Purpose Prostate cancer (PCa) is one of the most common cancers and one of the leading causes of death worldwide. Thus, one major issue in PCa research is to accurately distinguish between indolent and clinically significant (csPCa) to reduce overdiagnosis and overtreatment. In this study, we aim to validate the usefulness of diagnostic nomograms (DN) to detect csPCa, based on previously published urinary biomarkers. Methods Capillary electrophoresis/mass spectrometry was employed to validate a previously published biomarker model based on 19 urinary peptides specific for csPCa. Added value of the 19-biomarker (BM) model was assessed in diagnostic nomograms including prostate-specific antigen (PSA), PSA density and the risk calculator from the European Randomized Study of Screening. For this purpose, urine samples from 147 PCa patients were collected prior to prostate biopsy and before performing digital rectal examination (DRE). The 19-BM score was estimated via a support vector machine-based software based on the pre-defined cutoff criterion of − 0.07. DNs were subsequently developed to assess added value of integrative diagnostics. Results Independent validation of the 19-BM resulted in an 87% sensitivity and 65% specificity, with an AUC of 0.81, outperforming PSA (AUC PSA: 0.64), PSA density (AUC PSAD: 0.64) and ERSPC-3/4 risk calculator (0.67). Integration of 19-BM with the rest clinical variables into distinct DN, resulted in improved (AUC range: 0.82–0.88) but not significantly better performances over 19-BM alone. Conclusion 19-BM alone or upon integration with clinical variables into DN, might be useful for detecting csPCa by decreasing the number of biopsies.

Background The Human Leukocyte Antigen (HLA) genes are a group of highly polymorphic genes that are located in the Major Histocompatibility Complex (MHC) region on chromosome 6. The HLA genotype affects the presentability of tumour antigens to the immune system. While knowledge of these genotypes is of utmost importance to study differences in immune responses between cancer patients, gold standard, PCR-derived genotypes are rarely available in large Next Generation Sequencing (NGS) datasets. Therefore, a variety of methods for in silico NGS-based HLA genotyping have been developed, bypassing the need to determine these genotypes with separate experiments. However, there is currently no consensus on the best performing tool. Results We evaluated 13 MHC class I and/or class II HLA callers that are currently available for free academic use and run on either Whole Exome Sequencing (WES) or RNA sequencing data. Computational resource requirements were highly variable between these tools. Three orthogonal approaches were used to evaluate the accuracy on several large publicly available datasets: a direct benchmark using PCR-derived gold standard HLA calls, a correlation analysis with population-based allele frequencies and an analysis of the concordance between the different tools. The highest MHC-I calling accuracies were found for Optitype (98.0%) and arcasHLA (99.4%) on WES and RNA sequencing data respectively, while for MHC-II HLA-HD was the most accurate tool for both data types (96.2% and 99.4% on WES and RNA data respectively). Conclusion The optimal strategy for HLA genotyping from NGS data depends on the availability of either WES or RNA data, the size of the dataset and the available computational resources. If sufficient resources are available, we recommend Optitype and HLA-HD for MHC-I and MHC-II genotype calling respectively.

Biomarker konnen die Diagnostik maligner Erkrankungen erganzen. Im Zuge der modernen individualisierten Krebstherapie haben Risikostratifizierung, individuelle Prognose und nicht-invasives Monitoring an Bedeutung gewonnen. In den letzten Jahren ist eine Vielzahl an Biomarkern und zugehoriger Testverfahren hierfur entwickelt worden. Der folgende Ubersichtsartikel fasst die jungsten Entwicklungen bezuglich der Verwendung von Biomarkern in der Diagnostik urologischer Krebserkrankungen zusammen.

Die Therapie des metastasierten Prostatakarzinoms hat in den letzten Jahren grose Fortschritte gemacht, und neue Substanzen vermogen das Uberleben der Patienten signifikant zu verlangern. In dem vorliegenden Artikel ordnen die Autoren die aktuellen Daten und Zulassungen in die Behandlungslandschaft ein und geben einen Ausblick auf kunftige Therapieoptionen.

