We present male monozygotic twins who showed quite different clinical features. Blood chromosome analysis revealed 46, XY/46, XY, de1(7) (q32 qter) mosaicism in both twins. However, cultured skin fibroblasts from the twins showed different karyotypes. Twin 1, with a normal phenotype, had normal chromosomes and was 46,XY. Twin 2, on the other hand, had the characteristic manifestations of 7q — syndrome and chromosomes of 46, XY, del(7) (q32 qter). DNA fingerprint analysis of their peripheral blood samples revealed the same pattern. However, DNA fingerprint patterns of cultured skin fibroblasts and buccal mucosal cells were different when a 7q terminal marker, probe 83, was used. These identical twins with discordant phenotypes can be explained by the occurrence of twinning and simultaneous erroneous mitosis. In addition, there might be a vascular communication which probably resulted in blood exchange and chromosomal mosaicism of the lymphocytes of the monozygotic twins.

Medium-sized sulfur- or selenium-containing heterocyclic compounds were synthesized by the reductive cross-piece bond cleavage of bicyclic sulfonium and selenonium salts bearing a bridgehead sulfur or selenium atom with magnesium metal or sodium borohydride.

Thieno[3,2-f]morphans (4-7), (27) and (28) were prepared by cyclization of 2-thenyl-4-hydropiperidines (3) or 2-thenyltetrahydropyridine (26) with hydrobromic acid. Thieno[4,3-f]morphans (37) and (38) and 3-thiamorphinan (47) were similarly prepared. Some kinds of N-sub and a 3-thiamorphinan (48). Analgesic activities of the N-substituted compounds were investigated

1. In canine antrum, rhythmic electrical activity consists of a rapid upstroke phase followed by a plateau depolarization. In response to slow waves, cytosolic Ca2+ ([Ca2+]cyt) and tension increased. 2. Addition of sodium nitroprusside (SNP, 0.5 microM) decreased the amplitude of the plateau phase of slow waves without significant effects on the upstroke depolarization. SNP also inhibited changes in [Ca2+]cyt and tension associated with the plateau potential. SNP induced a negative chronotropic effect at concentrations above 0.1 microM. 3. Similar to the effects of SNP, illumination of muscles during slow waves with ultraviolet (UV) light caused premature repolarization. UV illumination is known to release NO in some tissues. 4. L-NG-monomethyl-arginine (L-NMMA, 300 microM), Methylene Blue (MB, 5 microM) and oxyhaemoglobin (oxy-Hb, 5 microM) increased the force of contractions. In contrast, L-arginine (L-Arg, 300 microM) decreased contractile force and antagonized the effects of L-NMMA. 5. During the upstroke phase, SNP caused a small reduction in [Ca2+]cyt and a large reduction in force, suggesting that SNP caused a decrease in Ca2+ sensitivity. 6. In muscles permeabilized by alpha-toxin, cyclic GMP (100 microM) and UV illumination inhibited Ca(2+)-induced contraction (at pCa 5.5). 7. These data suggest that NO or NO-related compounds are spontaneously released in gastric muscles. These agents have two effects on excitation-contraction coupling: (i) inhibition (directly and/or indirectly) of the voltage-dependent Ca2+ channels that participate in the plateau phase of slow waves, and (ii) reduction in the Ca2+ sensitivity of the contractile element.

Purpose: Final goal is to develop a technique to detect the 125I seed and measure its strength during implantation process. This will be a backup tool for QA by offering an additional measurement apart from the pre‐operation measurement. In actual implantation, the speed of the seed varies because it is conducted manually. In order to develop a simple and stable system usable in any speed, this study supposes to use single detector and repeat short‐time measurements. This report summarizes a feasibility test. Methods: The 125I seeds (Oncra Inc. Oncoseed 6711) was moved in speeds up to 200 mm/s by an electric actuator. Its strength was measured with a NaI detector (1″diam. × 1″) set at 170 mm from the needle guiding the seed. The requirement for the counting time was investigated to estimate the strength regardless the seed speed. Results: In the measurement where the seed was set static in needle, the change in counting rate of the NaI detector was 4 % at the 30 mm‐long region along the needle, while the counting rate rapidly decreased outside the region. It is desirable to start‐stop the measurement while the seed is within this region. Assuming 200 mm/s as the maximum speed, 7 to 8 measurements can be performed for the counting time of 20 ms, 3 measurements at 50 ms, 1 measurement at 100 ms. By using the average of the measured results while the seed was within the region, the estimated seed strength agreed within 10 % between the speeds 0 and 200 mm/s at counting time of 100 ms or less. Conclusion: This report shows the feasibility in principle to estimate the 125I seed strength during the implantation operation, regardless the seed velocity.

