Here we report the results of a retrospective germline analysis of 6941 individuals fulfilling the criteria necessary for genetic testing of hereditary breast- and ovarian cancer (HBOC) according to the German S3 or AGO Guidelines. Genetic testing was performed by next-generation sequencing using 123 cancer-associated genes based on the Illumina TruSight® Cancer Sequencing Panel. In 1431 of 6941 cases (20.6%) at least one variant was reported (ACMG/AMP classes 3–5). Of those 56.3% (n = 806) were class 4 or 5 and 43.7% (n = 625) were a class 3 (VUS). We defined a 14 gene HBOC core gene panel and compared this to a national and different internationally recommended gene panels (German Hereditary Breast and Ovarian Cancer Consortium HBOC Consortium, ClinGen expert Panel, Genomics England PanelsApp) in regard of diagnostic yield, revealing a diagnostic range of pathogenic variants (class 4/5) from 7.8 to 11.6% depending on the panel evaluated. With the 14 HBOC core gene panel having a diagnostic yield of pathogenic variants (class 4/5) of 10.8%. Additionally, 66 (1%) pathogenic variants (ACMG/AMP class 4 or 5) were found in genes outside the 14 HBOC core gene set (secondary findings) that would have been missed with the restriction to the analysis of HBOC genes. Furthermore, we evaluated a workflow for a periodic re-evaluation of variants of uncertain clinical significance (VUS) for the improvement of clinical validity of germline genetic testing.

Das kolorektale Karzinom (KRK) stellt in Deutschland die dritthaufigste Krebsart in der Gesamtbevolkerung dar. Bei etwa 5 % der Patienten mit einem KRK besteht ein gut definiertes erbliches Tumorsyndrom (hereditarer Darmkrebs). Es handelt sich um eine selektive Literaturrecherche und eigene Arbeiten zum Thema erblicher Darmkrebs und gastrointestinale Polyposis. Zu den hereditaren Darmkrebs-Syndromen gehoren insbesondere das Lynch-Syndrom/hereditare nichtpolypose Kolonkarzinom (HNPCC), die familiare adenomatose Polyposis (FAP), die MUTYH-assoziierte Polyposis (MAP) und verschiedene hamartomatose Polyposissyndrome (Peutz-Jeghers-Syndrom, juvenile Polyposis, Cowden-Syndrom). Ursachlich sind Keimbahnmutationen in verschiedenen Genen, die meist mit einem autosomal-dominanten Erbgang und einer auffalligen individuellen und/oder familiaren Tumoranamnese hinsichtlich Darmkrebs und anderer Karzinome einhergehen. Die Ursache des serratierten Polyposissyndroms (SPS) ist noch weitgehend unbekannt. Der Nachweis einer ursachlichen Keimbahnmutation bei einem erkrankten Familienmitglied und die Identifikation von weiteren Anlagetragern der Familie sind entscheidend, da Betroffenen eine an das jeweilige Tumorrisiko angepasste lebenslange Uberwachungsstrategie angeboten wird, die neben engmaschigen Vorsorgekoloskopien auch andere Fruherkennungsmasnahmen umfasst. Bei einigen hereditaren Syndromen kann eine prophylaktische Operation das Tumorrisiko deutlich senken.