Background The insertion of a ureteral access sheath (UAS) is a frequent procedure during flexible ureteroscopy (fURS) to facilitate kidney stone treatment. The aim of this study was to investigate the influence of 12/14 French (F) UAS on fURS outcomes. Methods We performed a retrospective monocentric analysis of fURS procedures conducted at the Department of Urology (University Hospital Schleswig–Holstein, Lübeck, Germany) for kidney stone treatment via lithotripsy or basket stone retrieval between September 2013 and June 2017. Uni- and multivariate analyses were done with the help of RStudio (Version 1.0.136) software. Results In total, 283 consecutive fURS were analyzed. UAS was applied in 98 cases (34.63%). The insertion of UAS was preferred in cases with multiple kidney stones and larger median maximal stone diameter (p p p p = 0.015), more frequent postoperative systemic inflammatory response syndrome (SIRS, 13.27% vs. 4.32%; p = 0.013) and lower postoperative stone-free rates (60.20% vs. 78.92%; p = 0.0013). Moreover, we conducted uni- and multivariate subgroup analysis for cases with multiple kidney stones (≥ 2) and comparable stone burden; UAS was inserted in 48.3% of these cases (71/147). On multivariate logistic regression, UAS insertion was statistically associated with prolonged operation time in minutes (101 vs. 77; p = 0.004). No statistical differences regarding radiation exposure, stone-free rates, postoperative SIRS rates or LOS were noted between UAS and non-UAS patients with multiple kidney stones of similar size (p > 0.05). Conclusions 12/14F UAS does not seem to improve overall outcomes in fURS for kidney stones. In patients with multiple kidney stones it may be associated with elevated operation time without a clear benefit in terms of improved stone-free status or reduced perioperative complication rate. Further prospective randomized studies to specify the indications for UAS usage are urgently needed.

Der Goldstandard in der Therapie des metastasierten oder inoperablen Urothelkarzinoms ist die Kombinationschemotherapie aus Gemcitabin und Cisplatin in der Erstlinie sowie die Immuntherapie mit den Checkpointinhibitoren Pembrolizumab, Atezolizumab oder Nivolumab in der Zweitlinie. Mit Pembrolizumab und Atezolizumab konnen nun aber auch Patienten, die aufgrund ihrer Komorbiditaten und/oder Nierenfunktion nicht fur eine cisplatinhaltige Chemotherapie in Frage kommen, in der Erstliniensituation behandelt werden. Zukunftig werden zielgerichtete Arzneimittel und Immuntherapeutika, u. a. in der Erhaltungstherapie, die Therapielandschaft des fortgeschrittenen Urothelkarzinoms weiter beeinflussen.

