The Mini-Mental State Examination (MMSE) is a long-established test to screen for dementia, estimate the severity and monitor the progression of cognitive impairment. The MMSE total score is dependent upon demographic factors, particularly education, but little is known about how education influences the 6 distinct MMSE cognitive domains. The present study aims to understand how the performances in the MMSE cognitive domains reflect clinical diagnosis and educational level. The study recruited 1043 participants, comprising 388 healthy controls, 360 patients with Mild Cognitive Impairment (MCI) and 295 patients with dementia. The association of the MMSE cognitive domains scores with clinical diagnosis (healthy, MCI, dementia) and educational level (primary education, middle school, high school and university/college) was analyzed with a multivariate ordinal regression model. The scores in all MMSE domains were generally higher in healthy controls as compared to patients with MCI, and higher in these as compared to patients with dementia. The MMSE domain Constructional ability was associated to the education level, the domains Orientation, Recall and Language were associated to diagnosis, Attention and calculation was associated to both education level and diagnosis, and Registration was not associated to either education or diagnosis. In conclusion, impairment in specific MMSE domains pinpoints cognitive decline, probably indicating brain areas affected by neurodegeneration, and impairment in others reflects lower education levels and the lack of acquisition of relevant schooling abilities. Introduction: Mental Time Travel (MTT) is the people's ability to remember themselves in the past and to imagine themselves in the future, and influence important life domains such as making decisions and planning future actions. It is widely recognized that patients with aMCI have deficits in episodic memory, but they also show impairments in semantic memory. It has been controversial whether MTT tasks are disturbed in aMCI mainly in relation to internal details related to episodic information, or external details, representing semantic and other extraneous information. The present study assessed whether patients with aMCI are affected in MTT regarding generation of internal details and external details, in past and future dimensions. Furthermore, it analyzed production in individual detail categories (internal: event details, thought/emotion, place, time, perceptual; external: extraneous events, semantic, other, repetitions). Method: Twenty-nine patients with aMCI and 29 healthy controls underwent a MTT task based on an Autobiographical Interview, where they had to generate past and future events in response to cue words. Transcriptions were segmented and classified into internal detail categories and external detail categories, and composite scores were obtained. Results: Patients with aMCI could globally produce significantly less details than controls. Similar to controls, patients with aMCI produced more internal details than external details, had more difficulty in generating details regarding the future as compared to the past, and scored higher in the detail categories event details and thought/emotion which represent internal detail types. Conclusions: Patients with aMCI showed widespread deficits in MTT, presumably reflecting deficiencies in the complex and multiple cognitive abilities required for MTT tasks. [ABSTRACT FROM AUTHOR]

Introduction: Patients with mild cognitive impairment (MCI) may make suboptimal decisions particularly in complex situations, and this could be due to temporal discounting, the tendency to prefer immediate rewards over delayed but larger rewards. The present study proposes to evaluate intertemporal preferences in MCI patients as compared to healthy controls.Method: Fifty-five patients with MCI and 57 healthy controls underwent neuropsychological evaluation and a delay discounting questionnaire, which evaluates three parameters: hyperbolic discounting (k), the percentage of choices for delayed and later rewards (%LL), and response consistency (Acc).Results: No significant differences were found in the delay discounting questionnaire between MCI patients and controls for the three reward sizes considered, small, medium, and large, using bothkand %LL parameters. There were also no differences in the response consistency, Acc, between the two groups.Conclusions: Patients with MCI perform similarly to healthy controls in a delay discounting task. Pure alexia (PA) has been associated with visual deficits or a failure to activate the visual word form area (VWFA). We report a patient with pure alexia due to posterior cortical atrophy, in whom event-related potentials revealed a delay in the P100 component and an absent N170 compared with controls. Furthermore, there was a tendency for a larger delay in P100 latencies associated with incorrectly read words. This suggests that some cases of PA might result from deficits in visual perception, signaled by the P100 early potential which could lead to an inability to consistently activate the VWFA, marked by the absent N170. Introduction: Prospective memory (PM) is a fundamental requirement for independent living which might be prematurely compromised in the neurodegenerative process, namely in mild cognitive impairment (MCI), a typical prodromal Alzheimer’s disease (AD) phase. Most encoding manipulations that typically enhance learning in healthy adults are of minimal benefit to AD patients. However, there is some indication that these can display a recall advantage when encoding is accompanied by the physical enactment of the material. The aim of this study was to explore the potential benefits of enactment at encoding and cue-action relatedness on memory for intentions in MCI patients and healthy controls using a behavioral PM experimental paradigm.Method: We report findings examining the influence of enactment at encoding for PM performance in MCI patients and age- and education-matched controls using a laboratory-based PM task with a factorial independent design.Results: PM performance was consistently superior when physical enactment was used at encoding and when target–action pairs were strongly associated. Importantly, these beneficial effects were cumulative and observable across both a healthy and a cognitively impaired lifespan as well as evident in the perceived subjective difficulty in performing the task.Conclusions: The identified beneficial effects of enacted encoding and semantic relatedness have unveiled the potential contribution of this encoding technique to optimize attentional demands through an adaptive allocation of strategic