The importance of the type of pain medication in spinal disease is an ongoing matter of debate. Recent guidelines recommend acetaminophen and NSAIDs as first-line medication for lumbar disc herniation. However, opioid pain medication is commonly used in patients with chronic pain, and therefore also in patients with sciatica. The aim of this study is to evaluate if opioids have an impact on the outcome in patients suffering from lumbar disc herniation. To assess this objectively quantitative sensory testing (QST) was applied. In total, 52 patients with a single lumbar disc herniation confirmed on magnetic resonance imaging (MRI) and treated by lumbar sequesterectomy were included in the trial. Patients were analysed according to their preoperative opioid intake: 35 patients who did not receive opioids (group NO) and 17 patients, who received opioids preoperatively (group O). Further evaluation included detailed medical history, physical examination, various questionnaires, and QST. No pre- and postoperative differences were detected in thermal or mechanical thresholds (p > 0.05). Wind-up ratio (WUR) differed significantly between groups 1 week postoperatively (p = 0.025). The NRS for low back pain was rated significantly higher in the non-opioid group (NO) after 1-week follow-up (p = 0.026). Radicular pain tended to be higher in the NO group after 12 months of follow-up (p = 0.023). Opioids seem to be a positive predictor for the postoperative pain outcome in early follow-up in patients undergoing lumbar sequesterectomy. Furthermore, patients presented with less radicular pain 1 year after surgery.

Purpose Stabilization of C1-2 using a Harms–Goel construct with 3.5 mm titanium (Ti) rods has been established as a standard of reference (SOR). A reduction in craniocervical deformities can indicate increased construct stiffness at C1-2. A reduction in C1-2 can result in C1-2 joint gapping. Therefore, the authors sought to study the biomechanical consequences of C1-2 gapping on construct stiffness using different instrumentations, including a novel 6-screw/3-rod (6S3R) construct, to compare the results to the SOR. We hypothesized that different instrument pattern will reveal significant differences in reduction in ROM among constructs tested. Methods The range of motion (ROM) of instrumented C1-2 polyamide models was analyzed in a six-degree-of-freedom spine tester. The models were loaded with pure moments (2.0 Nm) in axial rotation (AR), flexion extension (FE), and lateral bending (LB). Comparisons of C1-2 construct stiffness among the constructs included variations in rod diameter (3.5 mm vs. 4.0 mm), rod material (Ti. vs. CoCr) and a cross-link (CLX). Construct stiffness was tested with C1-2 facets in contact (Contact Group) and in a 2 mm distracted position (Gapping Group). The ROM (°) was recorded and reported as a percentage of ROM (%ROM) normalized to the SOR. A difference > 30% between the SOR and the %ROM among the constructs was defined as significant. Results Among all constructs, an increase in construct stiffness up to 50% was achieved with the addition of CLX, particularly with a 6S3R construct. These differences showed the greatest effect for the CLX in AR testing and for the 6S3R construct in FE and AR testing. Among all constructs, C1-2 gapping resulted in a significant loss of construct stiffness. A protective effect was shown for the CLX, particularly using a 6S3R construct in AR and FE testing. The selection of rod diameter (3.5 mm vs. 4.0 mm) and rod material (Ti vs. CoCr) did show a constant trend but did not yield significance. Conclusion This study is the first to show the loss of construct stiffness at C1-2 with gapping and increased restoration of stability using CLX and 6S3R constructs. In the correction of a craniocervical deformity, nuances in the surgical technique and advanced instrumentation may positively impact construct stability.

The negative impact on spinal diseases may apply not only to obesity but also to smoking. To investigate the influence of obesity and smoking on the development and recovery of lumbar disc herniation in young adults. Retrospective analysis of 97 patients who presented with lumbar disc herniation at the authors’ department between 2010 and 2017. Data were collected using the patients’ digital health records including demographics, clinical and neurological characteristics, treatment details, and outcomes. Ninety-seven patients between 17 and 25 years were included in this retrospective analysis. Patients were categorized into two groups according to their body mass index: obese (O, ≥ 30 kg/m2) and non-obese (NO, 2). The proportion of obese patients in our cohort vs. in the overall population differed significantly (19.4% vs. 3.8–7.1%, RR 3.17; p p = 0.067) and pain (p = 0.074). Also, the proportion of regular smokers differed significantly from the numbers of known smokers of the same age (62.4% vs. 30.2%, RR 2.0; p = 0.01). Obesity plus smoking showed a significantly negative impact on motor deficits postoperatively (p = 0.015) and at discharge (p = 0.025), as well as on pain values (p = 0.037) and on analgesic consumption (p = 0.034) at 6 weeks follow-up. The negative impact of obesity and smoking on the occurrence of lumbar disc herniation could be demonstrated for individuals aged 25 or younger. Furthermore, a trend to earlier recovery of motor deficits and significantly lower pain scales for non-obese and non-smoking patients could be shown.