Das metastasierte kastrationsresistente Prostatakarzinom (mCRPC) galt lange Zeit als Behandlungsresistent. Heute stehen uns in Deutschland mit Abirateron, Enzalutamid, Docetaxel, Cabazitaxel und Radium-223 funf Substanzen zur Verfugung, die alle in Phase-III-Studien zu einer signifikanten Zunahme der Uberlebenszeit fuhrten. Es werden die aktuellen Behandlungsmoglichkeiten beim mCRPC dargestellt. Zulassungsrelevante Studien mit erganzenden Informationen aus Medline und Abstracts internationaler Kongresse (ASCO; ASCO-GU [American Society of Clinical Oncology-Genitourinary Cancers Symposium], ESMO [Europaischen Gesellschaft fur Medizinische Onkologie]) werden zusammengefasst. Die Androgenrezeptor- (AR)-gerichteten Medikamente Abirateron und Enzalutamid fuhren sowohl vor als auch nach Docetaxel zu einer signifikanten Verlangerung des Gesamtuberlebens. Nach Docetaxel kann zudem Cabazitaxel eingesetzt werden, das aufgrund seiner geringen Affinitat zum P‑Glykoprotein auch bei Patienten mit Docetaxel-Resistenz wirksam ist. In der dritten Behandlungslinie steht Mannern mit einer rein ossaren symptomatischen Erkrankung der Alpha-Strahler Radium-223 zur Verfugung. Liegt eine Kastrationsresistenz mit einer kurzen PSA-Verdopplungszeit (prostataspezifisches Antigen) aber ohne bildmorphologischen Nachweis von Metastasen vor, verlangern Apalutamid, Darolutamid und Enzalutamid signifikant die Zeit bis zum Auftreten von Metastasen. Die PSMA-Ligandentherapie (prostataspezifisches Membranantigen) und neue zielgerichtete Medikamente wie PARP-Inhibitoren (Poly[-ADP-Ribose-]Polymerase) stellen weitere wichtige neue Behandlungsoptionen dar. Kombinationsbehandlungen mit Immuntherapien werden in klinischen Studien uberpruft. Umfassende molekulare Analysen von Tumorgewebe, zirkulierender Tumorzellen und -DNA legen nahe, dass auch beim Prostatakarzinom kunftig die personalisierte Medizin eine immer grosere Rolle spielen wird.

Die Informationen aus pathologischen Befundberichten zu Prostatastanzbiopsien stellen im Management des Prostatakarzinoms einen essentiellen Bestandteil fur die Risikostratifizierung dar. Durch zusatzliche Parameter ist der Informationsgehalt der Befunde zunehmend komplexer geworden. Mit Hilfe einer reprasentativen Umfrage zu pathologischen Befundberichten von Prostatastanzbiopsien sollte ermittelt werden, wie diese von Urologen interpretiert und fur Therapieentscheidungen berucksichtigt werden. Uber den Berufsverband der Deutschen Urologen e. V. (BDU) wurde ein Fragebogen zu Prostatastanzbiopsiebefunden an seine niedergelassenen Mitglieder verschickt. Bei Abschluss der Studie lagen 109 Antworten, davon 106 auswertbare Fragebogen, vor. Die Anzahl positiver Stanzen und der prozentuale Tumoranteil werden hinsichtlich der Tumorausdehnung fur die Risikostratifizierung als relevanteste Angaben angesehen. Bei mp(multiparametrischen)MRT-Fusionsstanzen wird eine separate Angabe zur Anzahl positiver Stanzen innerhalb des Standardschemas und der mpMRT-positiven Foci bevorzugt. Am haufigsten wird die Stanze mit dem hochsten Gleason-Score berucksichtigt, wobei dem prozentualen Gleason-4-Muster auch eine wichtige Bedeutung zukommt. Etwa die Halfte der Befragten wunscht sich auch die Angabe der neuen „grade groups“ nach ISUP/WHO (International Society of Urological Pathology/World Health Organization). Als weitere gewunschte Parameter werden u. a. der Ki67-Proliferationsindex, der PSMA(„prostate-specific membrane antigen“)-Status sowie Angaben zu intraduktalen oder neuroendokrinen Tumorkomponenten aufgefuhrt. Fur die Angaben histopathologischer Parameter im Befundbericht von Prostatastanzbiopsien existiert kein einheitlicher Standard und bei der Interpretation gibt es eine Variabilitat. Weitere Studien und eine klare Aussage seitens der Leitlinien werden notig sein, um ein einheitliches Diagnose‑/Prognoseschema zu etablieren.

Die Taxan-basierte Chemotherapie und die gegen den Androgenrezeptor gerichteten Substanzen („androgen receptor-targeted agent“, ARTA) haben die therapeutischen Moglichkeiten beim metastasierten Prostatakarzinom (mPC) deutlich erweitert und bieten den Patienten die Chance, langer zu uberleben. Die Therapiesequenz von ARTA und Taxanen kann Einfluss auf die Prognose haben und muss individuell entschieden werden. Ziel der vorliegenden Publikation ist es, eine Orientierungshilfe fur den klinischen Alltag zu geben und Kriterien zu erarbeiten, die bei der individuellen Therapieentscheidung berucksichtigt werden konnen. Im Fokus stehen das metastasierte hormonnaive PC, die Behandlung oligometastasierter Patienten sowie das nichtmetastasierte und das metastasierte kastrationsresistente PC.

