10. Active and satisfied? Physical activity and profession-related quality of life in psychologists

Zusammenfassung Hintergrund Klinisch tätige Psycholog:innen sind in ihrer Arbeit mit Menschen mit psychischen Erkrankungen oft zahlreichen Stressoren ausgesetzt; diese können sich in psychischer Belastung und einer verringerten Lebensqualität niederschlagen. Körperliche Aktivität könnte eine protektive Ressource darstellen, diesbezüglich fehlen jedoch bisher empirische Befunde. Methoden Im Rahmen einer querschnittlichen Online-Befragung über SoSci-Survey (Januar bis April 2020) wurden die körperliche Aktivität und die berufsbezogene Lebensqualität (positive Dimension: Mitgefühlszufriedenheit; negative Dimension: Mitgefühlsmüdigkeit) einer Gelegenheitsstichprobe aus 443 klinisch tätigen Psycholog:innen mithilfe etablierter Selbstberichtfragebogen (International Physical Activity Questionnaire – Short Form [IPAQ-SF], Professional Quality of Life Scale [ProQOL]) erfasst. Ergebnisse Der Großteil der befragten Psycholog:innen erfüllte das von der Weltgesundheitsorganisation empfohlene Mindestmaß an körperlicher Aktivität. Die Mitgefühlszufriedenheit war im Schnitt relativ hoch, während eher geringe bis moderate Ausprägungen an Burn-out-Symptomatik sowie sekundärem traumatischem Stress (Facetten der Mitgefühlsmüdigkeit) berichtet wurden. Außer vereinzelten kleinen negativen Korrelationen zwischen körperlicher Inaktivität und Mitgefühlszufriedenheit bei Psycholog:innen in Weiterbildung sowie Burn-out-Symptomen und körperlicher Gesamtaktivität bei Psychologischen Psychotherapeut:innen waren keine Zusammenhänge zwischen körperlicher Aktivität und berufsbezogener Lebensqualität ersichtlich. Schlussfolgerungen Psycholog:innen scheinen zu großen Teilen eine körperlich aktive und tendenziell zufriedene Berufsgruppe darzustellen. Zusammenhänge zwischen körperlicher Aktivität und Lebensqualität könnten teilweise durch Deckeneffekte verdeckt sein. Des Weiteren könnte eine Analyse der Ursachen für diese adaptiven Ausprägungen in Aktivität und Lebensqualität Anhaltspunkte für Maßnahmen zur Förderung anderer Professionen liefern.

11. Pandemieassoziierte Konsequenzen und Unterstützungsbedarf

Zusammenfassung Hintergrund Die COVID-19-Pandemie geht potenziell mit zahlreichen Veränderungen im Leben der Allgemeinbevölkerung einher. Dennoch gibt es bisher kaum Befunde dazu, welche Auswirkungen als besonders gravierend und negativ erlebt wurden, wie sich diese Auswirkungen und ihre Bewertungen im Verlauf der Pandemie änderten, und welche Unterstützungswünsche in diesem Kontext entstanden. Methode Längsschnittliche Daten einer Onlinestudie mit 10 Messzeitpunkten über die ersten 2 Jahre der Pandemie (März 2020 bis April 2022) wurden in einer Mixed-methods-Analyse betrachtet. Die Stichprobe aus 8337 Erwachsenen der Allgemeinbevölkerung in Deutschland beantwortete qualitative Freitextfragen zu den gravierendsten Konsequenzen der Pandemie auf ihr Leben sowie Unterstützungswünschen. Quantitativ wurden zudem die Bewertung der Konsequenzen im Pandemieverlauf und ihre Assoziationen mit psychischer Belastung betrachtet. Ergebnisse Die erlebten Konsequenzen und insbesondere ihre Bewertung veränderten sich im Pandemieverlauf. Sozial-gesellschaftliche und das allgemeine Leben betreffende Konsequenzen wurden im Schnitt besonders gravierend und negativ erlebt. Negativer erlebte Konsequenzen waren quer- und teilweise auch längsschnittlich mit stärkeren Angst- und depressiven Symptomen assoziiert. Psychotherapeutische sowie evaluativ-kommunikative Unterstützung wurde im Pandemiekontext besonders häufig erbeten. Schlussfolgerungen Subjektiv negativ erlebte Konsequenzen sollten möglichst durch adäquate Maßnahmen abgemildert werden. Die dynamischen Veränderungen der Konsequenzen und damit auch des Unterstützungsbedarfs im Pandemieverlauf sollten berücksichtigt werden. Die Unterstützungsmöglichkeiten reichen von sehr niedrigschwelligen Angeboten (z. B. Tipps online) bis hin zu einer Psychotherapie.

