However, little is known yet about Web use by health professionals as a formal learning strategy, let alone when dealing with the informal learning. It can be perceived that the actions in the field of education with the use, and, especially, to the use, of technology as a learning tool are still made in a very intuitive way, using a hit and miss method, given the very evolution of technology in a short period of time. Thus, the overall objective of this research is to understand the profile, perceptions and social representations on learning using the Web of doctors, nurses and dentists, and a possible influence of such use in daily work. To achieve the defined goal, we used the qualitative and quantitative methodology through the use of an online questionnaire with closed and open questions, answered by 277 students of the Specialization Course in Family Health offered by the State University of Rio de Janeiro (UERJ) center at the Open University of the Unified Health System (UNA-SUS). For the analysis of the closed questions we used descriptive statistics and nonparametric bivariate tests. The analysis of the open questions was made in the light of the theory of social representations with use of the techniques of content analysis and free evocations. The survey results were presented in three papers for presentation in events and four articles submitted for publication in high quality academic journals. Based on the results, we can highlight that there is a concern that the simple consumption of information is justifying the use of the internet to the subjects, and making this use restricted to it, in lieu of the educational possibilities of cyberculture. We believe the development of actions that support a more reflective practice is necessary, in order to reverse a possible reduced use of the potential of ICTs. No campo da educação permanente na área da saúde, podem ser citadas diferentes iniciativas que visam formar profissionais com o uso das Tecnologias de Informação e Comunicação (TICs). No entanto, pouco se sabe ainda sobre o uso da web por profissionais da saúde como estratégia da aprendizagem formal, menos ainda quando se aborda a aprendizagem informal. Percebe-se que as ações no campo da educação com uso e, sobretudo, para o uso da tecnologia como ferramenta de aprendizagem ainda são feitas de forma muito intuitivas, por acerto e erro, tendo em vista a própria evolução da tecnologia em um curto período temporal. Sendo assim, o objetivo geral da pesquisa é compreender o perfil, as percepções e representações sociais sobre aprendizagem na web de médicos, enfermeiros e cirurgiões-dentistas e uma possível influência desse uso no cotidiano profissional. Para atingir o objetivo delimitado, foi empregada a metodologia quali-quantitativa através da utilização de um questionário on-line, contendo questões fechadas e questões abertas, respondido por 277 alunos do Curso de Especialização em Saúde da Família oferecido pelo núcleo da Universidade do Estado do Rio de Janeiro (UERJ) da Universidade Aberta do Sistema Único de Saúde (UNA-SUS). Para análise das questões fechadas, foi utilizada a estatística descritiva e testes bivariados não paramétricos. A análise das questões abertas foi feita à luz da teoria da representações sociais com emprego da técnica da análise do conteúdo e das evocações livres. Os resultados da pesquisa foram apresentados em formato de três trabalhos para apresentação em eventos e quatro artigos submetidos para publicação em revistas de alta qualidade acadêmica. Com base nos resultados, destaca-se como preocupação que o simples consumo de informações esteja justificando e a ele esteja restrito o uso da internet para os sujeitos, em detrimento às possibilidades educacionais da cibercultura. Acredita-se ser necessário o desenvolvimento de ações que subsidiem uma prática mais reflexiva a fim de reverter um possível uso reduzido das potencialidades da TICs.

