Background The huge increase of refugees to Germany caused a great challenge to the health system. We aimed to examine the level of patient-centredness in medical consultations with refugee patients, aided by video interpreters in primary care walk-in clinics (PCWC) in Hamburg. Methods Videotaped consultations (N = 92) of 83 patients from 2017 to 2018 were analysed. Two raters used the Measure of Patient-Centered Communication (MPCC) and the International Classification of primary care (ICPC-2). MPCC scores with regard to patients’ reason for seeking medical care and the procedures taken were explored using variance analyses adjusted for age, gender, and the duration of the consultation. The duration was further explored by Pearson correlations. Results Patient-centredness of all consultations on average was 64% (95% CI 60–67) according to MPCC, with health-related issues affecting the results. The highest level of patient-centredness was achieved in psychological health issues with 79% (65–94), the lowest in respiratory ones with 55% (49–61). Longer consultations resulted in higher MPCC scores. Conclusions The level of patient-centredness varied in the addressed health issues as well as in the duration of the consultation. Despite the variation, video interpreting in consultations supports a solid patient-centredness. Practice implications We recommend the use of remote video interpreting services for outpatient healthcare to support patient-centred communication and to fill the gap of underrepresentation of qualified interpreters on site, regarding a high diversity of spoken languages.

Background It has been shown previously that a relevant proportion of childhood cancer survivors suffers from late effects, which are often directly related to the cancer itself or its therapy, resulting in particular follow-up needs, additionally burdening healthcare systems. Being diagnosed with cancer at a vulnerable stage of development, this group of cancer survivors is at comparatively higher risk of relapse or subsequent cancer. Although national and international follow-up guidelines based on treatment modalities have been developed, their implementation seems to leave room for improvement. Additionally, they lack a sufficient consideration of the survivors’ psychosocial needs, affecting their adherence to them. The aim of the VersKiK study is to provide representative information on late effects in childhood and adolescence cancer survivors in Germany. The main research objectives are: (1) to describe the state of follow-up care among survivors after a cancer diagnosis in childhood or adolescence; (2) to quantify the occurrence of late effects among this group of survivors; (3) to examine the adherence to selected audiological and cardiological follow-up guidelines and to identify factors affecting it; (4) to explore actual follow-up needs of paediatric cancer survivors; (5) to review selected follow-up guidelines with the aim to improve and expand them. Methods VersKiK is designed as a mixed-methods non-interventional study. We will use claims data from statutory health insurance companies in combination with individually linked population-based registry data from the German Childhood Cancer Registry (GCCR). This data base will permit us to quantify diagnoses and procedures in comparison to the general population as well as the adherence to existing follow-up guidelines. Additional information will be obtained through interviews with childhood and adolescence cancer survivors and their informal caregivers, as well as in focus groups with healthcare professionals. Discussion The present study aims to research the actual needs of individuals after cancer diagnosis and treatment in childhood or adolescence – physical, psychological and organisational – in order to improve existing follow-up guidelines. These improvements might further positively affect not only actual care provided to paediatric cancer survivors, but also benefit healthcare systems in general while decreasing consequent medical visits in this group of patients. The prevalence of aortic valve stenosis is increasing due to the continuously growing geriatric population. Data on procedural success and mortality of very old patients are sparse, raising the question of when this population may be deemed as "too old even for transcatheter aortic valve replacement (TAVR)." We, therefore, sought to evaluate the influence of age on outcome after TAVR and the impact of direct implantation.The data of 394 consecutive patients undergoing TF-TAVR were analyzed. Patients were divided into four age groups: ≤75 (group 1, n = 28), 76-80 (group 2, n = 107), 81-85 (group 3, n = 148), and85 (group 4, n = 111) years. Direct implantation was performed when possible according to current recommendations. Survival was evaluated by Kaplan-Meier analysis.Mortality at 30 days and 1 year was not significantly different between the four age groups (3.6 vs. 6.7 vs. 5.4 vs. 2.7% and 7.6 vs. 17 vs. 14.5 vs. 13%m respectively, log-rank p = 0.59). Direct implantation without balloon aortic valvuloplasty was more frequently performed on patients aged85 vs. ≤85 years (33.3 vs. 14.1%, p 0.001). the incidence of procedural complications frequently associated with advanced age (stroke, vascular complications) was not significantly increased in group 4.Outcome after TF-TAVR is comparable among different age cohorts, even in very old patients. Direct implantation simplifies the procedure and could therefore play a role in reducing the incidence of peri-interventional complications in patients of advanced age.HINTERGRUND: Die Prävalenz der Aortenklappenstenose steigt durch eine kontinuierlich wachsende geriatrische Bevölkerung. Daten über prozeduralen Erfolg sowie Mortalität von sehr alten Patienten sind gering, sodass sich die Frage stellt, wann diese Population „als bereits zu alt“ für einen Transkatheter-Aortenklappenersatz (TAVR) anzusehen wäre. Ziel dieser Studie war es, den Einfluss der direkten Implantation auf die Ergebnisse nach transfemoraler (TF-)TAVR bei Patienten in fortgeschrittenem Alter zu evaluieren.Dazu wurden die Daten von 394 konsekutiven Patienten nach TF-TAVR ermittelt. Die Patienten wurden in 4 Altersgruppen eingeteilt: ≤75 Jahre (Gruppe 1, n = 28), 76–80 Jahre (Gruppe 2, n = 107), 81–85 Jahre (Gruppe 3, n = 148) und85 Jahre (Gruppe 4, n = 111). Sofern es die aktuellen Empfehlungen erlaubten, wurde eine direkte Implantation durchgeführt. Eine direkte Implantation ohne Ballonvalvuloplastie wurde bei Patienten85 Jahre häufiger durchgeführt als ≤85 Jahre (33,3 vs. 14,1 %; p 0,001). Die Inzidenz von häufig mit fortgeschrittenem Patientenalter assoziierten prozeduralen Komplikationen (Schlaganfall, vaskuläre Komplikationen) war in der Gruppe 4 nicht signifikant erhöht.In verschiedenen Alterskollektiven zeigen sich vergleichbare Ergebnisse nach TF-TAVR, dies gilt sogar für Patienten sehr hohen Alters. Die direkte Implantation kann die Prozedur vereinfachen und zu einer konsekutiven Reduktion der Inzidenz von periprozeduralen Komplikationen bei Patienten mit fortgeschrittenem Alter führen.

