Objectives To evaluate the force delivered by removable thermoplastic appliances (RTAs, aligners), altered with Hilliard precision thermopliers, on an upper central incisor to tip it in the palatal and vestibular directions. Materials and methods A total of 10 aligners made from Ideal Clear® (polyethylene terephthalate glycol copolyester, PET-G) with a thickness of 1 mm were used in force analysis. Different-sized spot-thermoformed protuberances (bumps) were generated by activating the thermoplier (thin and thick) up to 30°, 60° and 90° in the centre of the palatal and vestibular surfaces of the aligner in 15° steps. The tipping (Fx) and intrusive (Fz) force components were measured on the isolated upper central incisor as part of a standardized resin model, with or without vertical loading by a weight equivalent. Results Thermoplier activation at 30°, 60° and 90° resulted in different bump heights. The analysis revealed significantly higher Fx and Fz values with increasing bump heights for every activation step in all cases (p Conclusions Aligners modified with Hilliard precision thermopliers showed altered biomechanical parameters. This approach could be an option for treatment modification. Clinical relevance The instrumental examination provided informative results for daily practice, as activation, force dosage and different force values under chewing pressure can be estimated more precisely based on the determined force levels.

Das multisektorale Versorgungszentrum WIR – Walk In Ruhr hat aufgrund seiner Besucher*innenstruktur Zugang zu Daten aus diversen Lebenswelten.Ziel der ArbeitAus verschiedenen WIR-internen Datenquellen sollen populationsbezogene Erkenntnisse zu Alter, Geschlecht, sexueller Orientierung, Sexual- und Risikoverhalten gewonnen und Bedarfe für Prävention abgeleitet werden. Einflüsse der SARS-CoV-2-Pandemie auf das Sexualverhalten sollen durch den Vergleich verschiedener Zeiträume untersucht werden.MethodenAusgewertete Datenquellen sind der Onlinerisikotest für HIV und STI, die COWIR- und PrEP-Studie sowie die Immunologische Ambulanz und das Gesundheitsamt im WIR.ErgebnisSexuell übertragbare Infektionen (STI) sind von 2019 auf 2020 trotz Kontaktbeschränkungen gestiegen. Generell haben junge Menschen, Männer, die Sex mit Männern, und Frauen, die Sex mit Frauen haben, ein erhöhtes STI-Risiko aufgrund der gewählten Sexualpraktiken und der Anzahl sexueller Kontakte. Microbial transglutaminase (TG) is an enzyme isolated on an industrial scale from Streptomyces mobaraensis. Technical TG, a formulated powder, is primarily used to restructure meat in the meat-processing industry, typically at a 1% concentration and is often referred to as “meat glue.” In the European Union, meat restructured with TG requires the indication “formed meat” on the label according to Regulation (EU) No 1169/2011. In order to detect food fraud like the undeclared TG usage in meat and meat products, a qualitative mass spectrometric method using specific tryptic marker peptides has been published in 2017. Here the successful inter-laboratory validation and first-time standardization of a proteomics method for food control is described, which was subsequently included into the Official Collection of Analysis Methods according to the German Food and Feed Code (§ 64 LFGB). Thirteen laboratories from governmental, academic, and private institutions participated in the study, whereas four laboratories did not meet the minimal quality criteria and therefore their results had to be excluded. Three different test materials containing between 0.2 and 2% technical TG as well as blank samples were produced and tested. The laboratories used triple-quadrupole mass spectrometers from several vendors as well as quadrupole time-of-flight instruments. The detection of TG was considered to be positive, if three mass transitions for the marker peptides VTPPAEPLDR (TG-1) and SPFYSALR (TG-2), each, showed a signal-to-noise ratio of at least 3. The level of detection LOD95% for the median laboratory with intermediate performance was 0.31%, the false-positive rate was 0% and the false-negative rate was 2.1%.