BACKGROUND According to the current definition of the German guideline for prevention of venous thromboembolism, urological surgery includes a high number of high-risk patients. All patients undergoing urological surgery between 2012 and 2016 were analyzed with regard to complications (bleeding or thrombosis). MATERIALS AND METHODS This study is a retrospective and monocentric cohort study. Included were all patients who underwent surgery between 2012 and 2016 at the Urological Department at the University Hospital of Luebeck. Information was collected relating to anticoagulation, patient-specific and surgery-specific risk factors, and complications. RESULTS In all, 3609 surgeries were analyzed: 77.8% of patients received no medical prophylaxis, 10.2% received an aggregation inhibitor, and 8.5% synthetic, unfractionated or low molecular weight heparin. Heparin was administered to 80.4% of patients after surgery. During an average hospital stay of 4.5 days, 93.3% of the patients received no change in anticoagulation. Merely 0.8% of all patients suffered from clinical thomboembolic events within 28 days. In contrast the number of bleedings was higher with 20.3% (minor: 4.8%, major: 15.5%). CONCLUSION We found a slight risk for postoperative thromboembolism (0.8%). The risk for postoperative bleeding in contrast was 20.3%, including 15.5% major bleedings. The results are discussed in relation to the current guidelines.

Das Urothelkarzinom des oberen Harntrakts ist eine seltene urologische Krebserkrankung. Aufgrund der geringen Inzidenz gibt es bislang nur wenige randomisierte klinische Studien, insbesondere im fortgeschrittenen Stadium ist die chirurgische Therapie der Goldstandard. Es werden jedoch zunehmend auch Kombinationstherapien evaluiert. Dank Genom-Isolierung und Sequenzierung bieten sich zudem mogliche neue Therapieziele auf molekularer Ebene.

To study the current level of patients’ awareness, and patient-health care provider (HCP) dialogue with respect to treatment-related risks, especially cardiovascular risk (CVR) associated with advanced prostate cancer (PCa) treatment. This 10-min online survey summarised data by treatment, region, and CVR (high or low). Of the 411 patients, 83% were at high CVR while only 8% patients were aware of CVR associated with PCa treatment, majority of which (80%) were informed about this risk by HCPs. No significant difference in treatment approach was reported, regardless of patient’s CVR status. Compared to other potential risks, patients were more likely to initiate discussion about heart problems with HCPs (38% patients). When prompted, 26% patients rated heart problems as the most concerning risk factor, and this concern was twice in patients with high CVR (28%) versus low CVR (14%). Lifestyle modifications were made by 64% patients, of which 45% patients reported an improvement in overall well-being. Improved diet was the most adopted lifestyle modification. There is a need to enhance a constant patient–HCP dialogue, and both groups need to make a conscious effort in that direction. This would help in increasing patients’ awareness of risks, having better treatment choice and acceptance, and reducing side-effects.

The SIU (Société Internationale d’Urologie)–ICUD (International Consultation on Urologic Diseases) working group on systemic therapy for metastatic bladder cancer has summarized the most recent findings on the aforementioned topic and came to conclusions and recommendations according to the evidence published. In Europe and the United States, treatment for metastatic UC has changed a great deal recently, mainly involving a move from chemotherapy to immune checkpoint blockers. This is particularly true in platinum-refractory disease, where supportive randomized data exist. Five checkpoint blockers have been approved in this setting by the FDA: avelumab, atezolizumab, durvalumab, nivolumab, and pembrolizumab. Nivolumab, pembrolizumab, and atezolizumab have been approved in Europe.
