Denture adhesives are designed to be moisture-sensitive through the inclusion of a blend of polymer salts with varying degrees of water-sensitivity. This enables the adhesive to mix with saliva in vivo and activate its high tack, through the formation of a mucilaginous layer. We report for the first time, the use of differential scanning calorimetry to study a series of hydrophobic and hydrophilic polymeric systems in order to correlate water-structuring behavior with adhesion strength. Adhesive bonding of the more hydrophobic variants was higher than that of a commercial-based control and a more hydrophilic polymer system in both lap shear and tensile configurations. Water-binding data suggested that increasing the hydrophobicity of the maleic acid copolymer substituents led to decreased levels of freezing water. In comparison, increasing the hydrophilic nature of the polymer backbone gave higher levels of freezing water within the hydrated samples. The results of this study emphasize the importance of varying the levels of hydrophobic and hydrophilic components within denture adhesive formulations, alongside the types of water present within the adhesive systems. This phenomenon has shown the potential to fine-tune the adhesive properties and failure mode against poly(methyl methacrylate), surfaces. © 2018 Wiley Periodicals, Inc. Long‐term care services are provided to help people manage the consequences of impairment, but their impact goes beyond the meeting of basic needs. Accordingly, the main aim was to explore the marginal effectiveness of care when measured in terms of people's overall care‐related quality of life (CRQoL) and assess changes in marginal effect for increasing intensity. The associated aim was to refine and apply an observational method to estimate marginal effectiveness. A “production function” approach was used with survey data, including Adult Social Care Outcomes Toolkit‐measured CRQoL, whereby we statistically modelled the expected relationship between service utilisation rates and CRQoL. This method seeks to limit endogeneity issues by controlling on observables and using instrumental variable. Using a survey of publicly funded long‐term care service users in England, we found that community‐based long‐term care significantly improved people's CRQoL but with diminishing marginal effects and effects differentiated by baseline impairment levels. There are implications for how the care system should respond to changes in global public budgets. For example, where there is unmet need, a system aimed to maximise (unadjusted) CRQoL would put more emphasis on access (more recipients) than intensity of support compared to a system operating on a needs basis.

Background This study reports the experiences of developing and pre-testing an Easy Read version of the Adult Social Care Outcomes Toolkit (ASCOT) for self-report by people with intellectual disabilities. Methods The study has combined survey development and pre-testing methods with approaches to create accessible information for people with intellectual disabilities. A working group assisted researchers in identifying appropriate question formats, pictures and wording. Focus groups and cognitive interviews were conducted to test various iterations of the instrument. Results Substantial changes were made to the questionnaire, which included changes to illustrations, the wording of question stems and response options. Conclusions The process demonstrated the benefits of involving people with intellectual disabilities in the design and testing of data collection instruments. Adequately adapted questionnaires can be useful tools to collect information from people with intellectual disabilities in survey research; however, its limitations must be recognized.

This study investigates the causes of full closure of care homes in the English care home/nursing home market. We develop theoretical arguments about two causes for closure that are triggered by errors or external shocks: poor economic sustainability and regulatory action. Homes aiming to operate with lower quality in the market are argued for a number of reasons to be more susceptible to errors/shocks in setting quality, especially negative errors, leading to an empirical hypothesis that observed quality should negatively affect closure chance. In addition, given quality, homes facing relatively high levels of local competition should also have an increased chance of closure. We use a panel of care homes from 2008 and 2010 to examine factors affecting their closure status in subsequent years. We allow for the potential endogeneity of home quality and use multiple imputation to replace missing data. Results suggest that homes with comparatively higher quality and/or lower levels of competition have less chance of closure than other homes. We discuss that the results provide some support for the policy of regulators providing quality information to potential purchasers in the market.

In many countries, public responsibility over the funding and provision of long-term care services is held at the local level. In such systems, long-term care provision is often characterised by significant local variability. Using a panel dataset of local authorities over the period 2002-2012, the paper investigates the underlying causes of variation in gross social care expenditure for older people in England. The analysis distinguishes between factors outside the direct control of policy makers, local preferences and local policy spillovers. The results indicate that local demand and supply factors, and to a much lesser extent local political preferences and spatial policy spillovers, explain a large majority of the observed variation in expenditure.

We investigate electronic band-structure images in reciprocal space of few-layer graphene epitaxially grown on SiC(000-1). In addition to the observation of commensurate rotation angles of the graphene layers, the k-space images recorded near the Fermi edge highlight structures originating from diffraction of the Dirac cones due to the relative rotation of adjacent layers. The 21.9° and 27° rotation angles between two sheets of graphene are responsible for a periodic pattern that can be described with a superlattice unit cells. The superlattice generates replicas of Dirac cones with smaller wave vectors, because of a Brillouin zone folding. Copyright © 2014 John Wiley & Sons, Ltd.

