Although the mean score of the Children's Sleep Habits Questionnaire (CSHQ) differs between countries, there are no normative data for the CSHQ of Japanese preschoolers based on a community sample. The aims of this study were therefore to present normative data for the CSHQ and determine the prevalence and characteristics of sleep problems in Japanese preschoolers. Parents or the primary caregiver of 482 preschoolers aged 4-5 years completed the CSHQ and the Strength and Difficulties Questionnaire. Approximately 80% of preschoolers scored above the cut-off for sleep disturbance on the CSHQ. In addition, co-sleeping was prevalent in Japanese preschoolers but the habit of co-sleeping contributed little to behavioral and emotional problems. Sleep problems appear to be prevalent in Japanese preschoolers based on the CSHQ, and could be associated with the Japanese sleep habit of co-sleeping.

Objective Although sexual dysfunction is believed to be caused by hormonal abnormalities, few reports have studied sexual dysfunction and its association with hormonal abnormalities in Asian populations with schizophrenia. Methods We employed a cross-sectional, case–control survey design to collect data from 191 (108 men) Japanese schizophrenia outpatients treated with antipsychotics and 182 (49 men) healthy subjects. Sexual dysfunction was evaluated using the Udvalg for Kliniske Undersogelser (UKU) Side Effect Rating Scale. We measured plasma concentrations of prolactin in both genders and testosterone in men and estradiol in women. A 77‐year‐old man who had been taking Chinese herbal drugs containing gardenia fruit for a long time visited our hospital following a positive fecal occult blood test. A computed tomography scan showed multiple linear calcifications distributed on the right‐side mesenteric veins, which is a typical finding of idiopathic mesenteric phlebosclerosis (IMP).

6. No association between bone mass and prolactin levels among patients with schizophrenia

Objective Decreased bone mineral density has been implicated in schizophrenic patients for long years. The purpose of this study was to assess the relationship between bone mass and prolactin levels in schizophrenic patients. Methods In this study, bone mass was measured using quantitative ultrasound densitometry of the calcaneus in 114 patients (49 males and 65 females). The osteosono-assessment index (OSI) was calculated as a function of the speed of sound and transmission index. Estradiol, testosterone, and prolactin levels were also measured. Factors that influenced prolactin levels and bone mass were determined via multiple linear regression analysis. Results Among the female patients, body mass index and estradiol levels were independently and significantly associated with the OSI. Neither prolactin levels nor duration of antipsychotic treatment was associated with bone mass for either gender. Conclusions These findings suggest that prolactin levels do not contribute to poor bone mass, as assessed using the OSI among schizophrenic patients. However, the interpretation of our results was hampered by lack of data including differences in lifestyle and type of antipsychotic medications used in schizophrenic patients. Association between prolactin levels and low bone mineral density was not completely ruled out. Future research exploring prolactin levels and bone mineral density is warranted. Recently a new sustained-release formulation of valproic acid has been developed in Japan. The sustained-release mechanism of the new formulation was different from the conventional formulation. The aim of the present study was to compare the pharmacokinetic characteristics of valproic acid in two sustained-release formulations. Different sustained-release formulations of valproic acid (Depakene R and Selenica R) were administered in a randomized cross-over fashion in repeated doses in 24 psychiatric patients. After > or = 4 weeks administration of valproic acid once daily, blood samples were taken just before (0 h) and 8, 12, 24 h after the morning dose. Blood sampling was performed in the same manner in the same patients 4 weeks after switching from one to the other formulation of valproic acid. Serum concentrations of valproic acid at 0 h (50.7 +/- 19.4 vs 44.9 +/- 21.8 microg/mL, P < 0.05) and 24 h (52.3 +/- 19.54 vs 6.2 +/- 22.2 microg/mL, P < 0.05) were significantly higher during Selenica R than during Depakene R treatment, whereas the serum concentration of valproic acid at 8 h (49.7 +/- 19.2 vs 62.4 +/- 25.6 microg/mL, P < 0.01) was significantly lower during Selenica R treatment than during Depakene R treatment. Serum concentrations of valproic acid at 12 h were not different. The present study demonstrated that steady-state serum concentrations were different because of the different dissolution profiles. When a prescription for valproic acid is switched from one drug to the other, prescribers should be aware that the therapeutic drug monitoring data are not consistent.

Polymorphic enzyme cytochrome P450 (CYP) 2C19 is expressed not only in the liver but also in the brain and mediates the biotransformation of 5-hydroxytriptamine (5-HT). We investigated possible association between genetic polymorphism of CYP2C19 and individual personality traits, possibly influenced by neurotransmitters. Mentally and physically healthy Japanese subjects were enrolled in this study (n = 352). Temperament and Character Inventory (TCI) and CYP2C19 genotyping were performed in all subjects. We detected CYP2C19*2 and *3 (http://www.imm.ki.se/CYPalleles/) using Amplichip CYP450 DNA tip. The number of genotypes classified as homozygous extensive metabolizer (EM), heterozygous EM, and poor metabolizer were 113, 181, and 58, respectively. Significant difference was found in TCI score in harm avoidance (HA; F = 3.138, P < 0.05). Post hoc analysis showed that TCI score in harm avoidance in homozygous EM was significantly lower than that in heterozygous EM (P < 0.05) or PM (P < 0.05). In sub-item analyses, HA3 (shyness with strangers, P < 0.01) and HA1 (anticipatory worry, P < 0.05) of TCI scores were significantly different among CYP2C19 genotypes. Meanwhile, there were no differences in TCI scores of novelty seeking (NS; F = 0.350, n.s.), reward dependence (RD; F = 1.080, n.s.), or persistence (P; F = 0.786, n.s.) among CYP2C19 genotypes. This study demonstrated that a significant association between CYP2C19 activity and HA is present in Japanese.

The aim of the present study was to investigate whether there is an association between the (TCAT)(n) repeat polymorphism in the tyrosine hydroxylase (TH) gene and personality. The (TCAT)(n) repeat polymorphism in the TH gene was genotyped in 898 healthy Japanese subjects. Personality traits were evaluated using the Temperament and Character Inventory (TCI). There was no significant difference in the TCI scores of subjects with and without the T9 allele. Furthermore, no significant association was found between each genotype and the TCI scores, even when the TCI scores were compared with the homozygous genotype. These findings suggest that the (TCAT)(n) repeat polymorphism in the TH gene does not contribute to the personality traits evaluated on the TCI in healthy Japanese subjects.

To obtain low polymeric polystyrene (PS), pyrolysis of high polymeric PS in solution was studied in the temperature range from 290 to 400°C by using additives or acid catalysts. The low polymeric PS targeted here was that with average molecular weight of 104. When the feed PS was pyrolyzed in tetralin by adding sulfur or diphenyl disulfide, the molecular weight of PS decreased greatly, even at lower temperatures, and the desired low polymeric PS was formed in a relatively large amount at the temperatures below 350°C. The degradation behavior was able to be explained in terms of a random polymer chain scission mechanism initiated by sulfur radicals formed from the additives. © 1998 John Wiley & Sons, Inc.
