While there have been notable advancements in child health in Egypt, disparities in child mortality still exist. Understanding these disparities is crucial to addressing them. The objective of this study is to explore the factors linked to child mortality in Egypt, providing a comprehensive understanding of the disparities in child mortality rates. The study utilises cross-sectional data from Egypt's Demographic and Health Survey (EDHS) in 2014 to examine child mortality. The dataset consists of 15,848 observations from mothers with children born within five years prior to the survey. The choice of explanatory variables was guided by the Mosely and Chen Framework and logistic multivariate regression was used to conduct the analyses. The study finds lower education, early childbearing, insufficient birth spacing, lack of breastfeeding, and absence of improved toilet facilities (proxy for living conditions) were all significantly linked to an increased likelihood of child loss. Additionally, poorer people in rural settings experienced the worst child mortality. The findings align with the World Health Organization's Conceptual Framework for Action on the Social Determinants of Health (CSDH). Pneumonia remains the leading infectious cause of global childhood deaths, despite the availability of pneumococcal conjugate vaccine (PCV) products and widespread evidence of their safety and efficacy. To map the landscape of countries that are yet to fully include PCV in their National Immunization Programs, we conducted an archetype analysis of country indicators related to barriers and facilitators for PCV decision-making. We created a country matrix focused on three key domains – health characteristics, immunisation factors, and policy framework, and identified ten related indicators. We scored countries based on indicator performance and subsequently ranked and grouped them into three archetypes of low-, moderate-, and high-barrier countries with regard to PCV introduction. Our results indicated 39 countries (33 low- and middle-income countries [LMICs] and 6 high-income countries) that are yet to introduce PCV. Among LMICs, 15 countries were classified as 'low-barrier,' indicating factors favourable for PCV introduction such as high immunisation coverage of common childhood vaccines, supportive governments, and substantial disease burden and eligibility for Gavi support. Countries classified in the 'moderate-barrier' (12) and 'high-barrier' (6) archetypes demonstrated adequate capacity in immunisation systems but had competing national priorities and cost barriers that impeded policy decision-making on PCV introduction. The current health and policy indicator-based categorisation provides an actionable framework to design tailored PCV advocacy within these last-mile countries. Policy approaches emerging from this framework can lead to strengthened decision-making on vaccine introduction and sustained vaccine access that can enhance child survival worldwide. Child health outcomes vary between Parliamentary Constituencies (PCs) in India. There are a total of 543 PCs in India, each of which is a geographical unit represented by a Member of Parliament (MP). MP characteristics, such as age, gender, education, the number of terms they have served, and whether they belong to a Scheduled Caste or Scheduled Tribe, might be associated with indicators of child malnutrition and child mortality. The purpose of this paper was to examine the associations between MP characteristics and measures of child malnutrition and mortality. We did not find any meaningful associations between MP characteristics and child anthropometry, anemia, and mortality. Future research should consider the size of a constituency served by an MP along with MP party affiliations as these factors might help explain between-PC variations in child health outcomes. Essential health, education and other service disruptions arising from the COVID-19 pandemic risk reversing some of the hard-won gains in improving child survival over the past 40 years. Although children have milder symptoms of COVID-19 disease than adults, pandemic control measures in many countries have disrupted health, education and other services for children, often leaving them without access to birth and postnatal care, vaccinations and early childhood preventive and treatment services. These disruptions mean that the SARS-CoV-2 virus, along with climate change and shifting epidemiological and demographic patterns, are challenging the survival gains that we have seen over the past 40 years. We revisit the initiatives and actions of the past that catalyzed survival improvements in an effort to learn how to maintain these gains even in the face of today's global challenges. Reducing child mortality is a key global health challenge. We examined reasons for greater or lesser success in meeting under-five mortality rate reductions, i.e. Millennium Development Goal #4, between 1990 and 2015 in Sub-Saharan Africa where child mortality remains high. We first examined factors associated with child mortality from all World Health Organization African Region nations during the Millennium Development Goal period. This analysis was followed by case studies of the facilitators and barriers to Millennium Development Goal #4 in four countries – Kenya, Liberia, Zambia, and Zimbabwe. Quantitative indicators, policy documents, and qualitative interviews and focus groups were collected from each country to examine factors within and across countries related to child mortality. We found familiar themes that highlighted the need for both specific services (e.g. primary care access, emergency obstetric and neonatal care) and general management (e.g. strong health governance and leadership, increasing community health workers, quality of care). We also identified methodological opportunities and challenges to assessing progress in child health, which can provide insights to similar efforts during the Sustainable Development Goal period. Specifically, it is important for countries to adapt general international goals and measurements to their national context, considering baseline mortality rates and health information systems, to develop country-specific goals. It will also be critical to develop more rigorous measurement tools and indicators to accurately characterize maternal, neonatal, and child health systems, particularly in the area of governance and leadership. Valuable lessons can be learned from Millennium Development Goal successes and failures, as well as how they are evaluated. As countries seek to lower child mortality further during the Sustainable Development Goal period, it will be necessary to prioritize and support countries in quantitative and qualitative data collection to assess and contextualize progress, identifying areas needing improvement. Background: Several evaluative studies demonstrate that a well-coordinated Integrated Management of Childhood Illnesses (IMCI) program can reduce child mortality. However, there is dearth of information on how frontline providers perceive IMCI and how, in their view, the program is implemented and how it could be refined and revitalized. Purpose: To determine the key challenges affecting IMCI implementation from the perspective of health care workers (HCWs) in primary health care facilities. Methods: A scoping review based on the five-step framework of Arskey and O'Malley was utilized to identify key challenges faced by HCWs implementing the IMCI program in primary health care facilities. A comprehensive search of peer-reviewed literature through PubMed, ScienceDirect, EBSCOhost and Google Scholar was conducted. A total of 1,475 publications were screened for eligibility and 41 publications identified for full-text evaluation. Twenty-four (24) published articles met our inclusion criteria, and were investigated to tease out common themes related to challenges of HCWs in terms of implementing the IMCI program. Results: Four key challenges emerged from our analysis: 1) Insufficient financial resources to fund program activities, 2) Lack of training, mentoring and supervision from the tertiary level, 3) Length of time required for effective and meaningful IMCI consultations conflicts with competing demands and 4) Lack of planning and coordination between policy makers and implementers resulting in ambiguity of roles and accountability. Although the IMCI program can provide substantial benefits, more information is still needed regarding implementation processes and acceptability in primary health care settings. Conclusion: Recognizing and understanding insights of those enacting health programs such as IMCI can spark meaningful strategic recommendations to improve IMCI program effectiveness. Based on the competing theories regarding the relationship between family structure and child health outcomes, this article examined the effects of polygynous family system (PFS) on under-five mortality (U5M) across different socio-economic and neighbourhood contexts in selected sub-Saharan African countries. Cox proportional regression analysis was performed on pooled data of children (n = 54 842) born in the five years before the Demographic and Health Surveys of selected countries. Results indicated differential effects of PFS on U5M across varying contexts, because risks of U5M were significantly higher for children of polygynous mothers in poor communities (hazard ratio: 2.98, 95% confidence interval: 2.23 to 3.95, p < 0.001) and children of monogamists in poor communities (hazard ratio: 2.24, 95% confidence interval: 1.69 to 2.98, p < 0.001) compared with the children of monogamists in rich communities. Given the worsening effects of polygyny on childhood survival across different contexts, this study stressed the need for marriage reforms and enforcement of a monogamous family system if significant U5M reduction would be achieved in sub-Saharan African countries during the post-2015 development era. Household environmental health hazards or simply household health hazards (HHH) are pathogens and chemicals in the household that can cause health problems. In this study, we assess their effect on under-five mortality (U5MR) in 12 sub-Saharan African countries, using data from the Demographic and Health Surveys. Referring to the principal component analysis approach, we measure the HHH by the following indicators: source of water and its location, type of toilet facility, flooring material, type of wall, type of roof and type of cooking fuel. In an unadjusted multilevel discrete-time hazard model, we find that HHH affect positively