Background:  Nerve transfers from one common donor nerve to recipient nerves with multiple target branches can yield slower and unpredictable recovery in the target nerves. Our hypothesis is that steal phenomenon exists when multiple nerve neurotization comes from one donor nerve., Methods:  In 30 Sprague-Dawley rats, the left ulnar nerve (UN) was selected as the donor nerve, and the musculocutaneous nerve (MCN) and median nerve (MN) as the recipient target nerves. The rats were separated into three groups (10 rats in each): group A, UN-to-MCN (one-target); group B, UN-to-MN (one-target); and group C, UN-to-MCN and MN (two-target). The right upper limbs were nonoperative as the control group. Outcome obtained at 20 weeks after surgery included grooming test, muscle weight, compound muscle action potential, tetanic muscle contraction force, axon counts, and retrograde labeling of the involved donor and target nerves., Results:  At 20 weeks after surgery, muscles innervated by neurotization resulted in significant worse outcomes than the control side. This was especially true in two-target neurotization in the parameter of muscle weight and forearm flexor muscle contraction force outcome when compared to one-target neurotization. Steal phenomenon does exist because flexor muscle contraction force was significantly worse during two-target neurotization., Conclusion:  This study proves the existence of steal phenomenon in multiple target neurotization but does not significantly affect the functional results. Background:  The restoration of finger movement in total brachial plexus injuries (BPIs) is an ultimate challenge. Pedicled vascularized ulnar nerve graft (VUNG) connecting a proximal root to distal target nerves has shown unpredictable outcomes. We modified this technique by harvesting VUNG as a free flap to reinnervate median nerve (MN). We analyzed the long-term outcomes of these methods., Methods:  From years 1998 to 2015, 118 acute total brachial plexus patients received free VUNG to innervate the MN. Patients were followed up at least 5 years after the initial surgery. Donor nerves included the ipsilateral C5 root (25 patients) or contralateral C7 root (CC7 = 93 patients). Recovery of finger and elbow flexion was evaluated with the modified Medical Research Council system. Michigan Hand Score and Quick-DASH were used to represent the patient-reported outcomes., Results:  For finger flexion, ipsilateral C5 transfer to MN alone yielded similar outcomes to MN + MCN (musculocutaneous nerve), while CC7 had significantly better finger flexion when coapted to MN alone than to MN + MCN. Approximately 75% patients were able to achieve finger flexion with nerve transfer alone. For elbow flexion, best outcome was seen in the ipsilateral C5 to MCN and MN., Conclusion:  In acute total BPI, the priority is to identify the ipsilateral C5 root to innervate MN, with concomitant innervation of MCN to establish the best outcomes for finger and elbow flexion. Aim:  The author presents a solicited "white paper" outlining her perspective on the role of nerve transfers in the management of nerve injuries., Methods:  PubMed/MEDLINE and EMBASE databases were evaluated to compare nerve graft and nerve transfer. An evaluation of the scientific literature by review of index articles was also performed to compare the number of overall clinical publications of nerve repair, nerve graft, and nerve transfer. Finally, a survey regarding the prevalence of nerve transfer surgery was administrated to the World Society of Reconstructive Microsurgery (WSRM) results., Results:  Both nerve graft and transfer can generate functional results and the relative success of graft versus transfer depended on the function to be restored and the specific transfers used. Beginning in the early 1990s, there has been a rapid increase from baseline of nerve transfer publications such that clinical nerve transfer publication now exceeds those of nerve repair or nerve graft. Sixty-two responses were received from WSRM membership. These surgeons reported their frequency of "usually or always using nerve transfers for repairing brachial plexus injuries as 68%, radial nerves as 27%, median as 25%, and ulnar as 33%. Background: Ulnar nerve decompression at the elbow traditionally requires regional or general anesthesia. We wished to assess the feasibility of performing ulnar nerve decompression and transposition at the elbow under local anesthesia., Methods: We examined retrospectively the charts of 50 consecutive patients having undergone ulnar nerve entrapment surgery either under general or local anesthesia. Patients were asked to estimate pain on postoperative days 1 and 7 and satisfaction was assessed at 1 year., Results: On day 1, pain was comparable among all groups. On day 7, pain scores were twice as high when transposition was performed under general anesthesia when compared with local anesthesia. Patient satisfaction was slightly increased in the local anesthesia group. The treatment of neuroma-in-continuity is controversial. To bypass neuroma-in-continuity with a nerve graft using end-to-side neurorrhaphy is considered to be theoretically a good option. To test this therapeutic modality, we performed a nerve bypass graft in a neuroma-in-continuity rat model. An obstructive neuroma-in-continuity was created in a transected peroneal nerve by interposition using the aponeurosis of the spinal muscles. In the experimental animals, (1) immediate, (2) 3-week delayed, or (3) no ulnar nerve bypass graft was performed. The peroneal functional index (PFI), conduction velocity, tibialis anterior muscle weight, and histomorphometric analyses were performed and compared with control (simply cut and repair) animals. On postoperative day 70, the recoveries of the PFI values, conduction velocity, and tibialis anterior muscle weight in the bypassed animals showed no significant differences as compared with the control animals, and the extent of these recoveries in the bypassed animals were significantly superior to those in the no-graft animals. In the histomorphometric analysis, the mean percent nerve in the bypassed animals was significantly larger than that in the no-graft animals. In conclusion, this technique may be a good alternative to the current therapeutic techniques for neuroma-in-continuity when there is a significant retained function.