The angiotensin-converting enzyme 2 (ACE2) is a viral receptor used by sarbecoviruses to infect cells. Fusion proteins comprising extracellular ACE2 domains and the Fc part of immunoglobulins exhibit high virus neutralization efficiency, but the structure and stability of these molecules are poorly understood. We show that although the hinge between the ACE2 and the IgG4-Fc is highly flexible, the conformational dynamics of the two ACE2 domains is restricted by their association. Interestingly, the conformational stability of the ACE2 moiety is much lower than that of the Fc part. We found that chemical compounds binding to ACE2, such as DX600 and MLN4760, can be used to strongly increase the thermal stability of the ACE2 by different mechanisms. Together, our findings reveal a general concept for stabilizing the labile receptor segments of therapeutic antiviral fusion proteins by chemical compounds.

While cytosine-C5 methylation of DNA is an essential regulatory system in higher eukaryotes, the presence and relevance of 6-methyladenine (m6dA) in human cells is controversial. To study the role of m6dA in human DNA, we introduced it in human cells at a genome-wide scale at GANTC and GATC sites by expression of bacterial DNA methyltransferases and observed concomitant reductions in cell viability, in particular after global GANTC methylation. We identified several genes that are directly regulated by m6dA in a GANTC context. Upregulated genes showed m6dA-dependent reduction of H3K27me3 suggesting that the PRC2 complex is inhibited by m6dA. Genes downregulated by m6dA showed enrichment of JUN family transcription factor binding sites. JUN binds m6dA containing DNA with reduced affinity suggesting that m6dA can reduce the recruitment of JUN transcription factors to target genes. Our study documents that global introduction of m6dA in human DNA has physiological effects. Furthermore, we identified a set of target genes which are directly regulated by m6dA in human cells, and we defined two molecular pathways with opposing effects by which artificially introduced m6dA in GANTC motifs can directly control gene expression and phenotypes of human cells.

Fusion of ACE2 to IgM-Fc results in hexameric ACE2-IgM-Fc proteins with potent virus neutralization efficiency of patient-isolated SARS-CoV and SARS-CoV-2 variants. Coronavirus infections are a world-wide threat to human health. A promising strategy to develop a broadly active antiviral is the use of fusion proteins consisting of an antibody IgG Fc region and a human ACE2 domain to which the viral spike proteins bind. Here we create antiviral fusion proteins based on IgM scaffolds. The hexameric ACE2-IgM-Fc fusions can be efficiently produced in mammalian cells and they neutralize the infectious virus with picomolar affinity thus surpassing monomeric ACE2-IgM-Fc by up to 96-fold in potency. In addition, the ACE2-IgM fusion shows increased neutralization efficiency for the highly infectious SARS-CoV-2 omicron variant in comparison to prototypic SARS-CoV-2. Taken together, these multimeric IgM fusions proteins are a powerful weapon to fight coronavirus infections.

Anlasse fur eine Diagnostik zum Ausschluss von sexuell ubertragbaren Infektionen (STI) konnen vielfaltig sein. Neben der Abklarung von Symptomen wie Ausfluss, Brennen beim Wasserlassen und genitalen Ulzerationen mussen risikoadaptierte Testangebote gemacht werden. Die Anamnese umfasst die sexuelle Orientierung und sexuelle Praktiken, Symptome und ggf. symptomfreie Intervalle, Symptome der Partner*innen, das Vorliegen vorheriger oder chronischer STI, vorherige Therapien und sonstiger Erkrankungen (insbesondere Antibiotikaallergien), den Impfstatus, Verhutungsmethode(n) und Kondomnutzung. Die klinische Untersuchung richtet sich nach dem Geschlecht, den sexuellen Praktiken und den berichteten Symptomen. Sie sollte die Inspektion der Genitalien und der Analregion, des Rachens und der Haut umfassen. Eine konsequente Diagnostik, immer auch oral und anal, und eine leitliniengerechte Therapie mit Erfolgskontrolle sind notwendig.