12. Affinity-based isolation of extracellular vesicles and the effects on downstream molecular analysis

Extracellular vesicles (EVs) are transport vesicles with diameters ranging from 30 to 1000 nm, secreted by cells in both physiological and pathological conditions. By using the EV shuttling system, biomolecular cargo such as proteins and genetic materials travels between cells resulting in intercellular communication and epigenetic regulation. Because the presence of EVs and cargo molecules in body fluids can predict the state of the parental cells, EV isolation techniques from complex biofluids have been developed. Further exploration of EVs through downstream molecular analysis depends heavily on those isolation technologies. Methodologies based either on physical separation or on affinity binding have been used to isolate EVs. Affinity-based methods for EV isolation are known to produce highly specific and efficient isolation results. However, so far, there is a lack of literature summarizing these methods and their effects on downstream EV molecular analysis. In the present work, we reviewed recent efforts on developing affinity-based methods for the isolation of EVs, with an emphasis on comparing their effects on downstream analysis of EV molecular cargo. Antibody-based isolation techniques produce highly pure EVs, but the harsh eluents damage the EV structure, and some antibodies stay bound to the EVs after elution. Aptamer-based methods use relatively mild elution conditions and release EVs in their native form, but their isolation efficiencies need to be improved. The membrane affinity-based method and other affinity-based methods based on the properties of the EV lipid bilayer also isolate intact EVs, but they can also result in contaminants. From the perspective of affinity-based methods, we investigated the influence of the isolation methods of choice on downstream EV molecular analysis.

Some jobs have a higher level of challenges and adversities. Individuals pursuing these jobs learn how to react to challenges and build up resilience. Within this study, we concentrated on the potential career path of top athletes as entrepreneurs, who are both expected to have a higher level of resilience than non-athletes. The purpose of this research was to examine if resilience is a determining factor on entrepreneurial intention and if the model based on the Theory of Planned Behavior (TPB) can be applied in general and for specific groups with a high level of resilience. To address the research questions, we collected data from a sample of 195 top athletes and 142 non-athletes. First, the level of resilience and entrepreneurial intention were compared with an analysis of variance (ANOVA). Subsequently, the structural equation model tested the influence of resilience on entrepreneurial intention, mediated by the TPB, first for the whole sample and then as a multigroup comparison for both groups. Resilience had an indirect influence on entrepreneurial intention, mediated by the explaining factors of the TPB (personal attitude, subjective norm, perceived behavioral control). The multigroup comparison revealed a difference in the influence of perceived behavioral control on entrepreneurial intention between top athletes and non-athletes. Based on these results, this research added further knowledge to the field of entrepreneurial intention by examining the specific role of resilience necessary for careers as top athletes and entrepreneurs. It also contributes by researching the specific group of top athletes compared to non-athletes and extrapolating recommendations in entrepreneurship education for both groups, as creating athletes' awareness of potential overconfidence or implementing resilience training in education for non-athletes.

Increasing physical activity is essential to improve psychiatric patients’ physical and mental health. This study aimed to characterise the physical activity levels of inpatients in a general psychiatric clinic and to determine the feasibility of using a simple tool in everyday practice to assess physical activity levels in standard patient documentation. We assessed the level of physical activity undertaken by patients treated on an inpatient basis in a psychiatric hospital over 20 months. A total of 328 patients were included in the analysis. Physical activity was measured using a slightly altered version of the Exercise as a vital sign (EVS) questionnaire. All information was extracted from letters of discharge. During inpatient treatment, moderate to vigorous activity levels increased, and more patients engaged in physical activity. Patients with mood or anxiety disorders displayed the most considerable increase in physical activity. Patients with other diagnoses, such as schizophrenia, benefitted less or not at all. Factors associated with physical activity included—among others—history of substance use, education and month of admission. Investigating the feasibility of standardised documentation of physical activity showed fluctuation in documentation rates throughout the study. The level of physical activity performed by psychiatric patients can be increased during inpatient treatment. Implementing physical activity level as part of standard patient documentation is a first step in gathering data to assess the need for interventions to achieve an optimal physical activity in psychiatric patients throughout inpatient treatment.

