Critics have noted a mode in Samuel Beckett's writing that can be characterised as the urge to purge. Obsessed with figuring language as both vomit and shit in the guts of an oral/anal textual system, many of his texts are constructed by an oscillation between force-feeding and the compulsive evacuations of emetic and enema. This article suggests that Beckett's persistent textual questioning of what it means to take in and process material from outside the self can usefully be understood in relation to psychoanalytic interpretations of eating disorders; it also suggests that it can be read alongside Beckett's desperate turn to psychoanalytic psychotherapy in the 1930s. Plagued by difficulties with ingestion and digestion (among other symptoms), Beckett sought treatment from Wilfred Bion – the man who would later become one of the most famous British psychoanalysts. Beckett spat out and abandoned the therapy relatively quickly; nevertheless, after his time with Bion there is a sense that the textual evacuations enacted in an uncontrolled fashion in the earlier writing could now be ruminated on, raised to the level of form, and consequently contained. Perhaps coincidentally, the later Bion insisted that what analysis offered, in the face of the patient's frantic desire to ingest and master the world by splitting off and evacuating parts of themselves, was a container – a container for thinking. He insisted that this capacity for thinking should be understood as a form of ‘digestion’ that might bear external material long enough for it to be used rather than purged. Quite properly, Bion never gave details of his therapeutic work with Beckett and there is no accessible archive of clinical material on Beckett's case. This article suggests, however, that an archive of Beckett's analysis can be found in his corpus – in the words and writing he used as food for thought.

4. Understanding the value of the arts in the education of mental health professionals: Georg Lukács, Samuel Beckett and the aesthetic category of specific particularity

The manner in which the arts can enhance the practical, therapeutic concerns of mental health professionals is becoming well established in the health care literature. What gets discussed less frequently, however, are those aesthetic frameworks that propose to give an account of the possible 'meaning' and 'purpose' of art. In response, this paper will elucidate the aesthetic theory of the Hungarian philosopher Georg Lukacs and will suggest that his concept of specific particularity enables an understanding of how art, and literature, poetry and drama in particular, can be employed as an educational resource that can contribute to the development of the 'emotional capabilities' of practitioners. However, insofar as Lukacs' works are philosophically complex and challenging, his concept of specific particularity will be discussed within the context of Samuel Beckett's dramatic work Ohio Impromptu. In doing so, it will be suggested that Ohio Impromptu is not only productive for the elucidation of Lukacs' aesthetics, but also illustrates how the arts provides practitioners with a valuable educative opportunity to engage with, and critically reflect upon, a multiplicity of affective dimensions, thereby enhancing the practitioner's ability to move towards achieving an empathic understanding of, and 'emotional resonance' with, those receiving mental health care.

5. A Subject After All - Rethinking the ‘personalized narrator’ of the self-reflexive first-person novels of O'Brien, Beckett and Banville

This essay suggests a way of getting beyond the ‘personalized’ narrator traditionally seen as defining the first-person novel, without giving up completely the idea of an existential relationship between narrator and character. It explores the construction of a subject after all in three self-reflexive first-person novels (The Third Policeman, Malone Dies, Ghosts). These self-reflexive first-person novels cannot be explained within the existing framework of theories about the first-person novel as they question and partly undermine the notion of the personalized narrator as a more or less unproblematic entity. To see how this subject is constructed in these self-reflexive novels, this essay rethinks the ‘experiencing I’ and the ‘narrating I’ respectively in the light of Paul Ricoeur's concept of ‘narrative identity’ and Rimmon-Kenan's concept of ‘access’. This leads to the notions of a ‘storied subject’ and a ‘speaking subject’. Furthermore it argues that we need to take into consideration a third aspect of the subject, the ‘linguistic subject’ (theoretically based on Benveniste) in order to comprehend the subject of the self-reflexive first-person novel.