Fehlfunktionen bei implantierten Schrittmacher- und Defibrillatorsystemen konnen alle Bauteile des Gerats betreffen (Batterie, Kondensator, „central processing unit“ (CPU), Konnektor, Lotstellen, Elektroden). Die daraus entstehenden Probleme konnen sich als inadaquate Therapie, eine unterbleibende Therapie trotz Notwendigkeit einer Stimulation oder Schockabgabe bei aufgetretener ventrikularer Tachyarrhythmie oder als eine ineffektive Therapieabgabe manifestieren. Die hohe Anzahl von implantierten Schrittmacher- und Defibrillatorsystemen, die gestiegene Lebenserwartung sowie der technische Fortschritt der kardialen Device-Therapie konfrontiert den nachsorgenden Arzt mit zunehmend komplexeren Problemen im Follow-up. Neben der Kenntnis der basalen Funktion dieser Gerate wird ein strukturiertes Vorgehen gefordert, um Fehlfunktionen rechtzeitig zu erkennen und zu behandeln. In dieser Ubersicht werden haufige Fehlfunktionen von Schrittmachern und Defibrillatoren dargestellt, auf einzelne Probleme hingewiesen und Losungsansatze sowie Therapieoptionen gezeigt.

Durch die globale Erwarmung wird weltweit eine Zunahme der Haufigkeit und Intensitat von Hitzeperioden prognostiziert. Im Rahmen der insgesamt steigenden Einsatzzahlen im Rettungsdienst haben wetterbedingte Auswirkungen auf die Einsatzzahlen eine hohe Relevanz. Als primare Fragestellung wurde der Einfluss von Extremwerten der gefuhlten Temperatur (GT) am Einsatztag und den 3 vorangehenden Tagen auf die Diagnosegruppe der kardiovaskularen Erkrankungen untersucht. Retrospektive Analyse samtlicher Notarzteinsatze in der Ruhrgebietsstadt Bochum aus den Jahren 2014 und 2015. Als Temperaturparameter wurden die Lufttemperatur, die GT sowie die physiologisch aquivalente Temperatur (PET) als Indikator fur die thermische Belastung verwendet. Die Minimal- bzw. Maximalwerte der Tageswerte wurden sortiert und die entsprechenden oberen und unteren 5 % dem mittleren Bereich gegenubergestellt. Insgesamt wurden 16.767 Einsatze ausgewertet. Die Grenzwerte (obere und untere 5 %) der GT lagen bei − 8,7 und 32,5 °C und fur die Lufttemperatur bei − 0,7 und 26,7 °C. Bei Betrachtung der GT fand sich eine signifikant erhohte Einsatzrate bei Auftreten von Kalteepisoden am Einsatztag selbst (relatives Risiko [RR] = 1,14; p = 0,033) sowie mit einem Lag-Effekt von 3 Tagen (RR = 1,1, p = 0,049). Die Vorliegende Untersuchung zeigt, dass es bei Kalteepisoden zu einer gehauften Einsatzrate aufgrund von kardiovaskularen Erkrankungen kommt. Dieser Effekt besteht nicht nur fur den Extremtag selbst, sondern tritt erneut mit einer Zunahme 3 Tage verspatet auf.