It is more common in females. It results from a visual-vestibular conflict that occurs during locomotion in various types of transportation such as car, bus, plane and boat. The most dramatic form occurs in shipping. It causes nausea, vomiting, sweating, increased salivation, loss of appetite, malaise and hypotension. It is usually produced by vestibular stimulation, but can also be induced by visual stimuli. Both linear as angular accelerations generate motion sickness if applied for long periods in susceptible individuals. Recently there has been increased interest in this condition because of increased use of flight and automotive steering simulation technology. The aim of this study was to analyze vestibular disorders in adults with motion sickness. This is a prospective, case series. Patients were approached at the otolaryngology offices in Pedro Ernesto University Hospital and were evaluated for general and specific medical history and general and ear nose throat physical examination. Those with a history of ear disease were excluded. Later they were scheduled to perform audiometry and vestibular function tests with the graphic recording of the nystagmus by electronystagmography. The results of caloric tests found 3,33% of patients with abnormal PDN, 6,67% with abnormal PL, 3,33% with left hyperreflexia, , 3,33% with left hyporreflexia and 3,33% with left hyporeflexia. We also found some variation in the analysis of saccades, optcokinetic nystagmus and tracking eye movement. Our results were in accordance with the literature findings. Given the results, we observed that the study of motion sickness was very important for the evaluation of patients with this condition. Our study brings benefits to the medical sciences in general and in particular to otolaryngology, as it contributes to a greater understanding of motion sickness and stimulates further research in this area. A cinetose tem elevada prevalência mundial, sendo mais frequente na infância e no sexo feminino, mas também pode acometer adultos. Resulta de um conflito vestíbulo-visual que incide durante a locomoção em diversos meios de transporte, como carro, ônibus, avião e barco. A forma mais dramática ocorre no transporte marítimo. Ocasiona náusea, vômito, sudorese, aumento da salivação, redução do apetite, hipotensão e mal-estar. Geralmente é produzida por estímulo vestibular, mas também pode ser induzida por estímulo visual. Tanto as acelerações lineares quanto angulares geram cinetose se persistirem por longo período em indivíduos susceptíveis. Recentemente ocorre maior interesse nessa afecção devido ao crescente uso de tecnologia de simulação de vôo e direção automobilística. O objetivo do estudo foi analisar as alterações vestibulares nos indivíduos adultos com cinetose. Trata-se de um estudo prospectivo, tipo série de casos. Os pacientes do ambulatório do Hospital Universitário Pedro Ernesto-UERJ foram avaliados através de anamnese geral e dirigida e exame fisico geral e otorrinolaringológico. Aqueles com histórico de doença otológica foram excluídos. Posteriormente realizaram audiometria e testes vestibulares com o registro gráfico dos nistagmos através da vectoeletronistagmografia. Nos resultados das provas calóricas encontramos 3,33% de pacientes com alteração de predomínio direcional, 6,67% com alteração de predomínio labiríntico, 3,33% com hiperreflexia esquerda, 3,33% com hiporreflexia direita e 3,33% com hiporreflexia esquerda. Encontrados algumas variações na análise dos movimentos sacádicos, nistagmo optocinético e rastreio pendular. Os resultados foram de encontro aos achados da literatura. Diante dos achados, foi observado que o exame otoneurológico com registro gráfico da cinetose mostrou-se muito importante para a avaliação dos pacientes com essa afecção. O estudo traz benefícios por contribuir para o melhor entendimento da cinetose e estimular novas pesquisas na área.

In the absence of an effective vaccine or treatment for the disease, the efforts have to be focused on prevention. Health policies adopted by the Brazilian government resulted in stabilization of the disease in the country on younger age groups, although this trend hasn`t been happening in other age groups. Check the incidence of AIDS in the elderly, in Niterói, RJ, according to sex, age, period and birth cohort of 1982-2011, in addition to analyzing the spatial dynamics of the AIDS epidemic in the elderly (individuals aged 60 or more) in the state of Rio de Janeiro in the period 1997-2011, are the goals of this study. Population data by age, sex and group were obtained from population censuses, population count (1996), inter-census projections and data from the Information System for Notifiable Diseases, Mortality and Control Laboratory Tests. The incidence rates per 100,000 were calculated for geographical units by counting the number of new AIDS cases in individuals aged 60 years or older and size of the local population in the same age group. To evaluate the spatial dependence of the global rate, Moran index was calculated. Moran Maps were used to show the spatial correlation schemes potentially different in different sub-regions. Probability distributions and empirical Bayesian analysis were also used. There were 575 cases of AIDS among residents of Niteroi with ≥ 50 years of age. Increasing trend of incidence rates over time were found in both sexes. Also, the study of the spatial dynamics of the incidence of the syndrome(AIDS) in the elderly, in Rio de Janeiro, in the period 1997-2011 immunodeficiency syndrome rates between men and women, remained buoyant throughout the period. In this study, it wasn`t possible to detect overall significant correlation using the global Moran index. On the southeast coast of the state, where there are the major metropolitan areas (Rio de Janeiro and Niterói), we observe groups of cities with rates up to 20 cases per 100 000 population. This