OBJECTIVE: To investigate (a) the magnitude of the independent associations of neighborhood‐level and person‐level social risk factors (SRFs) with quality, (b) whether neighborhood‐level SRF associations may be proxies for person‐level SRF associations, and (c) how the association of person‐level SRFs and quality varies by neighborhood‐level SRFs. DATA SOURCES: 2015–2016 Medicare Advantage HEDIS data, Medicare beneficiary administrative data, and 2016 American Community Survey (ACS). STUDY DESIGN: Mixed effects linear regression models (1) estimated overall inequities by neighborhood‐level and person‐level SRFs, (2) compared neighborhood‐level associations to person‐level associations, and (3) tested the interactions of person‐level SRFs with corresponding neighborhood‐level SRFs. DATA COLLECTION/EXTRACTION METHODS: Beneficiary‐level SES and disability administrative data and five‐year ACS neighborhood‐level SRF information were each linked to HEDIS data. PRINCIPAL FINDINGS: For all or nearly all HEDIS measures, quality was worse in neighborhoods lower in SES and in neighborhoods with higher proportions of residents with a disability. Quality by neighborhood racial and ethnic composition was mixed. Accounting for corresponding person‐level SRFs reduced neighborhood SRF associations by 25% for disability, 43% for SES, and 74%–102% for racial and ethnic groups. INTRODUCTION The Alzheimer's Disease Neuroimaging Initiative (ADNI) has accumulated 15 years of clinical, neuroimaging, cognitive, biofluid biomarker and genetic data, and biofluid samples available to researchers, resulting in more than 3500 publications. This review covers studies from 2018 to 2020. METHODS We identified 1442 publications using ADNI data by conventional search methods and selected impactful studies for inclusion. RESULTS Disease progression studies supported pivotal roles for regional amyloid beta (Aβ) and tau deposition, and identified underlying genetic contributions to Alzheimer's disease (AD). Vascular disease, immune response, inflammation, resilience, and sex modulated disease course. Biologically coherent subgroups were identified at all clinical stages. Practical algorithms and methodological changes improved determination of Aβ status. Plasma Aβ, phosphorylated tau181, and neurofilament light were promising noninvasive biomarkers. Prognostic and diagnostic models were externally validated in ADNI but studies are limited by lack of ethnocultural cohort diversity. DISCUSSION ADNI has had a profound impact in improving clinical trials for AD.

Aim Previous studies reported conflicting evidence on the effects of obesity on outcomes after gastrointestinal surgery. The aims of this study were to explore the relationship of obesity with major postoperative complications in an international cohort and to present a meta-analysis of all available prospective data.Methods This prospective, multicentre study included adults undergoing both elective and emergency gastrointestinal resection, reversal of stoma or formation of stoma. The primary end-point was 30-day major complications (Clavien-Dindo Grades III-V). A systematic search was undertaken for studies assessing the relationship between obesity and major complications after gastrointestinal surgery. Individual patient meta-analysis was used to analyse pooled results.Results This study included 2519 patients across 127 centres, of whom 560 (22.2%) were obese. Unadjusted major complication rates were lower in obese vs normal weight patients (13.0% vs 16.2%, respectively), but this did not reach statistical significance (P = 0.863) on multivariate analysis for patients having surgery for either malignant or benign conditions. Individual patient meta-analysis demonstrated that obese patients undergoing surgery for malignancy were at increased risk of major complications (OR 2.10, 95% CI 1.49-2.96, P Conclusions In our international data, obesity was not found to be associated with major complications following gastrointestinal surgery. Meta-analysis of available prospective data made a novel finding of obesity being associated with different outcomes depending on whether patients were undergoing surgery for benign or malignant disease.

OBJECTIVES Propofol has not been extensively studied as an acute migraine therapy; however, based on the limited evidence from outpatient and inpatient settings, propofol has been proposed as an option for patients who present to the emergency department (ED). The purpose of this review was to evaluate the existing literature regarding the safety and efficacy of propofol for acute migraine treatment in the ED. METHODS A systematic review of clinical studies of propofol treatment for acute migraine in the ED was performed using Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. Trials were identified through PubMed, Google Scholar, clinical trial registries, research registries, and key journals through May 2019. A modified Jadad scoring system was used to assess the methodologic quality of the included randomized controlled trials, and the Newcastle-Ottawa Scale was used for the retrospective cohort study. RESULTS Nine studies, including five case reports or series, one retrospective cohort study, and three randomized controlled trials, consisting of 290 patients, were reviewed. All studies in adults reported propofol to be an effective therapy for migraine, but the strength of these results was limited by dosing variations, small sample sizes, and limited generalizability. Pediatric studies produced mixed results. CONCLUSIONS Propofol may be an effective rescue therapy for patients presenting to the ED for acute migraine, but its place in therapy based on the limited available evidence is unknown. The safety of propofol for migraine management in the ED has not been adequately examined.

