Introduction: Preterm birth, defined as birth before 37 weeks of gestation, is the leading cause of death of children under the age of five years worldwide. Globally there has been no change in the preterm birth rate between 2010 and 2020, when rates were calculated to be 9.8% and 9.9% of all livebirths, respectively. The analysis of amniotic fluid has been advocated by many to identify the mechanisms driving threatened preterm labor. Amniocentesis is largely considered to be safe, but care provider concerns about complications in the setting of threatened preterm labor (tPTL) exist. This systematic review will critically review the data regarding safety and outcomes of amniocentesis in cases of PPROM and tPTL and help in the counseling and clinical decision-making of patients and care providers alike. Methods: This systematic review was conducted in accordance with the preferred reporting items for systematic reviews and meta-analyses (PRISMA). All English language, peer-reviewed human studies where amniocentesis was used to sample the amniotic fluid of patients presenting with tPTL or PPROM and published between January 1990 and March 2022 were included. Data on the uptake rates, success rates, and safety profile associated with amniocentesis in this context of tPTL or PPROM were analyzed as part of the systematic review. Included studies compromised of randomized control trials, cohort studies, and case-control studies. Conference abstracts and abstracts with no full text were excluded. Results: 10,215 studies were returned after searches were conducted in MEDLINE, EMBASE, EMCARE, Web of Science, and SCOPUS databases using free text and Medical Subject Headings (MESH). 399 studies were assessed for eligibility with 15 studies being included in the final review. The main reason for exclusion from the review was an absence of safety data. Four studies gave information on uptake rates of amniocentesis in cases of PPROM and tPTL, with a range of 55% in an observational study to >99% in centers offering it as part of routine care. Eleven studies detailed success rates of amniocentesis, with all centers reporting >90% success rates. However, in some centers, a "successful" amniocentesis was deemed to be a retrieval of >0.5 ml. Three studies reported risks associated with amniocentesis. There were four reported complications (all transitory) in a series of 1119 cases (0.35%). Discussion: This systematic review found that amniocentesis in cases of PPROM or threatened preterm labor is a safe and feasible procedure. In this article we introduce 'capital flow' and 'capital friction' as terms that characterize features and user behaviours on internet platforms which encourage and discourage spending, respectively. We explore these via leading live streaming platform Twitch, where numerous platform features and normative behaviours both promote and downplay spending. This makes it an ideal site for understanding conflicting economic and social platform dynamics, and the meta-friction between flows and frictions of capital on a platform. We examine these phenomena through three case studies: Channel Points, a non-monetary 'currency' exchangeable for in-stream rewards; Bits, a real-money platform currency for donating to streamers; and streamers' performative and verbal responses to financial support. Through capital flow and capital friction we present Twitch's platform economy as an ongoing negotiation both among users and between users and platform features, and one where capital friction does not necessarily lead to a reduction of income, but generates new streamer-spectator-platform relations and more circuitous routes for the flow of capital. Our theorization of capital flow and capital friction thus provides new insights into platform features, their uptake or resistance, and the frictions within platform designs and platform cultures. Congenital heart disease (CHD) is the most common cardiac disorder in pregnancy in the western world (around 80%). Due to improvements in surgical interventions more women with CHD are surviving to adulthood and choosing to become pregnant. Preconception counseling, antenatal management of CHDs and strategies to prevent maternal and fetal complications. Preconception counseling should start early, before the transition to adult care and be offered to both men and women. It should include the choice of contraception, lifestyle modifications, pre-pregnancy optimization of cardiac state, the chance of the child inheriting a similar cardiac lesion, the risks to the mother, and long-term prognosis. Pregnancy induces marked physiological changes in the cardiovascular system that may precipitate cardiac complications. Risk stratification is based on the underlying cardiac disease and data from studies including CARPREG, ZAHARA, and ROPAC. Women with left to right shunts, regurgitant lesions, and most corrected CHDs are at lower risk and can be managed in secondary care. Complex CHD, including systemic right ventricle need expert counseling in a tertiary center. Those with severe stenotic lesions, pulmonary artery hypertension, and Eisenmenger's syndrome should avoid pregnancy, be given effective contraception and managed in a tertiary center if pregnancy does happen. This study investigates the strategies video game streamers with disabilities employ to navigate their identity as gamers as it relates to their subject positions as persons with disabilities. Through an analysis of online videos featuring eight streamers with disabilities, this study reveals four themes around how streamers establish their identities regarding both disability and gaming: establishing gaming capital, acknowledging disability, gaming to overcome challenges, and feeling empowered to 'inspire'. Our analysis discusses how the four themes coalesce around a co-constitutive identity of 'disabled streamer' that is unique from both gamer and disability identities yet informed by and negotiated through each of these in various ways. This paper examines the ongoing gamblification of engagement mechanisms on the live streaming website Twitch.tv (Twitch). Twitch is the market-leading platform for live broadcast of digital games and digital gambling, with two million content creators reaching around one hundred and fifty million viewers per month. Streamers use a variety of monetization techniques to encourage fan engagement while generating revenue: this includes incorporating chance-based elements and unpredictable rewards, part of the ongoing broader convergence of gambling and gaming products. The primary research objective for this study is to investigate the chance-based mechanics in these stream interaction and engagement services, how they work, and how these mechanics fit within elements of traditional legal definitions of gambling: consideration, chance, and prize. Understanding how game spectators engage with streamers helps establish a foundation for understanding how emerging forms of media engagement fit within a policy landscape that might not be designed for technology-driven gambling and gaming consumption. The themes that emerge here have important implications for streamers who monetize, stream extension developers who operate in spaces where certain game mechanics may fall into gambling or other consumer protection oversight, and regulatory authorities who maintain that oversight. This article examines depictions of 'deep play' – play with outcomes substantial enough to affect life outside the game – and 'dark play' – play that is non-consensual – in contemporary cinema, and their relationship to the ideological trappings of late capitalism. The article analyses numerous films with such themes and notes a clear commonality in their depiction of hyper-capitalist societies or socioeconomic processes, and argues that this recurring element is a reflection of the current dominant global culture of axiomatic capitalism and the high-stakes game-like competition it creates and forces upon its citizens. The paper examines the forms of capitalism portrayed and the intersections between capitalistic and ludic competition, focusing on four themes: games as amusements for the powerful, games as agents of social control, games and the question of 'fairness', and forms of resistance to these capitalistic games. It concludes by summarising new understandings of our contemporary situation we might gain through a closer look at this distinctive set of cultural depictions of life under extreme capitalism, in which capitalism has shifted from metaphorically 'playing with lives' to doing so literally. Twitch.tv is one of the most successful online live streaming platforms in the world, with 200 million viewers, 2 million regular "streamers," and a market value of over $1billion. In this paper, we offer a first conceptualization of streamers as social media influencers, and how effectively they can perform strategic communication for sponsors. We draw on extensive ethnographic research and over a hundred semi-structured interviews with streamers to address two questions: first, how does Twitch operate as a platform for strategic communication; second, what skills do streamers need to be successful influencers? In the first case, we show Twitch is well suited to influencing, in large part due to its integration of data analytics, while streamers are using these tools to adopt a business-oriented mindset; in the second case, we show the importance of authenticity to both streamers and clients, and how channels of different sizes offer strategic communication opportunities. The article contributes to the emerging literature on Twitch, developing insights from influencing and strategic communication, but given the increasing scope of live streaming, we also argue the phenomenon – particularly when combined with the economic dynamics of influencing – is just as important for making sense of the wider media landscape today. This article examines consumer perspectives on match-fixing in esports – professionalized competitive video game play – and the implications of these perspectives for understanding game and gambling integrity. The relationship between match-fixing, game integrity and gambling is a close one, as gambling markets are reliant on strong game integrity, but has not yet been studied in detail in the context of esports. Drawing on extensive qualitative data collected from esports fans around the world, this article examines perceptions of gambling awareness, integrity and esports gambling to assess esports consumers' awareness of and attitudes towards gambling-related match-fixing. Results indicate that esports viewers are not deeply concerned by match-fixing. In addition, spectators typically view gambling as a cause of corruption among competitors, but also understand and accept some elements of the practice. Further, spectators tend to rely on rules to determine their assessment of what is 'wrong', rather than assessments based on ethics, and are often willing to forgive infractions through a range of reasons and justifications. We propose a need for education among esports spectators, extending existing anti-cheating programmes beyond just athletes to include the broader esports community. [ABSTRACT FROM AUTHOR]