Zusammenfassung Hintergrund Monogen erbliche Tumorsyndrome bzw. Tumordispositionssyndrome (TDS) beruhen auf Keimbahn-/konstitutionellen Mutationen in Schlüsselgenen der Karzinogenese. Ein frühes Erkrankungsalter und eine Häufung von Tumoren eines typischen Spektrums in der Eigen- und/oder Familienanamnese sind Hinweise für das Vorliegen einer hereditären Form. Insbesondere seltene spezifische Tumoren treten relativ häufig im Rahmen eines TDS auf. Methode Der vorliegende Artikel stellt basierend auf einer Literaturrecherche dar, welche TDS bei Vorliegen eines seltenen Tumors als Differentialdiagnose (DD) bedacht werden sollten. Ergebnisse Die Identifizierung einer ursächlichen Keimbahnmutation bei einer erkrankten Person der Familie dient der DD, Ermittlung des Wiederholungsrisikos und der prädiktiven Testung asymptomatischer Risikopersonen. Bei TDS mit autosomal-dominantem Erbgang lassen sich oft mehrere Hochrisikopersonen in den betroffenen Familien identifizieren. Schlussfolgerung Die frühe Erkennung und korrekte Einordnung ist von hoher klinischer Relevanz, da den Erkrankten und Risikopersonen häufig effektive präventive Maßnahmen (Früherkennungs-Untersuchungen, prophylaktische Operationen) angeboten werden können und z. T. spezielle therapeutische Optionen bestehen. TDS stehen paradigmatisch für ein äußerst erfolgreiches Konzept der präventiven Onkologie und personalisierten Medizin. Die Einführung neuer Methoden der Hochdurchsatz-Sequenzierung (Next-Generation-Sequencing) ermöglicht eine effektivere genetische Diagnostik, stellt aber auch eine Herausforderung für die Befundinterpretation und Beratung dar. Für die Betreuung der Familien ist die Anbindung an multidisziplinäre Expertenzentren sinnvoll.

Routine diagnostics for colorectal cancer patients suspected of having Lynch-Syndrome (LS) currently uses Next-Generation-Sequencing (NGS) of targeted regions within the DNA mismatch repair (MMR) genes. This analysis can reliably detect nucleotide alterations and copy-number variations (CNVs); however, CNV-neutral rearrangements comprising gene inversions or large intronic insertions remain undetected because their breakpoints are usually not covered. As several founder mutations exist for LS, we established PCR-based screening methods for five known rearrangements in MLH1, MSH2, or PMS2, and investigated their prevalence in 98 German patients with suspicion of LS without a causative germline variant or CNV detectable in the four MMR genes. We found no recurrence of CNV-neutral structural rearrangements previously described: Neither for two inversions in MLH1 (exon 1 and exon 16-19) within 33 MLH1-deficient patients, nor for two inversions in MSH2 (exon 1-7 and exon 2-6) within 48 MSH2-deficient patients. The PMS2 insertion in intron 7 was detected in one of 17 PMS2-deficient patients. None of the four genomic inversions constitutes a founder event within the German population, but we advise to test the rare cases with unsolved PMS2-deficiency upon the known insertion. As a next diagnostic step, tumour tissue of the unsolved patients should be sequenced for somatic variants, and germline analysis of additional genes with an overlapping clinical phenotype should be considered. Alternatively, full-length cDNA analyses may detect concealed MMR-defects in cases with family history.

In pathogenicity assessment, RNA-based analyses are important for the correct classification of variants, and require gene-specific cut-offs for allelic representation and alternative/aberrant splicing. Beside this, the diagnostic yield of RNA-based techniques capable to detect aberrant splicing or allelic loss due to intronic/regulatory variants has to be elaborated. We established a cDNA analysis for full-length transcripts (FLT) of the four DNA mismatch repair (MMR) genes to investigate the splicing pattern and transcript integrity with active/inhibited nonsense-mediated mRNA-decay (NMD). Validation was based on results from normal controls, samples with premature termination codons (PTC), samples with splice-site defects (SSD), and samples with pathogenic putative missense variants. The method was applied to patients with variants of uncertain significance (VUS) or unexplained immunohistochemical MMR deficiency. We categorized the allelic representation into biallelic (50 ± 10%) or allelic loss (≤10%), and >10% and 10% and 30% of VUS. Our method allows a standardized, systematic cDNA analysis of the MMR FLTs to assess the pathogenicity mechanism of VUS on RNA level, which will gain relevance for precision medicine and gene therapy. Diagnostic accuracy will be enhanced by detecting MMR defects in hitherto unsolved patients. The data generated will help to calibrate a high-throughput NGS-based mRNA-analysis and optimize prediction programs.