Background Coagulopathic bleeding is frequently present after major trauma. However, trauma-induced coagulopathy (TIC) remains incompletely understood. This laboratory analysis of blood samples derived from our completed trial on fibrinogen in the initial resuscitation of severe trauma (FiiRST) was conducted to evaluate TIC and associated responses to fibrinogen replacement. Study design and methods We conducted a retrospective evaluation of TIC in 45 FiiRST trial patients based on rotational thromboelastometry (ROTEM), international normalized ratio (INR), and biomarkers for hemostasis and endotheliopathy. Whole blood was analyzed by ROTEM. Plasma was analyzed for INR and biomarkers. Results Overall, 19.0% and 30.0% of the FiiRST trial patients were coagulopathic on admission defined by EXTEM maximum clot firmness out of the range of 40-71 mm and INR >1.2, respectively. The FiiRST patients showed lower fibrinogen, factor II and V levels, protein C and antiplasmin activities, higher activated protein C, tissue plasminogen activator, d-dimer, and thrombomodulin concentrations at admission than healthy controls. Most of the biomarkers changed their activities during 48-h hospitalization, but were at abnormal levels even 48-h after admission. The fibrinogen treatment reduced hypofibrinogenemia and increased factor XIII level, but had no significant effects on other biomarkers levels. Limited development of endotheliopathy was indicated by syndean-1, thrombomodulin, and sE-selectin. Conclusions About 19%-30% of the trauma patients in the FiiRST trial were coagulopathic on hospital admission depending on the definition of TIC. Analyses of the TIC biomarkers demonstrated that hemostasis would not return to normal after 48-h hospitalization, and fibrinogen replacement improved hypofibrinogenemia.

Background Hemorrhage is a leading cause of preventable death in civilian and military trauma. Freeze-dried plasma is promising for hemostatic resuscitation in remote prehospital settings, given its potential benefits in reducing blood loss and mortality, long storage at ambient temperatures, high portability, and rapid reconstitution for transfusion in austere environments. Here we assess the ex vivo characteristics of a novel Terumo's freeze-dried plasma product (TFDP). Study design and methods Rotational thromboelastometry (ROTEM) tests (INTEM, EXTEM, and FIBTEM) were conducted on plasma samples at 37°C with a ROTEM delta-machine using standard reagents and procedures. The following samples were analyzed: pooled plasma to produce TFDP, TFDP reconstituted, and stored immediately at -80°C, reconstituted TFDP stored at 4°C for 24 h and room temperature (RT) for 4 h before freezing at -80°C. Analysis of plasma concentrations of selected cytokines, chemokines, and vascular molecules was performed using a multiplex immunoassay system. One-way ANOVA with post hoc tests assessed differences in hemostatic and inflammatory properties. Results No significant differences in ROTEM variables (coagulation time [CT], clot formation time, α-angle, maximum clot firmness, and lysis index 30) between the TFDP-producing plasma and reconstituted TFDP samples were observed. Compared to control plasma, reconstituted TFDP stored at 4°C for 24 h or RT for 4 h showed a longer INTEM CT. Levels of immuno-inflammatory mediators were similar between frozen plasma and TFDP. Conclusions TFDP is equivalent to frozen plasma with respect to global hemostatic and immuno-inflammatory mediator profiles. Further investigations of TFDP in trauma-induced coagulopathy models and bleeding patients are warranted.