child mortality in 9 of the 12 countries, whereas this effect presented itself only in 4 countries when controlling for other covariates. However, using a model with interaction between the child’s age and HHH, we find it interesting that increasing levels of the HHH are consistently associated with increasing risk of death during 24–59 months after birth in eight countries. Future researches are needed to decipher the mechanisms behind these findings, whether explained by the accumulation of hazardous environment in early childhood, or frequent contact with noxious environments at a later stage of childhood, or both. In Nigeria, childhood mortality is very high, with almost a million children dying each year. A fifth of these die from vaccine-preventable diseases. The role of immunisation in preventing childhood morbidity and mortality is widely recognised. Unfortunately little, if any, account has been given of the law’s role in immunisation in legal discourse in Nigeria. Yet recent steps have been taken at both federal and state levels to compel immunisation using the instrument of legislation. For instance, the Child Rights Act, a domestication of the Convention of the Rights of the Child, passed in 2003 includes provisions compelling immunisation. Also, several states in Nigeria like Jigawa, Niger and Katsina States, have recently enacted legislation that makes childhood immunisation compulsory under the law. These pieces of legislation criminalise refusal to immunise children and also public statements that discourage immunisation. I identify the challenges that limit or obstruct uptake of immunisation in Nigeria and ask what role the law can play in providing solutions to these challenges. I examine the ways in which the law has been engaged to promote immunisation, the impact of law and the limits of legislative intervention. Introduction: The objective of this research was to spatially analyze linked health data for geographic trends in factors impacting children's health. Traditional linear regression analyses of countylevel data tend to inflate R2. Spatial regression represents a robust approach for improved analysis of geographic data. Methods: We used GeoDa 1.6.0 to regress 3,221 U.S. county-level child health outcomes (e.g., infant mortality, child mortality) on independent variables (e.g., low birth weight, percent race/ethnicity, uninsured, emotional support). Statistical analyses included spatial R2, Moran's I, and multicollinearity measures. The data source was the 2014 County Health Rankings. Results: Three spatial regression models (health, socioeconomic, and combined) were compared for infant and child mortality. The combined model for infant mortality rate yielded the largest adjusted R2 = 0.428 (F = 110.9, p < 0.001), similarly for child mortality rate R2 = 0.411 (F =94.3, p < 0.001). The strongest predictors in bothmodels were obesity, smoking, teen birth rate, severe housing problems, no social supports, and urbanicity. Discussion: The results demonstrate correlations between county-level conditions and child health outcomes, supporting previous research linking poor health/education and low socioeconomic conditions. Introduction: Health is multidimensional and affected by a wide range of factors, many of which are outside the health sector. To improve population health and reduce health inequality, it is important that we take into account the complex interactions among social, environmental, behavioural, and biological factors and design our health interventions accordingly. Objectives: This study examines mortality differentials in children of different age groups by key social determinants of health (SDH) including parental education and employment, mother's level of autonomy, age, asset index, living arrangements (utilities), and other geographical contextual factors (area of residence, road conditions). Design: We used data from the two rounds of Bangladesh Health and Demographic Survey, a nationally representative sample survey of the population residing in Bangladesh. Multilevel logistic models were used to study the impact of SDH on child mortality. Results: The study found that the mother's age, the education of both parents, the mother's autonomy to take decisions about matters linked to the health of her child, the household socio-economic conditions, the geographical region of residence, and the condition of the roads were significantly associated with higher risks of neonatal, infant, and under-five mortality in Bangladesh. Conclusion: The study findings suggest there are complex relationships among different SDH. Thus larger intersectoral actions will be needed to reduce disparities in child health and mortality and achieve meaningful progress towards equity-oriented universal health coverage. Background: Under-five mortality has been a major public health challenge from time immemorial. In response to this challenge, the World Health Organization and the United Nations Children's Fund developed the Integrated Management of Childhood Illnesses (IMCI) strategy and presented it to the whole world as a key approach to reduce child morbidity and mortality. Botswana started to implement the IMCI strategy in 1998. Reductions in the under-five mortality rate (U5MR) have been documented, although the reduction