Background: Throughout their careers, doctors and other healthcare professionals experience numerous transitions. When supporting transitions, opportunities for development and learning should be maximized, while stressors having negative impacts on well-being should be minimized. Building on our international data, this study aimed to develop a conceptual model of the trainee-trained transition (i.e. the significant transitions experienced by doctors as they complete postgraduate training moving from trainee/resident status to medical specialist roles). Methods: Employing Multiple and Multidimensional Transitions (MMT) theory and current conceptualizations of clinical context, this study undertook secondary analysis of 55 interviews with doctors from three countries (Netherlands, Cananda and the UK) undergoing trainee-trained transitions. Results: Through this analysis, the Transition-To-Trained-Doctor (T3D) conceptual model has been developed. This model takes into consideration the multiple contexts and multiple domains in which transitions take place. Discussion: This model is significant in that it has several uses and is applicable across countries: to remind doctors, managers and medical educators of the complexity of transitions; to frame and facilitate supportive conversations; and as a basis to teach about transitions. Empathy has been highlighted as a key concept in chaplaincy care, but its meaning has hardly been explored in depth within this field. This study aims to help develop stronger conceptual clarity by investigating humanist chaplains' conceptualizations of empathy. Data were collected through semi-structured interviews with twenty humanist chaplains working in health care, military, and prisons. A qualitative design was employed to clarify which components and features constitute empathy in humanist chaplaincy care. Empathy emerges as a multidimensional concept that is "fundamentally human." Chaplains distinguish between true and pseudo empathy based on different features including authenticity and concern. This article provides a conceptual model that combines the different components and features of empathy in humanist chaplaincy care and the relationship between them in light of empathy's humanizing quality. In this paper, we focus on task restructuring as one of the most frequently occurring types of change in our contemporary knowledge society. In spite of its evident prevalence, research on task restructuring and employee well-being has been scarce until now. Based on Conservation of Resources (COR) theory and the Job Demands-Resources (JD-R) model, we argue that task restructuring is negatively related with employee well-being (in terms of emotional exhaustion and vigour). Furthermore, we advance that opportunities for learning through reflection and experimentation, as well as recently acquired KSAOs can serve as buffers in the relationship between task restructuring and well-being. Hierarchical regression analyses and simple slope analyses were conducted in order to test the research hypotheses on a large sample of the Dutch working population (N = 1711). Task restructuring had a positive association with emotional exhaustion and a negative one with vigour. Furthermore, recently acquired KSAOs, as well as opportunities for reflection and experimentation, buffered the relationship between task restructuring and emotional exhaustion. Opportunities for reflection and experimentation moderated the relationship between task restructuring and vigour as well. After the decline of the compartmentalization of Dutch society, where healthcare was being organized along confessional/denominational lines, spirituality became neglected or implicit for decades in the Netherlands healthcare system. During the modernization of healthcare in the 1960s, the development of professional language concerning chaplaincy and psychosocial care in a secularizing society created a blind spot for this fundamental dimension of care. From the moment palliative care in the Netherlands became part of a national programme, healthcare providers, policymakers, and researchers were presented with the challenge of reassessing this complex concept. National policy and personal initiatives on research and education connected in an inspiring process that led to a Dutch, consensus-based national guideline for multidisciplinary spiritual care as part of palliative care, the adjustment of the professional standard of healthcare chaplaincy in the Netherlands, and the initiative for a European taskforce on spiritual care. In the appendices of this paper the first English-language summary of the Dutch guideline on spiritual care and the European Association for Palliative Care (EAPC) definition of spirituality are presented. Railway traffic experiences disturbances during operations that cause conflicts between train paths or even deadlock situations. Dispatchers need actions to restore feasibility and limit spreading of delays through the network. To help them in such a task, the dispatching support tool ROMA (Railway traffic Optimization by Means of Alternative graphs) has been implemented in a laboratory environment. This paper reports on enhancements to the underlying train dispatching model as well as to the solution algorithms studied in order to tackle the increased complexity of busy stations with multiple conflicting paths and high service frequencies. Advanced train reordering and rerouting techniques are