Despite the availability of various osteoporosis treatments, adherence remains suboptimal. One contributing factor may be patient experience with therapy. This US, multicenter, combined retrospective chart review and patient questionnaire study included postmenopausal women at high risk for fracture and is the first study to describe real‐world patient experience with abaloparatide (ABL) injection. Eight geographically diverse secondary care sites in the United States participated (n = 193). Mean ± SD age was 67.4 ±8.62 years. Most patients (86%) were satisfied with the ABL regimen, especially with ease of preparation (82%), ease of storage (87%), and storage convenience (89%), an attribute 83% of the patients thought was important. The majority of patients reported complete satisfaction with the ABL regimen allowing for their ability to conduct daily activities (85%) and convenience to fit into their daily schedule (84%). All reported taking ABL as directed, by injection in the lower abdomen, and 83% of patients reported medium or high adherence. Patients were satisfied with the needle size (76% completely satisfied), and 93% reported never deliberately missing a dose. Although injecting medication (18%) and higher out‐of‐pocket costs (17%) were deemed the most bothersome attributes, the majority (69%) noted their healthcare team understands how osteoporosis impacts their lives. In multivariable analyses, ease of preparation (OR = 2.62; 95% CI, 1.01–6.81; p = 0.048) and fracture history (OR = 1.72; 95% CI, 1.03–2.86; p = 0.037) were significantly associated with overall satisfaction. Ease of preparation was a predictor of higher satisfaction with treatment convenience (coefficient = 13.60; 95% CI, 8.08–19.12; p = 0.00). Remembering to take the medication was a significant predictor of self‐reported adherence (OR = 16.66; 95% CI, 3.30–84.24; p = 0.001). In conclusion, the majority of patients were satisfied with ABL and found it convenient/easy to prepare and store. High self‐reported adherence may be associated with positive patient experience including ease of use and adequate support from healthcare providers. © 2020 The Authors. Occupational exposure limits (OELs) have been previously proposed for diacetyl; however, most of these values are based on worker cohort studies that are known to have several limitations and confounders. In this analysis, an 8 hour time-weighted average (TWA) OEL for diacetyl was derived based on data from a chronic, 2 year animal inhalation study recently released by the US National Toxicology Program. In that study, complete histopathology was conducted on male and female mice and rats exposed to 0, 12.5, 25 or 50 ppm diacetyl. Several responses in the lower respiratory tract of rats (the more sensitive species) were chosen as the critical endpoints of interest. Benchmark concentration (BMC) modeling of these endpoints was used to estimate BMC values associated with a 10% extra risk (BMC10 ) and the associated 95% lower confidence bound (BMCL10 ), which were subsequently converted to human equivalent concentrations (HECs) using a computational fluid dynamics-physiologically based pharmacokinetic (CFD-PBPK) model to account for interspecies dosimetry differences. A composite uncertainty factor of 8.0 was applied to the human equivalent concentration values to yield 8 hour TWA OEL values with a range of 0.16-0.70 ppm. The recommended 8 hour TWA OEL for diacetyl vapor of 0.2 ppm, based on minimal severity of bronchiolar epithelial hyperplasia in the rat, is practical and health-protective.

OBJECTIVES The medial elbow is stabilized during valgus loading by the ulnar collateral ligament, the flexor pronator mass (FPM), and the radial head. Impairment of the FPM can occur as a result of repeated wrist flexion activity. The purpose of this study was to determine the effect of repeated elbow flexion exercise on medial elbow stability. METHODS Thirty volunteers were included in this study. Ultrasound images of the medial joint space of the nondominant elbow of the participants were collected during a valgus stress test. The images were collected before and immediately after a wrist flexion exercise protocol. RESULTS The wrist flexion strength measure decreased (7.5%) after the exercise protocol (t = 3.840; P = .001). Objectives An assessment of medial elbow stability is essential to the patient with an ulnar collateral ligament injury. Ultrasound imaging can be used to assess medial elbow stability. This study determined the effect of the elbow flexion angle on the medial elbow joint space during clinical tests of medial elbow stability. Methods Ultrasound images of the nondominant elbow were collected during 3 tests of medial elbow stability: valgus stress test, weighted valgus test, and milking maneuver. The elbow flexion angle increased between the valgus stress test and milking maneuver. The width of the medial joint space was measured on ultrasound images collected in unstressed and stressed conditions. Results Across test conditions, the medial joint width was greater in the stressed condition (mean ± SD, 3.7 ± 0.1 mm) than in the unstressed condition (2.9 ± 0.09 mm). The medial elbow joint space width was less (mean difference, 0.16 ± 0.01 mm; P = .01) in the milking maneuver position compared to the valgus stress test positions. Conclusions This study provides evidence that changes in the width of the medial elbow during clinical evaluation of the unimpaired elbow can be detected by ultrasound. Changing the elbow flexion angle did not affect the change in width of the medial elbow during valgus loading.