is not on par with the expected Millennium Development Goal 4 predictions. Design: A quantitative study was done to identify the problems IMCI implementers face when tending children under 5 years in the Gaborone Health District of Botswana. The study population was made up of all the IMCI-trained and registered nurses, and systematic sampling was used to randomly select study participants. Questionnaires were used to collect data. Results: The study findings indicated challenges related to low training coverage, health systems, and the unique features of the IMCI strategy. The world has made substantial progress in reducing maternal and child mortality, but many countries are projected to fall short of achieving their Millennium Development Goals (MDGs) 4 and 5 targets. The major objective of this paper is to examine progress in Pakistan in reducing maternal and child mortality and malnutrition over the last two decades. Data from recent national and international surveys suggest that Pakistan lags behind on all of its MDGs related to maternal and child health and, for some indicators especially related to nutrition, the situation has worsened from the baseline of 1990. Progress in addressing key social determinants such as poverty, female education and empowerment has also been slow and unregulated population growth has further compromised progress. There is a need to integrate the various different sectors and programmes to achieve the desired results effectively and efficiently as many of the determinants and influencing factors are outside the health sector. All the 11 members of the South-East Asia Region (SEAR) of the World Health Organization are categorised as low- and middle-income countries. This region has over a quarter of the world's total population but comprises about one-third of the world's unintentional injury-related deaths. There is a paucity of good-quality mortality and morbidity data from most of these countries. This is the first systematic review of community-based surveys on child injuries that summarises evidence from child injury studies from the SEAR countries. The included papers reported varying estimates of overall non-fatal unintentional injury rates across the countries, from 15/1000 children in Thailand to as high as 342/1000 children in India. The fatal injury rates were also found to be varying. This review revealed a need for strengthening child injury research using standard methodologies across the region and for promoting the dissemination of the results. [ABSTRACT FROM PUBLISHER]

Objectives: This paper examines social inequality in children's health following the election of Evo Morales. Bolivia is marked by dramatic socioeconomic, ethnic and geographic inequality in children's health. The Morales government initiated several public health reforms designed to provide more equal access to public health services. Methods: Analysis was conducted using Bolivia Demographic and Health Survey data from 2003 to 2008. Results: The analysis shows some improvements in children's health. However, inequality in access to doctors, child mortality and child nutritional status did not diminish. In contrast, inequalities in immunization declined dramatically. Conclusions: Results suggest that aspects of health that are easier to implement such as immunization are more accessible to public policy than are more complex health problems like malnutrition and high child mortality. Child and infant mortality in developing countries decreased 33.5 per cent and 30.9 per cent, respectively, during 1995–2009, while remittances and public health spending more than doubled. I examine how remittances and government health spending improve these child health outcomes. Neither government health spending nor remittances causally affect household health spending. Public health spending has an insignificant negative impact on mortality. The increase in remittances causally accounts for 32 per cent and 37 per cent of the decline in child and infant mortality, respectively. In June of 2012, representatives from more than 80 countries promulgated a Child Survival Call to Action, which called for reducing child mortality to 20 or fewer child deaths per 1,000 live births in every country by 2035. To address the problem of ending preventable child deaths, the U.S. Agency for International Development and the United Nations Children's Fund convened, on June 3–4, 2013, an Evidence Summit on Enhancing Child Survival and Development in Lower- and Middle-Income Countries by Achieving Population-Level Behavior Change. Six evidence review teams were established on different topics related to child survival and healthy development to identify the relevant evidence-based interventions and to prepare reports. This article was developed by the evidence review team responsible for identifying the research literature on caregiver change for child survival and development. This article is organized into childhood developmental periods and cross-cutting issues that affect child survival and healthy early development across all these periods. On the basis of this review, the authors present evidence-based recommendations for programs focused on caregivers to increase child survival and promote healthy development. Evidence-based behavior change