compared with straightforward rules and the current approach in the Netherlands. Extensive computational studies based on accepted statistical distributions of train delays for Utrecht Central Station assess the effectiveness of the ROMA tool in terms of solution quality and computation time. Workplace conflict is a potent stressor, but most previous research has focused on its effect on productivity and performance rather than on individual well-being. This paper examines the moderating roles of an individual's internal locus of control and a problem-solving conflict management strategy. In the cross-sectional study, among 774 health care workers in the Netherlands, employees' internal locus of control did moderate the relationship between experienced conflict at work and psychological strain, which was measured using a 13-item Dutch adaptation of the Occupational Stress Indicator. In addition, this moderation was mediated by the active conflict management strategy of problem solving; people with a more internal locus of control use a problem-solving conflict management strategy more often and, as a result, experience less psychological strain in cases of workplace conflict. Frequent spatial reorganization of administrative units is common in many countries. It may comprise the merging or division of spatial units, or boundary changes between units. These reorganizations prevent the effective assessment of longer-term population dynamics at a detailed spatial level. To deal with this problem in the Netherlands, we developed a new temporal correction method for the populations of municipalities. Rather than estimating the affected population, we used existing data on the number of persons affected by each spatial change. We assumed that before any boundary changes took place, population development was spatially uniform within a municipality. Systematically transferring proportions of the population from original to newly defined target municipalities back in time provided a corrected time series for 1988–2011, based on the 2011 municipal boundaries. Overall, our results correspond well with a detailed reconstruction for 1999–2009 based on data for individual households. This study proposes a risk analysis approach for complex healthcare processes that combines qualitative and quantitative methods to improve patient safety. We combine Healthcare Failure Mode and Effect Analysis with Computer Simulation (HFMEA-CS), to overcome widely recognised HFMEA drawbacks regarding the reproducibility and validity of the outcomes due to human interpretation, and show the application of this methodology in a complex healthcare setting. HFMEA-CS is applied to analyse drug adherence performance in the surgical admission to discharge process of pheochromocytoma patients. The multidisciplinary team identified and scored the failure modes, and the simulation model supported in prioritisation of failure modes, uncovered dependencies between failure modes, and predicted the impact of measures on system behaviour. The results show that drug adherence, defined as the percentage of required drugs received at the right time, can be significantly improved with 12%, to reach a drug adherence of 99%. We conclude that HFMEA-CS is both a viable and effective risk analysis approach, combining strengths of expert opinion and quantitative analysis, for analysing human-system interactions in socio-technical systems. Practitioner summary: We propose combining Healthcare Failure Mode and Effects Analysis with Computer Simulation (HFMEA-CS) for prospective risk analysis of complex and potentially harmful processes, to prevent critical incidents from occurring. Although it is recognised that face-to-face interactions are important for sharing interests and (new) knowledge, it remains unknown how and where students and university employees interact in academic buildings. Therefore, the aim of this study is to analyse the location choice for face-to-face interactions in an academic building, including several personal- and interaction characteristics. An Experience Sampling Method (ESM) was used to collect data on 643 face-to-face interactions during two weeks in the Flux building at Eindhoven University of Technology, the Netherlands. In general, students more often interacted in meeting rooms than teaching staff, and support staff interacted less in eat/drink areas and the hallways than other users. Unexpectedly, some of the lectures took place outside of traditional project-/lecture space. Real estate managers of university campuses could use these results to create better interactive work environments that stimulate face-to-face interactions among employees and students of different departments. Practitioner Summary: Based on longitudinal data of ftf interactions among students and employees in an academic building, results showed that ftf interaction characteristics, compared to personal characteristics, are most important for explaining the location choice of interactions. These insights could help to design academic work environments that optimise the support of interactions. Purpose: It has been argued that social security disability assessments should directly assess claimants’ work capacity, rather than relying on proxies such as on functioning. However, there is little academic discussion of how such assessments could be conducted. Method: The article presents an account of different models of direct disability assessments based on