Patients suffering from Long-COVID syndrome experience a variety of different symptoms on a physical, but also on a psychological and social level. Previous psychiatric conditions such as depression and anxiety have been identified as separate risk factors for developing Long-COVID syndrome. This suggests a complex interplay of different physical and mental factors rather than a simple cause–effect relationship of a specific biological pathogenic process. The biopsychosocial model provides a foundation for understanding these interactions and integrating them into a broader perspective of the patient suffering from the disease instead of the individual symptoms, pointing towards the need of treatment options on a psychological as well as social level besides biological targets. This leads to our conclusion, that the biopsychosocial model should be the underlying philosophy of understanding, diagnosing and treating patients suffering from Long-COVID syndrome, moving away from the strictly biomedical understanding suspected by many patients, treaters and the media while also reducing the stigma still associated with the suggestion of a physical–mental interplay.

The aim of this longitudinal study was to examine changes in COVID-19 and illness-related perceptions, gastrointestinal symptoms, coping, catastrophising, psychological distress, and QoL during the COVID-19 pandemic. A total of 831 adults with a gastrointestinal condition completed an online questionnaire at baseline (May—October 2020). Of those, 270 (32.5%) participants (85.2% female, mean age = 47.3 years) provided follow-up data (March—May 2021). Repeated-measures multiple analysis of variance and a cross-lagged panel model were used to test the study hypotheses. Gastrointestinal symptoms and COVID-19 perceptions at follow-up were strongly predicted by their baseline values, while illness perceptions were predicted by baseline gastrointestinal symptoms. Cross-lagged relationships indicated a reciprocal relationship between gastrointestinal symptoms and psychological distress. Moreover, gastrointestinal symptoms had substantial predictive utility, strongly predicting future gastrointestinal symptoms, and to a lesser extent, more negative illness perceptions, greater psychological distress, and greater use of adaptive coping strategies across time.

ZusammenfassungNach dem Zweiten Weltkrieg wurden weltweit Zwillingskohorten aufgebaut, die inzwischen ca. 1,5 Mio. Zwillinge umfassen und zwischen 1950 und 2012 über 2748 Zwillingsstudien hervorgebracht haben. Diese Zahl steigt jedes Jahr um weitere 500 bis 1000. Die Unterrepräsentanz deutscher Zwillingsstudien in diesen Datenbanken lässt sich nicht allein durch den Missbrauch medizinischer Forschung im Nationalsozialismus erklären. Entwicklung und Ausbau großer Zwillingskohorten sind ethisch und datenschutzrechtlich eine Herausforderung. Zwillingskohorten ermöglichen jedoch die Langzeit- und Echtzeiterforschung vieler medizinischer Fragestellungen; und die Zwillingsstudien tragen auch nach der Entschlüsselung des Humangenoms erheblich zur Beantwortung der Frage nach Anlage oder Umwelt als mögliche Erkrankungsauslöser bei.Derzeit gibt es 2 deutsche Zwillingskohorten: die biomedizinische Kohorte HealthTwiSt mit ca. 1500 Zwillingspaaren und TwinLife, eine soziologisch-psychologische Kohorte mit ca. 4000 Zwillingspaaren. Daneben gibt es krankheitsspezifische Kohorten. 2016 startete das TwinHealth-Konsortium der Medizinischen Fakultät der Universität Tübingen mit dem Ziel, eine forschungsoffene und nachhaltige Zwillingsforschung am Standort Tübingen zur Bearbeitung unterschiedlicher Fragestellungen zu etablieren.Der Artikel bietet mithilfe einer systematischen Literaturrecherche und einer medizinhistorischen Betrachtung einen Überblick über die weltweite und nationale Entwicklung von Zwillingsstudien und -datenbanken der letzten 100 Jahre. Anhand der Tübinger TwinHealth-Initiative beleuchtet er den Aufbau eines Zwillingskollektivs und dessen juristische, ethische und Datenschutzaspekte.

