Objective: To describe the initial effects of the large-scale vaccination campaign in June-July of 2002 (1-5- and 15-18-year-olds) and September-November of 2002 (6-14-year-olds) on the incidence of group-C meningococcal disease in the Netherlands., Design: Descriptive., Method: The incidence of meningococcal disease and the serogroup distribution were determined on the basis of the patient data associated with isolates of Neisseria meningitidis that were sent to the Netherlands Reference Laboratory for Bacterial Meningitis during the period from 1 January 1999 to 31 January 2003., Results: The highest monthly incidence of serogroup-C disease was reported in January-April 2002 (2.2-3.1/100,000), after which the incidence remained more or less unchanged after September at a level of 0.1-0.4/100,000. The incidence of meningococcal-C disease was 73% lower in August-October 2002 compared with the same period in 2001. In the same months, the incidence of meningococcal-C disease for the 0, 1-5, 6-14, 15-18 and > 18 year-olds was 49%, 80%, 89%, 89% and 42%, respectively, lower than in the same months in the previous year. The percentage of meningococcal disease caused by serogroup-C fluctuated from 35 to 49% in January-August 2002 and decreased to 9-19% in September-December 2002. There was also a decrease in the unvaccinated age groups., Conclusion: The vaccination campaign led almost immediately to a sharp decrease in the number of patients with meningococcal serogroup-C disease.

There has been growing interest in recent years in early switch therapy: antibiotics are administered intravenously during the early phase of the infection, and then continued orally. A large number of recent prospective and randomized studies justify the application of an early switch. There is consensus in the literature about the circumstances in which an early switch is justified: (a) the patient must show clinical improvement; (b) the oral therapy should result in sufficiently high levels at the infection site; (c) the patient must be capable of taking oral medication, there must be no signs of malabsorption and interactions with food or with other drugs should be taken into account; (d) if these rules are observed, switch to oral therapy as a rule is justified after 2 to 3 days' intravenous administration. An early switch is more comfortable to the patient, eases the load on the nursing staff and considerably reduces expenses.

Helicobacter pylori is the causative factor of chronic gastritis and ulcers of duodenum and stomach and relates to mucosa-associated lymphoid tissue (MALT) lymphomas. Host factors, bacterial factors and dietary habits conceivably play a part in the more severe mucosal lesions induced by H. pylori. H. pylori is a heterogeneous bacterial species, the various types differing in possession of a locus with genes coding for products that may intensify the inflammatory reaction. The cytotoxin-associated gene (cagA) protein is a marker for this locus; this protein and the vacuolating-associated cytotoxin A are clearly related to the ulcerative disease. Patients may be colonized by a mixed population of H. pylori consisting of cagA-positive and cagA-negative H. pylori. Also, a patient may be infected by a mixed population of H. pylori sensitive and resistant to metronidazole. Accordingly, a substantial part of the H. pylori population in a patient should be examined to arrive at an adequate conclusion concerning the production of H. pylori factors that may relate to a particular clinical picture and to the sensitivity to antibiotics of the H. pylori.

Two patients with an HIV-I infection, a man aged 47 with confusion, aphasia and diarrhoea, and a man aged 32 with dysphagia, a non-productive cough and diarrhoea, were diagnosed as having a disseminated Mycobacterium genavense infection. Both had low counts of CD4+ T lymphocytes. They responded to antimycobacterial treatment. M. genavense was recognized in Geneva in the early nineties as a causative agent of disseminated mycobacterial infections in HIV-seropositive patients with poor cellular immunity. The clinical picture resembles that of a generalized infection with M. avium-intracellulare. M. genavense is a slowly growing mycobacterium which can be isolated and identified using enriched nutrient media and molecular-biological techniques. The infection probably begins in the gastrointestinal tract after oral contamination. DNA of M. genavense can be demonstrated in 25% of the intestinal biopsy samples of non-HIV-seropositive patients.

Objective: To inventory both clinical and demographical data of patients with tuberculosis in the AMC, as well as diagnostic procedures, response to therapy and rate of resistance., Design: Retrospective, descriptive., Setting: The Academic Medical Center. Amsterdam, the Netherlands., Method: The medical records of all patients with a bacteriologically confirmed infection with Mycobacterium tuberculosis complex between January 1993 and December 1995 were studied., Results: 70 out of 100 patients with tuberculosis were not born in the Netherlands. Out of 50 patients tested 18 were HIV positive. The most common abnormality seen on X-rays of non-HIV positive patients were caverns. The X-ray of HIV positive patients showed no abnormalities in 39%; there was no correlation with CD4 cell count. In 74% of the patients with pulmonary tuberculosis the diagnosis was made by culture of the sputum. Treatment consisted of INH, rifampicin, pyrazinamide and ethambutol. Twelve patients were infected with resistant strains, of which two were multidrug resistant. Four patients died of tuberculosis., Conclusion: Tuberculosis was seen mostly among immigrants. Only half the patients with tuberculosis were tested for presence of HIV antibodies. Culture and staining of sputum played a key role in the diagnosis of tuberculosis. Multiresistant tuberculosis was present in 2% of the patients. Death due to tuberculosis in this population was 4%.