Summary Introduction On February 25, 2020, the first 2 patients were tested positive for severe acute respiratory syndrome coronavirus‑2 (SARS-CoV-2) in Tyrol, Austria. Rapid measures were taken to ensure adequate intensive care unit (ICU) preparedness for a surge of critically ill coronavirus disease-2019 (COVID-19) patients. Methods This cohort study included all COVID-19 patients admitted to an ICU with confirmed or strongly suspected COVID-19 in the State of Tyrol, Austria. Patients were recorded in the Tyrolean COVID-19 intensive care registry. Date of final follow-up was July 17, 2020. Results A total of 106 critically ill patients with COVID-19 were admitted to 1 of 13 ICUs in Tyrol from March 9 to July 17, 2020. Median age was 64 years (interquartile range, IQR 54–74 years) and the majority of patients were male (76 patients, 71.7%). Median simplified acute physiology score III (SAPS III) was 56 points (IQR 49–64 points). The median duration from appearance of first symptoms to ICU admission was 8 days (IQR 5–11 days). Invasive mechanical ventilation was required in 72 patients (67.9%) and 6 patients (5.6%) required extracorporeal membrane oxygenation treatment. Renal replacement therapy was necessary in 21 patients (19.8%). Median ICU length of stay (LOS) was 18 days (IQR 5–31 days), median hospital LOS was 27 days (IQR 13–49 days). The ICU mortality was 21.7% (23 patients), hospital mortality was 22.6%. There was no significant difference in ICU mortality in patients receiving invasive mechanical ventilation and in those not receiving it (18.1% vs. 29.4%, p = 0.284). As of July 17th, 2020, two patients are still hospitalized, one in an ICU, one on a general ward. Conclusion Critically ill COVID-19 patients in Tyrol showed high severity of disease often requiring complex treatment with increased lengths of ICU and hospital stay. Nevertheless, the mortality was found to be remarkably low, which may be attributed to our adaptive surge response providing sufficient ICU resources.

Depression and coronary heart disease (CHD) are prevalent and often co-occurring disorders. Both have been associated with a dysregulated stress system. As a central element of the stress system, the FKBP5 gene has been shown to be associated with depression. In a prospective design, this study aims to investigate the association of FKBP5 with depressive symptoms in CHD patients. N = 268 hospitalized CHD patients were included. Depressive symptoms were measured using the Hospital Anxiety and Depression Scale (HADS-D) at four time points (baseline, and after 1 month, 6 months, and 12 months). The functional FKBP5 single-nucleotide polymorphism (SNP) rs1360780 was selected for genotyping. Linear regression models showed that a higher number of FKBP5 C alleles was associated with more depressive symptoms in CHD patients both at baseline (p = 0.015) and at 12-months follow-up (p = 0.025) after adjustment for confounders. Further analyses revealed that this effect was driven by an interaction of FKBP5 genotype with patients’ prior CHD course. Specifically, only in patients with a prior myocardial infarction or coronary revascularization, more depressive symptoms were associated with a higher number of C alleles (baseline: p = 0.046; 1-month: p = 0.026; 6-months: p = 0.028). Moreover, a higher number of C alleles was significantly related to a greater risk for dyslipidemia (p = .016). Our results point to a relevance of FKBP5 in the association of the two stress-related diseases depression and CHD.

In context of the current COVID-19 pandemic the consumption of pandemic-related media coverage may be an important factor that is associated with anxiety and psychological distress. Aim of the study was to examine those associations in the general population in Germany. 6233 participants took part in an online-survey (March 27th–April 6th, 2020), which included demographic information and media exploitation in terms of duration, frequency and types of media. Symptoms of depression, unspecific anxiety and COVID-19 related anxiety were ascertained with standardized questionnaires. Frequency, duration and diversity of media exposure were positively associated with more symptoms of depression and unspecific and COVID-19 specific anxiety. We obtained the critical threshold of seven times per day and 2.5 h of media exposure to mark the difference between mild and moderate symptoms of (un)specific anxiety and depression. Particularly the usage of social media was associated with more pronounced psychological strain. Participants with pre-existing fears seem to be particularly vulnerable for mental distress related to more immoderate media consumption. Our findings provide some evidence for problematical associations of COVID-19 related media exposure with psychological strain and could serve as an orientation for recommendations—especially with regard to the thresholds of critical media usage.

