In this paper, a variant of the open shop scheduling problem is considered in which the intermediate storage is forbidden among two adjacent production stages (zero buffer or machine blocking constraint). The performance measure is to minimise the maximal completion time of the jobs (makespan). Since this is an NP-hard problem, a two-stage constraint programming approach is proposed as a new exact method. Computational experiments were carried out on 222 literature problem instances in order to test the performance of the proposed algorithm. The relative deviation is adopted as the performance criteria. Computational results point to the ability of the proposed method to solve large-sized instances in comparison with the developed mixed-integer linear programming model and a simple constraint programming model, both with user cuts. In all set of instances, the proposed two-stage method performed better than benchmarking methods and integer programming models, with average relative deviation regarding objective values as lower as 12%. In addition, the results point to a competitive efficiency in computational times of the proposed method with less than 200 s in the most instances to obtain the optimal solution, in comparison to competitive metaheuristics from literature of the problem, for the tested test instances. A review of several statistical methods that are currently in use for outlier identification is presented, and their performances are compared theoretically for typical statistical distributions of experimental data, considering values derived from the distribution of extreme order statistics as reference terms. A simple modification of a popular, broadly used method based upon box-plot is introduced, in order to overcome a major limitation concerning sample size. Examples are presented concerning exploitation of methods considered on two data sets: a historical one concerning evaluation of an astronomical constant performed by a number of leading observatories and a substantial database pertaining to an ongoing investigation on absolute measurement of gravity acceleration, exhibiting peculiar aspects concerning outliers. Background and purpose: The Sheba model of orthogerioatric medicine is a unique model of in-hospital care for elderly hip fractured patients, based upon the concept that a hip fracture represents a geriatric, rather than an orthopedic disease. The nature and feasibility of such a comprehensive orthogeriatric unit, taking care of all surgical, medical and rehabilitation needs, in a single geriatric-based setting (rather than orthopedic-based), were questioned. The aim of the study is to describe the results of its operation during a five-year period. Method: A retrospective charts analysis of consecutive older patients with hip fractures, admitted from the emergency unit directly to the orthogeriatric unit of a department of geriatric medicine. Results: A total number of 592 patients were admitted. Mean age of patients was 83.2 years, mostly women. A total of 538 (91%) were treated surgically. Delay to surgery was 3.6+/-2.9 days. A total of 65.6% were suitable for rehabilitation, and had a mean Functional Independence Measure (FIM) gain of 22.3+/-9. Mean total hospital length of stay was 29.9 days and 68.7% of patients returned to their previous living residence. Rates of major complications (4.1%) and in-hospital mortality (3.2%, equivalent to 30 days mortality) were low. Conclusions: Treatment within this unit was associated with low rates of major morbidity and mortality, short stay and acceptable functional outcomes. The data provide clinical evidence supporting the implementation of this model of comprehensive orthogeriatric care, being a practical, applicable and feasible service for elderly hip fractured patients, and covering the various needs of these patients. OBJECTIVE: Societal services after traumatic spinal cord injury in Sweden were investigated, including self-rated levels of satisfaction with the application process and resource allocation. DESIGN: Survey of an incidence population. SUBJECTS: Thirty-four persons of a total regional incidence population (n = 48) with traumatic spinal cord injury. METHODS: Structured interviews using a standardized questionnaire. RESULTS: About 25 separate services were identified being available for persons with traumatic spinal cord injury. The average number of applications per person was 5 (range 0-11). The most common service was 'transportation service'. Of the applications, 17% were partially or totally rejected. Most subjects received information about available services from a social worker. For 13 available services at least 1 subject claimed ignorance about its existence. CONCLUSIONS: In Sweden, significant resources are allocated for allowing independence and financial compensation for individuals with traumatic spinal cord injury. However, this support system sometimes also results in frustration and disappointment. Insufficient information and co-ordination are reported as weaknesses. The persons' efforts to acquire knowledge of how the system works take time which could be better used for rehabilitation and full integration into the community. [ABSTRACT FROM AUTHOR]

The effect of functional electrical stimulation (FES) training on body composition, assessed by computed tomography, and the effect of spasticity, assessed by both objective and subjective measures, are evaluated. Fifteen motor-complete spinal-cord-injured men participated in the study. Eight of the 15 subjects undertook FES cycling 3 times weekly for 6 months. Whole body computed tomography scans evaluated changes in body composition. Simultaneous Modified Ashworth Scale and electromyography (EMG) measurements, resistive torque (Kin-Com) and EMG measurements, and self-ratings with Visual Analogue Scale during four consecutive days were used to evaluate changes in spasticity. Lower extremity muscle volume increased by an average of 1300 cm3 (p < 0.001) in the training group compared to the control group, who experienced no change. Otherwise no changes in body composition were seen. Significant correlations (Spearman) were found between individual EMG activity recordings and movement-provoked Modified Ashworth Scale ratings in 26% of the test situations, irrespective of group and time. The objective and subjective evaluation of movement-provoked passive (viscoelastic) and active (spasticity-related) resistance remained unchanged. [ABSTRACT FROM AUTHOR]