Increased international travelling, an increased number of patients with immunosuppression caused by HIV infection, and renewed interest in known but little studied microorganisms, resulted in a more frequent finding of certain medically important parasites. Three emerging protozoal infection, Cyclospora cayetanensis, Dientamoeba fragilis and Microspora (Enterocytozoon bieneusi and Encephalitizoon) are causative agents of diarrhoea. Encephalitozoon infections are also associated with hepatitis, hepatoconjunctivitis and nephritis. C. cayetanensis infection was diagnosed in 28 patients in the years 1992-1995 in the Academic Medical Centre in Amsterdam, half of these patients returning from a visit to Indonesia. D. fragilis has a prevalence in the Netherlands of 8% among patients with diarrhoea lasting longer than one week referred for parasitological investigation. The prevalence of E. bieneusi in HIV positive patients with diarrhoea was 8-10% in the Academic Medical Centre in the last five years. Infection with Encephalitozoon appears to be endemic in the Netherlands.

Objective: To determine the two-year results of nationwide vaccination with Haemophilus influenzae type b (Hib) vaccine on the occurrence of Hib meningitis in the Netherlands., Design: Retrospective controlled study., Setting: The Netherlands., Method: Children born since April 1, 1993 are vaccinated at the age of 3, 4, 5 and 11 months to protect them from infections with Hib. The number of Hib meningitis patients in the period 1 April, 1993 to 1 April, 1995, among infants born in this period who were offered the Hib vaccine (study group), was compared with the number of Hib meningitis patients in the period 1 April, 1991 to 1 April, 1993 among children born in last-mentioned period (control group)., Results: Twenty-one cases of meningitis by Hib were observed in the study group. Twelve children who, as a consequence of their age, had only been vaccinated once or not at all; 7 children were not vaccinated for several reasons. In addition one patient was infected by H. influenzae type f strain and one by a non-typable strain. In the control group 185 cases of Hib meningitis occurred., Conclusion: Hib meningitis was not observed among infants who had been vaccinated at least twice.

Objective: To determine whether patients colonised with methicillin-resistant Staphylococcus aureus (MRSA) remain carriers after discharge from hospital and whether transmission of MRSA to the community occurs., Design: Prospective and retrospective study., Patients and Methods: Fifteen patients colonized with MRSA and admitted to the University Hospital of the University of Amsterdam between January 1991 and September 1992 were screened for MRSA after their discharge from hospital (350 days median). Their family members and domestic animals were screened at the same time., Results: Cultures from 50 people (15 ex-patients, 35 family members) and II animals were investigated for presence of MRSA. Two (13%; 95% confidence interval: 2-40%) patients still carried the MRSA strain. Both still had risk factors for carriage of the bacteria. No MRSA was found in cultures from family members or domestic animals., Conclusions: Long term carriage of MRSA outside the hospital occurs infrequently and is closely related to predisposing factors of the patient. Transmission of MRSA to healthy individuals could not be demonstrated.

Objective: To assess the secondary attack rate (SAR) of meningococcal disease among the household contacts of primary patients and to describe the use of chemoprophylaxis in the Netherlands., Design: Descriptive, nation-wide survey., Methods: Information was collected of patients with meningococcal disease, reported between April 1st, 1989 and April 30th, 1990, and their household contacts. A household contact suffering from meningococcal disease between 24 hours and 1 month after hospital admission of the primary patient, was considered to be a secondary case. Chemoprophylaxis was considered appropriate if rifampicin or minocycline had been prescribed to all household contacts within a maximum of one day after admission of the primary patient., Results: There were 5 secondary cases (SAR: 0.3%). Chemoprophylaxis was prescribed to 627 of 1130 household contacts (55%). Of those the prophylaxis was considered appropriate in 46%. 2 secondary cases were not given any prophylaxis, 2 received penicillin and 1 rifampicin. Of the primary patients, 6% were given prophylaxis during their hospital stay. All meningococci, isolated from pairs of secondary and primary patients, were rifampicin sensitive., Conclusions: The SAR of meningococcal disease in the Netherlands is similar to that in other countries. Although prescription of chemoprophylaxis is not recommended by the government, it is prescribed to 55% of the household contacts, and in almost half of these instances it was considered to be appropriate. Chemoprophylaxis is rarely prescribed to primary patients. Recommendations concerning chemoprophylaxis in the Netherlands are in need of reappraisal. Based on the results from this study and the literature, the prescription of chemoprophylaxis to all household contacts of a patient with meningococcal disease, and to the index patient, is recommended.