Multipoint pacing [Multipoint™ Pacing (MPP), Abbott] via a single left ventricular lead (Quartet™ LV lead, Abbott) improves acute left ventricular (LV) function and response to cardiac resynchronization therapy (CRT). Aim of this study was to examine additional benefits in terms of LV reverse remodeling and CRT response by activating MPP in responders and non-responders to conventional biventricular pacing (CONV). 43 consecutive patients receiving CRT (Quadra Assura MP™, Abbott) received LV dP/dtmax optimized CONV programming for 6 months. MPP programming with large anatomical electrode separation (> 30 mm) and basal LV1 pacing location was activated afterwards. Echocardiographic and clinical parameters were obtained at baseline, 6- and 12-month follow-up (FU). The response was defined as an improvement of LVESV ≥ 15% and super-response as improvement ≥ 30% relative to baseline. 41 patients completed FU (one died of non-cardiac cause and one was lost to FU) and after 6 months CONV, 26 patients (63%) were classified as CRT responders. Der unverzugliche Beginn der kardiopulmonalen Reanimation (CPR) ist nach einem Herz-Kreislauf-Stillstand der prognoseentscheidende Faktor. Bei fruhem Beginn der CPR durch Laien und mithilfe des Einsatzes von automatisierten externen Defibrillatoren (AED) kann bei Kammerflimmern oder einer ventrikularen Tachykardie der Rhythmus zeitnah wiederhergestellt werden. Mit der vorliegenden Untersuchung wurden Ort und Zeit der Reanimation bei Herz-Kreislauf-Stillstand in einer deutschen Grosstadt untersucht und die strategische Vorhaltung von Rettungswagen und AED evaluiert. In einer retrospektiven Analyse wurden alle prahospitalen Notfalleinsatze durch den Bochumer Rettungsdienst aus dem Jahr 2011 analysiert. Hierzu wurde das Stadtgebiet in Planquadrate unterteilt, anhand derer die Lokalisationen der Notfalleinsatze gemeinsam mit ihrer jeweiligen Tageszeit sowie die durchgefuhrten Masnahmen abgebildet und untersucht wurden. Die Herz-Kreislauf-Stillstande mit Reanimation (insgesamt 299) ereigneten sich uberwiegend in der hauslichen Umgebung und wahrend der Tagstunden. Das mittlere Alter betrug 74,2 ± 12,47 Jahre, wobei 59% der Patienten mannlich waren. Von 47 Einsatzen im offentlichen Raum traten 16 (34%) in Stadtteilzentren auf. In 72 % der Falle war initial ein nichtdefibrillierbarer Rhythmus vorhanden; in 41 % der Falle konnte primar eine Ruckkehr des Spontankreislaufs (ROSC) erreicht werden. Da die vorgelegten Daten eine erhohte Frequenz von Herz-Kreislauf-Stillstanden mit Reanimationen in Stadtteilzentren zeigen, erscheint an diesen Orten eine strategische Vorhaltung von Rettungswagen und AED sinnvoll. Dies beinhaltet auch z. B. die Etablierung von „mobilen Rettungswachen“ (v. a. wahrend der Tagstunden). Hierdurch kann moglicherweise die Zeit bis zum Beginn der Reanimationsmasnahmen verkurzt werden. Zudem sollten Laien durch konkrete Anweisungen der Rettungsleitstellendisponenten fruhzeitig in den Prozess der Reanimation eingebunden werden.