concentration becomes more pronounced in subsequent periods when it seems gradual spread of the epidemic occur south coast to the north of Rio de Janeiro. A síndrome de imunodeficiência adquirida (AIDS) é um problema de saúde pública que alcançou grandes proporções. Na ausência de uma vacina eficaz ou tratamento efetivo para a doença, esforços têm ser concentrados na prevenção. As políticas de saúde adotadas pelo governo brasileiro têm resultado em estabilização da enfermidade no país na faixa etária mais jovem, muito embora essa tendência não venha acontecendo nos outros grupos etários mais velhos. Verificar a incidência da AIDS em indivíduos idosos, no município de Niterói, RJ, de acordo com sexo, idade, período e coorte de nascimento de 1982-2011, além de analisar a dinâmica espacial da epidemia de AIDS em idosos (indivíduos com 60 anos ou mais) no estado do Rio de Janeiro no período de 1997-2011, são os objetivos deste estudo. Os dados da população por idade, sexo e grupo, foram obtidos a partir de: censos populacionais, contagem da população (1996), projeções intercensitárias, informações do Sistema de Informações de Agravos de Notificação, de Mortalidade e de Controle de Exames Laboratoriais. As taxas de incidência por 100 000 foram calculadas para as unidades geográficas através da contagem do número de novos casos de AIDS em indivíduos com 60 anos ou mais e tamanho da população do município no mesmo grupo etário. Para avaliar a dependência espacial das taxas foi calculado o índice de Moran global. Moran Mapas foram construídos para mostrar regimes de correlação espacial potencialmente distintos em diferentes subregiões. Distribuições de probabilidade e método Bayes empírico foram aplicados para a correção das taxas de incidência da AIDS. Ocorreram 575 casos de AIDS em residentes de Niterói com ≥50 anos de idade. Tendência crescente de taxas de incidência ao longo do tempo foi detectada em ambos os sexos. No estudo da dinâmica espacial da incidência da AIDS em idosos, Rio de Janeiro, no período de 1997 a 2011, as taxas entre homens e mulheres permaneceram flutuantes ao longo de todo o período. Não foi possível detectar correlação significativa global, usando o índice global de Moran. Na costa sudeste do Estado, onde se localizam as grandes áreas metropolitanas (Rio de Janeiro e Niterói), observaram-se grupos de cidades com taxas de até 20 casos por 100 000 hab. Esta concentração se torna mais pronunciada em períodos subsequentes, quando parece ocorrer propagação gradual da epidemia da costa sul até o norte do Rio de Janeiro.

This segmentation can be accomplished in different ways. This study aimed to evaluate the use of Hidden Markov Models (HMM) in the segmentation names and addresses of people and the efficiency of the segmentation on the record linkage process. Databases of the Information System on Mortality (SIM in portuguese) and Information Subsystem for High Complexity Procedures (APAC in portuguese) of the state of Rio de Janeiro between 1999 and 2004 were used. A method composed of eight stages has been proposed for segmenting the names and addresses using routines implemented in PL/SQL and a library called JAHMM, a Java implementation of HMM algorithms. A random sample of 100 records in each database was used to verify the correctness of the segmentation process using the hidden Markov model. In order to verify the effect of segmenting the names through the HMM, three record linkage process were applied on a sample of the aforementioned databases, each of them using a different segmentation strategy, namely: 1) dividing the name into first name , last name, and middle initials; 2) division of the name into five parts; 3) segmentation by HMM. The HMM segmentation mechanism was in good agreement when compared to a human observer. The three linkage processes produced very similar results, with the first strategy performing a little better than the others. This study suggests that the segmentation of Brazilian names by means of HMM is not more efficient than the traditional segmentation methods. A segmentação dos nomes nas suas partes constitutivas é uma etapa fundamental no processo de integração de bases de dados por meio das técnicas de vinculação de registros. Esta separação dos nomes pode ser realizada de diferentes maneiras. Este estudo teve como objetivo avaliar a utilização do Modelo Escondido de Markov (HMM) na segmentação nomes e endereços de pessoas e a eficiência desta segmentação no processo de vinculação de registros. Foram utilizadas as bases do Sistema de Informações sobre Mortalidade (SIM) e do Subsistema de Informação de Procedimentos de Alta Complexidade (APAC) do estado do Rio de Janeiro no período entre 1999 a 2004. Uma metodologia foi proposta para a segmentação de nome e endereço sendo composta por oito fases, utilizando rotinas implementadas em PL/SQL e a biblioteca JAHMM, implementação na linguagem Java de algoritmos de HMM. Uma amostra aleatória de 100 registros de cada base foi utilizada para verificar a correção do processo de segmentação por meio do modelo HMM.Para verificar o efeito da segmentação do nome por meio do HMM, três processos de vinculação foram aplicados sobre uma amostra das duas bases citadas acima, cada um deles utilizando diferentes estratégias de segmentação, a saber: 1) divisão dos nomes pela primeira parte, última parte e iniciais do nome do meio; 2) divisão do nome em cinco partes; (3) segmentação segundo o HMM. A aplicação do modelo HMM como mecanismo de segmentação obteve boa concordância quando comparado com o observador humano. As diferentes estratégias de segmentação geraram resultados bastante similares na vinculação de registros, tendo a estratégia 1 obtido um desempenho pouco melhor que as demais. Este estudo sugere que a segmentação de nomes brasileiros por meio do modelo escondido de Markov não é mais eficaz do que métodos tradicionais de segmentação.