Erst in jungster Zeit finden psychische Erkrankungen von Leistungssportlern mehr Beachtung, nachdem lange davon ausgegangen wurde, dass diese bei Leistungssportlern sehr viel seltener seien als in der Allgemeinbevolkerung. Aufbauend auf die aktuell verfugbare Literatur wird ein Uberblick uber psychische Erkrankungen bei Leistungssportlern sowie mogliche Besonderheiten in Diagnostik und Therapie gegeben. Selektive Literaturubersicht. Insgesamt kommen psychische Erkrankungen bei Leistungssportlern vergleichbar haufig vor wie in der Allgemeinbevolkerung. In Abhangigkeit von den Sportarten kann jedoch das Risiko sogar erhoht sein, z. B. fur Essstorungen in asthetischen Sportarten, beim Radfahren oder beim Laufen. Daneben gibt es sportspezifische psychopathologische Auffalligkeiten wie Anorexia athletica, korperdysmorphe Storung oder die chronische traumatische Enzephalopathie. Psychische Erkrankungen konnen auch bei Leistungssportlern auftreten, wobei sich die Risikofaktoren von denen in der Allgemeinbevolkerung unterscheiden konnen. Das Stigma psychischer Erkrankungen ist im Hochleistungssport noch weit verbreitet, sodass Betroffene und auch deren Umfeld uber psychische Erkrankungen, ihre Erkennung, aber insbesondere ihre Behandlungsmoglichkeiten noch besser intensiver informiert und aufgeklart werden mussen.

In den letzten Jahrzehnten zeigten sich in zahlreichen Studien enge Zusammenhange zwischen korperlicher Aktivitat und Angst. Aktuelle Forschungsergebnisse zum Zusammenhang zwischen korperlicher Aktivitat und Angsten in der Allgemeinbevolkerung werden dargestellt. Des Weiteren werden die praventive Wirkung von korperlicher Aktivitat sowie ihre Effektivitat in der Therapie von Angsterkrankungen, potenzielle Wirkmechanismen sowie Empfehlungen bezuglich Art und Intensitat korperlicher Aktivitat und Besonderheiten im klinischen Setting thematisiert. Auswertung aktueller Studienergebnisse, Ubersichtsarbeiten und Metaanalysen im Bereich von korperlicher Aktivitat und Angsterkrankungen. Die Wirkung von korperlicher Aktivitat zur Reduktion von Angsten in nichtklinischen Populationen ist gut belegt. Es finden sich erste Hinweise auf eine praventive Wirkung von regelmasiger korperlicher Aktivitat bezuglich der Entwicklung von Angsterkrankungen. Menschen mit Angsterkrankungen zeigen durchschnittlich ein geringeres Ausmas korperlicher Aktivitat als Menschen ohne Angsterkrankungen, wobei spezifische Barrieren bezuglich korperlicher Aktivitat eine Rolle spielen. Erste Studien zur Nutzung von korperlicher Aktivitat als Therapie bei Angsterkrankungen weisen auf eine Effektivitat hin. Bezuglich Art und Intensitat korperlicher Aktivitat fur Menschen mit Angsterkrankungen scheint vorerst eine Orientierung an den allgemeinen Bevolkerungsempfehlungen sinnvoll. Aufgrund der vorliegenden Befunde zu korperlicher Aktivitat in der Therapie von Angsterkrankungen sollte allen Patienten mit Angsterkrankungen korperliche Aktivitat als Therapieoption oder -erganzung empfohlen und angeboten werden.