In this paper I explore economic and inclusion opportunities for people with disabilities and mental health issues afforded by 'live streaming' ‒ the live broadcast of one's activities over the internet to a globally dispersed audience. In both 2016 and 2017, the leading live streaming platform Twitch.tv broadcast over 500,000 years of video, which were produced by over two million regular broadcasters ('streamers'), and consumed by an audience of several hundred million viewers. Streamers can profit, up to and including a full-time living 'wage' for those at the highest levels. Numerous successful streamers with chronic health issues have discussed the personal and professional benefits streaming brings them. Utilising data from a research project with 100 interviews, alongside approximately 500 hours of ethnographic observation, this paper examines the experiences of live streaming for broadcasters with disabilities, mental health issues, or physical health issues. Firstly, I explore the positive elements of streaming for these broadcasters, focusing on the many conditions represented in this demographic, and the benefits streaming gives for inclusion and community. Secondly, I consider the negative experiences of these streamers, focused on entanglements of health and technology that make their streaming lives potentially more challenging than their colleagues. Thirdly, I focus on the economic opportunities, and the potential for entrepreneurial activity, the platform affords. I conclude the analysis by exploring how these aspects make live streaming a potentially exemplary emancipatory and entrepreneurial space for these individuals, but not one without challenges. This paper explores the lives and careers of video game live broadcasters, especially those who gain their primary real-world income through this practice. We introduce the dominant market leader - the platform Twitch.tv - and outline its immensely rapid growth and the communities of millions of broadcasters, and tens of millions of viewers, it now boasts. Drawing on original interview data with professional and aspiring-professional game broadcasters ('streamers'), we examine the pasts, presents, and anticipated futures of streamers: how professional streamers began streaming, the everyday labour practices of streaming, and their concerns and hopes about the future of their chosen career. Through these examinations we explore the sociotechnical entanglements - digital intimacy, celebrity, content creation, and video games - that exemplify this new media form. Live streaming is an online practice expanding in both production and consumption at immense speed, and Twitch and its streamers appear to be at the forefront of that revolution. This paper examines the development of the ‘space tourism’ industry, a concept which primarily denotes the development of space technology for recreational or leisure purposes. It will first theoretically locate space tourism with relevant streams of the mobilities literature, primarily research on aeromobilities and tourism mobilities. It will then summarise existing literature on space tourism, focusing especially on the different models of space tourism that have been proposed and the subtle but important differences between them. The analysis then explores the perceptions of feasibility of space tourism from those within the space sector, the anticipated changes to living these forms of space tourism would bring with them, and the comparisons with existing forms of mobility that are drawn by many in the space sector when attempting to ‘sell’ the value and potential of space tourism. In the final part of the paper, we explore many of the implicit assumptions held by this nascent industry, the uncertain position of the passenger within these conceptions of future space tourism, and identify avenues for future research into this emerging form of mobility. Human chorionic gonadotrophin (hCG) is released within hours of fertilization and has a profound ability to downregulate maternal cellular immunity against trophoblastic paternal antigens. It also promotes angiogenic activity of the extravillous trophoblast, and impairment of this function may lead to inadequate placentation and an increased risk of preeclampsia. There is increasing evidence that hCG alters the activity of dendritic cells via an upregulation of indoleamine 2,3-dioxygenase activity. This reduces T-cell activation and cytokine production, as well as encouraging Treg cell recruitment to the fetal-maternal interface. These changes are critical in promoting maternal tolerance hCG is also able to increase the proliferation of uterine natural killer cells, while reducing the activity of cytotoxic peripheral blood natural killer cells. There are rare reports of autoantibodies directed against hCG or the luteinizing hormone/hCG receptor in women with recurrent miscarriage. These autoantibodies are more frequent in women with thyroid autoimmunity. This may explain the association between thyroid autoimmunity and impaired fertility. Downregulating these anti-hCG and anti-luteinizing hormone/hCG receptor autoantibodies may be helpful in some women with early miscarriage or recurrent failed in vitro fertilization. The article discusses the resiliency of normal-mode analysis (NMA) in its ability to represent functional macromolecular dynamics. It addresses the definition of normal modes by a harmonic potential-energy expansion about a particular configuration. The importance of NMA as a tool for analyzing biomolecular dynamics into functionally relevant motional modes is considered. The relevance of neutron scattering for NMA is also mentioned.

