Background: Chronic pelvic pain, in particular dysmenorrhoea, is a significant yet unresolved healthcare problem in gynaecology. As interoceptive sensitivity and underlying neural mechanisms remain incompletely understood, this functional magnetic resonance imaging (fMRI) study assessed behavioural and neural responses to visceral stimuli in primary dysmenorrhoea (PMD)., Methods: Women with PMD (N = 19) without psychological comorbidity and healthy women (N = 20) were compared with respect to visceral sensory and pain thresholds, and to neural responses to individually calibrated mildly painful and painful rectal distensions implemented during scanning. Trial-by-trial ratings of perceived intensity were accomplished with visual analogue scales (VAS)., Results: Although women with dysmenorrhoea reported significantly higher chronic pain intensity and pain interference with daily life activities (p < 0.01, assessed with the West Haven-Yale Multidimensional Pain Inventory), there were no differences between groups in visceral sensitivity and mean trial-by-trial VAS ratings were virtually identical. Analysis of neural responses revealed activation in brain regions previously shown to be involved in the processing of visceral stimuli with differences between painful and mildly painful stimulation, but no group differences were found even when using a liberal statistical threshold., Conclusions: Dysmenorrhoea patients show unaltered perceptual and neural responses to experimental interoceptive stimuli. Despite limited sample size, these negative results argue against a generalized sensitization towards interoceptive stimuli in patients without psychological comorbidities. Future studies should clarify the role of psychosocial factors in central sensitization using more pain region-specific models in larger and clinically more heterogeneous samples., Significance: Despite higher chronic pain and pain interference with daily life activities, women with primary dysmenorrhoea do not differ from healthy women with respect to visceral sensitivity or neural processing of aversive interoceptive stimuli induced by rectal distensions. It has been recently shown that 20 min of mechanical flutter stimulation induces lasting motor cortical excitability changes, as assessed by transcranial magnetic stimulation in relaxed hand muscles. The present functional magnetic resonance imaging (fMRI) study aims to examine if such neuromodulatory changes are reflected in the BOLD signal during a motor test. Therefore, two groups were recruited: one group receiving whole-hand flutter stimulation with a frequency of 25 Hz (FSTIM group, n = 22) and a second group receiving no stimulation (NOSTIM group, n = 22). As motor test finger-to-thumb tapping was performed to activate a wide sensorimotor network during the fMRI measurements. Three fMRI measurements were obtained with this test: before stimulation (PRE), after stimulation (POST1), and 1 h after stimulation (POST2). Three regions of interest (ROIs) were defined: primary motor area (M1), primary somatosensory area (S1), and supplementary motor area. In the absence of baseline differences between both groups, the FSTIM group showed increased movement-related brain activations compared with the NOSTIM group, both at POST1 and POST2. ROI analysis revealed increased blood-oxygenation-level-dependent (BOLD) responses within contralateral S1 (+20%) and M1 (+25%) at POST1, which lasted until POST2. These poststimulatory effects within S1 and M1 obviously reflect neuroplastic changes associated with augmented cortical excitability. As it is the case in brainstorming, each single idea a person generates to a specific problem may stimulate new ideas or solutions in others. In this fMRI study, we investigate the effects of cognitive stimulation via the exposure to other people's ideas on the originality of generated ideas. Participants are requested to generate alternative uses of conventional everyday objects subsequent to a short cognitive stimulation intervention in which they are exposed to other ideas, which were either common or highly original. In a control condition, meaningless pseudowords are shown. Results suggest that cognitive stimulation via common or moderately creative ideas was effective in improving creativity. At the neurophysiological level, temporo-parietal brain regions (primarily right-hemispheric) turned out to be particularly sensitive to cognitive stimulation, possibly indicating that cognitive stimulation via relevant memory cues results in a state of heightened focused attention to memory that facilitates efficient retrieval and recombination of existing knowledge., (Copyright © 2011 Wiley Periodicals, Inc.)

Dynamic susceptibility contrast-MRI is the most commonly used functional MRI-based method for studying changes in cerebral perfusion. However, several studies indicated a systematic overestimation of perfusion parameters compared with other imaging modalities related to the high sensitivity of dynamic susceptibility contrast-MRI for blood flow in large vessels. In this study, we therefore suggest an improved, automated, robust, and efficient method allowing for generating hemodynamic parameter maps where signal influence from large vessels is minimized. Based on independent component analysis, this fully automated approach corrects dynamic susceptibility contrast-MRI data without any user interaction, thus making a clinical applicability possible. The accuracy of the proposed method was tested in 10 patients with cerebrovascular disease. Application of our correction algorithm resulted in a significant reduction of the effect of macrovessel signal on hemodynamic parameters like the cerebral blood flow and the cerebral blood volume compared with uncorrected data. As desired, our method specifically corrected for macrovessel artifacts in cortical grey matter tissue, leaving white matter tissue parameters largely unaffected. Distinct regions in the left inferior frontal gyrus (IFG) preferentially support the processing of different word-types (e.g., real words, pseudowords) and tasks (e.g., lexical decisions, phonological decisions) in visual word recognition. However, the functional connectivity underlying the task-related specialisation of regions in the left IFG is not yet well understood. In this study we investigated the neural mechanisms driving the interaction of WORD-TYPE (real word vs. pseudoword) and TASK (lexical vs. phonological decision) in Brodmann's area (BA) 45 in the left IFG using dynamic causal modelling (DCM). Four different models were compared, all of which included left BA44, left BA45, and left inferior temporal gyrus (ITG). In each model, the visual presentation of words and pseudowords is assumed to directly evoke activity in the ITG and is then thought to be subsequently propagated to BA45 and to BA44 via direct intrinsic connections. The models differed with regard to which connections were modulated by the different tasks. Both tasks were assumed to either modulate the ITG&#95;BA45 connection (Model #1), or the BA44&#95;BA45 connection (Model #2), or both connections in parallel (Model #3). In Model #4 lexical decisions modulated the ITG&#95;BA45 connection, whereas phonological decisions modulated the BA44&#95;BA45 connection. Bayesian model selection revealed a superiority of Model #1. In this model, the strength of the ITG&#95;BA45 connection was enhanced during lexical decisions. This model is in line with the hypothesis that left BA 45 supports explicit lexical decisions during visual word recognition based on lexical access in the ITG.

Months, days of the week, and numbers differ from other verbal concepts because they are ordered in a sequence, whereas no order is imposed on members of other categories, such as animals or tools. Recent studies suggest that numbers activate a representation of their quantity within the intraparietal sulcus (IPS) automatically, that is, in tasks that do not require the processing of quantity. It is unclear, however, whether ordered verbal materials in general and not only numbers activate the IPS in such tasks. In the present functional magnetic resonance imaging study word generation of months, numbers, and animals were compared. Word generation of numbers and nonnumerical materials from an ordered category (months) activated the IPS more strongly than generating items from a not-ordered category such as animals or the verbal control conditions. An ROI analysis of three subregions within the anterior IPS revealed that the most anterior and lateral of these regions, human intraparietal area hIP2, shows a greater sensitivity to ordered materials than the other two areas, hIP1 and hIP3. Interestingly, no difference in activation was observed within the IPS between numbers and months suggesting that the activation of the IPS might not be modulated by the additional quantity information carried by numbers., ((c) 2007 Wiley-Liss, Inc.)