Verglichen mit dem profunden Wissen über die Erfahrungen und Bedürfnisse US-amerikanischer Soldaten, ist der Kenntnisstand über die psychische Gesundheit und die Bewältigungsmechanismen aktiver deutscher BundeswehrsoldatInnen eher gering. Über ehemalige EinsatzsoldatInnen, die inzwischen aus dem militärischen System ausgeschieden und Teil der deutschen Zivilgesellschaft geworden sind, ist noch viel weniger bekannt. Ziel der Pilotstudie war die Identifikation und Darstellung von Coping- und Abwehrmechanismen ehemaliger BundeswehrsoldatInnen mithilfe einer methodisch-strukturierten, qualitativen Datenerhebung. Zu diesem Zweck wurden 43 Interviews mit ehemaligen EinsatzsoldatInnen geführt, transkribiert und der thematischen Analyse unterzogen. Die identifizierten Coping- und Abwehrmechanismen wurden den Bereichen Verhalten, Beziehung, Emotion, Reflexion und Zeit zugewiesen und modellhaft in Achsenform veranschaulicht. Durch die bewusste Synthese von kognitiv-behavioralen und psychoanalytischen Perspektiven konnte ein differenziertes Bild erstellt sowie ein eigenständiges, schulenübergreifendes, integratives Achsenmodell zur Darstellung der bewussten und unbewussten Bewältigung belastender Erfahrungen entwickelt werden., Compared to the profound knowledge about the experiences and needs of American soldiers, in Germany the level of knowledge on mental health and coping mechanisms of active soldiers in the German Armed Forces (GAF) is relatively limited. Even less is known about former soldiers who are no longer on active duty and who, in the majority of cases, have become part of the German public healthcare system after discharge from the military. The aim of the pilot study was the identification and presentation of the coping and defence mechanisms of former GAF soldiers by using methodically structured, qualitative data collection. For this purpose, 43 interviews with former soldiers were conducted, transcribed and thematically analyzed. The coping and defence mechanisms identified were assigned to the areas of behavior, relationship, emotion, contemplation and time and visualized as an axis model. By consciously synthesizing cognitive-behavioral and psychoanalytical perspectives, it was possible to create a differentiated picture and develop an independent, integrative model across psychotherapeutic school boundaries to illustrate the conscious and unconscious coping with traumatic experiences.

This work strives to develop a typological classification of the use of conscious and unconscious defense and coping mechanisms based on methodically and structurally collected data from a qualitative survey of 43 former soldiers in Germany. Seven coping and defense types were identified: the Fighter, the Comrade, the Corpsman, the Strategist, the Partisan, the Self-Protector and the Infantryman. The types identified differed with regard to the accumulation, combination, and use of their conscious and unconscious defense and coping mechanisms in the superordinate areas of behaviour, relationships, emotions, reflexivity and time focus. The typological classification could offer psychotherapeutic interventions tailored to individuals and their defense and coping mechanisms, which could lead to improved therapy use and compliance.

d-cycloserine (DCS), a partial NMDA-receptor agonist, seems to be a promising enhancer for exposure therapy in anxiety disorders. It has been tested successfully in animal models of fear extinction, where DCS enhanced extinction learning. Applied in clinical studies, results of DCS-augmented exposure therapy remain ambiguous, calling for a deeper understanding of the underlying mechanisms of DCS and its exact effect on extinction learning and return of fear (ROF) in humans. In the present study, we investigated the effect of DCS-augmented extinction learning on behavioral, psychophysiological, and neural indices of ROF during a 24-h delayed recall test. Thirty-seven participants entered a randomized, placebo-controlled, double-blind, 3-day fear conditioning and delayed extinction fMRI design. One hour before extinction training, participants received an oral dose of 50 mg of DCS or a placebo. Behavioral arousal ratings revealed a generalized ROF during extinction recall in the placebo but not DCS group. Furthermore, participants receiving DCS compared to placebo showed attenuated differential BOLD responses in left posterior hippocampus and amygdala from extinction learning to extinction recall, due to increased hippocampal recruitment in placebo and trendwise decreased amygdala responding in DCS subjects. Our finding that DCS reduces ROF in arousal ratings and neural structures subserving defensive reactions support a role for NMDA receptors in extinction memory consolidation and encourage further translational research.