The repair of large peripheral nerve defects is not always possible, especially when the proximal stump is not available. In these cases, end-to-side nerve anastomosis has been proposed. In the present experiment, using the terminal branches of the rat brachial plexus, the authors studied behavioral responses after end-to-side nerve anastomoses using fibrin glue, 3 and 6 months after surgery. Rats were evaluated by results of a grasping test, a capsaicin test and a hot-plate test. The collected data demonstrated that there was no functional motor or sensory reinnervation after the end-to-side nerve anastomoses. The conjunctive layers of the peripheral nerve thus represented an effective barrier to reinnervation.

Twenty-five patients with severe brachial plexus lesions (having a rather poor prognosis in general), were subjected to a variety of split nerve graft procedures, with 22 achieving useful functional recovery. Thirty-eight nerves were reconstructed, with 32 of them achieving useful recovery. Results in these patients were no better nor worse than those obtained with other types of nerve grafts (e.g., free cutaneous nerve grafts, vascularized nerve grafts, etc.). The technique of splitting the nerve for the use of split fascicle groups as free nerve grafts is nevertheless recommended as an alternative to the application of the ulnar nerve as a vascularized nerve graft. The plexiform arrangement of the fascicles within the ulnar nerve apparently does not preclude the possibility of harvesting sufficiently long nerve grafts.

The concept of reconstruction to regain lost function after brachial plexus lesions has to be as broad and complex as possible. We have been exploring wider and more novel clinical concepts at the Clinic of Plastic, Hand, and Reconstructive Surgery at the Medical School of Hannover. Our ideas are supported by experience in 160 patients. We have attempted to combine the use of a vascularized nerve graft and a microvascularly-transferred autologous muscle. Patients undergoing the procedures have included those with late complete root avulsions and no functional return, as well as previously operated cases with poor recovery of biceps, wrist, and forearm function. The surgery is divided into two stages. In the first stage, the ulnar nerve is prepared as a vascularized nerve graft and is sutured to intercostal nerves 3 to 5 or 6. In stage 2, when the Tinel sign reaches the distal ends of the ulnar nerve graft (about six to eight months later), the latissimus dorsi muscle is harvested. The muscle is then placed as far distally as possible in the forearm and sutured to the deep finger flexors and flexor pollicis longus. Proximally, the insertion is performed similarly to Steindler's method. The vessels are connected to the brachial artery and vein and the thoracodorsal nerve is sutured to the graft. This method provides flexion of both the fingers and the elbow.

The ulnar nerve is supplied basically by the arteries accompanying it in its various locations: in the axillary section, by a branch of the lateral thoracic artery or directly by the axillary artery; in the upper arm, by branches originating from the collateral ulnar superior artery; in the supracondylar section and in the region of the groove for the ulnar nerve, by branches originating from the anastomosis of the collateral arteries and the posterior branch of the recurrent ulnar artery; and in the forearm, by branches of the recurrent ulnar artery and the ulnar artery. Venous return is by the venae comitantes. Since the ulnar nerve possesses a good arterial supply, it may be used with different techniques as a vascularized nerve transplant in traumatic lesions of the brachial plexus, to repair more important missing nerve paths.