Im April 2018 erschien die erste deutsche interdisziplinare S3-Leitlinie fur die Diagnostik, Therapie und Nachsorge der Patientinnen mit Endometriumkarzinom. Dieser Beitrag ist die Zusammenfassung des Kapitels 3: „Epidemiologie und Risikofaktoren, Pravention des Endometriumkarzinoms“ und des Kapitels 10: „Hereditare Endometriumkarzinome“ mit dem Ziel, dem Leser eine klinisch orientierte Kurzfassung an die Hand zu geben. Dargestellt sind die Statements und Empfehlungen zur Epidemiologie und den Risikofaktoren des Endometriumkarzinoms sowie zur Diagnose und Therapie hereditarer Endometriumkarzinome.

In 25% aller Kolonkarzinome besteht eine familiare Haufung. Das hereditare Non-Polyposis-assoziierte Kolorektalkarzinom (HNPCC)/Lynch-Syndrom ist eine autosomal-dominant vererbte Erkrankung, welche einen Anteil von 2–3% aller kolorektalen Karzinome ausmacht. Da der Ubergang von gutartigen Vorstufen zu bosartigen Tumoren und das Tumorwachstum im Vergleich zu sporadisch auftretendem Darmkrebs deutlich beschleunigt ablauft, ist ein grundlicher diagnostischer Nachweis von malignen Neoplasien und deren Vorstufen sowie die standardisierte Uberwachung der betroffenen Patienten und ihrer Familien notwendig. Um dies zu leisten, wurde 1999 das deutsche HNPCC Konsortium als ein multizentrisches und interdisziplinares Netzwerk von 7 Universitaten mit kooperierenden Gastroenterologen, Chirurgen, Humangenetikern, Pathologen und Epidemiologen gegrundet. Die Pathologie liefert die histologische Diagnose und uberpruft die Mikrosatelliteninstabilitat (MSI) und die Expression der Mismatch-Reparatur-Proteine im Tumorgewebe. Die Diagnose HNPCC gilt als gesichert, wenn bei MSI-Tumoren die Amsterdam-II-Kriterien erfullt sind oder wenn eine pathogene Keimbahnmutation in den Mismatch-Reparatur-Genen entdeckt wird. Ebenso gilt die Diagnose HNPCC als bestatigt, wenn eine Mutation fehlt, aber Bethesda-Kriterien erfullt sind und ein Tumor mit MSI oder Verlust der Mismatch-Reparatur-Proteine gefunden wird. Die Ziele des HNPCC-Konsortiums bestehen in einer verbesserten Uberwachung der Indexpatienten und ihrer Familien durch Definition optimaler diagnostischer Intervalle, in der Verbesserung der angewendeten Methoden, in der Entwicklung therapeutischer Ansatze sowie in der Detektion valider prognostischer Marker. Daruber hinaus wird die Integration des HNPCC-Programms in die allgemeine Gesundheitsversorgung angestrebt.

Missense mutations of the DNA mismatch repair gene MLH1 are found in a significant fraction of patients with Lynch syndrome (hereditary nonpolyposis colorectal cancer, HNPCC) and their pathogenicity often remains unclear. We report here all 88 MLH1 missense variants identified in families from the German HNPCC consortium with clinical details of these patients/families. We investigated 23 MLH1 missense variants by two functional in vivo assays in yeast; seven map to the ATPase and 16 to the protein interaction domain. In the yeast-2-hybrid (Y2H) assay three variants in the ATPase and twelve variants in the interaction domain showed no or a reduced interaction with PMS2; seven showed a normal and one a significantly higher interaction. Using the Lys2A 14 reporter system to study the dominant negative mutator effect (DNE), 16 variants showed no or a low mutator effect, suggesting that these are nonfunctional, three were intermediate and four wild type in this assay. The DNE and Y2H results were concordant for all variants in the interaction domain, whereas slightly divergent results were obtained for variants in the ATPase domain. Analysis of the stability of the missense proteins in yeast and human embryonic kidney cells (293T) revealed a very low expression for seven of the variants in yeast and for nine in human cells. In total 15 variants were classified as deleterious, five were classified as variants of unclassified significance (VUS) and three were basically normal in the functional assays, P603R, K618R, Q689R, suggesting that these are neutral.

