목적: 본 연구의 목적은 방광 요관 역류(vesicoureteral reflux) 환아에서 재발성 요로감염의 임상양상을 조사하고, 첫 번째 및 재발성 요로감염 간의 원인균과 항생제 감수성 결과를 비교하는 것이다. 방법: 방관 요관 역류로 진단된 소아 환자 중에서 요로감염이 발생한 환자의 의무기록을 후향적으로 검토하였다. 첫 번째와 두 번째 요로감염 건에서 같은 균종이 동정된 경우를 1군으로, 다른 균종이 동정된 경우를 2군으로 분류하여 분석하였다. 결과: 2005년 4월부터 2018년 2월까지 총 13년 간의 연구 기간 동안, 총 77명의 방관 요관 역류 환아가 적어도 한 번 이상의 요로감염을 경험하였다. 이 중 47명의 환자(61.0%)가 재발성 요로감염 소견을 보였다. 재발성 요로감염 환자 중에서 1군은 19명(40.4%)이었고, 2군은 28명(59.6%)이었다. Escherichia coli (n=37, 39.4%)는 재발성 요로감염 환자의 첫 번째 및 두 번째 요로감염 건에서 가장 흔하게 분리된 원인균이었고, 그 다음으로는 Klebsiella pneumoniae (n=18, 19.1%), Enterococcus faecalis (n=14, 14.9%), Enterobacter aerogenes (n=7, 7.4%), 및 Pseudomonas aeruginosa (n=4, 4.3%) 순이었다. 비록 통계적 차이는 없었지만, E. coli에 의한 재발 시, 같은 균에 의한 재발 임에도 ceftazidime, piperacillin/tazobactam과 trimethoprim-sulfamethoxazole에 대한 항생제 내성의 비율이 첫 번째보다 두 번째 감염에서 증가하였고, E. aerogenes에 의한 재발에서도 cefotaxime, ceftazidime, piperacillin/tazobactam 및 meropenem에 대한 항생제 내성의 비율이 첫 번째보다 두 번째 감염에서 증가하였다. 결론: 방관 요관 역류 환아의 재발성 요로감염에서 경험적 항생제를 선택할 때, 이전 감염의 병원균 및 항균제 감수성 결과가 재발 시에 항상 동일하지는 않다는 점을 염두에 두어야 한다.

목적: 최근 관찰연구들에서 낮은 혈중 25(OH)D 농도가 호흡기 감염과 관련이 있다는 결과들이 보고되고 있다. 하지만, 소아의 무작위 대조 연구에서 비타민 D의 투여가 호흡기 감염을 예방하는 효과에 대해서는 아직 논란의 여지가 있다. 이번 연구는 비타민 D 보충 요법이 호흡기 감염 예방에 미치는 영향을 체계적 문헌고찰과 메타분석을 통하여 알아보고자 한다. 방법: MEDLINE, EMBASE, the Cochrane Central Register of Controlled Trial을 이용하여 소아의호흡기 감염 예방을 위해 비타민 D 보충을 실험한 무작위 대조 연구들이 분석 대상에 포함되었다. 문헌의 비뚤림 위험도를 평가하기 위해 코크란 연합의 질 평가 방법(Cochrane Collaboration’s tool for assessing the risk of bias)을 사용하였다. 연구별로 상대위험도와 95% 신뢰구간을 추출한 후, ReviewManager 5.3을 이용하여 메타분석을 시행하였다. 결과: 총 7편의 무작위 대조 연구들이 메타분석 대상에 포함되었다. 임의효과모형을 사용하여 산출된전체 비타민 D 보충군에서 상대적 위험도는 0.82 (95% CI: 0.69-0.98)이었고, 이질성에 대한 I2=62%이었다. 연구 간의 이질성 원인을 파악하기 위해 시행한 하위 집단 분석에서 추적 관찰 기간이 1년 미만인경우, 대상군의 나이가 5세 이상인 경우, 대상군이 환자인 경우, 비타민 D 투여 요법이 매일 요법인 경우이질성이 감소하였다. 깔때기 그림에서 출판 비뚤림의 가능성을 시사하는 비대칭 소견을 보였다. 결론: 메타분석 결과는 소아에서 비타민 D 보충요법이 호흡기 감염을 예방하는 데 효과가 있을 수 있음을 보여준다. 하지만, 이번 연구에서는 분석대상에 포함된 연구들이 적고, 연구들 간에 이질성이 존재하며, 출판 비뚤림이 존재할 가능성이 있어 결과를 주의해서 해석할 필요가 있다.

