The case describes the interdisciplinary treatment of a 23-year-old woman with a Class III malocclusion, missing an upper right lateral incisor, abrasion of the maxillary incisal edges, anterior gingival margin discrepancies, and gingival recession. Initially, the patient was treated with fixed appliances combined with orthognathic surgery. The extraction of the upper left lateral incisor and bilateral canine substitution plan was chosen. At the end of the surgical and orthodontic treatment, the restorative treatment with six veneers was accomplished to improve smile esthetics. Despite the missing lateral incisors, the patient showed a natural, good-looking final result. A symmetric incisal plane was established, a functional occlusion with average vertical and horizontal overlap was set, and the bone scallop and consequently the gingival margins were leveled. The interdisciplinary approach hid all of the initial esthetic defects of the case. The result highlights how to obtain a remarkable improvement of the smile outcome with a well-functioning masticatory system.

ObjectivesTo compare the degree of accuracy of the Face Hunter facial scanner and the Dental Pro application for facial scanning, with respect to both manual measurements and each other.Materials and MethodsTwenty-five patients were measured manually and scanned using each device. Six reference markers were placed on each subject's face at the cephalometric points Tr, Na′, Prn, Pog′, and L–R Zyg. Digital measurement software was used to calculate the distances between the cephalometric reference points on each of the scans. Geomagic X Control was used to superimpose the scans, automatically determining the best-fit alignment and calculating the percentage of overlapping surfaces within the tolerance ranges.ResultsIndividual comparisons of the four distances measured anthropometrically and on the scans yielded an intraclass correlation coefficient index greater than .9. The t-test for matched samples yielded a P value below the significance threshold. Right and left cheeks reached around 60% of the surface, with a margin of error between 0.5 mm and −0.5 mm. The forehead was the only area in which most of the surface fell within the poorly reproducible range, presenting values out of tolerance of more than 20%.ConclusionsThree-dimensional scans of the facial surface provide an excellent analytical tool for clinical evaluation; it does not appear that one or the other of the measuring tools is systematically more accurate, and the cheeks are the area with the highest average percentage of surface in the highly reproducible range.

This case report describes orthodontic treatment including both skeletal maxillary expansion and unilateral distalization by means of a single bone-borne appliance followed by clear aligner therapy in a young adult patient. A surgical guide was digitally designed and three-dimensionally printed to facilitate the placement of four miniscrews in the palatal vault. The miniscrews were fitted and the bone-borne appliance was delivered in a single clinical appointment. The postexpansion photographic records and models demonstrate the opening of the palatal median suture, the pure skeletal expansion, and the resolution of the left crossbite after 40 activations. Specifically, left molar Class I was obtained in about 5 months without any loss of anterior anchorage, and the subsequent aligner phase achieved all of the objectives established in the treatment plan. This case report shows clearly how careful digital planning of miniscrew insertion and the delivery of a pure bone-borne appliance in a single sitting enabled good clinical outcomes to be achieved in an acceptable timeframe, without side effects, even in a young adult patient.

This case report describes the use of a miniscrew-assisted rapid palatal expander and aligners to correct bilateral cross-bite and crowding in an adult patient with a Class III skeletal pattern. A digitally designed surgical guide was three-dimensionally printed and used to accurately insert four miniscrews into the palate; these were employed to anchor a novel miniscrew-assisted rapid palatal expander appliance without any dental anchorage. Cone-beam computed tomograms before and after miniscrew-assisted rapid palatal expander treatment demonstrated the orthopedic expansion of the maxilla without dental tipping. The patient was then fitted with aligners to correct crowding and malocclusion. This case report demonstrates the successful treatment of an adult patient with a narrow maxilla and bilateral cross-bite using a nonsurgical, conservative treatment.

Objective: To investigate the stress release properties of four thermoplastic materials used to make orthodontic aligners when subjected to 24 consecutive hours of deflection. Materials and Methods: Four types of aligner materials (two single and two double layered) were selected. After initial yield strength testing to characterize the materials, each sample was subjected to a constant load for 24 hours in a moist, temperature-regulated environment, and the stress release over time was measured. The test was performed three times on each type of material. Results: All polymers analyzed released a significant amount of stress during the 24-hour period. Stress release was greater during the first 8 hours, reaching a plateau that generally remained constant. The single-layer materials, F22 Aligner polyurethane (Sweden & Martina, Due Carrare, Padova, Italy) and Duran polyethylene terephthalate glycol-modified (SCHEU, Iserlohn, Germany), exhibited the greatest values for both absolute stress and stress decay speed. The double-layer materials, Erkoloc-Pro (Erkodent, Pfalzgrafenweiler, Germany) and Durasoft (SCHEU), exhibited very constant stress release, but at absolute values up to four times lower than the single-layer samples tested. Conclusions: Orthodontic aligner performance is strongly influenced by the material of their construction. Stress release, which may exceed 50% of the initial stress value in the early hours of wear, may cause significant changes in the behavior of the polymers at 24 hours from the application of orthodontic loads, which may influence programmed tooth movement.

Objective: To evaluate the degree to which the height, width, and cross-section of rectangular and square orthodontic archwires affect the play between the archwires and the bracket slot. Materials and Methods: The stated measurements (height and width) of 43 archwires from six different manufacturers were compared with real values obtained using a digital gauge. The curvature (radius) of the edge bevels was also measured to calculate the play within the slot, and this measurement was compared with the ideal value. Results: The real height and width of the archwires differed from those stated by the manufacturers, falling within the range −6.47% and +5.10%. The curvature of each bevel on each archwire cross-section was shown to differ, and consequently increased the real play between the archwire and slot with respect to the ideal to different degrees. Conclusions: The archwire-slot play was greater than the ideal for each archwire considered, inevitably leading to a loss of information within the system.

OBJECTIVES: To investigate and compare the characteristics of commonly used types of traditional and heat-activated initial archwire by plotting their load/deflection graphs and quantifying three suitable parameters describing the discharge plateau phase. MATERIALS AND METHODS: Forty-eight archwires (22 nickel titanium [NiTi] and 26 heat-activated) of cross-sectional diameter ranging from 0.010 to 0.016 inch were obtained from seven different manufacturers. A modified three-point wire-bending test was performed on three analogous samples of each type of archwire at a constant temperature (37.0°C). For each resulting load/deflection curve, the plateau section was isolated, along with the mean value of the average plateau force, the plateau length, and the plateau slope for each type of wire obtained. RESULTS: Statistically significant differences were found between almost all wires for the three parameters considered. Statistically significant differences were also found between traditional and heat-activated archwires, the latter of which generated longer plateaus and lighter average forces. The increase in average force seen with increasing diameter tended to be rather stable, although some differences were noted between traditional and heat-activated wires. CONCLUSIONS: Although great variation was seen in the plateau behavior, heat-activated versions appear to generate lighter forces over greater deflection plateaus. On average, the increase in plateau force was roughly 50% when the diameter was increased by 0.002 inch (from 0.012 to 0.014 and from 0.014 to 0.016 inch) and about 150% when the diameter was increased by 0.004 inch (from 0.012 to 0.016), with differences between traditional and heat-activated wires noted in this case.
