[Background] The value of neoadjuvant chemotherapy in soft tissue sarcoma (STS) is not completely understood. This study investigated the benefit of neoadjuvant chemotherapy according to prognostic stratification based on the Sarculator nomogram for STS., [Methods] This study analyzed data from ISG-STS 1001, a randomized study that tested 3 cycles of neoadjuvant anthracycline plus ifosfamide (AI) or histology-tailored (HT) chemotherapy in adult patients with STS. The 10-year predicted overall survival (pr-OS) was estimated with the Sarculator and was stratified into higher (10-year pr-OS < 60%) and lower risk subgroups (10-year pr-OS ≥ 60%)., [Results] The median pr-OS was 0.63 (interquartile range [IQR], 0.51-0.72) for the entire study population, 0.62 (IQR, 0.51-0.70) for the AI arm, and 0.64 (IQR, 0.51-0.73) for the HT arm. Three- and 5-year overall survival (OS) were 0.86 (95% confidence interval [CI], 0.82-0.93) and 0.81 (95% CI, 0.71-0.86) in lower risk patients and 0.69 (95% CI, 0.70-0.85) and 0.59 (95% CI, 0.51-0.72) in the higher risk patients (log-rank test, P = .004). In higher risk patients, the 3- and 5-year Sarculator-predicted and study-observed OS rates were 0.68 and 0.58, respectively, and 0.85 and 0.66, respectively, in the AI arm (P = .04); the corresponding figures in the HT arm were 0.69 and 0.60, respectively, and 0.69 and 0.55, respectively (P > .99). In lower risk patients, the 3- and 5-year Sarculator-predicted and study-observed OS rates were 0.85 and 0.80, respectively, and 0.89 and 0.82, respectively, in the AI arm (P = .507); the corresponding figures in the HT arm were 0.87 and 0.81, respectively, and 0.86 and 0.74, respectively (P = .105)., [Conclusions] High-risk patients treated with AI performed better than predicted, and this adds to the evidence for the efficacy of neoadjuvant AI in STS., This was a nonfunded analysis. In the ISG-STS 1001 trial, PharmaMar provided trabectedin for the HG-MRLPS cohort. That trial was partially funded through a European Union grant (EUROSARC FP7 278472).

Human population is facing a revolutionary change in the demographic structure with an increasing number of elderly people requiring an unmet need to ensure a smooth aging process and dental care is certainly an important aspect that has to be considered. To date, dentistry has been conservative and the need of transferring the scientific models of regenerative dentistry into clinical practice is becoming a necessity. The aim of this study was to characterize the differentiation commitment (in vitro) and the clinical grafting ability (in vivo) of a population of progenitor stem cells obtained after mechanical digestion of dental pulp with an innovative system recently developed. This approach was successfully used in previous studies to obtain a clinical-grade ready to use dental pulp fragments that could be grafted in autologous tissues to obtain bone. We are thus showing that micro grafts resulting from mechanical digestion contain stem cells with a mesenchymal phenotype, able to differentiate toward different cell types and to generate new bone in patients. We are providing data for the establishment of standardized and routinely oral surgery approaches, having outlined the cellular properties of human stem cells obtained from the dental pulp. This method can represent a valid tool for both regenerative medicine and tissue engineering purposes not only applicable to the cranio-maxillofacial region but, likely, to different bone pathologies for a fastening and healing recovering of patients. Autologous graft is considered the gold standard of graft materials; however, this approach is still limited due to both small amount of tissue that can be collected and to reduced cell viability of cells that can be obtained. The aim of this preliminary study was to demonstrate the efficacy of an innovative medical device called Rigeneracons® (CE certified Class I) to provide autologous micro-grafts immediately available to be used in the clinical practice. Moreover, Rigeneracons® is an instrument able to create micro-grafts enriched of progenitors cells which maintain their regenerative and differentiation potential. We reported preliminary data about viability cell of samples derived from different kind of human tissues, such as periosteum, cardiac atrial appendage biopsy, and lateral rectus muscle of eyeball and disaggregated by Rigeneracons®. In all cases we observed that micro-grafts obtained by Rigeneracons® displayed high cell viability. Furthermore, by cell characterization of periosteum samples, we also evidenced an high positivity to mesenchymal cell markers, suggesting an optimal regenerative potential.

The egg, a fantastic little laboratory of molecular biology, has played a crucial role in redefining modern biology by moving it from the description of living things to the synthesis of living things (synthetic biology). Over the centuries, many hypotheses have been advanced concerning the egg's role in reproduction-from the preformation theory until von Baer's discovery to the present, with the 2012 Nobel Prize for Physiology or Medicine celebrating the egg as a totipotent stem cell able to reprogram fully differentiated somatic nuclei. The molecular dissection of its cytoplasmic components makes the egg an ideal bioreactor for several biotechnological applications, including pharmacological and food production sciences. In addition to its ubiquitous contribution to the worldwide diet, the egg, a powerful symbol, pervades philosophy, art, religion, and idiomatic expressions.

A gradual restriction in stem cells potency is observed during the different stages of mammalian embryonic development, starting with the zygote, the totipotent stem cell par excellence, up to the blastocyst stage. Stem cells of the inner cell mass are endowed with pluripotent characteristic, being able to form cells of the three germ layers (ectoderm, endoderm, and mesoderm). Pluripotent stem cells retain proliferative capacity throughout the life of an individual, and divide asymmetrically, wherein one cell is the copy of the original stem cell while the other is programmed to begin the differentiation process. In mammals, pluripotent stem cells can also be found in the fetus and in some adult tissues, including the umbilical cord blood

Using a semi-quantitative, single-cell sensitive RT-PCR method, we studied the expression of oogenesis specific genes (Nobox, Oct4, Bmp15, Gdf9, Oogenesin1 and Oogenesin2) in single oocytes collected from primordial, primary, secondary, preantral and antral follicles during natural and gonadotropin-induced mouse follicular development. We compared the number of transcripts of these genes, showing that they are differentially expressed, both in natural conditions and under gonadotropin-induction throughout the assessed developmental stages. Our data show a clear increase in the number of transcripts between the primordial until the preantral stages, with the exception of the Oogenesin1 transcripts under gonadotropin-induction. The number of transcripts starts decreasing at the antral stage and proceeds until the metaphase II stage, with values very similar to those obtained for the primordial oocytes in both analyzed conditions. Under exogenous gonadotropin-induction, oocyte recruitment to ovulation at the preantral stage is marked by an increase in Nobox and Oogenesin2 gene expression that is concomitant with a decrease in Oogenesin1 gene expression. Oocytes that are able to proceed into whole embryo development show a tight regulation of Nobox and Oct4 expression at the antral stage. A parallel immunocytochemical study at the protein level corroborates these findings.
