serum levels of 7-dehydrocholesterol and decreased levels of serum cholesterol. This article describes the clinical features and medical treatment of SLOS. A case report is included, with recommended guidelines for providing safe and comprehensive dental care for individuals with SLOS.

Uncoupling protein-3 (UCP3) is a mitochondrial inner-membrane protein abundantly expressed in rodent and human skeletal muscle which may be involved in energy dissipation. Many studies have been performed on the metabolic regulation of UCP3 mRNA level, but little is known about UCP3 expression at the protein level. Two populations of mitochondria have been described in skeletal muscle, subsarcolemmal (SS) and intermyofibrillar (IMF), which differ in their intracellular localization and possibly also their metabolic role. To examine if UCP3 is differentially expressed in these two populations and in different mouse muscle types, we developed a new protocol for isolation of SS and IMF mitochondria and carefully validated a new UCP3 antibody. The data show that the density of UCP3 is higher in the mitochondria of glycolytic muscles (tibialis anterior and gastrocnemius) than in those of oxidative muscle (soleus). They also show that SS mitochondria contain more UCP3 per mg of protein than IMF mitochondria. Taken together, these results suggest that oxidative muscle and the mitochondria most closely associated with myofibrils are most efficient at producing ATP. We then determined the effect of a 24-h fast, which greatly increases UCP3 mRNA (16.4-fold) in muscle, on UCP3 protein expression in gastrocnemius mitochondria. We found that fasting moderately increases (1.5-fold) or does not change UCP3 protein in gastrocnemius SS or IMF mitochondria, respectively. These results show that modulation of UCP3 expression at the mRNA level does not necessarily result in similar changes at the protein level and indicate that UCP3 density in SS and IMF mitochondria can be differently affected by metabolic changes.

OBJECTIVES The changing landscape of health care in this country has seen an increase in the delivery of care to critically ill patients in the emergency department (ED). However, methodologies to assess care and outcomes similar to those used in the intensive care unit (ICU) are currently lacking in this setting. This study examined the impact of ED intervention on morbidity and mortality using the Acute Physiology and Chronic Health Evaluation (APACHE II), the Simplified Acute Physiology Score (SAPS II), and the Multiple Organ Dysfunction Score (MODS). METHODS This was a prospective, observational cohort study over a three-month period. Critically ill adult patients presenting to a large urban ED and requiring ICU admission were enrolled. APACHE II, SAPS II, and MODS scores and predicted mortality were obtained at ED admission, ED discharge, and 24, 48, and 72 hours in the ICU. In-hospital mortality was recorded. RESULTS Eighty-one patients aged 64 +/- 18 years were enrolled during the study period, with a 30.9% in-hospital mortality. The ED length of stay was 5.9 +/- 2.7 hours and the hospital length of stay was 12.2 +/- 16.6 days. Nine (11.1%) patients initially accepted for ICU admission were later admitted to the general ward after ED intervention. Septic shock was the predominant admitting diagnosis. At ED admission, there was a significantly higher APACHE II score in nonsurvivors (23.0 +/- 6.0) vs survivors (19.8 +/- 6.5, p = 0.04), while there was no significant difference in SAPS II or MODS scores. The β3-adrenoceptor plays an important role in the adrenergic response of brown and white adipose tissues (BAT and WAT). In this study, in vitro metabolic responses to β-adrenoceptor stimulation were compared in adipose tissues of β3-adrenoceptor knockout and wild type mice. The measured parameters were BAT fragment oxygen uptake (MO2) and isolated white adipocyte lipolysis. In BAT of wild type mice (−)-norepinephrine maximally stimulated MO2 4.1±0.8 fold. Similar maximal stimulations were obtained with β1-,β2- or β3-adrenoceptor selective agonists (dobutamine 5.1±0.3, terbutaline 5.3±0.3 and CL 316,243 4.8±0.9 fold, respectively); in BAT of β3-adrenoceptor knockout mice, the β1- and β2-responses were fully conserved. In BAT of wild type mice, the β1/β2-antagonist and β3-partial agonist CGP 12177 elicited a maximal MO2 response (4.7±0.4 fold). In β3-adrenoceptor knockout BAT, this response was fully conserved despite an absence of response to CL 316,243. This unexpected result suggests that an atypical β-adrenoceptor, distinct from the β1-, β2- and β3-subtypes and referred to as a putative β4-adrenoceptor is present in BAT and that it can mediate in vitro a maximal MO2 stimulation. In isolated white adipocytes of wild type mice, (−)-epinephrine maximally stimulated lipolysis 12.1±2.6 fold. Similar maximal stimulations were obtained with β1-, β2- or β3-adrenoceptor selective agonists (TO509 12±2, procaterol 11±3, CL 316,243 11±3 fold, respectively) or with CGP 12177 (7.1±1.5 fold). In isolated white adipocytes of β3-adrenoceptor knockout mice, the lipolytic responses to (−)epinephrine, to the β1-, β2-, β3-adrenoceptor selective agonists and to CGP 12177 were almost or totally depressed, whereas those to ACTH, forskolin and dibutyryl cyclic AMP were conserved. Endurance exercise training has been shown to decrease diet-induced thermogenesis (DIT) in rats and humans. In rodents, most thermogenesis is thought to occur in brown adipose tissue via activation of the uncoupling protein-1 (UCP1) and in skeletal muscle. Since the level of UCP1 mRNA in rat BAT was reported to be unmodified by exercise training, the newly described uncoupling proteins UCP2 and UCP3 could be responsible for the decreased DIT in trained rats. UCP3 mRNA levels in endurance-trained rats were found to be reduced by 76% and 59% in tibialis anterior and soleus muscles, respectively. UCP2 mRNA levels were also decreased in tibialis anterior and in heart by 54% and 41%, respectively. Neither white adipose tissue UCP2 nor brown adipose tissue UCP1, UCP2, and UCP3 mRNA levels were modified. The results of this study show that a need for a higher metabolic efficiency is associated with decreased mRNA expression of the uncoupling proteins in skeletal and heart muscles, which would decrease energy dissipation in these tissues. The down-regulation of UCP3 and UCP2 expressions might also contribute to the rapid weight gain known to occur when exercise training ceased.