The aim is to describe and trial a pragmatic method to produce estimates of the incremental cost-effectiveness of care services from survey data. The main challenge is in estimating the counterfactual; that is, what the patient's quality of life would be if they did not receive that level of service. A production function method is presented, which seeks to distinguish the variation in care-related quality of life in the data that is due to service use as opposed to other factors. A problem is that relevant need factors also affect the amount of service used and therefore any missing factors could create endogeneity bias. Instrumental variable estimation can mitigate this problem. This method was applied to a survey of older people using home care as a proof of concept. In the analysis, we were able to estimate a quality-of-life production function using survey data with the expected form and robust estimation diagnostics. The practical advantages with this method are clear, but there are limitations. It is computationally complex, and there is a risk of misspecification and biased results, particularly with IV estimation. One strategy would be to use this method to produce preliminary estimates, with a full trial conducted thereafter, if indicated.

Ever since the failed 1999 Royal Commission, England has been attempting to reform its long-term care funding system. More than a decade later, significant changes to the means tested arrangements are yet to be introduced, whilst the pressure to achieve long-term reform mounts linked to increases in public expenditure and ever growing demand for better services. This paper examines the pros and cons of alternative options for reforming the English long-term care funding arrangements by examining the rationale for and consequences of the recent long-term care developments in Germany, Japan and France. In particular, the paper examines the implications of the reform options adopted in the different countries examined for equity and efficiency in the use of long-term care resources and for the sustainability of the long-term care system as a whole. Micron scale ferroelectric (FE) domains have been written into a 20-nm-thick epitaxial thin film of BaTiO3 (001) [BTO(001)] on a Nb-doped SrTiO3 substrate using a piezoforce microscope (PFM). The domain-dependent electronic structure has been studied using fully energy-filtered photoemission electron microscopy (PEEM) and synchrotron radiation. Shifts, induced by FE polarization, of up to 300 meV were observed in the work function of the sample. The surface is Ba-O terminated. Polarization-induced distortion of the electronic structure was observed in the valence band and on the Ba 3d, Ti 2p and O 1s core levels of BTO. Polarization-dependent surface adsorption was observed. A simple electrostatic model based on net surface charge is not sufficient to explain the observed modifications in the electronic levels. The allocation of central government funds is a critical element in the equitable provision of local authority-commissioned and -provided services. A variety of approaches to allocating funding for social services for older people have been used over the years, most recently founded on ‘needs-based’ formulae. In 2004, the Department of Health for England commissioned research to help inform the improvement and updating of the formula. The results of individual-level analyses were compared with the results obtained from analyses of small area (ward-level) data on service users. Both analyses were affected by problems of data availability, particularly the individual-level analysis, and the Department of Health and the (then) Office of the Deputy Prime Minister decided that the formula calculations should be based on the results of the small area analysis. However, despite the differences in approach, both methods produced very similar results. The correlation between the predicted relative needs weights for local authorities from the two models was 0.982. The article discusses the strengths and weaknesses of each approach and developments that could allow a normative approach that would incorporate future policy objectives into formulae that, to date, have inevitably been based on historical data and service patterns.

The redox properties of quinones play a key part in photosynthesis and oxidative phosphorylation. This mini-review focuses on the synthesis and properties of polymers that contain in-chain quinone units. Many of the syntheses exploit Yamamoto or Suzuki couplings. Most of the polymers that are soluble in organic solvents display interesting electrochemical and optical properties that may be useful for a wide range of applications. Conductive polymers are interesting materials for a number of biological and medical applications requiring electrical stimulation of cells or tissues. Highly conductive polymers (polypyrrole and polyaniline)/Antheraea mylitta silk fibroin coated fibers are fabricated successfully by in situ polymerization without any modification of the native silk fibroin. Coated fibers characterized by scanning electron microscopy confirm the silk fiber surface is covered by conductive polymers. Thermogravimetric analysis reveals preserved thermal stability of silk fiber after coating process. X-ray diffraction of degummed fiber diffraction peaks at around 2θ = 20.4 and 16.5 confirms the preservation of the β-sheet structure typical of degummed silk II fibers. This phenomenon implies that both polypyrrole and polyaniline chains form interactions with peptide linkages in degummed fiber macromolecules, without significantly disrupting protein assembly. Fourier transform infrared spectroscopy of coated fibers indicates hydrogen bonding and electrostatic interactions exist between silk fibroin macromolecules and conductive polymers. Resulting fibers display good conductive properties compared to corresponding conjugated polymers. In vitro analysis (live/dead assay) of the behavior of human immortalized keratinocytes (HaCaTs) on coated fibers demonstrates improved cell-adhesive properties and viability after polymers coating. Hence, polypyrrole- and polyaniline-coated A. mylitta silk fibers are suitable for application in cell culture and for tissue engineering, where electrical conduction properties are required.

A variety of contract types are used in the placement of elderly people in residential and nursing care homes in the UK. Contracts vary according to how and when providers are paid. Among other things, prices can be made contingent on the total quantity of service to be purchased and on production cost characteristics. They can be determined at the time of placement or in advance. The primary objective of this paper is to assess the impact of contract choices on the price of placements. Regression analysis was conducted on a final sample of 1780 publicly funded placements made in 18 local authorities in the UK over a 6-month period ending in early 1996. Controlling factors included in the price analysis were production cost indicators and those measuring market competitiveness. Choices of both quantity and cost contingent contracts were found to be significantly associated with placement prices. The findings support the hypothesis that contract payment arrangements have different risk, insurance and information properties, and so have implications for the performance of residential care providers.