Spinal infection (SI) is defined as an infectious disease affecting the vertebral body, the intervertebral disc, and/or adjacent paraspinal tissue and represents 2–7% of all musculoskeletal infections. There are numerous factors, which may facilitate the development of SI including not only advanced patient age and comorbidities but also spinal surgery. Due to the low specificity of signs, the delay in diagnosis of SI remains an important issue and poor outcome is frequently seen. Diagnosis should always be supported by clinical, laboratory, and imaging findings, magnetic resonance imaging (MRI) remaining the most reliable method. Management of SI depends on the location of the infection (i.e., intraspinal, intervertebral, paraspinal), on the disease progression, and of course on the patient’s general condition, considering age and comorbidities. Conservative treatment mostly is reasonable in early stages with no or minor neurologic deficits and in case of severe comorbidities, which limit surgical options. Nevertheless, solely medical treatment often fails. Therefore, in case of doubt, surgical treatment should be considered. The final result in conservative as well as in surgical treatment always is bony fusion. Furthermore, both options require a concomitant antimicrobial therapy, initially applied intravenously and administered orally thereafter. The optimal duration of antibiotic therapy remains controversial, but should never undercut 6 weeks. Due to a heterogeneous and often comorbid patient population and the wide variety of treatment options, no generally applicable guidelines for SI exist and management remains a challenge. Thus, future prospective randomized trials are necessary to substantiate treatment strategies.