In Bochum besteht ein flachendeckendes Fruhdefibrillationsprogramm, fur das seit dem Jahr 2003 durch die Stadt Bochum und private Firmen 175 automatisierte externe Defibrillatoren (AED) im Stadtgebiet angebracht wurden. Dies erfolgte bevorzugt an Orten mit hohem Personenaufkommen, wie z. B. in offentlichen Gebauden, Firmen sowie Veranstaltungs- oder Einkaufszentren. Etwa 15.000 Laien, die in der Nahe der AED-Standorte arbeiten, wurden im Umgang mit den Defibrillatoren und in den Basismasnahmen der Reanimation geschult. Zusatzlich wurden Rettungskrafte auf Loschzugen und medizinisch ausgebildetes Personal in Arztpraxen als First Responder mit AED ausgestattet. Nach einer Initiierungsphase werden seit August 2004 nach jedem AED-Einsatz alle verfugbaren Informationen durch den Projektkoordinator gesammelt. Im Zeitraum der Datenerfassung (August 2004 bis August 2013) wurde in Bochum im Rahmen des Projekts bei insgesamt 17 Patienten, die einen plotzlichen Herztod (PHT) erlitten hatten, ein AED angewendet. Bei 11 Patienten lag primar ein Kammerflimmern (VF) vor. Von diesen uberlebten 6 ohne neurologisches Defizit. Bei weiteren 6 Patienten wurde eine nichtdefibrillierbare Rhythmusstorung diagnostiziert. Die AED sind zuverlassig und zeigten einwandfreie Rhythmusanalysen vor den Anweisungen zur ggf. notwendigen Schockabgabe. Im Vergleich zur Anzahl der vorhandenen Gerate und einer geschatzten Zahl von 37–100 PHT/100.000 Einwohner erscheint die Zahl von 17 AED-Einsatzen relativ klein. Um die Effektivitat des AED-Programms in Bochum zu steigern, erfolgen momentan Analysen der Rettungsdiensteinsatze, die aufgrund eines plotzlichen Kreislaufzusammenbruchs notwendig wurden. Hierdurch konnen die Regionen mit einer erhohten Inzidenz von PHT identifiziert und die Grundlage fur eine strategische Anordnung von AED und Einsatzkraften geschaffen werden.

Die Uberlebensraten nach einem plotzlichen Herz-Kreislauf-Stillstand konnten durch sofortiges Einleiten der Wiederbelebungsmasnahmen gesteigert werden, wenn es gelingt Zeugen bis zum Eintreffen der Rettungskrafte zur Herzdruckmassage zu motivieren. Dieses sollte durch den Notruf annehmenden Disponenten der Leitstelle geschehen. Es soll geklart werden, ob die Zeit zwischen Notrufeingang und Beginn der Reanimation durch ein vereinfachtes Protokoll verkurzt werden kann und ob es einen Zusammenhang zwischen Intensitat des telefonischen Kontaktes und einer Verbesserung der Ergebnisse gibt. Bei bekannter vorliegender Bewusstlosigkeit erhielt Gruppe 1 (45 Probanden) durch den Disponenten eine kurze telefonische Anleitung zur Herz-Lungen-Wiederbelebung, wobei der Disponent so lange am Telefon blieb, bis der Rettungsdienst eintraf. Bei Gruppe 2 (45 Probanden) wurde das Telefonat nach identischer Anleitung sofort beendet. Gruppe 3 (29 Probanden) erhielt ausschlieslich den Hinweis, mit Herz-Lungen- Wiederbelebungsmasnahmen zu beginnen. Die Probanden in Gruppen 1 begannen im Mittel nach 68,0 ± 33,5 s mit den Reanimationsmasnahmen, in Gruppe 2 nach 68,3 ± 25,2 s und in Gruppe 3 nach 64,9 ± 34,4 s. Die erreichte mittlere Kompressionsfrequenz lag in Gruppe 1 bei 98,3/min, in Gruppe 2 bei 84,8/min und in Gruppe 3 bei 85,2/min. In Gruppe 1 erreichten 47,8 %, in Gruppe 2 44,2 % und in Gruppe 3 30,2 % der Probanden die korrekte Drucktiefe. Die Probanden fuhlten sich in allen Gruppen gut betreut. Die Frage, ob Handlungsanweisungen vermisst wurden, wurde von 70 der 90 Probanden verneint. In den Gruppen ergeben sich keine signifikanten Unterschiede in den Zielvariablen. Demnach werden Ersthelfer schon nach kurzer telefonischer Anleitung durch den Leitstellendispontenten bzw. allein durch den Hinweis, mit einer Herz-Lungen- Wiederbelebung zu beginnen, zur Hilfeleistung motiviert. Eine kontinuierliche telefonische Betreuung scheint nicht erforderlich zu sein.