Im letzten Jahr hatten in Deutschland 88 % der Manner und 86 % der Frauen mindestens einmal kondomlosen Sex. Analverkehr ist zunehmend verbreitet. Nicht nur die Chlamydienproktitis ist ein unterdiagnostiziertes Krankheitsbild. Auch anale Gonokokkeninfektionen oder Manifestationen der Syphilis werden oft nicht erkannt. Asymptomatische Infektionen fuhren daruber hinaus zu einer weiteren Verbreitung dieser Erkrankungen. Im Rahmen der HIV-Praexpositionsprophylaxe wird regelmasig ein risikoadaptiertes Screening auf sexuell ubertragbare Infektionen (STI) durchgefuhrt. Daruber hinaus ist das Bewusstsein fur anale STI in der Bevolkerung und bei Arzten und Arztinnen kaum vorhanden. Eine konsequente Diagnostik analer STI und eine leitliniengerechte Therapie mit Erfolgskontrolle sind anzustreben. Kostenlose risikoadaptierte Testangebote sollten ausgeweitet werden.

Zusammenfassung Hintergrund Zur HIV(„human immunodeficiency virus“)-Präexpositionsprophylaxe (PrEP) wird am WIR – Walk In Ruhr – Zentrum für Sexuelle Gesundheit und Medizin ein innovatives Modellprojekt zur intersektoralen PrEP-Versorgung durchgeführt. Forschungsziele Die vorliegende Studie beschreibt, wer mit der PrEP versorgt wurde und wie sich im Verlauf der PrEP-Einnahme das sexuelle Risikoverhalten änderte sowie welche sexuell übertragbaren Infektionen (STI) und unerwünschte Arzneimittelwirkungen auftraten. Methoden In die Studie wurden bisher N = 139 Probanden eingeschlossen, die zwischen 10/2017 und 12/2018 mit einer PrEP begonnen haben. Alle PrEP-Nutzer erhielten Fragebögen, zudem wurden Nebenwirkungen, HIV und andere STI mittels klinischer Laborwerte über einen Zeitraum von 13 Monaten erfasst. Ergebnisse Die PrEP-Nutzer hatten ein durchschnittliches Alter von 38 Jahren, waren zu 98,6 % Männer, die Sex mit Männern (MSM) hatten, und zeichneten sich durch einen hohen Bildungsstatus und geringe Arbeitslosigkeit aus. Die durchschnittliche Partneranzahl pro Proband innerhalb der letzten 6 Monate stieg im Verlauf signifikant an, während die Kondomnutzung signifikant zurückging. Innerhalb der ersten 4 Monate nach PrEP-Beginn traten 44 STI bei 34 Patienten auf. Es wurde keine HIV-Infektion festgestellt. In den ersten 4 Wochen nach PrEP-Beginn zeigten sich bei 38,8 % der Probanden unerwünschte Arzneimittelwirkungen, hauptsächlich Symptome des Magen-Darm-Trakts. Schlussfolgerung Die PrEP-Nutzer hatten einen guten Bildungsstatus und einen festen Arbeitsplatz bzw. eine Ausbildungsstelle. Das sexuelle Risikoverhalten nahm im Verlauf zu, verbunden mit einem gehäuften Auftreten von STI. Unerwünschte Arzneimittelwirkungen zeigten sich insbesondere zu Anfang der PrEP-Einnahme.