interventions addressing health systems must be identified and disseminated to improve child health outcomes. Studies of the efficacy of such interventions were identified from systematic searches of the published literature. Two hundred twenty-nine of the initially identified references were judged to be relevant and were further reviewed for the quality and strength of the evidence. Studies were eligible if an intervention addressed policy or health systems interventions, measured relevant behavioral or health outcomes (e.g., nutrition, childhood immunization, malaria prevention and treatment), used at least a moderate quality research design, and were implemented in low- or middle-income countries. Policy or systems interventions able to produce behavior change reviewed included media (e.g., mass media, social media), community mobilization, educational programs (for caregivers, communities, or providers), social marketing, opinion leadership, economic incentives (for both caregiver and provider), health systems strengthening/policy/legislation, and others. Recommendations for policy, practice, and research are given based on fairly strong data across the areas of health service delivery, health workforce, health financing, governance and leadership, and research. As part of a broader evidence summit, USAID and UNICEF convened a literature review of effective means to empower communities to achieve behavioral and social changes to accelerate reductions in under-5 mortality and optimize early child development. The authors conducted a systematic review of the effectiveness of community mobilization and participation that led to behavioral change and one or more of the following: child health, survival, and development. The level and nature of community engagement was categorized using two internationally recognized models and only studies where the methods of community participation could be categorized as collaborative or shared leadership were eligible for analysis. The authors identified 34 documents from 18 countries that met the eligibility criteria. Studies with shared leadership typically used a comprehensive community action cycle, whereas studies characterized as collaborative showed clear emphasis on collective action but did not undergo an initial process of community dialogue. The review concluded that programs working collaboratively or achieving shared leadership with a community can lead to behavior change and cost-effective sustained transformation to improve critical health behaviors and reduce poor health outcomes in low- and middle-income countries. Given the high morbidity and mortality among children in low- and middle-income countries as a result of preventable causes, the U.S. government and the United Nations Children's Fund convened an Evidence Summit on Enhancing Child Survival and Development in Lower- and Middle-Income Countries by Achieving Population-Level Behavior Change on June 3–4, 2013, in Washington, D.C. This article summarizes evidence for technological advances associated with population-level behavior changes necessary to advance child survival and healthy development in children under 5 years of age in low- and middle-income countries. After a rigorous evidence selection process, the authors assessed science, technology, and innovation papers that used mHealth, social/transmedia, multiplatform media, health literacy, and devices for behavior changes supporting child survival and development. Because of an insufficient number of studies on health literacy and devices that supported causal attribution of interventions to outcomes, the review focused on mHealth, social/transmedia, and multiplatform media. Overall, this review found that some mHealth interventions have sufficient evidence to make topic-specific recommendations for broader implementation, scaling, and next research steps (e.g., adherence to HIV/AIDS antiretroviral therapy, uptake and demand of maternal health service, and compliance with malaria treatment guidelines). While some media evidence demonstrates effectiveness in changing cognitive abilities, knowledge, and attitudes, evidence is minimal on behavioral endpoints linked to child survival. Population level behavior change is necessary to end preventable child deaths. Donors and low- and middle-income countries are encouraged to implement recommendations for informing practice, policy, and research decisions to fully maximize the impact potential of mHealth and multimedia for child survival and development. Through a systematic review of the literature, this article summarizes and evaluates evidence for the effectiveness of mass media interventions for child survival. To be included, studies had to describe a mass media intervention; address a child survival health topic; present quantitative data from a low- or middle-income country; use an evaluation design that compared outcomes using pre- and postintervention data, treatment versus comparison groups, or postintervention data across levels of exposure; and report a behavioral or health outcome. The 111 campaign evaluations that met the inclusion criteria included 15 diarrheal disease, 8 immunization, 2 malaria, 14 nutrition, 1 preventing mother-to-child transmission of HIV, 4 respiratory disease, and 67 reproductive health interventions. These evaluations were then sorted into weak (n =33), moderate (n = 32), and stronger evaluations (n = 46) on the basis of the sampling method, the evaluation design, and efforts to address threats to inference of mass media effects. The moderate and stronger evaluations provide evidence that mass media-centric campaigns can positively impact a wide range of child survival health behaviors. [ABSTRACT FROM PUBLISHER]