The use of sample surveys in biostatistics arises from the need to answer questions about health issues in the community. The earliest uses of survey methodology in health research in the United States were motivated more by policy issues than by epidemiologic questions and paralleled the use of surveys by the US Census Bureau and other Federal agencies to address social and economic concerns. More recently, it has been recognized that research into the causes and outcomes of disease can benefit from studies in the entire community, rather than just in the formal health care system, particularly in countries such as the United States, where access to health care is not uniform. The Affordable Care Act, passed in 2010 with the goals of increasing access to and reducing the costs of health care in the United States, has motivated both government and nongovernment studies to assess its impact. A number of practical concerns about conducting studies in the community then lead to the use of sample surveys, rather than complete enumeration. In this article, we review the history of sample surveys in the health sciences, present an overview of the major elements and features in the design of sample surveys and analysis of data from such studies, and discuss major methodological advances especially in the last decade. Finally, we provide a large set of references that might be of use to the reader wishing to delve more deeply into specific areas. We develop a multivariate cure survival model to estimate lifetime patterns of colorectal cancer screening. Screening data cover long periods of time, with sparse observations for each person. Some events may occur before the study begins or after the study ends, so the data are both left-censored and right-censored, and some individuals are never screened (the 'cured' population). We propose a multivariate parametric cure model that can be used with left-censored and right-censored data. Our model allows for the estimation of the time to screening as well as the average number of times individuals will be screened. We calculate likelihood functions based on the observations for each subject using a distribution that accounts for within-subject correlation and estimate parameters using Markov chain Monte Carlo methods. This guideline aims to ensure that patients admitted to hospital for elective surgery are known to have blood pressures below 160 mmHg systolic and 100 mmHg diastolic in primary care. The objective for primary care is to fulfil this criterion before referral to secondary care for elective surgery. The objective for secondary care is to avoid spurious hypertensive measurements. Secondary care should not attempt to diagnose hypertension in patients who are normotensive in primary care. Patients who present to pre-operative assessment clinics without documented primary care blood pressures should proceed to elective surgery if clinic blood pressures are below 180 mmHg systolic and 110 mmHg diastolic.

Aim To assess Acute Pain Service and paediatric pain management efficacy in a UK specialist paediatric hospital to inform wider recommendations for future sustainability. Background UK paediatric acute pain services vary. Although comprehensive pain management guidelines exist, consensus on the best model of care is lacking. Worldwide, medical and pharmacological advances and rapid patient turnover have increased the challenges of managing hospitalized children's pain. Simultaneously nurses, who deliver the bulk of pain management, have experienced reduction in skill mix and training opportunities. Specialist Acute Pain Services have evolved to meet these demands; their overall efficacy is unknown. Design This mixed-methods study explores pain management practice at a UK paediatric hospital to assess current efficacy and future sustainability. Method A 2013 case note review of all Acute Pain Services referrals over 14 days were compared with an interval sample of concurrent non-referred inpatient children; seven semi-structured interviews were conducted with a range of clinical staff. Results Twenty-two referrals of 15 children were made; 15 comparison children were identified. All 30 children (100%) were appropriately referred/non-referred. Acute Pain Services cases experienced higher pain levels, were more likely to have long term conditions, longer hospital stay and repeat admissions. Three key themes emerged through interview analysis: ‘addressing pain’, ‘changing contexts’ ‘pain as an “expert” skill’. Increased specialization, reduced clarity between different pain modalities and decreased training opportunities had resulted in potentially unsustainable APS dependence.

Objective To evaluate the effect of intramedullary rod (IMR) diameter on the mechanical behavior of string of pearls (SOP) plate-rod constructs. Study Design In vitro mechanical study. The use of endoscopy in anthropological and archaeological research was been well documented in the literature. This article explores the varied settings in which endoscopy is beneficial in gathering visual data for interpretation related to cultural remains and artifacts. Endoscopic data may be used to assist in the pursuit of answering such bioanthropological questions as sex, age at death, presence of paleopathologies, dental conditions, and cultural practices. Endoscopy is often used to guide and document biopsy procedures as well as the retrieval of artifacts from within poorly accessible locations such as body cavities, coffins, or tombs. In addition, endoscopic data is used to examine such archaeological features as tomb structure and design. A contrast between the medical and anthropological approach is described. Endoscopic research is enhanced when applied in conjunction with additional varied imaging modalities. While invasive, endoscopy is a nondestructive methodological approach. As with all methods, endoscopy has application and interpretational limitations, which can be described as limitations resulting from instrumentation, and those arising from personnel less familiar with the various approaches to endoscopy in both field and laboratory settings.

