Abstract

OBJECTIVE To investigate whether obese patients with binge eating (BE) have higher alexithymic features; to explore the different relationships between psychological features (alexithymia, depression, and anxiety) and BE. METHOD Three hundred sixty one obese BE-patients were evaluated for alexithymia, psychological distress, and BE. Alexithymia was measured with the 20-item Toronto Alexithymia Scale (TAS-20); BE was assessed with the BE Scale (BES), and depression and anxiety symptoms were evaluated with the Hospital Anxiety and Depression Scale (HADS). RESULTS Patients with BE reported significantly higher TAS-20 total scores than those without BE (p < .001). The SEM analysis showed that the difficulty in identifying feelings (DIF) and difficulty in describing feelings (DDF) components of alexithymia affected BE along different pathways. DIF was found as a major factor influencing altered eating both directly (p = .20*) and above all through the mediation of psychological distress (p = .19***), whereas DDF affected BE only through psychological distress at a lesser extent (p = .09**). DISCUSSION Alexithymic difficulties in affective awareness may play an important role in the onset and maintenance of BE, especially when patients experienced anxiety and depression symptoms. Clinicians involved in the management of obesity should address the combination of alexithymic traits and emotional distress by planning effective client-focused interventions.

Abstract

Obesity is associated with increased cardiovascular disease. Metabolic syndrome (MS) identifies substantial additional cardiovascular risk beyond the individual risk factors, and is a powerful predictor of cardiovascular events even regardless of body mass index, thus suggesting a common downstream pathway conferring increased cardiovascular risk. Platelet hyper-reactivity/activation plays a central role to accelerate atherothrombosis and is the result of the interaction among the features clustering in obesity and MS: insulin resistance, inflammation, oxidative stress, endothelial dysfunction. Interestingly, the same pathogenic events largely account for the less-than-expected response to antiplatelet agents, namely low-dose aspirin. The proposed explanations for this phenomenon, besides underdosing of drug and/or reduced bioavailability, subsequent to excess of adipose tissue, include enhanced platelet turnover, leading to unacetylated COX-1 and COX-2 in newly formed platelets as a source of aspirin-escaping thromboxane formation; extraplatelet sources of thromboxane, driven by inflammatory triggers; and enhanced lipid peroxidation, activating platelets with a mechanism bypassing COX-1 acetylation or limiting COX-isozyme acetylation by aspirin. This review will address the complex interactions between platelets and the pathogenic events occurring in obesity and MS, trying to translate this body of mechanistic information into a clinically relevant read-out, in order to establish novel strategies in the prevention/treatment of atherothrombosis.

Abstract

Newer therapeutic approaches are being employed for the treatment of diabetes mellitus, and among these, acarbose, an α-glucosidase inhibitor, has proven effective in lowering postprandial blood glucose levels and glycosylated hemoglobin. The aim of the present study was to evaluate the effects of acarbose in non-insulin-dependent diabetes mellitus (NIDDM) patients. Thirty-four NIDDM outpatients were enrolled who were followed at our Diabetic Clinic of the Internal Medicine Institute-Chieti University. They were randomized according to a case-control study protocol. The observation period lasted 3 months and tests were performed on the first, second, and third month. Group 1 patients received acarbose 50 mg, three times a day for the first month and 100 mg three times a day for the remaining 2 months. Subjects treated with acarbose showed a reduction of 22% in blood glucose (BG) (P < 0.001) and of glycosylated hemoglobin (HbA1c) (17%-P < 0.001) at the end of the study, while the control subjects presented only a slight reduction (10%) of BG (P < 0.05). After case-control analysis, BG and HbA1c were significantly lower in the case subjects (P < 0.005 and P < 0.02, respectively). In patients with NIDDM, acarbose is a well-tolerated drug with short-term effects mainly on blood glucose levels, while the lowering effects on HbA1c require longer observation periods.