No. of bitstreams: 1 Rigoleta Dutra Dias.pdf: 2695713 bytes, checksum: c725fff21da433bcb6d6bf0bbc1ea262 (MD5) Previous issue date: 2011-06-03 In 2005 the Brazilian National Healthcare Agency (Agência Nacional de Saúde Suplementar (ANS), in Portuguese) published the TISS standard, a mandatory electronic exchange claims between the health insurance (approximately 1500 registered at ANS) and healthcare providers (approximately 200 000) about the healthcare events provided to the beneficiaries. The TISS standard was developed following the structure of the ISO/TC215 Committee for Health Informatics and is divided into four parts: data structure that encompasses the forms; semantic content that refers to the vocabularies and terminologies; data interchange that includes the electronic messages; and security issues in healthcare information following a recommendation of the Federal Council of Medicine (Conselho Federal de Medicina (CFM), in Portuguese). To improve the TISS standard s evolution, this study analyses the levels of interoperability in accordance with ISO 20514 (ISO 20514, 2005) and the dual model of the openEHR Foundation, which proposes open standards for Electronic Health Records (EHR) architecture and healthcare information. The dual model of the openEHR Foundation specifications is composed, on the first level, by a generic reference model and on the second by an archetype model that details the concepts and attributes. Two studies were conducted in this thesis: the first one refers to a set of demographic archetypes developed as a proposal representation of the demographic information, based on the reference model of the openEHR Foundation. The second study proposes a generic reference model, as an improvement of the openEHR specifications, to represent the concept of submission of claims, as well as a set of archetypes. Finally, a new architecture for building the TISS standard is proposed based on the dual model of the openEHR Foundation and envisioning a patient-centered EHR Em 2005, a Agência Nacional de Saúde Suplementar (ANS) estabelece o padrão TISS (Troca de Informação na Saúde Suplementar), intercâmbio eletrônico obrigatório entre as operadoras de planos de saúde (cerca de 1500 registradas na ANS) e prestadores de serviços (cerca de 200 mil) sobre os eventos de assistência prestados aos beneficiários. O padrão TISS foi desenvolvido seguindo a estrutura do Comitê ISO/TC215 de padrões para informática em saúde e se divide em quatro partes: conteúdo e estrutura, que compreende a estrutura das guias em papel; representação de conceitos em saúde, que se refere às tabelas de domínio e vocabulários em saúde; comunicação, que contempla as mensagens eletrônicas; e segurança e privacidade, seguindo recomendação do Conselho Federal de Medicina (CFM). Para aprimorar sua metodologia de evolução, essa presente tese analisou o grau de interoperabilidade do padrão TISS segundo a norma ISO 20514 (ISO 20514, 2005) e a luz do modelo dual da Fundação openEHR, que propõe padrões abertos para arquitetura e estrutura do Registro Eletrônico de Saúde (RES). O modelo dual da Fundação openEHR é composto, no primeiro nível, por um modelo de referência genérico e, no segundo, por um modelo de arquétipos, onde se detalham os conceitos e atributos. Dois estudos foram realizados: o primeiro se refere a um conjunto de arquétipos demográficos elaborados como proposta de representação da informação demográfica em saúde, baseado no modelo de referência da Fundação openEHR. O segundo estudo propõe um modelo de referência genérico, como aprimoramento das especificações da Fundação openEHR, para representar o conceito de submissão de autorização e contas na saúde, assim como um conjunto de arquétipos. The anatomopathologic study of the liver biopsy is considered the gold standard for the evaluation of architectural distortion and degree o fibrosis of the liver s parenchyma, important prognostic factors for the patients with chronic viral hepatitis. In the actual histopathological evaluation, in addition to subjective reports of histological alterations, semiquantitative scores that correlate morphological findings with numeric degrees are used, such as the renowned Ishak and METAVIR scores. However, in all of these systems there is the disadvantage of the subjectiveness of the examinator and the incorporation of categorical description of architectural changes without reference to quantitative changes in liver collagen. Digital Image Analysis techniques (DIA) that provide objective quantification of the degrees of liver fibrosis in histological samples have been developed. But the high cost and worldwide unavailability of the technologies described restrained their