CAPES - Coordenação de Aperfeiçoamento de Pessoal de Nível Superior CNPq - Conselho Nacional de Desenvolvimento Científico e Tecnológico As lesões do manguito rotador geram incapacidade funcional e dor no ombro e suas possíveis causas são relacionadas com trauma agudo, processo degenerativo do tendão e impacto subacromial. O tratamento cirúrgico para a reparação da lesão é realizado, em geral, por artroscopia utilizando implantes tipo âncoras de sutura. Estes implantes podem ser metálicos ou não e suportam fios de sutura. A função primordial destes dispositivos é fornecer uma fixação estável do tendão ao osso favorecendo assim o processo de cicatrização. No entanto, o inconveniente desta técnica é a dificuldade da passagem e amarração dos fios que aumenta o tempo cirúrgico e os custos. Por este motivo, implantes que não utilizam fios de sutura foram desenvolvidos, entretanto, sua utilização é controversa, visto que são importados sendo pouco conhecida a sua eficiência no tratamento destas lesões. O principal objetivo deste trabalho foi desenvolver um implante alternativo às âncoras de sutura, que não utilize fios de sutura e que seja aplicado de forma direta e rápida. O implante proposto foi projetado no formato de um grampo de titânio onde a fixação do tendão ao úmero é feita por uma haste horizontal e a resistência ao arrancamento é fornecida por filetes posicionados nas hastes verticais do grampo. As etapas de validação do novo implante consistiram na verificação da resistência do grampo e na estabilidade em reparos do manguito rotador utilizando modelagem por elementos finitos. Para a realização das simulações numéricas foi necessária à determinação de parâmetros de forças atuantes na união do tendão ao osso através de ensaios de tração utilizando amostras de tendão e úmero bovinos. O reparo do tendão neste caso foi realizado usando âncora de sutura. As forças estimadas serviram como base para a determinação das condições de carregamentos que poderiam vir a atuar no reparo do manguito rotador utilizando o grampo projetado. Um modelo em elementos finitos do reparo com âncora de sutura também foi feito para entender o fenômeno físico do processo de amarração dos fios. A simulação considerando ambos os implantes de fixação foi feita utilizando um modelo tridimensional simplificado do úmero e tendão supraespinhal. Os resultados obtidos com a simulação mostraram que o grampo, nas condições analisadas, forneceu uma fixação estável do tendão ao osso. The rotator cuff tears generate functional disability and shoulder pain and their possible causes are related to acute trauma, tendon degeneration and subacromial impingement. The surgical treatment is performed through arthroscopy using suture anchors. These implants can be metallic or non-metallic and have suture threads. The main purpose of soft tissue fixation devices is to achieve a close approximation between the tendon and the bone to allow the former to heal securely to the latter. However, the disadvantage of this technique is the difficulty of guiding sutures through tendon and tying secure knots resulting in increased surgical time and costs. For this reason sutureless screw anchors were developed, however, there are little data regarding the mechanical properties or clinical outcomes of how well these implants perform in rotator cuff repairs. The main aim of this study was to develop an alternative implant to suture anchors, which does not use suture threads and can be applied directly and quickly to reattach tendons to bone. The proposed implant was designed in the shape of a titanium staple. The tendon attachment to the humerus with this new device is made by a cross-bar portion of the staple and the pull out strength is provided by the barbs disposed at the legs of the staple. The new implant was evaluated to check its strength and stability in rotator cuff repairs using finite element analysis. To carry out the numerical simulations it was necessary to determine parameters of forces acting on the attachment of the tendon to the bone through tensile tests using bovine tendon and humerus specimens. The predicted forces were used as the basis to determine the loading conditions of the rotator cuff repair with the new device. A finite element model of repair with suture anchor was also made to understand the physical phenomenon of the tie sutures process. The simulation considering both attachment implants using a simplified three-dimensional model of the humerus and supraspinatus tendon carried out. The results of the simulation showed that the staple, under the analyzed conditions, provided a stable fixation of tendon to bone. Dissertação (Mestrado)