case studies of the Netherlands, Germany, Denmark, Norway, the United States of America, Canada, Australia, and New Zealand, utilising over 150 documents and 40 expert interviews. Results: Three models of direct work disability assessments can be observed: (i) structured assessment, which measures the functional demands of jobs across the national economy and compares these to claimants’ functional capacities; (ii) demonstrated assessment, which looks at claimants’ actual experiences in the labour market and infers a lack of work capacity from the failure of a concerned rehabilitation attempt; and (iii) expert assessment, based on the judgement of skilled professionals. Conclusions: Direct disability assessment within social security is not just theoretically desirable, but can be implemented in practice. We have shown that there are three distinct ways that this can be done, each with different strengths and weaknesses. Further research is needed to clarify the costs, validity/legitimacy, and consequences of these different models. Implications for rehabilitation: It has recently been argued that social security disability assessments should directly assess work capacity rather than simply assessing functioning - but we have no understanding about how this can be done in practice. Based on case studies of nine countries, we show that direct disability assessment can be implemented, and argue that there are three different ways of doing it. These are “demonstrated assessment” (using claimants’ experiences in the labour market), “structured assessment” (matching functional requirements to workplace demands), and “expert assessment” (the judgement of skilled professionals). While it is possible to implement a direct assessment of work capacity for social security benefits, further research is necessary to understand how best to maximise validity, legitimacy, and cost-effectiveness. Various child welfare organizations are changing services by adopting child- and family-centered approaches and moving away from redundant bureaucracy, top-down strategies, and fragmented networks. This shift inevitably poses challenges. This article uses the case of intensive family case management in the Netherlands to explore conflicts perceived by professionals and associated coping strategies. Findings are that internal conflicts (leading to a relapse into old routines or tomisinterpretation of purpose) and boundary conflicts (leading to a relapse into old collaboration agreements) cause challenges. Objective: This study investigated the perspectives of professionals from the Dutch audiological centres on the definition and care pathways of children with suspected auditory processing disorders (susAPD).Design: focus group interviews.Study sample: In total, 45 professionals from 6 disciplines, representing 22 different audiological centres and one ambulatory service, participated in five parallel focus group interviews. Participants had a variety of experience in diagnosing and advising children with suspected APD.Results:Qualitative analysis (open and thematic) identified four themes (“Definition”, “Causes”, “Diagnostic Procedures” and “Clinical Reasoning”) expressing a variety of perspectives. Differences in perspectives were mainly affected by two debates: (1) whether or not APD exists as a pure (auditory) disorder and (2) whether or not current AP-tests are suitable in diagnosing children with listening difficulties. They also expressed a need for more guidance from the literature in their clinical decision making process.Conclusions: Professionals from the Dutch audiological centres share a broad perspective on children with APD. The ICF framework supports this perspective, thereby diminishing the need for a clear definition. The use of AP-tests should be limited to children where broader developmental disorders are first ruled out; a possible “pure” APD could then be diagnosed in a limited number of children. Previous studies in an e-commerce context suggest that there are biological differences (i.e. whether one is a man or a woman) with respect to perceptions of websites. In other research, there is evidence that psychological gender (i.e. values such as masculinity or femininity) likewise influences website perceptions. It is the aim of the current investigation to explore the possibility that both biological sex and psychological gender influence user perceptions, and to examine the predictive power of each on our model. To test these assumptions, we use an augmented technology acceptance model (TAM). To maximise variance in our sample, participants are selected from two countries that are significantly different in masculinity–femininity. As expected, psychological gender better predicted trust and TAM than biological sex. Moreover, this study validates that the masculinity–femininity dimension as originally developed by Hofstede [1980.Culture’s Consequences: International Differences in Work-Related Values. Background: Over the past six decades, the concept of patient-centred care (PCC) has been discussed in health research, policy and practice. However, research on PCC from a patients' perspective is sparse and particularly absent in outpatient psychiatric services. Aim: to gain insight into what patients with bipolar disorder and ADHD consider "good care" and what this implies for the conceptualisation of PCC. Method: A literature review on the different conceptualisations of PCC was complemented with qualitative