Branchiogenic cysts occuring within the parotid gland are rare developmental anomalies and can be preoperatively misdiagnosed as benign parotid tumors.A 54-year-old female had a painless swelling in the right parotid region. The swelling was elastic hard, nontender, and slightly mobile. It had a smooth surface and was 33×33mm in size. Ultrasonography, CT scan and MRI suggested a benign cystic tumor in the superficial lobe of the parotid gland. A superficial lobectomy of the parotid gland was performed, with care not to injure the facial nerve. Subsequent histopathological examination revealed a branchial cyst.