Background Health care associated infections are a major contributor to avoidable harm experienced by patients in modern health care settings. Recent reports suggest that electronic checklists for the documentation of a central line bundle may significantly enhance documented process compliance and help to reduce catheter-related bloodstream infection rates. Aims This paper describes the use of our electronic tool to monitor and feedback process compliance in conjunction of introducing bespoke central line insertion packs to tackle catheter-related bloodstream infections in our intensive care unit in a medium-sized district general hospital. Design and methods Continuous quality improvement programme with ‘Plan-Do-Study-Act’ cycles was implemented. The central venous catheter insertion and maintenance bundle was rolled out in 2007. To monitor compliance with the bundle elements, an electronic tool was designed as part of our bedside Clinical Information System. From 2009, regular quarterly feedback was provided on the number of central venous catheter lines inserted, compliance with the insertion and maintenance bundle and catheter-related bloodstream infection rate using the data collected through the Clinical Information System. We have also introduced dedicated line insertion trolleys and factory-prepared insertion packs. We used segmented regression analysis to assess the changes in the catheter-related bloodstream infection rate before and after implementation of the central venous catheter bundle. Results Bundle compliance increased during the implementation period and reached over 95% within 6 months. We observed a significant reduction in the catheter-related bloodstream infection rate from 15·6/1000 days to 0·4/1000 days. Regression analysis showed that only the compliance had significant effect on the number and prevalence of catheter-related bloodstream infections. Conclusion/Implications Implementation of evidence-based care bundles reinforced by real-time feedback on the performance of caregivers can significantly reduce the rate of catheter-related bloodstream infection in the intensive care unit. Ensuring that change processes are seamlessly integrated in the workflow with minimal administrative burden is crucial to the quality improvement process.

Objective Measure HCAHPS improvement in hospitals participating in the second and fifth years of HCAHPS public reporting; determine whether change is greater for some hospital types. Data Surveys from 4,822,960 adult inpatients discharged July 2007–June 2008 or July 2010–June 2011 from 3,541 U.S. hospitals. Study Design Linear mixed-effect regression models with fixed effects for time, patient mix, and hospital characteristics (bedsize, ownership, Census division, teaching status, Critical Access status); random effects for hospitals and hospital-time interactions; fixed-effect interactions of hospital characteristics and patient characteristics (gender, health, education) with time predicted HCAHPS measures correcting for regression-to-the-mean biases. Data Collection Methods National probability sample of adult inpatients in any of four approved survey modes. Principal Findings HCAHPS scores increased by 2.8 percentage points from 2008 to 2011 in the most positive response category. Among the middle 95 percent of hospitals, changes ranged from a 5.1 percent decrease to a 10.2 percent gain overall. The greatest improvement was in for-profit and larger (200 or more beds) hospitals. Conclusions Five years after HCAHPS public reporting began, meaningful improvement of patients' hospital care experiences continues, especially among initially low-scoring hospitals, reducing some gaps among hospitals.

In article number https://doi.org/10.1002/gch2.201900064 by Marcos Cruz, Brenda Parker, and co‐workers, a 1000 × 500 mm fibrous panel is robotically extruded from alginate‐based hydrogels immobilized with microalgal cells. Additively deposited layer by layer with three different hydrogels with varying water percentages, the biocompatible scaffold is hierarchically constructed and has mechanical stability. This bioprinting technique demonstrates the potential of creating large‐scale biohybrid structures for applications such as bioremediation within architecture and the built environment.

Objectives To assess the magnitude, interest, purpose and validity of vaccination-related information on Facebook and to determine whether information varies by site viewpoint. Methods The 10 largest vaccination-focused Facebook® pages, groups and places in each category were identified and classified by viewpoint (i.e. anti-, pro-, neutral) and purpose. Number of members, posts per week, likes, comments and shares per post were recorded. Posts were assessed for concordance with CDC and FDA recommendations. Results Of 30 sites, 43% (n = 13) were anti-vaccination, 7% (n = 2) neutral and 50% (n = 15) pro-vaccination. Most sites were most popular with American users. Median members were similar between anti-vaccination (2703 members, range 337–33 631 members) and pro-vaccination sites (2142 members, range 456–61 565 members, P = 0.262); however, anti-vaccination sites accumulated more posts per week by authors (median 15 vs. 3, P = 0.031) and members (median 33 vs. 1, P

Objective The purpose of this study is to examine the association of the clock drawing test (CDT) with incident dementia, cardiovascular events and mortality in very elderly hypertensive patients. Method All participants were hypertensive and aged 80 years and over. The CDT was administered at baseline and annually thereafter. Data on incident cardiovascular, fatal events and dementia were collected over follow-up. Results There were 3845 participants recruited and followed up for a mean of 2.1 years. Of these, 2701 completed a CDT with 2259 available at baseline. Of this group, 6.6% had a cardiovascular event, 6.1% died, and 10% were diagnosed with dementia. There was no relationship between baseline CDT score and subsequent cardiovascular events or mortality. For incident dementia, the hazard ratio was 0.88 (95% confidence intervals 0.83–0.94) suggesting that better performance on the baseline CDT was associated with a lower risk of dementia. Conclusion These results provide tentative support for the CDT alongside other cognitive screening tools in a hypertensive elderly population. Copyright © 2014 John Wiley & Sons, Ltd.
