Comparable performance indicators for breast cancer screening in the European Union (EU) have not been previously reported. We estimated adjusted breast cancer screening positivity rate (PR) and detection rates (DR) to investigate variation across EU countries. For the age 50-69 years, the adjusted EU-pooled PR for initial screening was 8.9% (cross-programme variation range 3.2-19.5%) while DR of invasive cancers was 5.3/1,000 (range 3.8-7.4/1,000) and DR of ductal carcinoma in situ (DCIS) was 1.3/1,000 (range 0.7-2.7/1,000). For subsequent screening, the adjusted EU-pooled PR was 3.6% (range 1.4-8.4%), the DR was 4.0/1,000 (range 2.2-5.8/1,000) and 0.8/1,000 (range 0.5-1.3/1,000) for invasive and DCIS, respectively. Adjusted performance indicators showed remarkable heterogeneity, likely due to different background breast cancer risk and awareness between target populations, and also different screening protocols and organisation. Periodic reporting of the screening indicators permits comparison and evaluation of the screening activities between and within countries aiming to improve the quality and the outcomes of screening programmes. Cancer Screening Registries would be a milestone in this direction and EU Screening Reports provide a fundamental contribution to building them.

The general aim of this systematic review is to mitigate breast cancer (BC) overdiagnosis and overtreatment. The specific aim is to summarize available data on the occurrence and features of indolent invasive or in situ (DCIS) BC, and precisely survival of untreated cases, prevalence of occult cancers found in autopsies, frequency of regressive BC. PubMed, Embase and Cochrane Library were systematically searched up to 3/31/2014. Eligibility criteria were: cohort studies, case-control studies, uncontrolled case series assessing survival in women with a diagnosis of BC who did not receive treatment compared to treated women; case series of autopsies estimating the prevalence of undiagnosed BC; cohort studies, case-control studies, uncontrolled case series, case reports assessing the occurrence of spontaneous regression of BC in women with a confirmed histology diagnosis. Untreated BC: 8 cohort studies and 12 case series (3593 BC) were included. In three controlled cohort studies (diagnoses 1978-2006), the 5-years overall survival was 19-43%. Occult BC: 8 case series (2279 autopsies) were included. The prevalence of invasive BC undiagnosed during lifetime range was 0-1.5%, while for DCIS the range was 0.2-14.7%. Spontaneous regression: 2 cohort studies, 3 case reports, 1 case series included. In the cohort studies the relative risk of regression for screen detected compared with nonscreened BC was estimated as 1.2 and 1.1. It seems plausible that around 10% of invasive BC are not symptomatic during life, and that one fith of BC patients if untreated would be alive after 5 years. Around 1 of 10 screen-detected BC may regress according two studies.

The synthesis, electrochemical, and photophysical properties of five multicomponent systems featuring a Zn(II) porphyrin (ZnP) linked to one or two anilino donor-substituted pentacyano- (PCBD) or tetracyanobuta-1,3-dienes (TCBD), with and without an interchromophoric bridging spacer (S), are reported: ZnP-S-PCBD (1), ZnP-S-TCBD (2), ZnP-TCBD (3), ZnP-(S-PCBD)2 (4), and ZnP-(S-TCBD)2 (5). A EuIII complex, tris-dibenzoylmethane mono-1,10-phenanthroline- europium(III) [Eu(DBM)3(Phen)], can be easily adsorbed in situ via hydrophobic interactions to single-walled carbon nanotube (SWNT) surfaces from a methanol solution. The EuIII containing material has been comprehensively characterized via thermogravimetric analysis (TGA), UV-vis-NIR absorption and luminescence spectroscopy, Raman spectroscopy, atomic force microscopy (AFM), high-resolution transmission electron microscopy (HR-TEM)), Z-contrast scanning transmission electron microscopy (STEM) imaging, and energy dispersive X-ray spectroscopy (EDS). The photophysical investigations revealed that the presence of a SWNT framework does not affect the lanthanide-centered luminescence stemming from the characteristic electronic transitions within the 4f shell of the EuIII ions. Such straightforward synthetic route leads to the preparation of luminescent SWNTs without significantly affecting the electronic and structural properties of the carbon framework, opening new possibilities of designing new classes of CNTs for biomedical applications.

Let there be light, let it be white: Recent developments in the use of chromophore-based gels as scaffolds for the assembly of white-light-emitting soft materials have been significant. The main advantage of this approach lies in the facile accommodation of selected luminescent components within the gel. Excitation-energy-transfer processes between these components ultimately generate the desired light output.