Despite almost a quarter of a century during which the global community pursued the goal of child survival, together with targets for improving child health and nutrition, Millennium Development Goal 4 (MDG4) had but one target: the reduction of child mortality by two-thirds between 1990 and 2015. MDG4 was untethered from the Millennium Declaration and from the Convention on the Rights of the Child. This paper analyzes the unintended consequences of framing MDG4 in this reductive manner, showing that doing so shrunk the child health agenda, and ignored earlier incipient efforts to embed human rights principles in the pursuit of child survival. The paper also analyzes the evidence of whether the remarkable 47% decline in child mortality since 1990 was a consequence of the mobilizing Power of MDG4's numbers. Change seems due to veterans of the child-survival revolution of the late twentieth century coalescing to overcome MDG4's limitations. By 2010, the coalition began to address the distortions that flowed from disconnecting MDG4 from a human rights framework. Using data from the Indian National Family Health Surveys (1992–93, 1998–99, 2005–06), this study examined how the relationship between household wealth and child health evolved during a time of significant economic change in India. The main predictor was an innovative measure of household wealth that captures changes in wealth over time. Discrete-time logistic models (with community fixed effects) were used to examine mortality and malnutrition outcomes: infant, child, and under-5 mortality; stunting, wasting, and being underweight. Analysis was conducted at the national, urban/rural, and regional levels, separately for boys and girls. The results indicate that the relationship between household wealth and under-5 mortality weakened over time but this result was dominated by infant mortality. The relationship between wealth and child mortality stayed strong for girls. The relationship between household wealth and malnutrition became stronger over time for boys and particularly for girls, in urban and (especially) rural areas. [ABSTRACT FROM PUBLISHER]

The Millennium Development Goals (MDGs) are eight international development goals to be achieved by 2015 addressing poverty, hunger, maternal and child mortality, communicable disease, education, gender inequality, environmental damage and the global partnership. Most activities worldwide have focused on maternal and child health and communicable diseases, while less attention has been paid to environmental sustainability and the development of a global partnership. Up to now, several targets have been at least partially achieved: hunger reduction is on track, poverty has been reduced by half, living conditions of 200 million deprived people enhanced, maternal and child mortality as well as communicable diseases diminished and education improved. Nevertheless, some goals will not be met, particularly in the poorest regions, due to different challenges (e.g. the lack of synergies among the goals, the economic crisis, etc.). The post-2015 agenda is now under discussion. The new targets, whatever they will be called, should reflect today's political situation, health and environmental challenges, and an all-inclusive, intersectoral and accountable approach should be adopted. Annually, 1,000–2,000 child welfare workers experience the death of a child client. The child welfare field has paid increasing attention to the impact of workplace trauma events on the psychological distress of child welfare workers. In this article the author assesses the relationship between experiencing a maltreatment fatality and workers' posttraumatic stress among a multi-state sample of 385 child welfare workers. Results indicate that a maltreatment fatality is not associated with higher rates of posttraumatic stress. Among child welfare workers who experienced a fatality, those who feel greater culpability for the death report higher levels of posttraumatic stress. The Millennium Development Goal 4 (MDG4) calls for the reduction in under-five mortality by two-thirds between 1990 and 2015. Only 16 of the 68 countries with 97% of the world's child deaths are on track to achieve this. This paper reviews the current evidence regarding proven interventions for reducing child mortality in high-mortality, resource-poor settings. All of these interventions require implementation within communities rather than implementation confined to only those attending health facilities with strong community participation in order to be effective. The evidence strongly suggests that facility-based interventions require a strong community-based component in order to improve child mortality in the surrounding population. We provide specific information about common community-based approaches used in the implementation of interventions with documented improvements in child mortality. A stronger emphasis on community-based approaches will be needed in order to accelerate progress in reaching MDG4. The Millennium Development Goals are a set of eight goals drafted by the United Nations in 2000 with the aim of improving the health and welfare of people worldwide. The goals provide specific targets to be met by 2015, using the 1990 basis as a standard. This review presents these goals as they relate to children, discussing progress and future aims. Although not all eight goals specifically address children, each has its own impact on global child health. Thus far, much progress has been made, but increased rates of improvement must be achieved in order to meet the goals by 2015 and improve the health of children worldwide. [ABSTRACT FROM AUTHOR]