Hybrid stabilization with a dynamic implant has been suggested to avoid adjacent segment disease by creating a smoother transition zone from the instrumented segments to the untreated levels above. This study aims to characterize the transition zones of two-level posterior instrumentation strategies for elucidating biomechanical differences between rigid fixation and the hybrid stabilization approach with a pedicle screw-based dynamic implant. Eight human lumbar spines (L1–5) were loaded in a spine tester with pure moments of 7.5 Nm and with a hybrid loading protocol. The range of motion (ROM) of all segments for both loading protocols was evaluated and normalized to the native ROM. Thoracic myelopathy is often caused by vertebral body fractures resulting from neoplastic conditions, traumatic events, or infectious diseases. One of the preferred procedures for treating it is the lateral extracavitary approach (LECA) with single-level or multilevel decompressive corpectomy and reconstruction. The aim of this retrospective study was to analyze the thoracic lateral extracavitary approach with corpectomy using vertebral body replacement systems (VBR-S) and dorsal reconstruction. Twenty-four patients with metastatic or primary lesions of thoracic vertebrae T2–T12 underwent spinal decompression and ventral column reconstruction with correction of spinal deformity via a LECA. One-level to four-level corpectomies were performed with additional navigated dorsal pedicle screw fixation at an average of two levels above and below the corpectomy lesion. None of the patients received preoperative spinal embolization, and the majority of the patients were admitted to radiotherapy postoperatively. Their mean age was 56 years (± 15), with a female-to-male sex ratio of 8 to 16. Patients with a minimum follow-up period of 16 months were included. The Karnofsky index, preoperative and postoperative numeric rating scale (NRS), and Frankel scale were measured. In addition, intraoperative loss of blood (LOB), units of packed red blood cell (PRBC) transfusions, the duration of the operation, and the hospitalization period were evaluated and correlated with preoperative and postoperative values. The majority of the patients were suffering from metastatic lesions and were treated with a 1 level corpectomy (median 1 level, range 1 to 4). The mean duration of surgery was 288 min (± 121) and the mean LOB was 1626 mL (± 1486 mL), with approximately two PRBC units per patient used. All patients were transferred to the intensive care unit (ICU) postoperatively, with a mean ICU stay of 2.0 days (± 1 day). The mean hospitalization period was 13 days (± 7 days). No implant-related failures or procedure-related deaths were observed. Significant differences were noted between the preoperative and postoperative Karnofsky index (74 vs. 84%) and NRS (4 vs. 2). One patient required revision surgery due to a superficial wound infection, and another needed revision surgery due to a dural tear. In another patient, an iatrogenic dural tear was repaired during the same surgical procedure and did not lead to postoperative complications. Four pleural effusions and one pneumothorax were observed, so that the overall complication rate was approximately 33%. Four of the patients died within 2 years of the operation due to progression of the primary disease. Lateral corpectomy and sagittal reconstruction of the thoracic spine using VBR-S conducted via a navigated LECA approach yields favorable results, despite the burden of neoplastic disease. These challenging procedures are accompanied by increased LOB and hospitalization periods, with moderate transfusion requirements. Surgery-related complications are low and local tumor control is satisfactory, despite the progression of the underlying neoplastic disease. Surgical correction of fixed thoracolumbar deformity is usually achieved by estimating the preoperatively planned correction angles during surgery and is therefore prone to inaccuracy. This is particularly problematic in biplanar deformities. To overcome these difficulties, 3D model for planning, preparation, and simulation of an asymmetric pedicle subtraction osteotomy (aPSO) was printed and used to realign coronal and sagittal balance in case of rigid degenerative kyphoscoliosis. A 59-year-old woman presented with severe back pain and spinal claudication and was diagnosed with a rigid kyphoscoliosis with multilevel spinal stenosis. Spino-pelvic parameters were measured preoperatively (pelvic incidence 47° [PI], lumbar lordosis 18° [LL]; pelvic tilt 42° [PT], T1 pelvic angle 40° [TPA], Cobb angle 33°, sagittal vertical axis 10.5 cm [SVA]). To aid the complex deformity in the sagittal and coronal plane, a 1:1 3D model of the spine was printed according to the preoperative computed tomography (CT). With the use of a rebalancing software, the spine was prepared in vitro as a model for intraoperative realignment and the correction was preoperatively simulated. Surgery was accomplished according to the preoperative software-guided plan. Asymmetric pedicle subtraction osteotomy (aPSO) of L3 identical to the 3D model was performed. Additionally, a Smith-Peterson osteotomy of L4/5 with transforaminal lumbar interbody fusion (TLIF) and laminectomy of L2-S1 with pedicle screw instrumentation TH12-S1 was accomplished. Postoperative radiological parameters revealed good success (LL 40°, SVA 6 cm, PT 19°, TPA 22°, and a Cobb angle of 8°). Improvement of the Oswestry disability index (ODI) of 42 to 18, the visual analog scale (VAS) of 8 to 1, and walking distance 100 to 8000 m compared to preoperatively resulted at 24 months follow-up. The precise coronal and sagittal correction of a rigid degenerative kyphoscoliosis presents a major challenge. Asymmetric PSO is able to realign the thoracolumbar spine in both the coronal and sagittal planes. The creation of an in vitro 3D-printed model of a patient's spinal deformity in combination with a software to calculate the correction angles facilitates preoperative planning and implementation of aPSO.

Cervical corpectomies are increasingly used to treat degenerative, metastatic, inflammatory and traumatic multisegmental diseases. The postoperative results are thought to correlate mainly with the number of resected vertebral bodies. Thus, the aim of the study was to analyse complications and early outcome of these procedures to document the implant-related complications in order to set up a prospective clinical trial. Forty-five patients, who were treated in our department from 2011 to 2014 and who were available for a minimum follow-up of 1 year, were consecutively included in this retrospective evaluation. The median age was 61 (±11) years with a female to male sex ratio of 19 to 26, respectively. In these patients, cervical corpectomies (one-, two- and three-level procedures) were performed. The average number of resected levels was 1.2 levels. The intraoperative loss of blood (LOB), the red cell transfusions (rcT), the length of operation (LOO) and the usage of drains were investigated and correlated with intra- and postoperative complications. The mean LOO was 244 min (±68) with a mean LOB of 511 ml (±531). The overall complication rate was 22.9% (10 patients). Six patients (13.3%) had implant-related complications due to loosening and toggling of the screws and/or cage subsidence. Two patients (4.4%) had a postoperative haematoma and another two patients (4.4%) suffered from neurological deterioration due to an ongoing and severe myelopathic syndrome. All these patients received revision surgery. The average time from the first to revision surgery was 90 days. Cervical corpectomies still remain procedures with a high complication rate mainly represented by implant-related failures. These implant-related complications range from screw/plate loosening or toggling to graft dislocation with subsidence and might be associated with constructs extended to the C7 vertebral body. In our study population, the rate of implant failure was comparable to the literature, but not obviously correlated with the number of vertebral bodies resected. This may be attributed to the different disease entities. Thus, our results support the use of circumferential approaches for selected instability scenarios (metastatic or inflammatory diseases, kyphosis, osteoporosis, etc.) of one- and two-level corpectomies.

