This paper develops two numerical methods for solving a system of fractional differential equations based on hybrid shifted orthonormal Bernstein polynomials with generalized block-pulse functions (HSOBBPFs) and hybrid shifted orthonormal Legendre polynomials with generalized block-pulse functions (HSOLBPFs). Using these hybrid bases and the operational matrices method, the system of fractional differential equations is reduced to a system of algebraic equations. Error analysis is performed and some simulation examples are provided to demonstrate the efficacy of the proposed techniques. The numerical results of the proposed methods are compared to those of the existing numerical methods. These approaches are distinguished by their ability to work on the wide interval 0,a, as well as their high accuracy and rapid convergence, demonstrating the utility of the proposed approaches over other numerical methods.

This article proposes a collocation approach based on a redefined quintic B-spline basis for solving the time-fractional Whitham-Broer-Kaup equations. The presented method involves discretizing the time-fractional derivatives using an L1-approximation scheme and then approximating the spatial derivatives using the redefined quintic B-spline basis. The von Neumann technique has been used to demonstrate that the proposed method is unconditionally stable. The error estimates are discussed and show that the proposed method is third-order convergent. The results demonstrate the potential of the proposed method as a reliable tool for solving fractional differential equations.

Abstract The internet has become one of the main sources of news spread as it unleashed the information dissemination space, where the news websites express opinions on entities while also reporting on recent or unusual security risks. Recently, many research studies have focused on sentimental reflection on the views and impressions of people utilising natural language processing and analytical linguistics. Therefore, we have collected corpus from popular Arabic websites that publish articles related to recent security issues, and we provide light weight preprocessing techniques where data is term matrix is transformed. We also present an intensive lexical‐driven data analysis with visualised data views, as our topic modelling technique can effectively extract significant topics from all the collected text from different websites. Our experiments validate the k‐means clustering algorithm with and without the latent Dirichlet allocation topic modelling method, and we adopted various validation techniques to measure the topic clustering internally and externally. As shown in the experiments' results, our proposed combined method has a high round index rate of 87.2%, with a large number of topics and clusters.

Background Chronic obstructive pulmonary disease (COPD) is a common comorbidity in heart failure with reduced ejection fraction, associated with undertreatment and worse outcomes. New treatments for heart failure with reduced ejection fraction may be particularly important in patients with concomitant COPD. The objective of this study was to evaluate the Cd availability and toxicity as affected by NaCl-induced salinity and farmyard manure addition. The Cd availability and toxicity were investigated in greenhouse pot and incubation experiments were conducted on a calcareous loamy sand soil contaminated with Cd (0.5, 1.5, 3, 6, 12, and 24 mg kg−1 of soil) and amended with two rates of 0.0 and 30 g farmyard manure (FYM) kg−1. Barley seeds (Hordeum vulgare L.) were sown in pots and irrigated with water containing different levels of salinity (0, 30, 60, and 120 mM NaCl). The results revealed that the DTPA-extractable Cd and its content in barley plant shoots tended to increase in line as Cd was applied and salt levels increased. Elevated decreases in the soil basal respiration with increased Cd applied and NaCl-induced salinity were found. However, applying FYM significantly reduced Cd availability and increased plant growth and soil respiration activity. The results clearly showed that adding farmyard manure as soil organic amendment decreased the availability of Cd to barley plants and mitigated the toxicity of both Cd and salinity to soil microbial activity.