Modern pacemakers and implantable defibrillators provide a multitude of technical algorithms and parameters, which can be programmed individually and can treat different forms of arrhythmias. Stored electrograms offer the possibility to obtain valuable information during follow-up (but also in real-time) about arrhythmias and device function or malfunction. This results in improved treatment of cardiac arrhythmias and heart disease. Due to constant innovation and development of these systems, it requires, however, profound biomedical and technical knowledge, since stored electrograms may display complex arrhythmias and device reactions that are not easy to interpret. Understanding of stored device information improves follow-up and facilitates individual care for the patient.

Background and Purpose: Sudden cardiac arrest appears in 1-5 patients/1,000 clinical admissions. In spite of different research approaches, the prognosis after in-hospital resuscitation has not significantly improved in the last 40 years. This account presents the experiences with a hospital-wide emergency plan using the concept of defibrillation by first responders. □ Methods: In 2003, a hospital-wide emergency plan was implemented. The concept comprised the setup of 15 "defibrillator points", training of the entire hospital personnel as first responder, and the introduction of an emergency team. Over the following 3 years, the concept was optimized. In a period from May 2006 to April 2008, the data of all patients who received an in-hospital resuscitation were collected. □ Results: Within 24 months, a total of 41 resuscitations were conducted. Out of these, 24 patients (58%) were under intensive monitoring when the event occurred. Initially, 15 patients (36%) showed ventricular fibrillation, 15 (36%) a pulseless electrical activity, and eleven (27%) an asystoly. A total of twelve patients (29%) left hospital alive. About half of them (42%) experienced ventricular fibrillation and were under observation at the time of event. □ Conclusion: The data collected since the implementation of the hospital-wide emergency plan in 2003 reflect the daily clinical routine. The results show that there is a better outcome especially in patients with ventricular fibrillation when receiving first-responder defibrillation.

After implanting a CRT device, consistent and scheduled patient follow-up is mandatory. Besides determining electrode parameters and reviewing arrhythmic episodes, these follow-ups focus on monitoring and optimizing congestive heart failure therapy. Therefore new CRT devices present methods for heart failure surveillance and telemetric transmission of the acquired data, which allows the physician to respond immediately to the varying needs of the respective heart failure patient. In addition to cardiac resynchronization, optimization of atrioventricular (AV) and interventricular (VV) delay provide major hemodynamic benefits. As echocardiographic optimization of AV and VV delay is time consuming it is often not feasible during daily clinical practice. Therefore implemented algorithms that automatically determine and adapt AV and VV delays with respect to the fluctuating needs of the patients are essential. This article presents the current state of monitoring and optimization methods in CRT devices.