Spondylodiscitis may arise primarily via hematogenous spread or direct inoculation of virulent organisms during spine surgery. To date, no comparative data investigating the differences between primary and postoperative spondylodiscitis is available. Thus, the purpose of this retrospective study was to investigate differences between these two etiologies. One hundred fifty-nine patients that were treated at our department were included in the retrospective analysis. The patients were categorized into two groups based on the etiology of spondylodiscitis: group NS, primary spondylodiscitis without prior spinal surgery; group S, spondylodiscitis following spinal surgery. Biomechanical investigation. Cervical two-level corpectomies with anterior-only instrumentation are associated with a high rate of implant-related complications. These procedures, therefore, often require an additional dorsal instrumentation to prevent screw loosening. Cement augmentation of the anterior screws in two-level corpectomies might stabilize the construct, so that a second dorsal procedure could be avoided. To evaluate the screw anchorage in cervical anterior-only procedures, an ex vivo evaluation of the range of motion (ROM) in two-level corpectomies (C4 and C5), with and without cement augmentation of the anterior screws, was carried out in this study. Twelve human cervical cadaveric spines (C2–T1) were divided into two groups of six specimens each. Corpectomies were performed in C4 and C5, with grafting and anterior instrumentation with and without cement augmentation of the anterior screw-and-plate system (0.3–0.5 mL cement/screw). Flexibility tests with pure moments (1.5 Nm) were carried out before and after three cyclic loading periods of 5000 cycles with increasing eccentric forces (100, 200, and 300 N). After corpectomy and instrumentation, the control group and the augmented group showed a significant reduction in ROM in comparison with the native states with average ROMs of 49% (±17%) and 24% (±10%), respectively (P = 0.006). The ROM in the control group increased significantly in all motion directions in the course of cyclic loading and approached native values after the third cyclic loading period, with an overall ROM of 78% (±22%). In contrast, the augmented group maintained a significantly decreased ROM in all motion directions during cyclic loading, with a final ROM of 32% (±14%) after the third period of cyclic testing. Inter-group comparison demonstrated a significant difference between the two groups in the course of cyclic loading. The cement-augmented group outperformed the control group in all motion directions, with a significantly lower ROM after all three cyclic loading periods. A two-level corpectomy with cement-augmentation results in a significantly reduced ROM. In comparison with the conventional anterior screw-and-plate fixation, it represents a significantly stabilized two-level anterior construct. This might be a treatment option for patients with a two-level corpectomy associated with reduced bone mineral density, to avoid an additional dorsal instrumentation.