Sporttherapie hat sich in der Behandlung vieler psychischer Erkrankungen als nachweislich wirksame Therapieform erwiesen. Aufgrund der hohen individuellen Belastung fur Betroffene sowie der immensen Kosten fur das Gesundheitssystem, die durch psychische Erkrankungen entstehen, sollten alle Bestrebungen dahin gehen, eine moglichst gute Versorgung zu gewahrleisten, die dem aktuellen Forschungsstand entspricht. Erste Studien ergaben jedoch bereits 2016 Hinweise darauf, dass nur ein Bruchteil der Patienten wahrend ihrer stationaren Behandlung Sporttherapie erhalt. Die hier vorgestellte Studie hatte daher zum Ziel, die reale Versorgungssituation stationar psychiatrisch behandelter Patienten mit Sporttherapie in Deutschland zu erfassen und somit eine wissenschaftliche Grundlage fur die Forderung nach einer Verbesserung der Versorgung zu bilden. In einer retrospektiven Analyse der Abrechnungsdaten von 4 Universitatskliniken wurden hierfur 2693 Patientendaten auf die Frage nach der Versorgungssituation mit Sporttherapie hin untersucht. Dabei zeigte sich, dass nur 23 % der Patienten wahrend des Beobachtungszeitraums von einer Woche an Sporttherapie teilnehmen mit einer durchschnittlichen wochentlichen Therapiedauer von 36 min. Patienten mit Schizophrenie sowie stark komorbide Patienten nahmen sogar noch seltener und weniger Sporttherapie in Anspruch. Insgesamt konnte in der vorliegenden Studie also gezeigt werden, dass die Versorgungslage mit Sporttherapie in der stationar-psychiatrischen Behandlung mangelhaft ist und somit optimiert werden sollte. Vor dem Hintergrund der guten Evidenz von Sporttherapie in der Behandlung psychischer Erkrankungen, aktueller Leitlinienempfehlungen sowie des gunstigen Nebenwirkungsprofils im Vergleich zur Pharmakotherapie sollte die Etablierung von Sporttherapie in den Kliniken gezielt verbessert werden.

Several technical methods are currently discussed to meet the objectives of the United Nations Framework Convention on Climate Change 21st Conference of the Parties, Paris, France (Paris Agreement) in terms of carbon dioxide (CO2) concentration in the Earth’s atmosphere. In addition to efficiency improvements, reduction of energy consumption, and the utilization of renewable energy sources, the application of carbon capture and storage (CCS) technologies seems to be unavoidable. Whereas all these measures aim on the reduction of CO2 that is being newly released, there is the approach to remove CO2 from the atmosphere that has already been emitted. This can be achieved by the utilization of bioenergy in CCS processes. Within this paper, the utilization of alternative fuels in two second-generation CCS processes is assessed. In this regard, chemical looping combustion (CLC) and calcium looping (CaL) are two promising technologies. Both processes have proven their feasibility already in semi-industrial scale. The assessment includes three different types of fuel namely coal, biomass, and solid recovered fuel (SRF). The analysis is twofold: first, a heat and mass balance calculation reveals the specific CO2 emissions of each power system; second, a cost analysis points out the feasibility from an economic point of view. The highest CO2 removal can be achieved by a biomass-fired CLC unit (− 696 gCO2/kWhe). Furthermore, it was found that the co-combustion of SRF even at moderate co-firing rates allows for noteworthy improved economics of the CCS system. Therefore, the utilization of waste-derived fuels in the context of CCS processes should be put more into focus in future research activities.

Traditional models of future alcohol use in adolescents have used variable-centered approaches, predicting alcohol use from a set of variables across entire samples or populations. Following the proposition that predictive factors may vary in adolescents as a function of family history, we used a two-pronged approach by first defining clusters of familial risk, followed by prediction analyses within each cluster. Thus, for the first time in adolescents, we tested whether adolescents with a family history of drug abuse exhibit a set of predictors different from adolescents without a family history. We apply this approach to a genetic risk score and individual differences in personality, cognition, behavior (risk-taking and discounting) substance use behavior at age 14, life events, and functional brain imaging, to predict scores on the alcohol use disorders identification test (AUDIT) at age 14 and 16 in a sample of adolescents (N = 1659 at baseline, N = 1327 at follow-up) from the IMAGEN cohort, a longitudinal community-based cohort of adolescents. In the absence of familial risk (n = 616), individual differences in baseline drinking, personality measures (extraversion, negative thinking), discounting behaviors, life events, and ventral striatal activation during reward anticipation were significantly associated with future AUDIT scores, while the overall model explained 22% of the variance in future AUDIT. In the presence of familial risk (n = 711), drinking behavior at age 14, personality measures (extraversion, impulsivity), behavioral risk-taking, and life events were significantly associated with future AUDIT scores, explaining 20.1% of the overall variance. Results suggest that individual differences in personality, cognition, life events, brain function, and drinking behavior contribute differentially to the prediction of future alcohol misuse. This approach may inform more individualized preventive interventions.

