We describe the New Zealand Parkinson's Progression Programme (NZP3), its goals, findings, and future plans. To date, 354 people with Parkinson's disease and 89 healthy older controls have participated over a 14-year period. A major focus of the programme has been the characterisation of current cognitive impairment, and the identification of biomarkers for its future emergence in people with Parkinson's. The programme has made significant contributions to the concept of mild cognitive impairment (MCI) in Parkinson's and the development and validation of standardised criteria for it. Brain imaging, both MRI and PET, has also been a focus, showing associations between increasing brain pathology and declining cognitive function. Additional biomarkers such as genetics, fluid biomarkers, eye movement, speech, and quantitative electroencephalography (EEG) are also under investigation. The programme has become a platform supporting many other avenues of research, from investigating the personal impacts of caregiver burden through to national-level epidemiology. To date, the programme has led to multiple journal publications and 17 completed and 9 ongoing PhDs, and many other postgraduate theses. It has led to the development of a skilled core of early-career through to senior researchers and clinicians. We discuss the future directions for the programme. [ABSTRACT FROM AUTHOR]

Aotearoa New Zealand's population is ageing. Increasing life expectancy is accompanied by increases in prevalence of Alzheimer's Disease (AD) and ageing-related disorders. The multicentre Dementia Prevention Research Clinic longitudinal study aims to improve understanding of AD and dementia in Aotearoa, in order to develop interventions that delay or prevent progression to dementia. Comprising research clinics in Auckland, Christchurch and Dunedin, this multi-disciplinary study involves community participants who undergo biennial investigations informed by international protocols and best practice: clinical, neuropsychological, neuroimaging, lifestyle evaluations, APOE genotyping, blood collection and processing. A key research objective is to identify a 'biomarker signature' that predicts progression from mild cognitive impairment to AD. Candidate biomarkers include: blood proteins and microRNAs, genetic, neuroimaging and neuropsychological markers, health, cultural, lifestyle, sensory and psychosocial factors. We are examining a range of mechanisms underlying the progression of AD pathology (e.g. faulty blood–brain barrier, excess parenchymal iron, vascular dysregulation). This paper will outline key aspects of the Dementia Prevention Research Clinic's research, provide an overview of data collection, and a summary of 266 participants recruited to date. The doctorate and doctoral writing remain popular areas of inquiry and discussion, and yet very little research has empirically investigated the trends in dissertation types and how these trends might indicate broader changes in dissertation writing practices. This article builds on our recent work that investigated the macrostructures and research designs of 1,373 education-based PhD dissertations from five major Canadian research universities. In this current article, we more deeply explore the emergence in popularity of two 'alternative' or non-traditional dissertation macrostructures in education fields: the manuscript-style dissertation and the topic-based PhD dissertation. We highlight the popularity of these two dissertation types as evidence of shifting notions of what doctoral research and dissertations can (and do) look like in contemporary PhD programs. We focus specifically on these two dissertation macrostructures that were prevalent in our analysis, yet which are scarcely addressed in education-based dissertation resources. We provide a deeper reflection on the popularity of these dissertation models from our large-scale study, the ways these types of dissertations are organized at the global (macrostructural) level, and the chosen research designs, number of chapters, word counts, and authorship status (as either single-authored, partially co-authored, or mostly co-authored texts). This study examined the role of written feedback in the academic discourse socialization of second language doctoral students at a Canadian research university. Using a second language socialization framework, and foregrounding the sociocultural and interactional contexts of language learning and use, this article considers written feedback to be a form of social practice that socializes students and discursively positions them into a range of identity categories, academic discourse practices, and communities. Data sources include interviews, student-produced narratives, and written feedback artifacts. The analysis focuses on the students' interpretations of, responses to, and stated preferences and dispreference towards the feedback they received. Findings reveal that feedback played a formative role in their broader socialization and contributed to the co-construction of academic identities and (in)access to preferred discourse practices in their departments and disciplines. Purpose: Our objective was to explore the intersection between mild traumatic brain injury (MTBI) recovery experiences and injury understandings, using both quantitative and qualitative methods. Materials and Methods: The quantitative component was a descriptive case–control study comparing participants (n = 76) who had recovered or not recovered after an MTBI, across demographic and psychological variables. A subset of participants (n = 10) participated in a semi-structured interview to explore experiences of recovery in more detail. We followed threads across the datasets to integrate findings from component methods. Results: The quantitative analyses revealed differences between the two groups in terms of injury recovery understandings and expectations. The qualitative analyses suggested that achieving consistency across information sources was important. By tracing threads back and forth between the component datasets, we identified a super-ordinate meta-theme that captured participants' experiences of wrestling with uncertainty about their recovery and the impacts in terms of heightened anxiety, confusion, and feelings of invalidation. Conclusion: The effectiveness of psychoeducation and reassurance after MTBI may be optimized when content is tailored to the individual. Clinicians are urged to attend both to the subjective interpretations patients make of information gained from formal and informal, internal and external sources, and where information across these sources conflicts and creates uncertainty. Effectiveness of psychoeducation and reassurance after injury may be optimized when content is tailored to the individual rather than being generic. Effectiveness of such interventions may also be optimized by understanding the subjective interpretations individuals make of injury knowledge gleaned from formal and informal, internal and external sources. Conflicting information from such multiple sources may create uncertainty with associated increased distress as an individual negotiates their recovery from injury. Postsecondary institutions in the global north have rapidly internationalized, driven mainly by the proliferation of international students and responses to these changing demographics. This internationalization has captured attention across various platforms, including increased media focus directed toward these students and the primary and ancillary influences of their participation in PSIs and surrounding communities. The project explored in this paper examines this phenomenon by connecting internationalization of higher education research with insights from critical media studies and framing theory to investigate news media representations of international and refugee students' participation in Canadian universities and colleges. A critical thematic analysis was performed on 391 news media texts published between 2000 and 2017. Findings reveal the Canadian news media's tendency to construct issues related to international students and internationalization into one or a combination of four broad macrothemes: (1) Canada as benevolent and ideal; (2) international students and internationalization as commodified assets; (3) international students and internationalization as threats; and (4) the strategic neutrality of data. Objectives: Post-concussion-like symptoms (PCS) are common in patients without a history of brain injury, such as those with chronic pain (CP). This exploratory study examined neuro-cognitive and psychological functioning in patients with PCS following mild traumatic brain injury (mTBI) or CP, to assess unique and overlapping phenomenology.Methods: In this case-control study, participants (n = 102) with chronic symptoms after mTBI (n = 45) were matched with mTBI recovered (n = 31) and CP groups (n = 26), on age, gender, ethnicity and education. Psychological status, cognitive functioning, health symptoms, beliefs and behaviours were examined.Results: Participants who had not recovered from an mTBI and participants with CP did not differ in terms of PCS symptoms, quality of life, distress or illness behaviours, however, the CP group endorsed fewer subjective cognitive problems, more negative expectations about recovery and more distress (p < 0.05). On cognitive testing participants who had not recovered from an mTBI demonstrated greater difficulties with attention (p < 0.01) although differences disappeared when depression was controlled in the analyses.Conclusions: Unique patterns associated with each condition were evident though caution is required in attributing PCS and cognitive symptoms to a brain injury in people with mTBI presenting with chronic pain and/or depression. Human development should be seen as a broad, emancipatory and social process, rather than the mere expansion of individual choices or 'capabilities'. In post-colonial nations, a strong 'human development enabling' state is necessary to build basic human capacity, such as in health, education, shelter and nutrition; and then to promote popular participation. This requirement can be seen in a range of historical experience and is reflected in the declaration on the right to development. This investigation evaluated the familiarisation conditions required to promote subsequent and more long-term improvements in perceptual processing of dysarthric speech and examined the cognitive-perceptual processes that may underlie the experience-evoked learning response. Sixty listeners were randomly allocated to one of three experimental groups and were familiarised under the following conditions: (1) neurologically intact speech (control), (2) dysarthric speech (passive familiarisation), and (3) dysarthric speech coupled with written information (explicit familiarisation). All listeners completed an identical phrase transcription task immediately following familiarisation, and listeners familiarised with dysarthric speech also completed a follow-up phrase transcription task 7 days later. Listener transcripts were analysed for a measure of intelligibility (percent words correct), as well as error patterns at a segmental (percent syllable resemblance) and suprasegmental (lexical boundary errors) level of perceptual processing. The study found that intelligibility scores for listeners familiarised with dysarthric speech were significantly greater than those of the control group, with the greatest and most robust gains afforded by the explicit familiarisation condition. Relative perceptual gains in detecting phonetic and prosodic aspects of the signal varied dependent upon the familiarisation conditions, suggesting that passive familiarisation may recruit a different learning mechanism to that of a more explicit familiarisation experience involving supplementary written information. It appears that decisions regarding resource allocation during subsequent processing of dysarthric speech may be informed by the information afforded by the conditions of familiarisation. Primary objective: To examine fatigue prevalence, severity, predictors and co-variates over 6 months post-mild traumatic brain injury (MTBI). Research design: Longitudinal prospective study including 263 adults with MTBI. Procedures: Participants completed the Fatigue Severity Scale (FSS), Rivermead Post-concussion Symptoms Questionnaire (RPSQ), Hospital Anxiety and Depression Scale (HADS) and the Short Form 36 Health Survey-Version 2 (SF-36v2). Complete data were available for 159 participants. Key measures; prevalence-RPSQ Item 6: severity-FSS. The effect of time on fatigue prevalence and severity was examined using ANOVA. Multiple regression analysis identified statistically significant covariates. Main outcomes and results: Post-MTBI fatigue prevalence was 68%, 38% and 34% at 1 week, 3 and 6 months, respectively. There was a strong effect for time over the first 3 months and moderate-to-high correlations between fatigue prevalence and severity. Early fatigue strongly predicted later fatigue; depression, but not anxiety was a predictor. Fatigue was seen as laziness by family or friends in 30% of cases. Conclusions: Post-MTBI fatigue is a persistent post-concussion symptom, exacerbated by depression but not anxiety. It diminishes in the first 3 months and then becomes relatively stable, suggesting the optimum intervention placement is at 3 months or more post-MTBI. [ABSTRACT FROM AUTHOR]