Intraoral use of ultrasonic scalers may generate aerosols that contain infectious microorganisms and therefore pose a hazard to the health of the dental professional. The purpose of this in vivo study was to determine if an aerosol reduction device for an ultrasonic scaler would be effective in reducing the amount of contaminated aerosols produced during ultrasonic instrumentation. Twelve adult subjects participated in the study. A split-mouth design was utilized, and two treatments (in separate rooms) were performed on each subject: 1) ultrasonic scaling for 5 minutes with the aerosol reduction device; and 2) ultrasonic scaling for 5 minutes without the aerosol reduction device. The right or left side of the subject's mouth was randomly assigned to one of the two treatment groups. After instrumentation, the subject and operator remained motionless for 25 minutes during collection of aerosol particles. Air samples were collected on blood agar plates 6 inches from the subject's mouth. Replicate organism detection and counting plates were used to sample microorganisms present on the investigator's face shield. All plates were incubated for 3 days at 37 degrees C. The results, using the paired t-test (P < 0.001), indicate that the ultrasonic scaler without the aerosol reduction device had a significantly greater quantity of mean colony forming units (CFUs) 6 inches from the subject's mouth (45.1 +/- 28.9) than the ultrasonic scaler with the aerosol reduction device (2.6 +/- 3.6). No significant difference was evident in the number of CFUs found on the investigator's face shield. These data suggest that an aerosol reduction device is effective in reducing the number of microorganisms generated during ultrasonic scaling, therefore decreasing the risk of disease transmission.

A consistent finding in the literature on measures of clinical problem-solving scores is that there are very low correlations across different problems. This phenomenon is commonly labelled 'content-specificity', implying that the scores differ because the content knowledge necessary to solve the problems differs. The present study tests this hypothesis by presenting groups of residents and clinical clerks with a series of simulated patient problems in which content was systematically varied. Each subject also completed a multiple choice test with questions linked to each diagnosis presented in the clinical problems. Three of the four problem-solving scores showed low correlations, even to two presentations of the same problem, and no relationship to content differences. None of the scores were related to performance on the multiple choice test. The results suggest that variability in problem-solving scores is related to factors other than content knowledge, and several possibilities are discussed.
