1. Evaluation of a 3‐year teledermoscopy project in primary healthcare centres in Belgium

Abstract

Abstract Background With the increasing incidence of skin cancer and limited access to specialised care, teledermoscopy (TDS) may represent a useful triage tool for skin cancer detection. Objectives An evaluation of a 3‐year TDS project in primary healthcare centres (PHCs) in Belgium (TELESPOT project). Methods A total of nine PHCs were trained to use an in‐house developed smartphone‐based application for macroscopic and dermoscopic acquisition of skin lesions, subsequently analysed independently by two investigators in a tertiary university skin cancer centre. The primary outcome was the proportion of high‐priority management (HPM) recommendations. Secondary outcomes included the TDS diagnoses, the quality of image acquisition, the mean time between HPM recommendations and subsequent surgery, the correlation between HPM reports and histopathology after surgery as well as patient and general practitioner satisfaction scores. All the endpoints were compared between the initial year of the TDS project and the subsequent 2‐year extension period of the study. Results Over 3 years, a total of 478 lesions were analysed in 335 patients: initial phase (105 lesions from 76 patients in six PHCs) and extension phase (373 lesions from 259 patients in nine PHCs). An HPM was recommended in 9.2% (initial and extension phases: 7.6% and 15.7%, respectively). The dermoscopic‐histological correlation achieved 84.1%. The median delay between HPM and surgery was 9 days. Conclusions This TDS project avoided unnecessary tertiary care visits in about 9 out of 10 cases, increased the HPM by a ninefold in comparison with the conventional care pathway and provided excellent satisfaction levels for PHCs and patients. Long‐term participation improved the triage quality for suspect skin lesions by 2.24‐fold.

Abstract

Abstract Background Oxygen‐flow‐assisted topical administration of 5% methotrexate (OFAMTX5%) was recently described as a painless therapeutic option with good cosmetic results for superficial basal cell carcinoma (sBCC). Objectives To assess the efficacy and cosmetic outcome of OFAMTX5% for small (2 cm) sBCCs of the trunk. Methods Patients with a least two sBCCs, as diagnosed by clinical, dermatoscopic and optical coherence tomography (OCT) examination, were included. One target lesion was treated 2×/week for 2 weeks. Complete (CR), partial (PR) and no (NR) responses, respectively defined as a total absence, improvement or a similar OCT aspect compared to baseline, were evaluated by OCT at 3 and 12 months postprocedure. Cosmetic outcome was evaluated at 1 year. Results At 3 months in a cohort of 32 patients (mean age: 54 years, min: 44 years, max: 89 years), the small sBCC lesion group presented a 94.4% (17/18) CR and a 5.5% PR (1/18), whereas the large lesion group revealed a CR of 64.3% (9/14) and PR of 35.7% (5/14). At 12 months, 3/14 large lesions presented recurrent sBCC at the border of the target lesion, but none in the small lesion group (0/18). No treatment‐related cutaneous nor systemic adverse events were observed. No OCT changes were noted at 3 and 12 months in control sBCC lesions. Cosmetic results were scored as good and excellent in 19.4% and 80.1%, respectively. Conclusions OFAMTX5% represents an interesting therapeutic alternative for sBCC on the trunk, without pain sensations and treatment‐related adverse effects. The place of OFAMTX5% in the therapeutic armamentarium against sBCC remains to be determined in larger series.