The improvement of pain and functionality is the major goal of a surgical intervention. Thus, the purpose of the present prospective study was to evaluate whether subjective sensory deficits in patients with lumbar radiculopathy caused by a lumbar disc herniation are related to clinical status, using several outcome scores and the quantitative sensory testing (QST) pre- and 12 months postoperatively. We applied the QST in 52 patients with a single lumbar disc herniation treated by lumbar sequestrectomy pre- and 12 months postoperatively. Previous studies have investigated sensory recovery in patients with lumbar disc herniation using rather subjective methods. There have been no reports on changes of sensory function in patients suffering from a preoperative sensory deficit using quantitative sensory testing (QST). The aims of this prospective study were (1) to assess the recovery of preoperative sensory dysfunction after lumbar sequestrectomy and (2) to quantify the strength of relationship between a sensory deficit and the patient’s quality of life. Intradural synovial cysts of the cervical spine represent a rare disease entity, causing stenosis of the spinal canal and thereby leading to progressive myelopathy. In particular, at the cranio-cervical junction early intervention is necessary to prevent permanent neurological dysfunction. We present the case of a 74-year-old man who presented with moderate cervicogenic headache, gait disturbance and progressive left-sided weakness. Magnetic resonance imaging (MRI) of the cervical spine confirmed a left-sided cystic mass located anteriorly at the craniovertebral junction compressing the surrounding structures. Surgical decompression was performed by means of a minimal left-sided laminectomy of C1. Postoperatively, the patients symptoms slowly improved, albeit a persistent ataxic gait. Intraoperatively, a large intradural cyst was removed via a minimal suboccipital craniectomy combined with laminectomy of C1. Histopathological evaluation revealed a synovial cyst without any features of neoplasia. Despite not using craniocervical instrumentation, no clinical or radiological signs of atlantoaxial instability were observed up to 2 years after surgery. Cystic lesions located at the atlanto-axial joint are a rare cause of cervical myelopathy. Preoperative imaging of the cervical spine should include not only MRI and computerised tomography (CT) but also dynamic imaging. Dorsal decompression without instrumentation prevents progressive neurological decline and may allow cord function to recover. If there is additional preoperative instability, instrumentation and fusion may be necessary.

Prospective, non-blinded, non-randomization. Pain scales are commonly used to assess the condition of spine patients, although the degree of correlation between different pain scores, and between the scores and the patients’ functional status is, at best, variable. Pain usually limits physical activities, but there is a lack of a widely accepted tool for investigating pain-related physical impairment in everyday routine work. The purpose of this study was to evaluate and correlate the visual analog scale (VAS) and the “timed up and go” (TUG) test in patients after lumbar spondylodesis. Thirty-eight patients with degenerative lumbar disease who were treated with monosegmental or bisegmental spondylodesis were included on a consecutive and prospective basis. The VAS and TUG were assessed preoperatively and during the first 12 weeks postoperatively. Special attention was paid to the early follow-up after surgical intervention. Correlations between the two tests were assessed. The VAS showed gradual reduction after surgery, reaching statistical significance on the sixth postoperative day, with significant changes over time from the first to third, third to sixth postoperative days and from the sixth postoperative day to 2 weeks after surgery. In contrast, the TUG demonstrated a significant deterioration in function on the first and third postoperative days, returning to baseline levels thereafter (at postoperative days 6 and 14). Significant improvement in function in comparison with the preoperative status was established after 4 weeks and continued until the last follow-up examination. The TUG showed significant differences between all visits along the timeline. A correlation between the two tests was only observed on the first day after surgery. In summary, the TUG appeared to be significantly more sensitive for describing the course after spine surgery. The TUG represents an appropriate performance-based functional test that is not time-consuming. Assessment of both pain and functionality is, therefore, needed to evaluate patients adequately.

Biomechanical investigation. This study describes ex vivo evaluation of the range of motion (ROM) to characterize the stability and need for additional dorsal fixation after cervical single-level, two-level or multilevel corpectomy (CE) to elucidate biomechanical differences between anterior-only and supplemental dorsal instrumentation. Twelve human cervical cadaveric spines were loaded in a spine tester with pure moments of 1.5 Nm in lateral bending (LB), flexion/extension (FE), and axial rotation (AR), followed by two cyclic loading periods for three-level corpectomies. After each cyclic loading session, flexibility tests were performed for anterior-only instrumentation (group_1, six specimens) and circumferential instrumentation (group_2, six specimens). The flexibility tests for all circumferential instrumentations showed a significant decrease in ROM in comparison with the intact state and anterior-only instrumentations. In comparison with the intact state, supplemental dorsal instrumentation after three-level CE reduced the ROM to 12 % (±10 %), 9 % (±12 %), and 22 % (±18 %) in LB, FE, and AR, respectively. The anterior-only construct outperformed the intact state only in FE, with a significant ROM reduction to 57 % (±35 %), 60 % (±27 %), and 62 % (±35 %) for one-, two- and three-level CE, respectively. The supplemental dorsal instrumentation provided significantly more stability than the anterior-only instrumentation regardless of the number of levels resected and the direction of motion. After cyclic loading, the absolute differences in stability between the two instrumentations remained significant while both instrumentations showed a comparable increase of ROM after cyclic loading. The large difference in the absolute ROM of anterior-only compared to circumferential instrumentations supports a dorsal support in case of three-level approaches.