The cardio-circulatory system of chronotropic incompetent patients is unable to adapt heart rate adequately to the level of strain. A common therapy is the implantation of a pacemaker that stimulates the heart at a rate derived from a strain related sensor signal. The paper describes a new pacemaker concept that uses a well-defined time interval in the electrogram as a sensor parameter, the atrioventricular conduction time (AVCT). Identification experiments with patients delivered the stationary and dynamic behaviour of AVCT subject to variations in the exercise rate and the pacing frequency. Furthermore, it was demonstrated that AVCT is perturbed by respiratory activity. The new rate-responsive algorithm, which uses the internal model control principle, explicitly takes into account the closed-loop nature of the underlying system. The major design objectives were: extended range of the individual heart rate; effective attenuation of the respiratory-related disturbance; and stability. Seven patients undergoing incremental exercise were paced with this AVCT-based algorithm. The experiments confirmed the suitability of this concept with respect to the design goals. The patients' peak intrinsic heart rate at exercise was 100 +/- 20min(-1). When paced with the AVCT algorithm, the paced heart rate was 126 +/- 12min(-1). The increase was significant (26 +/- 13min(-1); 15 to 53 min(-1)), which clearly demonstrated the potential of this concept to restore chronotropic competence.

Innovative fibre-optic sensor technology for measuring the movement of the myocardial walls, and from this the heart chamber volumes, was developed. An optical fibre, with a mirror at its end, is inserted into a catheter located in the heart. An opto-electrical control unit positioned outside the heart contains both the light source and the signal receiver. It generates and couples the light into the fibre and transforms and analyses the reflected signal. With such a system, the movement of the cardiac wall can be continuously measured during each cycle, because the fibre moves synchronously with the heart, and this movement bends the fibre, changing the optical attenuation. Experiments where the fibres were wound around metal cylinders of different diameters revealed a maximum sensitivity of 4% mm(-1), diameter. The noise signal corresponded to about 1% of the diameter. First tests in a working pig heart showed a high correspondence of the fibre signal with cardiac parameters. Although these tests are promising, further long-term, extensive experiments in preclinical test devices, and later in clinical tests, must be carried out before the new sensor is used in clinical practice. The fibre-optic technique could be used in monitoring devices, assist devices, pacemaker systems or cardioverter defibrillators.

SDSL (Symmetrical Digital Subscriber Line) is a future-proof wideband access technology for small business and residential users via twisted pair. The original motive for development of new xDSL technologies was the desire on faster Internet access for classic telephone users via copper line. Especially the development of SDSL with its symmetrical nature presents new possibilities itself. For instance SDSL copes excellently with the growing demand for carrying several voice connections via a single copper line always called VoDSL (Voice over DSL), the new generation of voice services for small enterprises and the small office/home office (SOHO) market. We will summarize the features of SDSL and its implementation and have a short look at the actual standardization items. Different Voice over DSL approaches and the use of SDSL as in-house installation will be discussed.

Background Immediate bystander cardiopulmonary resuscitation (CPR) significantly improves survival after a sudden cardiopulmonary collapse. This study assessed the basic life support (BLS) knowledge and performance of high school students before and after CPR training. Methods This study included 132 teenagers (mean age 14.6 ± 1.4 years). Students completed a two-hour training course that provided theoretical background on sudden cardiac death (SCD) and a hands-on CPR tutorial. They were asked to perform BLS on a manikin to simulate an SCD scenario before the training. Afterwards, participants encountered the same scenario and completed a questionnaire for self-assessment of their pre- and post-training confidence. Four months later, we assessed the knowledge retention rate of the participants with a BLS performance score. Results Before the training, 29.5% of students performed chest compressions as compared to 99.2% post-training (P < 0.05). At the four-month follow-up, 99% of students still performed correct chest compressions. The overall improvement, assessed by the BLS performance score, was also statistically significant (median of 4 and 10 pre- and post-training, respectively, P < 0.05). After the training, 99.2% stated that they felt confident about performing CPR, as compared to 26.9% (P < 0.05) before the training. Conclusions BLS training in high school seems highly effective considering the minimal amount of previous knowledge the students possess. We observed significant improvement and a good retention rate four months after training. Increasing the number of trained students may minimize the reluctance to conduct bystander CPR and increase the number of positive outcomes after sudden cardiopulmonary collapse.

Dadurch haben wir ein zusatzliches Signal, welches fur die Entwicklung eines faseroptisch gesteuerten „inotropen” Schrittmachers herangezogen werden kann. Weiterhin ist an einen Einsatz in antitachykarden Systemen, wie den implantierbaren Defibrillatoren zur verbesserten Tachykardiedetektion zu denken.