use to a few specialized centers. This study aims at developing an affordable technique for liver s fibrosis DIA in chronic viral hepatitis. Three hundred and four core needle biopsies obtained by Menghini needles from patients with virus B and virus C hepatitis were studied. All samples had at least 15 mm in length or five complete portal tracts and were stained with the Masson s Trichrome Method. The staging by Ishak s and METAVIR s scores was done by a single experienced hepatopathologist, without the knowledge patient s clinical data. The microscopic images were digitalized. The fibrosis indexes were calculated in an automated way by a new technique developed in the Adobe® Photoshop® software. For the Ishak score the following median Fibrosis Indexes (FI) were observed: 0,8% ± 0,0 (stage 0), 2.4% ± 0,6 (stage 1), 4,7% ± 1,6 (stage 2), 7,4% ± 1.4 (stage 3), 14,9% ± 3.7 (stage 4), 23,4% ± 2.9 (stage 5) e 34,5% ± 1,5 (stage 6). For the METAVIR classification: 0,8% ± 0,1 (stage F0), 3,8% ± 1.8 (stage F1), 7,4% ± 1.4 (stage F2), 20,4% ± 5,2 (stage F3) e 34,5% ± 1,5 (stage F4). It has been used since 2007 at UFJF. For an effective applicability within the HU-UFJF, it was necessary to inquire into the work processes within the HU and do an action of enhancing these processes. It was relevant to avoid incorporation of information that does not match reality. A development of user requirements for the adequacy intended to be an instrument of intermediation for discussion about the development of SIGA- Adm for HU/UFJF. Use of Methodology for Analysis and Process Improvement - MAMP - in the supply chain of HU during 2004 / 2005, allowed to observe the requirements of users. A monitoring of material management is a strategy used by the Sentinel Network for the development of post-marketing actions of health products such as research use, product registration, withdrawal of product from the market and so on. University Hospital of UFJF is a member of the Brazilian Network of Sentinel Hospitals, implemented by the National Sanitary Surveillance Agency, since 2001. Shares of post-marketing products for health (in Technical surveillance) are shared by members of the ANVISA and the Sentinel Network through information on technical defects and / or adverse events that may cause harm to the population. As a result it is observed that SIGA-Adm can store information about products that allow tracking in normal and / or abnormal use by the institution and this contributes to the Technical surveillance exercise. Moreover, it is expected that construction of SIGA-Adm/HUUFJF be integrated into existing systems at UH. Esse trabalho aborda a elaboração de requisitos dos usuários como parte da adequação do sistema de informação-SISTEMA DE INFORMAÇÃO DE GESTÃO ACADÊMICA-Administrativo- SIGA-Adm, proposto para o Hospital Universitário da Universidade Federal de Juiz de Fora. SIGA-Adm está em utilização, desde 2007, na Universidade Federal de Juiz de Fora. No entanto, para a efetiva aplicabilidade no âmbito do Hospital Universitário, torna-se necessário uma investigação acerca dos processos de trabalho, dentro do HU e uma ação de melhoria desses processos, a fim de evitar a incorporação de um sistema de informação que não corresponda a realidade apresentada. A elaboração dos requisitos dos usuários para a adequação visa a ser um instrumento de intermediação para discussão acerca da elaboração do SIGA-Adm para o HU/UFJF. Os requisitos apresentados originaram-se no processo de gerenciamento de materiais, como resultado da utilização da Metodologia de Análise e Melhoria de Processos-MAMP, aplicada na cadeia de suprimentos do Hospital Universitário, no período de 2004 a 2005. O acompanhamento do gerenciamento de materiais é uma estratégia utilizada pela Rede Sentinela para o desenvolvimento de ações de pós-comercialização de produtos para a saúde tais como: investigação de uso, registro de produtos, retirada de produto do mercado e etc... O Hospital Universitário da UFJF é integrante da Rede Brasileira de Hospitais Sentinela, implementada pela Agência Nacional de Vigilância Sanitária, desde 2001. As ações Pós-Comercialização de Produtos para a Saúde, em Tecnovigilância, são compartilhadas pela ANVISA e pelos integrantes da Rede Sentinela, através de informações relativas a queixas técnicas e/ou eventos adversos que possam causar algum dano à população. A pesquisa indica a possibilidade do SIGA-Adm armazenar informações sobre os produtos que permitam o rastreamento em condições normais e/ou anormais de uso pela instituição, contribuindo para o exercício em Tecnovigilância. Além disso, espera-se que construção do SIGA-Adm/HU-UFJF permita integrar os sistemas existentes no HU.