Abstract

Introduction Lesion volume measurement provides an objective and quantitative assessment of stroke severity and it is often used as a surrogate endpoint of clinical outcome in therapeutic trials. ACTIMIS (NCT03803007) was a randomized phase 1b/2a clinical trial evaluating glenzocimab, a monoclonal antibody fragment targeting platelet receptor glycoprotein VI, versus placebo in patients with acute ischemic stroke treated by thrombolysis alone or associated with mechanical thrombectomy (MT), both subgroups being well balanced. Primary analysis demonstrated a significant reduction in intracranial hemorrhage occurrence stroke‐related mortality and a trend towards reduction in severe disability. In this sub‐analysis, Artificial Intelligence (AI) imaging biomarkers were used to assess efficacy of glenzocimab in the subgroup undergoing MT in the pooled phase 1b (dose escalation) and phase 2a (dose confirmation) studies. Methods In the phase 1b study, patients were randomized to an escalating dose of glenzocimab or placebo (4:1, n=12 per group, 125mg, 250mg, 500mg, 1000mg). In the phase 2 dose confirmation study, patients were randomized (1:1) with 1000mg glenzocimab or placebo. CT scan or MRI was acquired at baseline with CT at 24 hours and MRI at 7 days (CT if MRI not available) for safety and efficacy analysis, respectively. Baseline and follow up imaging were processed using Brainomix software (Oxford, UK). AI output was reviewed for accuracy by an expert clinician (DC) blinded to treatment allocation. Results Of 166 patients in the trial, 81 patients underwent MT and had follow up non‐contrast CT imaging available at 24 hours (48 glenzocimab, 33 placebo) and 72 had Day‐7 imaging (47 glenzocimab, 25 placebo). Day‐7 imaging was available for 8 fewer placebo patients and 1 glenzocimab patient than at 24 hours. All except 2 patients (1 placebo, 1 glenzocimab) with missing data at Day‐7 died during the study. Multivariate regression modelling showed a significant interaction between patients undergoing MT and receiving glenzocimab. Glenzocimab had a greater effect on reducing the risk of haemorrhagic transformation in the MT subgroup (exploratory p=0.001). Presenting acute infarct volume was similar between glenzocimab and placebo arms (mean [SD]; 16.27mL [30.2] vs. 19.62mL [28.98], non‐significant). Abstract

Abstract Scabies is a very common ectoparasitosis and is responsible for a real scourge worldwide. The infestation leads to a highly contagious and pruritic dermatosis. Clinical diagnosis is not always easy; dermoscopy is a very useful diagnostic tool. Treatment relies on hygienic measures and specific acaricidals, either for topical application or oral administration. Ivermectin, permethrin and benzyl benzoate are the most frequently used therapies. Treatment failure is on the rise and could be attributed to noncompliance, treatment costs or to resistance to these agents. A series of novel agents are currently under investigation.

Abstract

Abstract Background Nutritional impairment is common in cancer patients and is associated with poor outcomes. Only few studies focused on cachexia. We assessed the prevalence of cachexia in older cancer patients, identified associated risk factors, and evaluated its impact on 6 month overall mortality. Methods A French nationwide cross‐sectional survey (performed in 55 geriatric oncology clinics) of older cancer patients aged ≥70 referred for geriatric assessment prior to treatment choice and initiation. Demographic, clinical, and nutritional data were collected. The first outcome was cachexia, defined as loss of more than 5% of bodyweight over the previous 6 months, or a body mass index below 20 kg/m2 with weight loss of more than 2%, or sarcopenia (an impaired Strength, Assistance with walking, Rise from chair, Climb stairs and Falls score) with weight loss of more than 2%. The second outcome was 6 month overall mortality. Abstract The comprehensiveness of data collected by “omics” modalities has demonstrated the ability to drastically transform our understanding of the molecular mechanisms of chronic, complex diseases such as musculoskeletal pathologies, how biomarkers are identified, and how therapeutic targets are developed. Standardization of protocols will enable comparisons between findings reported by multiple research groups and move the application of these technologies forward. Herein, we describe a protocol for parallel proteomic and metabolomic analysis of mouse intervertebral disc (IVD) tissues, building from the combined expertise of our collaborative team. This protocol covers dissection of murine IVD tissues, sample isolation, and data analysis for both proteomics and metabolomics applications. The protocol presented below was optimized to maximize the utility of a mouse model for “omics” applications, accounting for the challenges associated with the small starting quantity of sample due to small tissue size as well as the extracellular matrix‐rich nature of the tissue.

Abstract

AbstractNumerous policy and international frameworks consider that “destructive fishing” hampers efforts to reach sustainability goals. Though ubiquitous, “destructive fishing” is undefined and therefore currently immeasurable. Here we propose a definition developed through expert consultation: “Destructive fishing is any fishing practice that causes irrecoverable habitat degradation, or which causes significant adverse environmental impacts, results in long‐term declines in target or nontarget species beyond biologically safe limits and has negative livelihood impacts.” We show strong stakeholder support for a definition, consensus on many biological and ecological dimensions, and no clustering of respondents from different sectors. Our consensus definition is a significant step toward defining sustainable fisheries goals and will help interpret and implement global political commitments which utilize the term “destructive fishing.” Our definition and results will help reinforce the Food and Agricultural Organization's Code of Conduct and meaningfully support member countries to prohibit destructive fishing practices.