We have determined the sequence and extent of maternal renal and uterine adaptation to pregnancy and examined the role of hormonal factors in their regulation. Renal bipolar diameter (RBD), renal artery resistance index (RARI), uterine artery pulsatility index (UAPI), and plasma relaxin, human chorionic gonadotropin (hCG), progesterone, estradiol, urea, and creatinine were measured longitudinally in women with normal spontaneous singleton pregnancies, in vitro fertilization (IVF) singleton pregnancies, ovum donation (OD) singleton pregnancies, and multiple pregnancies from prepregnancy to postpartum. There was a progressive increase in the RBD and the RARI and a decrease in the UAPI during pregnancy. These changes reversed toward prepregnancy levels by 6 weeks post delivery. There was no difference in the rate of change of RBD, RARI, and UAPI between spontaneous singleton, IVF singleton, OD singleton, and multiple pregnancies ( p < 0.05), but relaxin was directly correlated to the RARI ( r == 0.654, p == 0.015), and progesterone was inversely correlated to uterine artery PI ( r == 0.554, p == 0.049). These data show that renal size and resistance to blood flow increase with advancing gestation, whereas the uterine artery resistance declined with gestation. Objective: To determine the sequence and extent of maternal cardiovascular adaptation in an integrated way in spontaneous and assisted conception pregnancies Methods: Cardiovascular assessments were performed pre-pregnancy, during pregnancy and 6 weeks post delivery in women with normal spontaneously conceived singleton pregnancies (n = 13); singleton IVF pregnancies (n = 5); singleton OD pregnancies (n = 4); and multiple pregnancies (n = 6). Results: The OD singleton pregnancies had a significantly higher diastolic and mean blood pressure (BP) than the normal singleton pregnancies at 10 weeks gestation (p = 0.010 and 0.008, respectively), and the multiple pregnancy group had a higher systolic and mean BP at 16 weeks gestation than the IVF singleton pregnancy group (p = 0.045 and 0.033, respectively). The multiple pregnancy group had higher heart rate (HR) at 16 weeks gestation (p = 0.010), higher fractional shortening (FS) at 16 weeks gestation (p = 0.006), and higher left ventricular (LV) mass at 26 weeks gestation (p = 0.049) than the IVF singleton pregnancy group. The rate of change of LV mass was correlated to the rate of change of the blood pressure (r = 0.725, p = 0.005 for mean BP) and inversely correlated to the rate of change of fractional shortening (r = 0.767, and p = 0.002). Conclusion: Ovum donation pregnancies are associated with higher BP in early pregnancy, whilst multiple pregnancies are associated with higher BP, FS and LV mass in later pregnancy. This paper presents key findings from the first major study of the provision of infertility services to South Asian communities in the UK. The research aimed to explore the social meanings of infertility and to examine the experiences of couples receiving fertility treatment. Focus groups with people from Pakistani, Bangladeshi and Indian communities (n = 93) revealed a strongly pro-natalist ideology and a relatively limited knowledge of infertility and treatments. Interviews with 50 participants from the same communities revealed a general satisfaction with secondary level infertility services. However, a minority felt inadequately informed about their condition, tests undertaken and treatment options; only one-third were given any written information about treatment; many were concerned about delays and waiting times; a minority felt that staff could be more sympathetic in their response to ‘failed’ treatment and several couples suggested that additional emotional support would be helpful. No information or resources were available in any South Asian language and the arrangements for communication support for non-English speakers were generally less than adequate. This paper briefly outlines the housing careers of Vietnamese households since their arrival in the UK in the late 1970s and draws on a series of focus groups to examine the role that ethnicity has played, and may continue to play, in their housing careers. It notes that the first arrivals lacked the infrastructure of an existing Vietnamese community in the UK and were further disadvantaged by the dispersal policy adopted by the voluntary agencies responsible for their resettlement. The research indicates that ethnicity has been a crucial influence on the housing careers of Vietnamese households, and in particular on the location of the communities. It is argued that it is essential that researchers pay attention to the role of ethnicity as a resource and a constraint within a housing career. [ABSTRACT FROM AUTHOR]

There is clear evidence of inequalities in the health status of minority ethnic groups in Britain. This includes a number of conditions which may be alleviated or prevented through increased levels of physical exercise. Other research has shown lower levels of activity and raised body mass among Asian groups. There remains some scope for argument about the reasons for these differences. It is, for example, suggested that health education materials are poorly targeted at South Asian communities, or that certain cultures discourage involvement in recreational physical activities. It is known that both motivation and self-image are important to physical activity behaviour, from school age upwards. A lifestyle survey sponsored by the Health Education Authority, and qualitative research, provide an opportunity to examine the degree to which there are distinctive 'ethnic' barriers to exercise or other healthy physical activity amongst communities of South Asian origin living in England. While there are many similarities with the reasons given by members of the white population for 'not exercising', there are some subtle differences between communities, and a number of issues which appear to be specific to the minority populations. Some are particular to a religion, gender or generation. Attention to these, including questions of modesty, gender segregation, and safety, would improve the accessibility of recreational activities to this target group. At the same time, it is clear that individuals from these communities are well informed and well motivated, and do take advantage of local facilities when they can. The question of both institutional and personal racism cannot be excluded from a strategy for promoting healthy physical activity; equally, it is important to pay attention to the characteristics of particular communities to gain their confidence.