Background: In 2007, the Intergovernmental Panel on Climate Change (IPCC) presented a report on global warming and the impact of human activities on global warming. Later the Lancet commission identified six ways human health could be affected. Among these were not environmental factors which are also believed to be important for human health. In this paper we therefore focus on environmental factors, climate change and the predicted effects on maternal and newborn health. Arctic issues are discussed specifically considering their exposure and sensitivity to long range transported contaminants. Methods: Considering that the different parts of pregnancy are particularly sensitive time periods for the effects of environmental exposure, this review focuses on the impacts on maternal and newborn health. Environmental stressors known to affects human health and how these will change with the predicted climate change are addressed. Air pollution and food security are crucial issues for the pregnant population in a changing climate, especially indoor climate and food security in Arctic areas. Results: The total number of environmental factors is today responsible for a large number of the global deaths, especially in young children. Climate change will most likely lead to an increase in this number. Exposure to the different environmental stressors especially air pollution will in most parts of the world increase with climate change, even though some areas might face lower exposure. Populations at risk today are believed to be most heavily affected. As for the persistent organic pollutants a warming climate leads to a remobilisation and a possible increase in food chain exposure in the Arctic and thus increased risk for Arctic populations. This is especially the case for mercury. The perspective for the next generations will be closely connected to the expected temperature changes; changes in housing conditions; changes in exposure patterns; predicted increased exposure to Mercury because of increased emissions and increased biological availability. Conclusions: A number of environmental stressors are predicted to increase with climate change and increasingly affecting human health. Drawing from various bodies of social scientific literature and research, the authors assess the extent to which infant and child mortality rates in less-developed countries are affected by the percent of domestic populations living in urban slum conditions. Results of first-difference panel model estimates of 80 less-developed countries from 1990 to 2005 indicate that growth in the percent of populations living in urban slum conditions positively affects both forms of mortality, and the effects are much more pronounced for African countries than for less-developed countries in Latin America and Asia. These findings hold, net of economic development, fertility rates, world-economic integration, and other factors. Cross-sectional analyses of infant and child mortality rates in 2005 that include additional controls provide further evidence of the mortality/urban slum relationships found in the panel model estimates. Most microlevel studies in the social sciences have focused on the impact of different measured variables. While some studies have also dealt with unobserved variation, it has usually only been controlled for to perfect the estimates of the observables. In this article, the authors applied a modified version of a recently developed method designed to quantify the effect of unobserved variation in continuous time multilevel models, called a median hazard ratio. It allows a direct comparison of the effect of unobserved heterogeneity with standard relative risks. The method is used in an analysis of infant and child mortality in southern Sweden during the period 1766–1895. The article discusses the impact of environmental degradation from the cotton monoculture industry to high rates of child mortality in Central Asia. It is said that exposure to environmental externalities contributes to high rates of child mortality. Children are adversely affected by unsafe and unhealthy environmental conditions including air pollution, water, sanitation and chemical contamination which cause child mortality and morbidity. Environmental variables linked to child mortality are access to water and sanitation, due to ease of measurement. Agricultural activities are never linked to health of the local environment and mortality where long-term exposure to chemical and water pollution is abundant.

