Objective To analyze clinical and radiological features of preeclampsia combined with reversible posterior encephalopathy syndrome (RPES) and screen related risk factors. Methods Clinical data of 111 preeclampsia patients were recorded, including age, number of pregnancies, systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), hemoglobin, blood platelet count, serum creatinine (Cr) and plasma D?dimer. Head MRI were examined in all patients. Univariate and multivariate forward Logistic regression analysis were used to screen related risk factors of preeclampsia combined with RPES. Results In 111 patients, 42 were combined with RPES. Logistic regression analysis showed headache (OR = 70.958, 95%CI: 6.578-765.447; P = 0.000), visual disturbance (OR = 3.966, 95%CI: 1.290-12.191; P = 0.016), SBP (OR = 1.034, 95%CI: 1.006-1.064; P = 0.019) and serum Cr (OR = 1.060, 95%CI: 1.000-1.123; P = 0.048) were independent risk factors of preeclampsia combined with RPES. MRI showed the lesions of preeclampsia combined with RPES were located in frontal lobe (17 cases, 40.48% ), temporal lobe (6 cases, 14.29% ), parietal lobe (32 cases, 76.19% ), occipital lobe (35 cases, 83.33%), brain stem (4 cases, 9.52%), cerebellum (7 cases, 16.67%) and basal ganglia (22 cases, 52.38%). MRI showed T1WI hypointensity, T2WI and FLAIR hyperintensity, diffusion ? weighted imaging (DWI) hyperintensity and apparent diffusion coefficient (ADC) hypointensity. Conclusions Headache, visual disturbance, SBP and serum Cr level are independent risk factors of reeclampsia combined with RPES, which are closely related to the onset of RPES. The clinical and imaging features of 11 cheiro-oral syndrome (COS) cases were retrospectively analyzed. All patients were unilaterally involved, including 10 cases of acute lacunar cerebral infarction, and one case of cerebral hemorrhage. Thalamic ischemic stroke (5 cases) was most common, and secondly were parietal lobe infarction (3 cases) and brainstem infarction (3 cases). Three of 11 cases developed progressive stroke during treatment. All patients were followed up for 3 months after discharge, 2 cases still presented ipsilateral fingertips and perioral numbness, one progressive case left hemi-dysesthesia. Damage of nerve conduction from medulla oblongata to parietal lobe can induce COS. Although the prognosis of COS is good, progressive risk can be seen in some cases. Clinical and imaging data of 4 cases of primary Sjögren's syndrome (pSS) combined with central nervous system (CNS) lesions were retrospectively analyzed. Clinical symptoms of 4 patients were as follows: 2 cases had fever; 2 cases had blurred vision; 2 cases had lower extremity weakness; one case had single lower limb weakness; one case had unilateral limb weakness; one case had hemisensory reduction; one case had bilateral abducens paralysis; 3 cases had peripheral neuropathy. Analysis of cerebrospinal fluid (CSF) revealed increased CSF pressure (210-270 mm H2O) in 3 cases, increased number of leukocyte [(50-380) × 10 6/L] in 3 cases, and increased protein (1.30-2.56 g/L) in 3 cases. Brain MRI showed multiple lesions in white matters. After high-dose steroid therapy, 3 cases had good prognosis, and one had recurrent lower extremity weakness after 6 months. pSS combined with CNS lesions is rare. Lumbar puncture and brain MRI is important for diagnosis, and high-dose steroid therapy may have good therapeutic effect. DOI: 10.3969/j.issn.1672-6731.2015.04.014

Acquired hepatocerebral degeneration (AHD) is a rare chronic encephalopathy, and its etiology is usually correlated with cirrhosis and portal hypertension. In this study, clinical and radiological data of 326 patients with cirrhosis of the liver were retrospectively analyzed, among whom 11 patients were diagnosed as AHD, with the incidence of 3.37% . The most common neurological symptoms were static tremor (9 cases) and ataxia (7 cases). Cerebral MRI showed T1WI hyperintensity in bilateral globus pallidus of all 11 patients. The lesions were also involved in bilateral thalamus (one case), tegment of midbrain (one case), bilateral posterior crus of internal capsule (one case) and bilateral putamen (one case). Anti-Parkinson's disease drugs, such as levodopa and benserazide, had not presented satisfactory clinical effect. DOI: 10.3969/j.issn.1672-6731.2015.02.014