Reduced occlusal function is a main characteristic of orthognathic patients. The present study aimed to investigate the extent of therapy-induced functional improvements in occlusal function using a digital diagnostic method.This prospective clinical study included 41 orthognathic patients (24 women and 17 men, median age 27.26 ± 8.2 years) and 10 control patients (5 women and 5 men, median age 29.8 ± 13.5 years) with neutral skeletal and dental configurations. The patients were divided into classes I, II and III based on their cephalometry. Digital occlusal registrations in habitual occlusion in an upright sitting position were taken before (T1) and after (T2) therapy using the T‑Scan Novus (Tekscan, South Boston, MA, USA) application.Class II and III patients showed a significantly less efficient occlusal pattern than the untreated controls regarding total antagonism (p 0.001), time of occlusion (p = 0.004), occlusal asymmetry (p = 0.001), anterior antagonism (p 0.001) and posterior antagonism (p 0.001). After therapy, the occlusal pattern increased in both therapy groups, where class III patients became indistinguishable from the controls, and class II patients differed only in posterior antagonism (p = 0.035).The digital occlusal registration method proved to be a useful diagnostic tool and provided new insights into therapeutic effects in orthognathic patients. By precisely adjusting the occlusal function, masticatory performance improved significantly.Severe malocclusion leads to a significantly lower masticatory performance for patients, which can be improved by orthognathic therapy and captured by digital occlusal registration.ZIELSETZUNG: Die eingeschränkte okklusale Funktion ist ein Hauptmerkmal von Dysgnathiepatient/-innen. Ziel der vorliegenden Studie war es, das Ausmaß von therapiebedingten funktionellen Verbesserungen der okklusalen Funktion mithilfe eines digitalen Registrierverfahrens zu untersuchen.An dieser prospektiven klinischen Studie nahmen 41 Dysgnathiepatient/-innen (24 Frauen, 17 Männer, medianes Alter 27,26 ± 8,2 Jahre) und 10 Kontrollpatient/-innen (5 Frauen, 5 Männer, 29,8 ± 13,5 Jahre) mit skelettal und dental neutraler Konfiguration teil. Die Patienten/-innen wurden anhand ihrer Kephalometrie in die Angle-Klassen I, II und III eingeteilt. Digitale okklusale Registrierungen in habitueller Okklusion und aufrechter Sitzposition wurden vor (T1) und nach (T2) der Therapie unter Anwendung des T‑Scan Novus (Tekscan, South Boston, MA, USA) durchgeführt.Patienten/-innen der Angle-Klassen II und III zeigten ein signifikant weniger effizientes okklusales Muster als die unbehandelten Kontrollen in Bezug auf den Gesamtantagonismus (p 0,001), die Dauer der Okklusion (p = 0,004), die okklusale Asymmetrie (p = 0,001) sowie den anterioren (p 0,001) und den posterioren Antagonismus (p 0,001). Nach der Therapie nahm das okklusale Muster in beiden Therapiegruppen zu, wobei sich die Patient/-innen der Klasse III nicht mehr von den Kontrollen unterschieden, während die Patient/-innen der Klasse II sich nur im posterioren Antagonismus unterschieden (p = 0,035).Die digitale Okklusionsregistrierung erwies sich als nützliches Diagnoseinstrument und lieferte neue Erkenntnisse über die therapeutischen Auswirkungen bei Dysgnathiepatient/-innen. Durch die präzise Einstellung der okklusalen Funktion verbesserte sich die Kaufunktion signifikant.Schwere Zahnfehlstellungen führen bei Patient/-innen zu einer deutlich geringeren Kauleistung, die durch eine kombiniert kieferorthopädisch-kieferchirurgische Therapie verbessert und durch eine digitale Bissregistrierung erfasst werden kann.

Background Anorexia nervosa (AN) has consistently been found to be associated with poor cognitive flexibility and central coherence. These two cognitive functions have been considered important maintenance factors in AN and are addressed by specific treatment approaches such as cognitive remediation therapy. While there is clear empirical evidence that difficulties in such cognitive functions are related to impaired daily functioning in schizophrenia and bipolar disorder, this potential association has received only little attention in AN research so far. Therefore, the aim of this study was to examine potential relationships between weak cognitive flexibility, central coherence, and poor quality of life (QoL) in AN. Methods Cognitive flexibility and central coherence were measured by both neuropsychological (i.e., performance based) and self-report measures alongside with self-reported QoL in a sample of 138 adult patients with AN. Results Self-report but not performance based measures of cognitive flexibility and central coherence were associated with QoL. Weaker cognitive flexibility and central coherence were correlated with poorer QoL. These associations were independent of comorbid depression. The link between weak central coherence and poor QoL was particularly strong in patients with the restricting subtype of AN. The link between cognitive flexibility and QoL, however, was independent of AN subtype. Conclusions Weak cognitive flexibility and central coherence are associated with low QoL in AN, especially in patients with the restrictive subtype. However, this relationship is dependent on the method of measurement, where self-report measures seem to be more relevant than performance based measures.