Barbados was the only British sugar colony in the Caribbean to have a self-reproducing slave society by the time the slave trade was abolished in 1807. However, the estimated infant mortality rate was also among the highest in the region. High rates of infant and child mortality were particularly critical in times of food shortage and economic distress. The children of the labouring population occupied an ambiguous position in Barbadian society when their status changed during the transition from slavery to apprenticeship. This had direct consequences on their health. This paper explores the origins of some of these issues and links them to the wider socio-economic tensions in the colony in the early nineteenth century. The paper also introduces some challenges to studying children in former slave societies as well as investigates their shifting social, political and economic roles in early nineteenth century Barbados. This paper reviews experience since governments first began, through the United Nations, setting time-bound quantitative goals to serve as guidelines and benchmarks for national and international action and development assistance. It argues that, contrary to much opinion, many of these goals have had a major influence on subsequent action and many have been largely or considerably achieved. It discusses approaches to implementation adopted in the United Nations Development Decades as well as by the World Health Organization and the United Nations Children's Fund (UNICEF), and the Bretton Woods' structural adjustment programmes. It underlines the need for a more nuanced and critical approach to what is meant by goal achievement, drawing on the experience of the Water Decade and the child survival revolution. It examines the ways in which global goals were costed, and draws lessons for the pursuit of the Millennium Development Goals. Child survival from ages one to five in Matlab, Bangladesh is related to the length of the birth-to-subsequent conception interval (BCI), family composition, health and family planning interventions, and socioeconomic conditions. The BCI relationship consists of short-term effects (related to the mother's pregnancy or the presence of a very young sibling) and long-term effects (associated with having a 6-18 month old younger sibling). Their impact is apparent at longer intervals than previously thought. Girls with at least one sister are especially vulnerable and all girls have greater risks when the next child comes along. We conclude that the health and family planning programme improves life chances of children, especially girls, directly through provision of care, and indirectly through prevention of unwanted births and longer child spacing. The differential allocation of resources to children, indicated by the strong relationship of child survival to the composition of the older sibling set, may indicate previously unrecognized receptivity to family planning. Background: Social, cultural, and behavioral factors are often potent upstream contributors to maternal, neonatal, and child mortality, especially in low- and middle-income countries (LMICs). Social autopsy is one method of identifying the impact of such factors, yet it is unclear how social autopsy methods are being used in LMICs. Objective:This study aimed to identify the most common social autopsy instruments, describe overarching findings across populations and geography, and identify gaps in the existing social autopsy literature. Methods: A systematic search of the peer-reviewed literature from 2005 to 2016 was conducted. Studies were included if they were conducted in an LMIC, focused on maternal/neonatal/infant/child health, reported on the results of original research, and explicitly mentioned the use of a social autopsy tool. Results: Sixteen articles out of 1950 citations were included, representing research conducted in 11 countries. Five different tools were described, with two primary conceptual frameworks used to guide analysis: Pathway to Survival and Three Delays models. Studies varied in methods for identifying deaths, and recall periods for respondents ranged from 6 weeks to 5+ years. Across studies, recognition of danger signs appeared to be high, while subsequent care-seeking was inconsistent. Cost, distance to facility, and transportation issues were frequently cited barriers to care-seeking, however, additional barriers were reported that varied by location. Gaps in the social autopsy literature include the lack of: harmonized tools and analytical methods that allow for cross-study comparisons, discussion of complexity of decision making for care seeking, qualitative narratives that address inconsistencies in responses, and the explicit inclusion of perspectives from husbands and fathers. Conclusion: Despite the nascence of the field, research across 11 countries has included social autopsy methods, using a variety of tools, sampling methods, and analytical frameworks to determine how social factors impact maternal, neonatal, and child health outcomes. Background: Reducing child mortality was one of the Millennium Development Goals. In the current Sustainable Development Goals era, achieving equity is prioritized as a major aim. Objective: This study aims to provide a comprehensive and updated picture of inequalities in child health intervention coverage and child health outcomes by wealth status, as well as their trends between 2000 and 2014. Methods: Using data from Demographic Health Surveys and Multiple Indicator Cluster Surveys, we adopted three measures of inequality, including one absolute inequality indicator and two relative inequality indicators, to estimate the level and trends of inequalities in three child health outcome variables and 17 intervention coverages in 88 developing countries. Results: While improvements in child health outcomes and coverage of interventions have been observed between 2000 and 2014, large inequalities remain. There was a high level of variation between countries’ progress toward reducing child health inequalities, with some countries significantly improving, some deteriorating, and some remaining statistically unchanged. Among child health interventions, the least equitable one was access to improved sanitation (The absolute difference in coverages between the richest quintile and the poorest quintile reached 49.5% [42.7, 56.2]), followed by access to improved water (34.1% [29.5, 38.6]), and skilled birth attendant (SBA) (34.1% [28.8, 39.4]). The most equitable intervention coverage was insecticide-treated bed net for children (1.0% [−3.9, 5.9]), followed by oral rehydration therapy for diarrhea ((8.0% [5.2, 10.8]), and vitamin A supplement (8.4% [5.1, 11.7]). These findings were robust to various inequality measurements. Conclusions: Although child health outcomes and coverage of interventions have improved largely over the study period for almost all wealth quintiles, insufficient progress was made in reducing child health inequalities between the poorest and richest wealth quintiles.