resources. Our objective was to test whether data mining techniques, through an unsupervised learning approach, support the three-group diagnostic model of primary progressive aphasia (PPA) versus the existence of two main/classic groups. A series of 155 PPA patients observed in a clinical setting and subjected to at least one neuropsychological/language assessment was studied. Several demographic, clinical and neuropsychological attributes, grouped in distinct sets, were introduced in unsupervised learning methods (Expectation Maximization, K-Means, X-Means, Hierarchical Clustering and Consensus Clustering). Results demonstrated that unsupervised learning methods revealed two main groups consistently obtained throughout all the analyses (with different algorithms and different set of attributes). One group included most of the agrammatic/non-fluent and some logopenic cases while the other was mainly composed of semantic and logopenic cases. Clustering the patients in a larger number of groups (k > 2) revealed some clusters composed mostly of non-fluent or of semantic cases. However, we could not evidence any group chiefly composed of logopenic cases. Background:Quality of life (QoL) is affected in patients with dementia, but it is not clear whether it is already disturbed in more initial phases of cognitive decline, like Mild Cognitive Impairment (MCI). Aim:Compare the QoL in MCI patients with controls without cognitive impairment, and ascertain whether there are differences in the reports of QoL made by the subjects and by their informants. Methods:Two hundred participants were enrolled, divided into MCI patients (n = 50), MCI informants (n = 50), recruited from a memory clinic and a dementia outpatient clinic, and controls (n = 50) and controls informants (n = 50), recruited in a family practice clinic. QoL was assessed with the QoL in Alzheimer disease (QOL-AD) scale. Results:The total scores of the QOL-AD questionnaire were 32.1 ± 6.9 for MCI patients self-report, 27.2 ± 6.7 for MCI patients in the opinion of their informants, 35.3 ± 4.9 for controls self-report and 35.6 ± 4.9 for controls in the opinion of their informants. MCI patients had lower QOL-AD scores than controls. The QoL reported by patients with MCI was more favorable than the opinion of their informants. Conclusion:The QoL is affected at early stages of cognitive decline. The QoL reported by patients with MCI is better than the opinion of their informants, similarly to what is known in Alzheimer's disease patients. Serial position effects in word list learning have been used to differentiate normal aging and dementia. Prominent recency and diminished primacy have consistently been observed in Alzheimer's disease (AD). We examined serial position effects in patients with mild cognitive impairment (MCI), in patients with AD, and in normal healthy controls. Additionally, we classified MCI patients into those who progressed to AD (MCI-p) and those who did not (MCI-np). We compared two serial position measures: regional and standard scores. Regional scores, mainly the primacy effect, improved discrimination between MCI and controls and between MCI-np and MCI-p, proving to be more sensitive and specific than the recency effect. The Algarve, the most renowned Portuguese destination, is usually associated with its main tourist product, sun and beach. In 2007, the Portuguese government launched a program of events called Allgarve, intending to reposition the destination image. Conceived from the tourists' perspective, the Allgarve program was not initially welcomed by residents. Based on data from 384 questionnaires applied to residents in the region, this study examines the dimensions of the Algarve image and the Allgarve image, and shows how participation in Allgarve events is affecting both. Several studies examine residents' attitudes towards tourism development. Similarly, significant research explores the determinants of tourist satisfaction and its effects in planning policies for tourism. The purpose of this study is to establish a connection between these lines of research, showing how the local population may impact upon the way tourists perceive a destination. This study was conducted in the tourist region of Algarve, Portugal, and involves four municipalities bearing different characteristics. Results from statistical analyses show that tourists lodging in municipalities where residents are more positive to tourism experience greater receptiveness and are more satisfied with the tourist experience. Algarve is Portugal's top tourism destination. In looking to develop the traditional sun and beach tourism, the Portuguese government launched the Allgarve program in 2007 with the aim of positioning the region as a premier tourism destination, promoting the cultural industry as a modern and competitive component of the regional economy. Using partial least squares modelling (PLS), the study explores the relationship between the image projected by Allgarve and the image of Algarve as a tourism destination. Results show that the program has had a positive and reasonably strong influence on the destination image. In terms of affective elements, findings show that these were not easily perceived by tourists, as anticipated by the Allgarve program. The relationship between memory complaints and objective memory performance remains poorly understood, particularly in young and middle aged people. We studied the relationship between reports of memory complaints and objective memory performance, and the possibility of differentiating good and poor reporters across the lifespan based on concordance between reported abilities and objectively assessed performance. This cross-sectional study enrolled 292 healthy individuals, aged 18 to 87 years, able to perform common activities of daily living and without neurological or psychiatric conditions or systemic diseases likely to interfere with cognition. No correlation between memory complaints, as assessed by the Subjective Memory Complaints scale (SMC) score and the objective memory performance, evaluated by the long-delay free recall (LDFR) score of the California Verbal Learning Test (CVLT), was found, even when grouping the participants by decade. The SMC score was influenced by the presence of depressive symptoms. Participants who were more educated, female and younger tended to have a higher CVLT-LDFR score. Younger subjects were more likely to have good memory performance and report few memory complaints than older subjects. In conclusion, there are differences in the reliability of memory reporting across the lifespan, younger subjects being more likely to correctly report good memory than older subjects.