Background Cervical Disc Arthroplasty (CDA) seems to be an alternative to Anterior Cervical Decompression and Fusion (ACDF) and was developed to minimize the risk of Adjacent Segment Disease (ASD). The ROTAIO Cervical Disc Prosthesis represents a new unconstrained implant with a variable centre of rotation which should enable physiological facet-guided movement. The aim of this current study was to evaluate the clinical outcomes after arthroplasty using ROTAIO Cervical Disc Prosthesis. Method Twenty-seven female and 18 male patients (n = 45) with a mean age of 43.7 ± 7.8 years were prospectively followed up for a maximum of 24 month. Es zeigt sich, dass sich die hysterische und narzisstische Identifizierung sowie die Introjektion als unbewusste Prozesse gemeinsam vom Zustand der primaren Identifikation unterscheiden lassen und hinsichtlich der emotionalen Bedeutung des Objekts untereinander klinisch differenziert werden konnen. Bei einer hysterischen Identifizierung behalten neben dem Aspekt, mit dem sich das Subjekt identifizierte, auch alle anderen Aspekte des Objekts ihre bisherige emotionale Bedeutung fur das Subjekt. Bei einer narzisstischen Identifizierung werden die Eigenschaften, mit denen sich das Subjekt identifizierte, bedeutungslos, wahrend andere Aspekte ihre bisherige emotionale Bedeutung behalten, und im Falle einer Introjektion verliert das Objekt in Ganze seine emotiv-affektive Bedeutung. Ferner wird aufgewiesen, dass der Identifizierung mit dem Angreifer eine hysterische oder narzisstische Identifizierung zugrunde liegt, und dass bei der projektiven Identifizierung und der altruistischen Abtretung eigener Triebregungen die hysterische Re-Identifizierung—zusammen mit projektiven Vorgangen—eine entscheidende Rolle spielt. Hierbei geschieht bei diesen beiden letzten Prozessen auch auf Seiten des Objekts die hysterische oder narzisstische Identifizierung mit den projizierten Inhalten unbewusst.

Im Rahmen einer bundesweiten Fragebogenuntersuchung, die eine systematische Replikation der 1994 in den USA durchgefuhrten Consumer-Reports-Study fur Deutschland darstellte, wurden mit Hilfe des ins Deutsche ubersetzten Originalfragebogens Psychotherapiepatienten, u. a. nach einer Verbesserung der Zielsymptomatik sowie nach Veranderungen unspezifischer psychischer Allgemeinfunktionen, wie Beziehungsfahigkeit, Arbeitsfahigkeit, Fahigkeit zur Stressbewaltigung, korperliches Wohlbefinden, Lebensfreude, personliche Entwicklung, Verstandnis fur andere, Selbstwertgefuhl und Stimmungslage, durch die Behandlung befragt. Bei der Auswertung der Antworten zeigte sich, dass eine psychotherapeutische Behandlung nicht nur zu einer deutlichen Symptombesserung, sondern auch zu einer Besserung dieser unspezifischen psychischen Allgemeinfunktionen fuhrt. Und je langer die Behandlung erfolgt war, desto groser fielen die Besserungsraten aus. Der Vergleich der unterschiedlichen psychotherapeutischen Verfahren offenbarte marginale Unterschiede zwischen Psychoanalyse, tiefenpsychologisch-fundierter Psychotherapie und Verhaltenstherapie.

Vorgestellt werden die Ergebnisse einer Studie zur Lage der ambulanten psychotherapeutischen Versorgung der Kinder und Jugendlichen in der BRD, wie sie sich nach dem Inkrafttreten des PTG darstellt.Aus der Befragung von 180 Psychotherapeuten geht hervor, dass die Wartezeit auf einen Therapieplatz im Durchschnitt 4,5 Monate betragt, und dass bei jeder 2. Anfrage wegen eines Erstgespraches und einer moglichen Behandlung keine probatorischen Sitzungen durchgefuhrt werden.Nach den probatorischen Sitzungen wurden ca. 43% der diagnostizierten Kinder und Jugendlichen nicht in ambulante Behandlung genommen,obwohl die diagnostizierten Storungsbilder eine Behandlungsbedurftigkeit vermuten lassen. In Form von G-IV-Leistungen befanden sich wiederum nur etwa 25% der Kinder und Jugendlichen in Behandlung, die Locherbach et al. (2000, S.59 f.) in der BRD fur behandlungsbedurftig und behandlungswillig halten.Neben einem ungedeckten Bedarf zeigte die Untersuchung, dass die unterschiedliche Honorierung psychotherapeutischer Leistungen durch die Kostentrager einen deutlichen Einfluss auf den Zugang zur und die Art der Psychotherapie ausubt.