Background Sufficient analgesia is an obligation, but oligoanalgesia (NRS> 3) is frequently observed prehospitally. Potent analgesics may cause severe adverse events. Thus, analgesia in the helicopter emergency medical service (HEMS) setting is challenging. Adequacy, efficacy and administration safety of potent analgesics pertaining to injured patients in HEMS were analysed. Methods Observational study evaluating data from 14 year-round physician-staffed helicopter bases in Austria in a 12-year timeframe. Results Overall, 47,985 (34.3%) patients received analgesics, 26,059 of whom were adult patients, injured and not mechanically ventilated on site. Main drugs administered were opioids (n=20,051; 76.9%), esketamine (n=9082; 34.9%), metamizole (n=798; 3.1%) and NSAIDs (n=483; 1.9%). Monotherapy with opioids or esketamine was the most common regimen (n=21,743; 83.4%), while opioids together with esketamine (n= 3591; 13.8%) or metamizole (n=369; 1.4%) were the most common combinations. Females received opioids less frequently than did males (n=6038; 74.5% vs. n=14,013; 78.1%; p 95%), but females more often had moderate to severe pain on arrival in hospital (n=34; 5.0% vs. n=59; 3.2%; p=0.043). Administration of potent analgesics was safe, as indicated by MEES, SpO2 and respiratory rates. On 10% of all missions, clinical patient assessment was deemed sufficient by HEMS physicians and monitoring was spared. Conclusions Opioids and esketamine alone or in combination were the analgesics of choice in physician-staffed HEMS in Austria. Analgesia was often sufficient, but females more than males suffered from oligoanalgesia on hospital arrival. Administration safety was high, justifying liberal use of potent analgesics in physician-staffed HEMS.

Epi- und Pandemien sowie die Masnahmen, um ihre Verbreitung einzudammen, gehen mit zahlreichen Stressoren einher, die insbesondere auch zu starken Angsten fuhren konnen. Der Artikel erlautert Charakteristika derartiger Angste, potenzielle Resilienz- und Risikofaktoren sowie entsprechende Handlungsempfehlungen. Forschungsergebnisse bezuglich COVID-19 und fruherer Epi- und Pandemien (z. B. HIV, SARS, MERS, Ebola und Schweinegrippe) sowie offizielle Handlungsempfehlungen werden dargestellt. Angstsymptome treten in Epi- und Pandemien haufig auf – insbesondere zu Beginn – und verringern sich meist im weiteren Verlauf. Auch wenn sich verschiedene Infektionskrankheiten unterscheiden, ahneln sich die damit assoziierten Angste und beinhalten z. B. die Furcht vor gesundheitlichen, sozialen und wirtschaftlichen Konsequenzen. Resilienz- und Risikofaktoren fallen ebenfalls vergleichbar aus. Selbstwirksamkeitserwartung, Unsicherheitstoleranz, Normalisierung, Routine, Sicherheit und soziale Unterstutzung wirken meist protektiv. Erhohter Medienkonsum, weibliches Geschlecht, Arbeit im medizinischen Kontext, Suppression, vorbestehende Erkrankungen, gesundheitsschadliches Verhalten und eine unmittelbare Betroffenheit durch das Virus gehen dagegen in der Regel mit starkeren Angsten einher. Angste sollten beobachtet und aufgefangen werden, um pathologische Verlaufe zu verhindern – insbesondere bei vulnerablen Gruppen. Es empfiehlt sich, mittels praventiver und therapeutischer Masnahmen Resilienzfaktoren zu fordern und Risikofaktoren entgegenzuwirken. Daraus ergibt sich die Notwendigkeit der Entwicklung und empirischen Uberprufung spezifischer Interventionen sowie weiterer Langsschnittstudien.