목적: 2000년부터 식품의약품안전처 주관 하에 국내 주요 백신의 안전성과 면역원성에 대한 임상평가자료를 확보하기 위한 백신 평가 연구를 시작하였다. 본 연구는 백신의 면역원성, 효능, 효과, 안전성 및 다른 평가분야에 대해 수행된 연구 및 보고서들을 고찰하여 각 백신 별 유용성 평가 자료를 찾아 분석하고자 하였다. 방법: 지난 2000년부터 2014년까지 여러 연구자들에 의해 “백신 유용성 평가” 과제가 수행되었다. 백신 유용성 평가 자료들의 결과 및 성과들을 고찰하여 향후 국내 백신 정책에 활용할 수 있는 가능성을 분석하였다. 각 백신은 백신 평가 영역에 따라 자료를 분석하고 체계화하였다. 결과: 2000년부터 2014년까지 주요 백신에 대해 총 83개의 연구과제가 수행되었다. 각 백신 별로는 BCG 8개, DTaP/Td 14개, 폴리오 1개, Hib 5개, 폐구균 3개, 인플루엔자 11개, A형 간염 3개, MMR11개, 수두 11개, 일본뇌염 16개였다. 평가 영역은 안전성, 면역원성, 면역도, 면역지속능, 효능평가, 효과평가, 유효성 평가기술, 품질평가 및 기타로 나누어 분석하였다. 결론: 우리나라에서 수행된 백신 유용성 평가 연구를 통해 유용한 자료가 도출되어 향후 국내 백신 평가 사업, 백신 정책 수립 및 대중/전문가 교육에 필요한 기초자료로 활용될 수 있을 것이다.

This study was conducted to examine the relationship between experience in child care and common infectious diseases in children under 5 years of age. Purpose: Human bocavirus (hBoV), a recently discovered virus, has been detected in children with respiratory tract infections worldwide. The aim of this study was to analyze the frequency and molecular phylogeny of hBoV in the respiratory samples of children with acute respiratory tract infections in 2010. Methods: Nasopharyngeal samples were collected from 953 children with lower respiratory tract infections at Severance childrens hospital in Korea from January 2010 to December 2010. We applied the multiplex PCR technique for the identifi ' - cation of 12 respiratory viruses from the samples. Among the total specimens, hBoV positive samples were subjected to phylogenetic analysis by sequencing a fragment of the VP1/VP2 gene junction. Results: hBoVwas detected in 141 (14.8%) among 953 patients. The 61.7%of hBoV-positive samples were found to co-exist with other respiratory viruses. The results of phylogenetic analysis showed that all 141 hBoV-positive isolates were identified as hBoV 1, revealing a high similarity among the isolates (>98%). Conclusion: hBoV1 with minimal sequence variations circulated in children with acute respiratory infections during 2010. More research is needed to determine the clinical severity and outcomes of the minimal sequence variations. (Korean J Pediatr Infect Dis 2012;19:71-78)

Purpose : Enteroviral infection is a common viral illness in children. We undertook this study in attempt to comprehend the epidemiologic and clinical features of enteroviral infections, particularly EV71 in children. Methods : We enrolled 63 children with enteroviral infection at Severance Children's Hospital in Seoul between May and August 2009. Reverse transcriptase-polymerase chain reaction (RT-PCR) was performed from stool or cerebrospinal fluid samples, which were then tested for enteroviral infection. Viral isolation and serotype identification also were performed by RT-PCR. Results : A total of 63 patients with clinical diagnosis of enteroviral infections were enrolled; of those, 38 (60%) were positive for enterovirus. The mean age of the patients was 2 years and 7 months and the sex ratio of male to female was 0.9 :1. Their clincal manifestations included aseptic meningitis (21 cases, 55%), HFMD (16 cases, 42%), herpangina (5 cases, 13%), neonatal fever (2 cases, 5%), encephalitis (1 case, 3%), and myocarditis (1 case, 3%). Serotypes of isolated enteroviruses were EV71 (8 cases, 21%), coxsackievirus B1 (8 cases, 21%), coxsackievirus A16 (2 cases, 6%), coxsakievirus A2 (1 case, 3%), coxsakievirus A5 (1 case, 3%), and echovirus 9 (1 case, 3%). Clinical symptoms of EV71 infection included HFMD (5 cases, 63%), aseptic meningitis (3 cases, 38%), encephalitis (1 case, 13%), and myocarditis (1 case, 13%). A positive rate of C-reactive protein in EV71 was higher than those in other enterviral infections. However, there was no statistically significant difference in other laboratory findings. Conclusion : We reported on identified enteroviruses, including EV71, during a period of 3 months in the summer of 2009. In this study, EV71 infection frequently occurred in male and clinical manifestation caused by EV71 was a more severe disease than that due to other enterviral infections. There is a need for continuous surveillance of enteroviral infection and its clinical manifestations for diagnosis and treatment of enteroviral infection.