Hereditary non-polyposis colorectal cancer (HNPCC) is an autosomal dominant tumour predisposition syndrome caused by germline mutations in mismatch repair (MMR) genes. In contrast to MLH1 and MSH2, germline mutations in MSH6 are associated with a milder and particularly variable phenotype. Based on the reported interaction of the MMR complex and the base excision repair protein MUTYH, it was hypothesised that MUTYH mutations serve as phenotypical modifiers in HNPCC families. Recently, a significantly higher frequency of heterozygosity for MUTYH mutations among MSH6 mutation carriers was reported. We examined 64 MSH6 mutation carriers (42 truncating mutations, 19 missense mutations and 3 silent mutations) of the German HNPCC Consortium for MUTYH mutations by sequencing the whole coding region of the gene. Monoallelic MUTYH mutations were identified in 2 of the 64 patients (3.1%), no biallelic MUTYH mutation carrier was found. The frequency of MUTYH mutations was not significantly higher than that in healthy controls, neither in the whole patient group (P=0.30) nor in different subgroups regarding mutation type. Our results do not support the association between MSH6 mutations and heterozygosity for MUTYH mutations.

Introduction Defects in the DNA mismatch repair (MMR) protein MLH1 are frequently observed in sporadic and hereditary colorectal cancers (CRC). Affected tumors generate much less metastatic potential than the MLH1 proficient forms. Although MLH1 has been shown to be not only involved in postreplicative MMR but also in several MMR independent processes like cytoskeletal organization, the connection between MLH1 and metastasis remains unclear. We recently identified non-erythroid spectrin αII (SPTAN1), a scaffolding protein involved in cell adhesion and motility, to interact with MLH1. In the current study, the interaction of MLH1 and SPTAN1 and its potential consequences for CRC metastasis was evaluated. Methods Nine cancer cell lines as well as fresh and paraffin embedded colon cancer tissue from 12 patients were used in gene expression studies of SPTAN1 and MLH1. Co-expression of SPTAN1 and MLH1 was analyzed by siRNA knock down of MLH1 in HeLa, HEK293, MLH1 positive HCT116, SW480 and LoVo cells. Effects on cellular motility were determined in MLH1 deficient HCT116 and MLH1 deficient HEK293T compared to their MLH1 proficient sister cells, respectively. Results MLH1 deficiency is clearly associated with SPTAN1 reduction. Moreover, siRNA knock down of MLH1 decreased the mRNA level of SPTAN1 in HeLa, HEK293 as well as in MLH1 positive HCT116 cells, which indicates a co-expression of SPTAN1 by MLH1. In addition, cellular motility of MLH1 deficient HCT116 and MLH1 deficient HEK293T cells was impaired compared to the MLH1 proficient sister clones. Consequently, overexpression of SPTAN1 increased migration of MLH1 deficient cells while knock down of SPTAN1 decreased cellular mobility of MLH1 proficient cells, indicating SPTAN1-dependent migration ability. Conclusions These data suggest that SPTAN1 levels decreased in concordance with MLH1 reduction and impaired cellular mobility in MLH1 deficient colon cancer cells. Therefore, aggressiveness of MLH1-positive CRC might be related to SPTAN1.

MLH1 promoter methylation characterisation by MSP, bisulphite pyrosequencing (useful for diagnostic purpose, not for predictive testing). 1.7 Analytical validation Confirmation of mutation in an independent biological sample of the index case or an affected relative. In case of deletion/duplication of one exon, confirm with a second technique/kit based on different primers.