Objective: To determine the prevalence of complement system deficiencies in patients who have survived a Neisseria meningitidis infection., Design: Retrospective., Setting: Reference laboratory for bacterial meningitis of the University of Amsterdam and the National Institute of Public Health and Environmental Protection., Method: Out of the files of the laboratory 187 patients who had experienced a meningococcal infection in the Netherlands between 1959-1990 were selected in two groups according to the infecting bacterial strain: 97 patients with a serogroup X, Y, Z, W135, 29E, or non-groupable strains and 90 patients with an infection due to serogroup A or C. The patients were asked for their cooperation by their family doctor and one of us visited the patients at home to take blood samples. The complement activity was studied with a haemolysis in gel test and with an assay of haemolytic activity in free solution., Results: Complement deficiency was present in 18% of the 187 patients who had experienced a meningococcal infection. The highest prevalence was found in patients older than 10 years who had developed infections due to serogroups X, Y, W135, or non-groupable strains (45%). Of the patients with a serogroup A or C infection, 3% had an complement deficiency. Of the complement deficiencies, 42% concerned a component of the alternative pathway, 12% a deficiency of C3, and 46% a component of the terminal route. The most commonly found deficiencies were properdin deficiency (39%) and C8 deficiency (18%). 30% of the complement deficient patients reported other family members having experienced meningitis. Recurrent meningitis was only observed in patients with terminal route deficiencies., Conclusion: We recommend that patients with a meningococcal infection due to serogroups X, Y, W135 or non-groupable strains should be screened for complement deficiency.

Objective: Determination of the frequency of pulmonary tuberculosis in an Indian community in Ecuador among symptomatic patients and contacts of patients with tuberculosis. Pulmonary tuberculosis was defined as the presence of acid fast rods in sputum., Setting: Pontificia Universidad Catolica del Ecuador, Quito, Ecuador., Design: Sputum samples were collected with the aid of an educational drama. About 5000 persons were informed on tuberculosis by this performance. The sputum samples from 241 persons were examined for presence of acid-fast rods with a direct Ziehl-Neelsen (ZN) stain and a ZN stain after concentration of the sputum. If ZN stain was negative, the sputum was cultured on Löwenstein-Jensen medium or on Coletsos medium. The Löwenstein-Jensen medium was past the expiration date., Results: Bacteriological examination of the sputum samples from 241 patients and controls brought to light 65 (26.9%) individuals with acid-fast rods in the sputum. Forty per cent of the affected patients were between the ages of 20 and 34. Investigation of 82 contact persons revealed 19 (23.2%) positive cases. The results of culturing of the sputum samples were disappointing: only 6 of 152 (3.9%) samples showed growth of acid-fast rods., Conclusions: The frequency of pulmonary tuberculosis (as defined by the presence of acid fast rods in sputum) is high in the community investigated, much higher than the official data on the prevalence of tuberculosis indicate. It is important to investigate contacts of patients with pulmonary tuberculosis to trace a source and to trace new cases.

In order to assess the correlation between bacteria isolated from liver abscesses and factors predisposing to liver abscesses, a retrospective study of clinical and bacteriological data on 21 patients with 27 episodes of pyogenic abscesses was carried out at the University Medical Centre, Amsterdam. Out of 27 episodes, 15 (55%) were associated with biliary or pancreatic disease; in seven of these 15 episodes more than one microorganism was cultured. Enterobacteriaceae were isolated in 13 (85%) of 15 episodes. Anaerobic bacteria were recovered only when operations in the pancreatico-biliary area had been performed previously. Seven episodes (26%) were related to extrahepatic disease; anaerobic bacteria (Bacteroides and Fusobacterium spp.) were isolated in five of these seven episodes. Streptococcus milleri seemed especially prominent, since this bacterium was cultured in six of seven episodes. Enterobacteriaceae were not involved in these seven episodes. Other factors predisposing to liver abscesses (15%) were diabetes mellitus n = 2), paraproteinaemia (n = I), and metastatic carcinoid (n = I). Blood cultures were positive in 65% of 23 episodes, but 40% of the positive cultures contained a smaller number of bacterial species than could be cultured from the abscess itself. Bacteria isolated from liver abscesses are related to the underlying predisposing condition. For diagnosis of the underlying condition and antibiotic therapy, puncture and bacteriologic examination of the abscess is essential.