The number of wireless mobile communication service subscribers reached 4.6 billion worldwide in 2009, and mobile revenues are expected to be over US$1 trillion around 2012 according to the International Telecommunication Union (ITU). A significant number of studies have been made to forecast the growing market and evaluate the new generation technology, the 'beyond 3rd generation' (B3G). However, there is no study that forecasts when any of these new technologies will be commercialised. This paper presents a technical framework for forecasting the commercialisation timeline of B3G technologies and provides insight on technology trajectories from 1G to 4G. The results show that a combination of technical parameters can explain heterogeneous wireless mobile communication technologies. The OECD's Guidelines on Poverty and Health are compromised by neoliberal assumptions and corrupted by several major conflicts of interest. These conflicts of interest are embodied in the DAC guidelines, whereby specific OECD commercial interests influence the public policies of developing countries. Commercial agendas are being incorporated into developing country health policy through development assistance doctrine. Yet better critical health outcomes generally occur in the well-coordinated, public health systems, rather than those with high levels of private supplementation. Objective: This study examined motor impairments over 1 year following mild closed head injury (CHI). It is the first study to serially assess long-term oculoinotor and upper-limb visuomotor function following mild head trauma. Methods: Thirty-seven patients with mild CHI and 37 matched controls were compared at 1 week, 3 months and 6 months and 31 available pairs at 12 months post-injury on measures of saccades, oculomotor smooth pursuit, upper-limb visuomotor function and neuropsychological performance. Symptomatic recovery was sampled using the Rivermead Postconcussion Symptoms Questionnaire. Results: At 1 week, the group with CHI reported high levels of post-concussional symptoms and exhibited prolonged saccade latencies, increased directional errors, decreased saccade accuracy and impaired fast sinusoidal smooth pursuit concomitant with increased arm movement reaction time, decreased arm movement speed and decreased motor accuracy on upper-limb visuomotor tracking tasks. Neuropsychological testing identified deficits only in verbal learning and speed of processing while attention, short-term/working memory and general cognitive performance were preserved. At 3 and 6 months, the group with CHI continued to show deficits on several oculomotor and upper-limb visuomotor measures in combination with some deficits on verbal learning and improved, yet abnormal, levels of post-concussional symptoms. At 12 months, the group with CHI had no cognitive impairment but residual deficits in eye and arm motor function and continued to show elevated levels of post-concussional symptoms. Conclusions: The findings indicate that multiple motor systems are measurably impaired up to 12 months following mild CHI and that instrumented motor assessment may provide sensitive and objective markers of cerebral dysfunction during recovery from mild head trauma independent of neuropsychological assessment and patient self-report. Objective : Mild closed head injury (CHI) can impair performance on volitional saccades (fast eye movements), with poorer saccade accuracy being one of the principal deficits. Assessing a patient group with known deficits of volitional saccades, the authors investigated whether mild CHI similarly impairs the implicit adaptation of visually-guided (reflexive) saccades, an important process which maintains saccadic accuracy. Methods : Within 2 weeks following mild CHI, 30 patients and 30 matched controls were compared on a computerized paradigm, which artificially induced saccadic adaptation. In response to an initial stimulus, subjects made a saccade during which the stimulus was displaced centripetally causing the initial (primary) saccade to be inaccurate. While these intra-saccadic changes remained unnoticed by the subjects, the displacements gradually caused adaptive saccadic hypometria. Results : No differences in adaptation were found between the CHI group and the controls ( F (1, 29) = 0.51, p = 0.48). This finding indicates that mild CHI does not impair implicit reflexive saccade adaptation and suggests that cerebellar function and functions of deeper brain structures such as the thalamus, superior colliculus and the basal ganglia may be largely preserved following mild CHI. The current results support the notion that the profile of oculomotor function after mild CHI reflects a centripetal gradient of impairment and relates closely to the functional integrity of the injured brain. This article considers the capacity of small island and independent nationstates to recover from serious external economic crises by looking at the case of Cuba in the 1990s and comparing Cuban adjustment with adjustment in Thailand under an essentially IMF-controlled plan in 1997. Cuba had to restructure its socialist economy in the wake of the collapse of its trading relationship with the old Soviet Union and other Council for Mutual Economic Assistance (CMEA or COMECON) countries. Thailand had to restructure its capitalist economy after a collapse of its currency and the widescale failure of financial institutions.