Abstract Cyclodipeptide oxidases (CDOs) perform dehydrogenations on diketopiperazines and play an important role in the cyclodipeptide diversification. In this study, we investigated the two known CDOs AlbA/B and Ndas_1146/7 and one new member, CDO-Np. LC-MS monitoring of 32 cyclodipeptide biotransformations in E. coli revealed good consumption of cyclodipeptides containing aromatic amino acids. Cyclodipeptides consisting solely of aliphatic amino acids were poor substrates. In vitro assays of 34 substrates with crude enzyme extracts and product identification proved that the CDO-Np-containing extract catalyzes the formation of two C–C double bonds in many cases. The extracts containing the two other enzymes had lower activities and catalyzed mainly didehydrogenations. For didehydrogenation, the phenylalanyl or tyrosyl site was usually preferred. No or very low acceptance of benzodiazepinediones and a 2,6-diketopiperazine proved the importance of the 2,5-diketopiperazine ring. N-Methylation at the diketopiperazine ring or prenylation of the tryptophan-containing cyclodipeptides influences the enzyme activity and product spectrum. Schnelltests fur sexuell ubertragbare Infektionen wurden ursprunglich fur den Einsatz in der dritten Welt entwickelt, wo Kuhlketten und diagnostische Laborstandards nicht garantiert werden konnen. Doch auch in Deutschland werden die „Point-of-Care-Tests“ fur eine flachendeckende und niederschwellige Diagnostik immer wichtiger.

Autophagy mediates the degradation of damaged proteins, organelles and pathogens, and plays a key role in health and disease. Thus, the identification of new mechanisms involved in the regulation of autophagy is of major interest. In particular, little is known about the role of lipids and lipid-binding proteins in the early steps of autophagosome biogenesis. Using target-agnostic, high-content, image-based identification of indicative phenotypic changes induced by small molecules, we have identified autogramins as a new class of autophagy inhibitor. Autogramins selectively target the recently discovered cholesterol transfer protein GRAM domain-containing protein 1A (GRAMD1A, which had not previously been implicated in autophagy), and directly compete with cholesterol binding to the GRAMD1A StART domain. GRAMD1A accumulates at sites of autophagosome initiation, affects cholesterol distribution in response to starvation and is required for autophagosome biogenesis. These findings identify a new biological function of GRAMD1A and a new role for cholesterol in autophagy.

The complex Chiari, characterized by abnormal craniocervical bony anatomy in addition to Chiari tonsillar herniation, is a relatively recent addition to the concepts surrounding the Chiari literature. The primary findings of complex Chiari include craniocervical kyphosis and retroflexed odontoid, both of which can be described with radiographic measurements. This manuscript will outline the background literature regarding Chiari craniocervical morphometrics and supply an algorithm for the general management of complex Chiari patients.