explorative research on the experiences and needs of adults with ADHD and with bipolar disorder with mental healthcare in the Netherlands using focus group discussions and interviews. Results: The elements addressed in literature are clustered into four dimensions: "patient", "health professional", "patient–professional interaction" and "healthcare organisation". What is considered "good care" by patients coincided with the four dimensions of PCC found in literature and provided refinement of, and preferred emphasis within, the dimensions of PCC. Conclusions: This study shows the value of including patients' perspectives in the conceptualisation of PCC, adding elements, such as "professionals listen without judgment", "professionals (re)act on the fluctuating course of the disorder and changing needs of patients" and "patients are seen as persons with positive sides and strengths". Objectives:In the Netherlands, many community-dwelling people with dementia and behavioral disturbances and their family caregivers receive mental health care from a community psychiatric nurse (CPN). To promote continuity of care for these persons after moving to a nursing home, a transfer intervention was developed. The aim of this explorative study was to evaluate this intervention and its implementation. Method:A qualitative explorative study design was used. CPNs visited professional nursing home carers, people with dementia and family caregivers six weeks after moving, advised on how to manage behavioral problems of their former clients and provided support to family caregivers. Twenty-two interviews were conducted with participants exposed to the intervention (5 CPNs, 5 family and 12 nursing home carers) and with 11 stakeholders (i.e., nursing home and mental health care managers, professional caregivers) to identify facilitators and barriers to the implementation. Data were collected in 2012 and 2013. Results:The follow-up visit at six weeks met the need for background information of new admitted patients and helped family caregivers close off the period prior to the move. It did not meet the original purpose of providing nursing home staff with advice about problem behaviors on time: six weeks after the move was experienced as too late. Conclusion:The transfer intervention increased the awareness of nursing home staff about personal and behavioral characteristics of residents with dementia and supported caregivers in coping with the new situation. Background: Postgraduate medical education training programs strive to prepare their trainees optimally for independent practice. Several studies have shown, however, that new consultants feel inadequately prepared for practice, and that this increases the risk of stress and burnout. Aim: To analyze across specialties for which tasks and themes new consultants feel inadequately prepared. And, to identify themes that need improved attention in postgraduate medical education programs or after registration. Methods: 330 New consultants from all hospital specialities with accredited training programs who completed their training in the north-eastern educational region of The Netherlands between 2004 and 2010 received a questionnaire which was based on a previously validated generic task inventory. Results: 143 respondents (43%) returned the questionnaire. They felt excellently prepared for 40 tasks, well prepared for 25 tasks, marginally sufficiently prepared for 18 tasks and insufficiently prepared for 8 tasks. Preparedness scores were lowest for tasks concerning management administration and leadership, research, end-of-life care, and patient safety-related communication. Surgical specialists felt better prepared for practice than medical specialists, which could not be explained by differences in general self-efficacy. Conclusion: Although new consultants felt well prepared for medical tasks, the scores of more generic tasks indicate that the alignment between the different phases of the medical education continuum and independent practice needs improvement. Using stochastic actor-based models for longitudinal network analysis, this study examines the role of friends' smoking attitudes and behavior for Dutch adolescents' smoking behavior in four secondary schools ( N = 875). The data were collected in two waves in two small suburban towns under second graders in 2008 to 2009 by means of a standardized questionnaire. Stochastic actor-based models for longitudinal network analysis can control for friendship selection while examining the effect of friends' attitudes and smoking behavior on the smoking behavior of a student. The findings suggest that friends tend to select each other on similar smoking behavior. Influence of friends' smoking behavior seems to play no role. In one school, an effect of friends' attitudes towards smoking on the smoking behavior is found. The implications for future research are to consider attitudes when examining the influence of friendship network on smoking behavior. Immigration can help to lessen the burden of ageing for the welfare states of most Western economies. To show this, we develop a decomposition framework for Gross Domestic Product (GDP) per capita which deals with the impact of both ageing and immigration on economic growth. Using a Vector Error Correction Model (VECM) for the Netherlands during 1973 to 2009, we demonstrate the empirical relevance of some crucial interactions between elements of that decomposition. The conclusion