Aim, Patients and Methods: Through a defined time period, all consecutive vascular surgical patients with indicated and initiated medication with cilostazol (PAOD, stage II b) were registered and controlled clinically within 3-month time intervals to investigate the therapeutic effect in a representative, specifically vascular surgical group of patients using a systematic prospective, unicentre clinical observational study. In particular, maximum walking distance, subjective (semiquantitative) assessment of the quality of life, impact of accompanying diseases as well as the occurrence of adverse effects and their impact onto the treatment were studied., Results: Out of the 146 registered patients with initiated medication of cilostazol, 93 subjects were finally evaluated. Three months after initiation of cilostazol medication, maximum walking distance increased by 108 m (159.7 %) based on the initial 181 m. After 6 and 9 months, walking distance increased by 181 m (200 %) and 168 m (192.8 %), respectively. After 12 months, there was no further increase of the walking distance by 126 m (169.6 %) compared with months 6 and 9 but still with a significant difference to the initial distance. There was a trend of an improved walking distance from the 3rd to the 6th month (p = 0.1055) and a significant difference between the 3rd and the 9th month (p = 0.0094; no further significant differences between the time points 3rd/12th, 6th/9th, 6th/12th and 9th/12th month). While an improved quality of life was reported in 80.7 % of patients after 3 months, the rate varied between 82.6, 72.9 and 80.9 % at 6, 9 and 12 months, respectively, i.e., always in the majority of cases. There were no hints for safety concerns with regard to severe adverse effects, in particular, bleeding episodes. Subgroup analyses on accompanying diseases such as diabetes, hyperlipidemia or nicotine abuse did not reveal any impact on the improved walking distance (ABI - no feasible parameter)., Conclusion: Medication with cilostazol led to a significant increase of the maximum walking distance (p < 0.0001) compared to the initial distance, which was further increased through one year (however, the effect became weaker) accompanied by a dominating improvement of the quality of life. With regards to the rapid progress of interventional therapy in the treatment of PAOD the revision of the published guidelines (TASC Recommendations 2000) is more than necessary. The current options in therapy are modified constantly. Therefore, published recommendations might be not up-to-date. In consideration of the number of interventions and operations which are performed throughout Europe a trend towards less invasive treatment becomes apparent. Guidelines and recommendations from the Charing Cross and the ACC/AHA (ACC/AHA Guidelines for the Management of Patients With Peripheral Arterial Disease 2005) are reflecting this development. Regardless of the complexity of the planned procedure interventional treatment is almost always first choice therapy. A yearly revision of the actual guidelines would be useful, leading to an up-to-date modality. Although in the diagnostic pathway MRI and CTA gain more importance, the gold standard in diagnosis of PAOD remains the standardised measurement of the walking capacity, ankle-brachial index (ABI) and duplex ultrasound.

Purpose: Assessment of the value of contrast-enhanced MR angiography (ceMRA) using an automatic tracking technique at 1.0 Tesla in comparison with digital subtraction angiography (DSA) of the pelvic and lower limb arteries., Materials and Methods: In 15 patients with peripheral arterial occlusive disease a ceMRA (3D FLASH, TR/TE = 6.2/2.3 ms, Flip angle alpha = 30 degrees, Matrix 170 x 256) using a new automatic tracking technique was accomplished. The reference method was DSA. Twenty-one vessel segments of each patient were graded as normal, stenosed (> 50%) or occluded. Image material was evaluated independently by two radiologists., Results: In comparison, the interobserver agreement showed a good (Cohen's kappa > 71%) concordance for 38% of the 21 vessel segments, a moderate (Cohen's kappa between 31%-70%) for 21% and a poor (Cohen's kappa < 30%) concordance for 38% of the 21 vessel segments. Regarding the MRA and DSA results, a good concordance was achieved for 62% of the 21 vessel segments, 14% showed a moderate concordance and 24% only a poor concordance., Conclusion: In patients with peripheral arterial occlusive disease ceMRA using an automatic tracking technique enables a reliable evaluation of pelvis and upper limb arteries at 1.0 Tesla. However, a reliable evaluation of lower limb arteries is not yet possible.

Unlabelled: Neurolytic blocks of the lumbar sympathetic trunk enable a less invasive sympathetic denervation if compared to standard surgical procedures., Methods: 34 patients undergoing chemical lumbar sympathectomy (CLS) for advanced arterial occlusive disease were prospectively studied. An actual indication for reconstructive vascular surgical intervention had been excluded in all patients. The sympatholytic efficacy, different haemodynamic variables, the relief of rest pain and the effect on ischaemic ulcers were quantified before, 3 and 21 days after CLS., Results: The mean intensity of rest pain, as assessed on a visual analogue scale (0-100 mm), significantly decreased from 60 mm to 31 and 34 mm, respectively. 9 of 12 patients with ischaemic ulcers showed partial or complete remission within the study period. Overall, in 88% of patients sympathetic denervation could be demonstrated by ablation of the sympatho-galvanic skin response. The mean difference in skin temperature between the treated and the untreated leg significantly increased from -0.73 degrees C to +0.34 and +0.39 degrees C, respectively. Doppler-sonographic measurements of the ankle-pressure-index showed a minor increase from 0.34 to 0.36 and 0.42; however, these changes did not reach significance. Neither changes of the different physiologic variables nor data of the patient history could predict the relief of rest pain after CLS. The only side effect of CLS was transient neuralgia of the genitofemoral nerve occurring in 19% of patients., Conclusion: Considering the relatively short follow-up period, the results of this study support evidence of the therapeutic efficacy of CLS for rest pain and ischaemic ulcers in patients with advanced arterial disease. The prognostic value of standard variables for estimation of sympathetic reactivity and skin blood flow, however, could not be confirmed.

The revascularization of the profunda femoris artery, the minimal vascular operation, is described in this paper. After the anatomic and pathological relations of the femoral arteries the indications and the position of the profunda-revascularization in vascular surgery of the thigh are reported. During a period of 15 years (1959-1974) a profundaplasty was performed in 357 cases on 308 patients in the Munich University Surgical Clinic. The result of the operation could be controlled in 320 cases. The lethality-rate of operation is 3,8%. The operation was successful after 5 years in 75,3%, after 10 years in 62,6% in the whole period in 60% of the living patients. If there is a vascular disease of the deep femoral artery, the profundaplasty proves to be a standard technique in the vascular surgery of the thigh.