The use of intraoperative MRI (iMRI) during treatment of gliomas may increase extent of resection (EOR), decrease need for early reoperation, and increase progression-free and overall survival, but has not been fully validated, particularly in the pediatric population. To assess the accuracy of iMRI to identify residual tumor in pediatric patients with glioma and determine the effect of iMRI on decisions for resection, complication rates, and other outcomes. We retrospectively analyzed a multicenter database of pediatric patients (age ≤ 18 years) who underwent resection of pathologically confirmed gliomas. We identified 314 patients (mean age 9.7 ± 4.6 years) with mean follow-up of 48.3 ± 33.6 months (range 0.03–182.07 months) who underwent surgery with iMRI. There were 201 (64.0%) WHO grade I tumors, 57 (18.2%) grade II, 24 (7.6%) grade III, 9 (2.9%) grade IV, and 23 (7.3%) not classified. Among 280 patients who underwent resection using iMRI, 131 (46.8%) had some residual tumor and underwent additional resection after the first iMRI. Of the 33 tissue specimens sent for pathological analysis after iMRI, 29 (87.9%) showed positive tumor pathology. Gross total resection was identified in 156 patients (55.7%), but this was limited by 69 (24.6%) patients with unknown EOR. Analysis of the largest multicenter database of pediatric gliomas resected using iMRI demonstrated additional tumor resection in a substantial portion of cases. However, determining the impact of iMRI on EOR and outcomes remains challenging because iMRI use varies among providers nationally. Continued refinement of iMRI techniques for use in pediatric patients with glioma may improve outcomes.

The eastern oyster, Crassostrea virginica, has been classified as a keystone species as well as an ecosystem engineer because of the significant benefits that oysters and oyster reefs provide. Oyster reefs are being recognized as valuable for shoreline protection within coastal areas. The severe loss of oyster reef coverage has encouraged different types of conservation, mapping, monitoring and restoration efforts throughout its native range. Intertidal oyster reefs are an important habitat as well as an important industry in Florida, which makes accurate habitat monitoring a key element of resource management for this species. This project focused on creating a continuous intertidal oyster reef habitat map in northeast Florida. Existing aerial photography was used to identify oyster reef signatures, and map the distribution of intertidal oyster reefs throughout the study region using ArcGIS software. When accuracy assessment was completed, we found the number of observed agreements was 97% of the total observations. This mapping effort represents the first successful attempt at fine-scale oyster reef mapping across the entire northeast Florida region and resulted in a total of 17,953 individual reefs being mapped, with a total reef coverage of 651.86 ha. By using existing aerial photography, this methodology represents a low-cost method for reef mapping, compared to other methods such as drone imagery or field-based mapping. This baseline map of current oyster distribution will serve many functions for management this particular ecosystem. Future changes to reef distribution can be mapped and used to identify potential negative or positive impacts to the habitat.

Massenspektrometrische Verfahren verfugen uber ein groses Potenzial fur die Bestimmung von Proteinen in Lebensmitteln. Die Kombination von Flussigchromatografie und massenspektrometrischen Fragmentierungstechniken kann dazu genutzt werden, eine Vielzahl an Proteinen simultan in einem Lebensmittel zu bestimmen, sei es zur Authentizitatsprufung oder zum Nachweis von Allergenen. Jedoch benotigt man fur einen routinemasigen und qualitatsgesicherten Einsatz in der Lebensmitteluberwachung standardisierte Methoden. Aus diesem Grund wurde die § 64 LFGB Arbeitsgruppe „Massenspektrometrische Proteinanalytik“ gegrundet. Die Ziele der Arbeitsgruppe sind die Aufnahme von validierten Flussigchromatografie-Massenspektrometrie-Methoden in die „Amtliche Sammlung von Verfahren zur Probenahme und zur Untersuchung“ und die Entwicklung von Leitfaden fur die Validierung dieser Methoden. Am 13. Marz 2018 fand auf Einladung des Bundesamts fur Verbraucherschutz und Lebensmittelsicherheit in Berlin das erste Treffen der Arbeitsgruppe statt. Auf der Sitzung wurden Methoden zur Proteinbestimmung in Lebensmitteln mittels LC-MS vorgestellt. Anschliesend diskutierten die Mitglieder der Arbeitsgruppe uber das Anwendungspotenzial der Methoden und uber mogliche Validierungskonzepte. Fur eine Methode zur Authentizitatsprufung von Fischproben und fur eine Methode zum Nachweis von Schalenfruchten und Erdnuss wurde beschlossen, eine Uberprufung der Methoden durch laborubergreifende Vorringversuche durchzufuhren, um daran anschliesend Methodenvalidierungsstudien mit mindestens acht Laboren zu realisieren und auszuwerten.
