Abstract

The connection between adipose glucocorticoid action and whole-body metabolism is incompletely understood. Thus, we generated adipose tissue–specific glucocorticoid receptor–knockout (Ad-GcR(−/−)) mice to explore potential mechanisms. Ad-GcR(−/−) mice had a lower concentration of fasting plasma nonesterified fatty acids and less hepatic steatosis. This was associated with increased protein kinase B phosphorylation and increased hepatic glycogen synthesis after an oral glucose challenge. High-fat diet (HFD)–fed Ad-GcR(−/−) mice were protected against the development of hepatic steatosis and diacylglycerol-PKCε–induced impairments in hepatic insulin signaling. Under hyperinsulinemic-euglycemic conditions, hepatic insulin response was ∼10-fold higher in HFD-fed Ad-GcR(−/−) mice. Insulin-mediated suppression of adipose lipolysis was improved by 40% in Ad-GcR(−/−) mice. Adipose triglyceride lipase expression was decreased and insulin-mediated perilipin dephosphorylation was increased in Ad-GcR(−/−) mice. In metabolic cages, food intake decreased by 3 kcal/kg per hour in Ad-GcR(−/−) mice. Therefore, physiologic adipose glucocorticoid action appears to drive hepatic lipid accumulation during stressors such as fasting. The resultant hepatic insulin resistance prevents hepatic glycogen synthesis, preserving glucose for glucose-dependent organs. Absence of adipose glucocorticoid action attenuates HFD-induced hepatic insulin resistance; potential explanations for reduction in hepatic steatosis include reductions in adipose lipolysis and food intake.—Abulizi, A., Camporez, J.-P., Jurczak, M. Abstract

Cholesterol modification of synthetic and biological polymers is achieved using of thiocholesterol (TC) and thiol-disulfide exchange. TC is reacted with Ellman's reagent to produce a mixed disulfide (TC-ER) which is activated towards thiol-disulfide exchange. TC-ER is used to obtain an inclusion complex with methyl-β-cyclodextrin, which is then employed to achieve cholesterol functionalization of a model peptide, synthetic polymers, and physical hydrogels based on poly(vinyl alcohol). It is anticipated that the established techniques will significantly broaden the use of cholesterol in bio- and nanotechnology and specifically biomedicine.

Abstract

The current mode of administration of recombinant tissue-type plasminogen activator (rt-PA) in acute myocardial infarction is rather complex, although the rationale for the different components of this scheme is not clearly established. We compared pharmacokinetics of a continuous infusion of 38.5 MU of Burroughs Wellcome t-PA (duteplase) over 90 minutes in nine patients (phase I) with a scheme including a 0.04 MU/kg bolus, a 60-minute 0.36 MU/kg lytic infusion and a 180-minute 0.21 MU/kg maintenance infusion in 15 patients with acute myocardial infarction (Phase II). t-PA activity and antigen were fitted in a one-compartment model from which model-dependent and model-independent parameters were derived. Clearance of t-PA activity was 1020 +/- 465 (mean +/- SD) ml/min in phase I and 1359 +/- 590 ml/min in phase II. Clearance of t-PA antigen was 666 +/- 230 ml/min in phase I and 704 +/- 199 ml/min in phase II. Clearance of activity was significantly (p less than 0.01) higher than of antigen. Clearance and steady-state plasma levels showed a large interindividual variability (coefficient of variation, 56.4%), but this was significantly reduced by dosing by weight (coefficient of variation, 28.9%; p = 0.031). A 10% bolus in phase II shortened the time to reach 75% and 90% of the steady-state plasma level by 4 and 5 minutes, respectively, not significantly different from phase I. A simulation study showed that a bolus should be approximately 15% of the lytic dose to achieve a maximal level in the shortest period.