is that even temporary immigration may help to alleviate the ageing problem through a positive long-term contribution to employment, wages and GDP per capita, as long as the immigrants are able to participate in the labour force in tandem with the native population. The predominant species in maize in temperate climates is Fusarium graminearum, which produces the mycotoxins deoxynivalenol and zearalenone. Projected climate change is expected to affect Fusarium incidence and thus the occurrence of these mycotoxins. Predictive models may be helpful in determining trends in the levels of these mycotoxins with expected changing climatic conditions. The aim of this study was to develop a model describing fungal infection and subsequent growth as well as the formation of deoxynivalenol and zearalenone in maize in The Netherlands. For this purpose, a published Italian model was used as a starting point. This model is a mixed empiric–mechanistic model that describes fungal infection during silking (based on wind speed and rainfall) and subsequent germination, growth and toxin formation (depending on temperature and water availability). Model input uses weather parameters and crop management factors, such as maize hybrid, sowing date, flowering period and harvest date. Model parameter values were obtained by fitting these parameters to deoxynivalenol and zearalenone measurements in Dutch maize, using national mycotoxin data from the years 2002–2007. The results showed that the adapted model is capable of describing the trend in average deoxynivalenol and zearalenone levels over these years. Validation with external data is needed to verify model outcomes. It is expected that the current model can be used to estimate the effect of projected climate change on trends in deoxynivalenol and zearalenone levels in the coming years. [ABSTRACT FROM PUBLISHER]

Background/Study Context: Occupational activity is associated with cognitive functioning in older age. The mental exercise hypothesis attributes this association to differences in mental exercise at work. Methods: A case-control design was used to test the mental exercise hypothesis. Primary and secondary school teachers (aged between 25.29 and 79.01 years) and non-teacher controls were matched for level of occupation, educational level, age, and gender. Regression analyses were used to analyze the data. Possible confounders (such as precareer intelligence and depressive status) were taken into account. In 2000 disaster struck Enschede in The Netherlands. Due to explosions at a fireworks facility 22 people were killed. This study aims to describe the developments in the media coverage of this disaster from a media amplification perspective. Media amplification was assumed to have taken place in case: (1) events in the aftermath of the disaster resulted in increased media coverage, (2) the media framed the disaster in terms of conflict and responsibility and (3) the framing was in a negative tone-of-voice regarding governmental authorities. Content analysis was performed on 3942 articles, derived from four newspapers. Three peaks in the magnitude of the coverage were distinguished. The content of the coverage during these peaks differed from the period of gradual coverage decline. During the peaks, coverage more often was framed in terms of conflict and responsibility, government was referred to more often and the tone-of-voice on government was less positive. It was concluded that the media coverage of the Enschede fireworks met the conditions of media amplification. Home advantage is a pervasive phenomenon in sport. It has been established in team sports such as basketball, baseball, American football, and European soccer. Attention to home advantage in individual sports has so far been limited. The aim of this study was to examine home advantage in professional tennis. Match-level data are used to measure home advantage. The test used is based on logit models, and consistent specification is addressed explicitly. Depending on the interpretation of home advantage, restrictions on the specification of the model need to be imposed. We find that although significant home advantage exists for men, the performance of women tennis players appears to be unaffected by home advantage. [ABSTRACT FROM AUTHOR]

Following the work of Bon, this article compares the stability of two types of input‐output models. These are, the Leontief (or demand‐driven) model and the Ghosh (or supply‐driven) model. The empirical results are based on a times series of annual input‐output tables for the Netherlands, covering the period 1948–84. Three types of analysis are carried out. The first to evaluate the stability of the models' underlying coefficients over time. The second to investigate the forecasting behaviour when there is perfect foresight with respect to the models' exogenous variables. The article explores the retail planning research for larger regions based on a shopping study carried out by the Department of Geography at the University of Utrecht in the Netherlands. To perform this study, a new research strategy was developed to reduce time and costs by using computer facilities for data collection as well as data handling. The retail planning research is intended to make a forecast of the spatial distribution of retail spending in the future. The application of a shopping model presents a useful device to calculate the effect of different sized centers in different locations.