Aim: There is an association between heart failure with preserved ejection fraction (HFpEF) and insulin resistance, but less is known about the diabetic continuum, and in particular about pre-diabetes, in HFpEF. We examined characteristics and outcomes of participants with diabetes or pre-diabetes in PARAGON-HF., Methods and Results: Patients aged ≥50 years with left ventricular ejection fraction ≥45%, structural heart disease and elevated N-terminal pro-B-type natriuretic peptide (NT-proBNP) were eligible. Patients were classified according to glycated haemoglobin (HbA1c): (i) normal HbA1c, <6.0%; (ii) pre-diabetes, 6.0%-6.4%; (iii) diabetes, ≥6.5% or history of diabetes. The primary outcome was a composite of cardiovascular (CV) death and total heart failure hospitalizations (HFH). Of 4796 patients, 50% had diabetes and 18% had pre-diabetes. Compared to patients with normal HbA1c, patients with pre-diabetes and diabetes more often were obese, had a history of myocardial infarction and had lower Kansas City Cardiomyopathy Questionnaire scores, while patients with diabetes had more clinical evidence of congestion, but similar NT-proBNP concentrations. The risks of the primary composite outcome (rate ratio [RR] 1.59, 95% confidence interval [CI] 1.35-1.88), total HFH (RR 1.67, 95% CI 1.39-2.02) and CV death (hazard ratio [HR] 1.35, 95% CI 1.07-1.71) were higher among patients with diabetes, compared to those with normal HbA1c. Patients with pre-diabetes had a higher risk (which was intermediate between that of patients with diabetes and those with normal HbA1c) of the primary outcome (HR 1.27, 95% CI 1.00-1.60) and HFH (HR 1.35, 95% CI 1.03-1.77), but not of CV death (HR 1.02, 95% CI 0.75-1.40). Patients with diabetes treated with insulin had worse outcomes than those not, and those with 'lean diabetes' had similar mortality rates to those with a higher body mass index, but lower rates of HFH., Conclusion: Pre-diabetes is common in patients with HFpEF and is associated with worse clinical status and greater risk of HFH., Clinical Trial Registration: ClinicalTrials.gov Identifier NCT01920711., (© 2021 The Authors. Aim: To describe the baseline characteristics and treatment of the patients randomized in the PARADIGM-HF (Prospective comparison of ARNi with ACEi to Determine Impact on Global Mortality and morbidity in Heart Failure) trial, testing the hypothesis that the strategy of simultaneously blocking the renin-angiotensin-aldosterone system and augmenting natriuretic peptides with LCZ696 200 mg b.i.d. is superior to enalapril 10 mg b.i.d. in reducing mortality and morbidity in patients with heart failure and reduced ejection fraction., Methods: Key demographic, clinical and laboratory findings, along with baseline treatment, are reported and compared with those of patients in the treatment arm of the Studies Of Left Ventricular Dysfunction (SOLVD-T) and more contemporary drug and device trials in heart failure and reduced ejection fraction., Results: The mean age of the 8442 patients in PARADIGM-HF is 64 (SD 11) years and 78% are male, which is similar to SOLVD-T and more recent trials. Despite extensive background therapy with beta-blockers (93% patients) and mineralocorticoid receptor antagonists (60%), patients in PARADIGM-HF have persisting symptoms and signs, reduced health related quality of life, a low LVEF (mean 29 ± SD 6%) and elevated N-terminal-proB type-natriuretic peptide levels (median 1608 inter-quartile range 886-3221 pg/mL)., Conclusion: PARADIGM-HF will determine whether LCZ696 is more beneficial than enalapril when added to other disease-modifying therapies and if further augmentation of endogenous natriuretic peptides will reduce morbidity and mortality in heart failure and reduced ejection fraction., (© 2014 The Authors. Aims: Although the focus of therapeutic intervention has been on neurohormonal pathways thought to be harmful in heart failure (HF), such as the renin-angiotensin-aldosterone system (RAAS), potentially beneficial counter-regulatory systems are also active in HF. These promote vasodilatation and natriuresis, inhibit abnormal growth, suppress the RAAS and sympathetic nervous system, and augment parasympathetic activity. The best understood of these mediators are the natriuretic peptides which are metabolized by the enzyme neprilysin. LCZ696 belongs to a new class of drugs, the angiotensin receptor neprilysin inhibitors (ARNIs), which both block the RAAS and augment natriuretic peptides., Methods: Patients with chronic HF, NYHA class II-IV symptoms, an elevated plasma BNP or NT-proBNP level, and an LVEF of ≤40% were enrolled in the Prospective comparison of ARNI with ACEI to Determine Impact on Global Mortailty and morbidity in Heart Failure trial (PARADIGM-HF). Patients entered a single-blind enalapril run-in period (titrated to 10 mg b.i.d.), followed by an LCZ696 run-in period (100 mg titrated to 200 mg b.i.d.). A total of 8436 patients tolerating both periods were randomized 1:1 to either enalapril 10 mg b.i.d. or LCZ696 200 mg b.i.d. The primary outcome is the composite of cardiovascular death or HF hospitalization, although the trial is powered to detect a 15% relative risk reduction in cardiovascular death., Perspectives: PARADIGM-HF will determine the place of the ARNI LCZ696 as an alternative to enalapril in patients with systolic HF. PARADIGM-HF may change our approach to neurohormonal modulation in HF., Trial Registration: NCT01035255.

It has been recently postulated from our laboratory that Arabic gum (AG) offers a protective effect in the kidney of rats against nephrotoxicity induced by gentamicin via inhibiting lipid peroxidation. It has also recently shown a powerful antioxidant effect through scavenging superoxide anions. In this study we utilized a rat model of cisplatin (CP)-induced nephrotoxicity to determine its peak time following (1, 2, 5, and 7 days) of a single CP (7.5 mg/kg, i.p.) injection. Also, a possible protective effect of cotreatment with AG (7.5 g/kg/day p.o.) on CP-induced nephrotoxicity was investigated. Biochemical as well as histological assessments were carried out. CP-induced nephrotoxicity was manifested by significant elevations of the functional parameters blood urea, serum creatinine, and kidney/body weight ratio. Maximum toxic effects of CP were observed 5 days after its injection, while it started after day 1 in the biochemical parameters, such as glutathione depletion in the kidney tissue with concomitant increases in lipid peroxides and platinum content. Additionally, severe necrosis and desquamation of tubular epithelial cells in renal cortex as well as interstitial nephritis were observed after 5 days in CP-treated animals. Five days after AG cotreatment with CP did not protect the kidney from the damaging effects of CP. However, it significantly reduced CP-induced lipid peroxidation. These findings suggest that lipid peroxidation is not the main cause of CP-induced nephrotoxicity but it is rather more dependent on other factors such as platinum disposition in renal interstitial tubules., (Copyright 2003 Wiley Periodicals, Inc. Arabic gum (AG) is a naturally occurring compound that has been proposed to possess potent antioxidant activity. In this study, the possible effects whereby AG could protect against cardiotoxicity induced by doxorubicin (DOX) in mice were carried out. Administration of single dose of DOX (15 mg/kg, i.p.) induced cardiotoxicity 72 h, manifested biochemically by a significant elevation of serum creatine kinase (CK) (EC 2.7.3.2). In addition, cardiotoxicity was further confirmed by the significant increase in lipid peroxides measured as malondialdehyde (MDA). Administration of AG (25 g/kg) orally for 5 days before and 72 h after DOX injection produced a significant protection against cardiotoxicity induced by DOX. This was evidenced by significant reductions in serum CK and cardiac lipid peroxides. The effect of AG was examined on the superoxide anion radical generated by enzymatic and nonenzymatic methods. The results indicate that AG is a potent superoxide scavenger. The superoxide scavenging effect of AG may explain, at least in part, the protective effect of AG against cardiotoxicity induced by DOX., (Copyright 2002 Wiley Periodicals, Inc.)
