Purpose Digital breast tomosynthesis (DBT) is becoming increasingly used in clinical practice. In DBT, the microcalcification clusters may span across multiple slices, which makes it difficult for radiologists to directly assess these distributed clusters for diagnosis. We investigated a radiomics method to classify microcalcification clusters in DBT based on a semiautomatic segmentation process. Methods We performed a retrospective study on a cohort of 275 patients (including 79 benign and 196 malignant cases) with a total of 550 DBT volumes. Our method consisted of three steps. The initial step was to semiautomatically segment the microcalcification clusters. Then, radiomics features were extracted from the initially segmented microcalcification clusters. Finally, the benign and malignant microcalcification clusters were differentiated by the random forest (RF) classifier using selected subset features. The radiomics models were evaluated both on view-based and case-based modes with features selected from different domains. The receiver operating characteristic (ROC) curve and area under the ROC curve (AUC) were used to evaluate the classification performance. Results Twenty-six key features were selected from a total of 170 radiomics features and these features show promising classification performance. The highest AUC was 0.834 for view-based mode and 0.868 for case-based mode when using features selected from the 3D-domain. The 2D-domain radiomics features showed a statistically similar performance to the 3D features (P > 0.05). Conclusion Radiomics models can provide encouraging performance in classification between malignant and benign microcalcification clusters which are semiautomatically segmented in DBT.

Crosslinked polymer nanocapsules (CPNCs) are hollowed nanoparticles with network-like polymeric shells stabilized by primary bonds. CPNCs have drawn broad and significant interests as nanocarriers for biomedical applications in recent years. As compared with conventional polymeric nanoparticles systems without cavity and/or crosslinking architectures, CPNCs possess significant biomedical relevant advantages, including (a) superior structural stability against environmental conditions, (b) high loading capacity and ability for region-specific loading of multiple cargos, (c) tuneable cargo release rate via crosslinking density, and (d) high specific surface area to facilitate surface adsorption, modification, and interactions. With appropriate base polymers and crosslinkages, CPNCs can be biocompatible and biodegradable. While CPNC-based biomedical nanoplatforms can possess relatively stable physicochemical properties owing to their crosslinked architectures, various biomedically relevant stimuli-responsivities can be incorporated with them through specific structural designs. CPNCs have been studied for the delivery of small molecule drugs, genes, proteins, and other therapeutic agents. They have also been investigated as diagnostic platforms for magnetic resonance imaging, ultrasound imaging, and optical imaging. Moreover, CPNCs have been utilized to carry both therapeutics and bioimaging agents for theranostic applications. This article reviews the therapeutic, diagnostic and theranostic applications of CPNCs, as well as the preparation of these CPNCs, reported in the past decade. Interleukin-8 (IL-8) is a pro-angiogenic cytokine associated with aggressive prostate cancer (CaP). We detected high levels of IL-8 in sera from patients with CaP compared with healthy controls and patients with benign prostatic hypertrophy. This study examines the role of IL-8 in the pathogenesis of metastatic prostate cancer. We developed a biocompatible, cationic polylactide (CPLA) nanocarrier to complex with and efficiently deliver IL-8 small interfering RNA (siRNA) to CaP cells in vitro and in vivo. CPLA IL-8 siRNA nanocomplexes (nanoplexes) protect siRNA from rapid degradation, are non-toxic, have a prolonged lifetime in circulation, and their net positive charge facilitates penetration of cell membranes and subsequent intracellular trafficking. Administration of CPLA IL-8 siRNA nanoplexes to immunodeficient mice bearing human CaP tumours produced significant antitumour activities with no adverse effects. Systemic (intravenous) or local intra-tumour administration of IL-8 siRNA nanoplexes resulted in significant inhibition of CaP growth. Magnetic resonance imaging and ultrasonography of experimental animals demonstrated reduction of tumour perfusion in vivo following nanoplex treatment. Staining of tumour sections for CD31 confirmed significant damage to tumour neovasculature after nanoplex therapy. These studies demonstrate the efficacy of IL-8 siRNA nanotherapy for advanced, treatment-resistant human CaP.

Polymer nanocapsules (PNCs) are an important class of nanocarriers, but applications of conventional non-crosslinked PNCs have been significantly limited because they are susceptible to environmental conditions. Synthesis and applications of crosslinked PNCs (CPNCs) with robust covalently stabilized nanostructures have attracted great interest over the past decade. Three major categories of template synthesis approaches for the preparation of CPNCs have been established: (1) cavitation of shell-crosslinked nanostructures, (2) vesicle-based crosslinking and (3) emulsion interfacial crosslinking. This article provides a critical and comprehensive review of these approaches. Several special cases of the synthesis of CPNCs are also reviewed. © 2016 Society of Chemical Industry

Purpose: To design and develop a new mobile cone-beam CT (CBCT) system for head imaging with good soft-tissue visibility, to be used bedside in ICU and neurosurgery department to monitor treatment and operation outcome in brain patients. Methods: The imaging chain consists of a 30cmx25cm amorphous silicon flat panel detector and a pulsed, stationary anode monoblock x-ray source of 100kVp at a maximal tube current of 10mA. The detector and source are supported on motorized mechanisms to provide detector lateral shift and source angular tilt, enabling a centered digital radiographic imaging mode and half-fan CBCT, while maximizing the use of the x-ray field and keep the source to detector distance short. A focused linear anti-scatter grid is mounted on the detector, and commercial software with scatter and other corrective algorithms is used for data processing and image reconstruction. The gantry rotates around a horizontal axis, and is able to adjust its height for different patient table positions. Cables are routed through a custom protective sleeve over a large bore with an in-plane twister band, facilitating single 360-degree rotation without a slip-ring at a speed up to 5 seconds per rotation. A UPS provides about 10 minutes of operation off themore » battery when unplugged. The gantry is on locked casters, whose brake is control by two push handles on both sides for easy reposition. The entire system is designed to have a light weight and a compact size for excellent maneuverability. Results: System design is complete and main imaging components are tested. Initial results will be presented and discussed later in the presentation. Conclusion: A new mobile CBCT system for head imaging is being developed. With its compact size, a large bore, and quality design, it is expected to be a useful imaging tool for bedside uses. The work is supported by a grant from Chinese Academy of Sciences.« less