2, 6-Epithio-3-benzazocines (9-thiabenzomorphans) in which the carbon atom at the 11-position is replaced by a sulfur atom, were synthesized by treatment of 1-(2-ethoxycarbonylaminoethyl)isothiochroman asulfoxides (16) with acetic anhydride or by heating 3-acetoxyisothiochromans (17) in Dowtherm A.The hetero-acetal moiety of this novel heterocycle (15) was stable to lithium aluminum hydride and boron tribromide. Reduction of 15 with lithium aluminum hydride gave the N-methyl derivative (19) and demethylation of the 8-methoxy derivative (19b) with boron tribromide gave the 8-hydroxy derivative (20).

This is a retrospective study of 163 patients with Hodgkin disease and 364 patients with other lymphomas who were admitted to one Cancer Hospital (1940--1971). All the histological slides were reviewed and reclassified by one pathologist. In patients who had multiple and sequential biopsies (16 with Hodgkin disease and 97 with non-Hodgkin lymphomas), the histological subtypes remained the same in over 60% of the cases. When changes did occur, the neoplasms progressed toward histologically more malignant forms.

Carcinomatous involvement of the liver mimicking cirrhosis is a rare complication of metastatic carcinoma, most frequently seen with scirrhous carcinoma of the breast. The case of a 46-year-old woman with breast carcinoma presenting with ascites, jaundice, spider angiomata, and portal hypertension is reported. On hepatic scan the liver appeared to be almost the same size as the spleen, a finding frequently seen in cirrhosis. A chemotherapeutic program was introduced, but was of no benefit, and the patient died with uncontrolled esophageal bleeding. The prominent histologic feature on autopsy was intense stromal fibrosis and intravascular tumor infiltration with compression of vessels of the portal system.

A rare intrapulmonary tumor (chemodectoma) detected by routine chest x-ray is found in a 61-year-old white female. This is the fifth case of this tumor in this location reported in the literature. The tumor is of interest because it further documents the presence of chemoreceptors in the lung.

There were 30 cases of cylindroma at Ellis Fischel State Cancer Hospital from 1940 to 1969. Of these 30 cases, 22 were seen initially at Ellis Fischel. Fourteen patients died of cylindroma; the average survival from diagnosis to death was 66 months. Reappearance occurred in 20 of the 30 cases, with the interval between the first reappearance of tumore being 32 months. Tumor was located in the major salivary gland in 9 cases, minor salivary gland in 8 cases, and paranasal sinuses in 3 cases. Tumor frequently invaded bone, became fixed to surrounding structures, and not infrequently metastasized widely. Treatment consisted of surgery and/or radiation therapy. Of the 22 cases seen initially at Ellis Fischel, 16 were pure cylindromas, 2 were mixed with mucoepidermoid, 1 was mixed with adenocarcinoma, and 3 were mixed with epidermoid carcinoma. Four patients were cured of their cylindormas, as determined by survival at 10 or more years. All these cures were arrived at through surgical extirpation of the tumor. Radiotherapy and infusion chemotherapy gave good palliation but no cures. Adequacy of margin around the tumor was the single most important factor in cure. It is found that biopsy of the tumor did not alter survival. Patients with parotid tumors did much better than tumors located in other areas. Fixation of the tumor mass did not alter survival and nodal involvement was an ominous sign. The study indicated the way to improve survival was by resecting the tumor re-resecting until all margins are adequate.

The purpose of this investigation was to initiate the study of dental fear in Japan. 415 college students, aged 18-22 yr were surveyed. A standardized questionnaire which has been used in the United States was translated into Japanese and was administered to the students. More than 80% of those surveyed reported some dental fear. Six to 14% of the students reported extreme fear of the dentist. The majority of the subjects admitted that they delayed making dental appointments due to fear. Muscle tension was the most common physiological symptom reported. The dental drill and needle were the most fear-provoking stimuli.

The reaction of 2-methylisoselenochromanium salt with Grignard reagents afforded the reductive ring-opened product by the single electron transfer (SET) mechanism, not by the self-decomposition of σ-selenurane. The same reduction was observed in the reaction of the selenonium salt with magnesium metal. Some other selenonium salts were easily reduced by magnesium metal to give ring-opened products.