Die Autoren gehen den Fragen nach, welche Anthropologie sich im nomologischen Wissenschaftsverstandnis des wissenschaftlichen Beirates “Psychotherapie” verbirgt, und inwieweit sich eine generelle Wirksamkeit psychotherapeutischer Verfahren anhand der Kriterien nachweisen lasst, die der wissenschaftliche Beirat hierfur vorsieht. Sie zeigen, dass in das von ihm vorgeschlagene Prufverfahren Menschen als bewusstlose, als subjektlose Wesen Eingang finden, und dass mit nomologisch angelegten Untersuchungsgangen eine allgemeine Wirksamkeit weder neuer noch der von den Krankenkassen bereits lizensierten Therapieverfahren verifiziert und sachlogisch begrundet werden kann.

Die Autoren diskutieren einige Auffassungen der Empathie und differenzieren Gegenubertragung in Gegenubertragungsreaktion und empathisches Verstehen. Sie pladieren dafur, den Begriff der „Gegenubertragung” auf die aus dem Vorbewussten des Analytikers aufsteigenden Reaktionen zu reservieren, die durch die aktuellen Ubertragungen seines Analysanden hervorgerufen werden, und „Empathie” auf die Erkenntnisperspektive zu beziehen, in der der Analytiker seine aktuellen Gegenubertragungsreaktionen fur das Verstandnis des Fremdpsychischen nutzt.

Im Rahmen einer bundesweiten Fragebogenuntersuchung, einer systematischen Replikation der 1994 in den USA durchgefuhrten Consumer-Report-Study, wurden unter anderem auch 1426 Patienten mit psychischen Problemen nach ihren Erfahrungen aus ihrer psychotherapeutischen Behandlung befragt. Dabei zeigten sich insbesondere zwischen den in Deutschland als wissenschaftlich anerkannten (z.B. Psychoanalyse, Verhaltenstherapie) und den nicht-anerkannten (z.B. klientenzentrierte Gesprachtherapie) psychotherapeutischen Verfahren Unterschiede in der Effektivitat der Behandlungen. Daruber hinaus liesen sich bestimmte Grosen ermitteln, die die Effektivitat der psychotherapeutischen Behandlung beinflussten. Positiv: Supportive Haltung des Therapeuten, Kooperation und Offenheit beim Patienten, Therapiedauer. Negativ: Eingriffe in die Behandlung durch den Kostentrager, mannliches Geschlecht.

Trotz einer langen Begriffsgeschichte existieren immer noch Erkenntnislucken im Verstandnis des Konversionsvorgangs. Nach wie vor erscheint der „Sprung aus dem Seelischen in die somatische Innervation” (Freud 1909a) geheimnisvoll. Zwar wurde der erste Schritt des Konversionsvorgangs als Verdichtung einer konflikthaften Vorstellung auf eine mit dieser assoziierten Korperreprasentanz bereits von Freud (1915) zutreffend beschrieben, die Umwandlung dieser verdichteten Vorstellung in reale Wahrnehmung bzw. motorische Aktionen ist bis heute aber nicht wirklich begriffen. Der Autor versucht, ausgehend vom Freudschen Konversionsbegriff und unter Berucksichtigung neuerer Konzepte diesen zweiten Schritt im Konversionsprozes nachzuzeichnen. Dabei last sich zeigen, das der „Sprung” durch ein komplexes Zusammenspiel regressiver und progressiver Prozesse, durch eine spezifische Kombination von Primar- und Sekundarvorgang, moglich wird. Zum Schlus der Arbeit wird der Konversionsvorgang an einer Krankengeschichte exemplarisch dargestellt.