Durch die Umsetzung neuer Beleuchtungskonzepte konnte die Verwertung von Energie deutlich effizienter gestaltet werden. Alle derzeit entwickelten Technologien beruhen auf elektrolumineszierenden Festkorpern und lassen sich allgemein als Festkorperbeleuchtung (solid-state lighting, SSL) einstufen. Die beiden wichtigsten Zweige der SSL-Technologie sind Leuchtdioden (LEDs) und organische Leuchtdioden (OLEDs), doch seit kurzem bilden auch leuchtende elektrochemische Zellen (LECs) eine Alternative, die als aktive Materialien entweder lumineszierende Polymere in Kombination mit ionischen Salzen oder lumineszierende ionische Spezies wie ionische Ubergangsmetallkomplexe (iTMCs) enthalten. Cyclometallierte IrIII-Komplexe werden bei weitem am haufigsten als iTMCs in LECs verwendet. Hier zeigen wir, wie diese Komplexe hergestellt werden konnen, und wir diskutieren ihre einzigartigen elektronischen, photophysikalischen und photochemischen Eigenschaften. Schlieslich werden die wichtigsten Fortschritte bezuglich Einschaltzeit, Stabilitat, Effizienz und Farbe von iTMCs-LECs prasentiert.

Molecular tweezers built from sawhorse-type dinuclearruthenium units and carboxyphenyl porphyrin derivatives{5-(4-carboxyphenyl)-10,15,20-triphenyl-21H,23H-porphyrin,[ 5-(4-carboxyphenyl)-10,15,20-triphenylporphyrin]zinc, 5,10-bis(4-carboxyphenyl)-15,20-diphenyl-21H,23H-porphyrin, and [5,10-bis(4-carboxyphenyl)-15,20-diphenylporphyrin]zinc} are presented. Molecular modeling shows that in all cases the two porphyrin moieties are adequately positioned to encapsulate guest molecules. Therefore, the hostguest interactions between the sawhorse-type tetracarbonyldiruthenium tweezers and different guest molecules [fullerene (C60), pyridine (pyr), 4,4'-bipyridine (bpy), 1,2-bis(4-pyridyl)ethylene (bpe), 5,10-bis(4-pyridyl)-15,20-diphenyl-21H,23H-porphyrin (5,10-dpp), and 5,15-bis(4-pyridyl)-10,20-diphenyl-21H,23H-porphyrin (5,15-dpp)] have been studied in solution by using optical spectroscopic methods.

Introduction: Corticosteroid treatment is the standard of care in Duchenne muscular dystrophy (DMD), but the optimal age to initiate treatment and dosage pattern remain a matter of discussion. Methods: We performed a long-term study of alternate-day corticosteroids in five 2- to 4-year-old DMD patients. The primary outcome measure was prolongation of the ability to walk. Results: One patient lost ambulation at age 10. Four patients, aged 16 to 18 were fully ambulant, and 3 of them could still climb stairs. Respiratory function was moderately reduced in 2. Left ventricular ejection fraction was > 45%. Short stature and delayed puberty were the most relevant side effects. Although the negative impact of corticosteroid treatment on growth rate remained their major concern, parents and patients stated that they preferred corticosteroid therapy. Conclusions: Long-term corticosteroid treatment is effective in prolonging function but not in recovering lost function, and its early use seems appropriate. Muscle Nerve 45: 796–802, 2012

Hydrogen is often proposed as the fuel of the future, but the transformation from the present fossil fuel economy to a hydrogen economy will need the solution of numerous complex scientific and technological issues, which will require several decades to be accomplished. Hydrogen is not an alternative fuel, but an energy carrier that has to be produced by using energy, starting from hydrogen-rich compounds. Production from gasoline or natural gas does not offer any advantage over the direct use of such fuels. Production from coal by gasification techniques with capture and sequestration of CO2 could be an interim solution. Water splitting by artificial photosynthesis, photobiological methods based on algae, and high temperatures obtained by nuclear or concentrated solar power plants are promising approaches, but still far from practical applications. In the next decades, the development of the hydrogen economy will most likely rely on water electrolysis by using enormous amounts of electric power, which in its turn has to be generated. Producing electricity by burning fossil fuels, of course, cannot be a rational solution. Hydroelectric power can give but a very modest contribution. Therefore, it will be necessary to generate large amounts of electric power by nuclear energy of by renewable energies. A hydrogen economy based on nuclear electricity would imply the construction of thousands of fission reactors, thereby magnifying all the problems related to the use of nuclear energy (e. g., safe disposal of radioactive waste, nuclear proliferation, plant decommissioning, uranium shortage). In principle, wind, photovoltaic, and concentrated solar power have the potential to produce enormous amounts of electric power, but, except for wind, such technologies are too underdeveloped and expensive to tackle such a big task in a short period of time. A full development of a hydrogen economy needs also improvement in hydrogen storage, transportation and distribution. Hydrogen and electricity can be easily interconverted by electrolysis and fuel cells, and which of these two energy carriers will prevail, particularly in the crucial field of road vehicle powering, will depend on the solutions found for their peculiar drawbacks, namely storage for electricity and transportation and distribution for hydrogen. There is little doubt that power production by renewable energies, energy storage by hydrogen, and electric power transportation and distribution by smart electric grids will play an essential role in phasing out fossil fuels.