Hintergrund: Die Antibiotika-assoziierte Diarrho (AAD) stellt die haufigste intestinale Nebenwirkung einer Antibiotikatherapie dar. Verschiedene Probiotika oder Probiotikakombinationen werden oft wahrend der Antibiotikaeinnahme praventiv angewendet, um der Entstehung einer AAD vorzubeugen. Methode: Die Ubersichtsarbeit basiert auf einer systematischen Literaturrecherche in MEDLINE und EMBASE. Es werden 7 Probiotika hinsichtlich ihrer Wirksamkeit und Evidenz bei der Pravention der AAD dargestellt. Nur Praparate, die von der World Gastroenerology Organisation (WGO) mit Evidenzlevel 1-3 zur Pravention der AAD eingestuft waren, wurden berucksichtigt. 37 klinische Studien, davon 33 RCTs wurden ausgewertet. Ergebnisse: Saccharomyces boulardii CNCM I-745 ist das am umfangreichsten untersuchte Probiotikum hinsichtlich der Pravention der AAD. Es zeigte in allen Patientengruppen (ambulant und stationar behandelte Kinder und Erwachsene) eine evidenzbasierte Wirksamkeit auf. Auch Lactobacillus rhamnosus GG besitzt eine gute Studienlage bei Kindern und ambulant behandelten Erwachsenen. Die weiteren Probiotika und Probiotikakombinationen, welche in der vorliegenden Arbeit bewertet wurden, zeigen lediglich eine Wirksamkeit bei hospitalisierten Patienten auf oder es liegt nur eine sehr eingeschrankte Evidenz fur die Wirksamkeit hinsichtlich der Pravention einer AAD vor, beispielsweise aufgrund des zugrundeliegenden Studiendesigns bzw. der geringen Anzahl an Patienten. Schlussfolgerung: Probiotika wirken stammspezifisch, uber ihre Wirksamkeit bei der Pravention einer AAD kann keine allgemeine Aussage getroffen werden. Grundsatzlich empfiehlt es sich, zur Pravention der AAD ein Probiotikum mit evidenzbasierter Wirkung auszuwahlen wie beispielsweise Saccharomyces boulardii CNCM I-745 oder Lactobacillus rhamnosus GG. Adipositas ist eine schwerwiegende chronische Erkrankung mit ausgepragten Folgeerkrankungen sowie erhohter Mortalitat. Das Therapiekonzept sollte multimodal und interdisziplinar ausgerichtet sein und ist unter anderem vom Ausmas des Ubergewichts, korperlicher und psychischer Komorbiditat, bisherigen Therapieversuchen sowie von Motivations- und Personlichkeitsfaktoren abhangig. Psychotherapeutische Interventionen und Strategien zur Verhaltensmodifikation werden vorwiegend bei einem Body-Mass-Index zwischen 30 und 40 kg/m2 eingesetzt.

Acyl-ghrelin administration increases food intake, body weight, and blood glucose. In contrast, mice lacking ghrelin or ghrelin receptors (GHSRs) exhibit life-threatening hypoglycemia during starvation-like conditions, but do not consistently exhibit overt metabolic phenotypes when given ad libitum food access. These results, and findings of ghrelin resistance in obese states, imply nutritional state dependence of ghrelin’s metabolic actions. Here, we hypothesized that liver-enriched antimicrobial peptide-2 (LEAP2), a recently characterized endogenous GHSR antagonist, blunts ghrelin action during obese states and postprandially. To test this hypothesis, we determined changes in plasma LEAP2 and acyl-ghrelin due to fasting, eating, obesity, Roux-en-Y gastric bypass (RYGB), vertical sleeve gastrectomy (VSG), oral glucose administration, and type 1 diabetes mellitus (T1DM) using humans and/or mice. Our results suggest that plasma LEAP2 is regulated by metabolic status: its levels increased with body mass and blood glucose and decreased with fasting, RYGB, and in postprandial states following VSG. These changes were mostly opposite of those of acyl-ghrelin. Furthermore, using electrophysiology, we showed that LEAP2 both hyperpolarizes and prevents acyl-ghrelin from activating arcuate NPY neurons. We predict that the plasma LEAP2/acyl-ghrelin molar ratio may be a key determinant modulating acyl-ghrelin activity in response to body mass, feeding status, and blood glucose.

Functional dyspepsia is one of the most prevalent functional gastrointestinal disorders. Functional dyspepsia comprises three subtypes with presumed different pathophysiology and aetiology: postprandial distress syndrome (PDS), epigastric pain syndrome (EPS) and a subtype with overlapping PDS and EPS features. Functional dyspepsia symptoms can be caused by disturbed gastric motility (for example, inadequate fundic accommodation or delayed gastric emptying), gastric sensation (for example, sensations associated with hypersensitivity to gas and bloating) or gastric and duodenal inflammation. A genetic predisposition is probable but less evident than in other functional gastrointestinal disorders, such as irritable bowel syndrome (IBS). Psychiatric comorbidity and psychopathological state and trait characteristics could also play a part, although they are not specific to functional dyspepsia and are less pronounced than in IBS. Possible differential diagnoses include Helicobacter pylori infection and peptic ulceration. Pharmacological therapy is mostly based on the subtype of functional dyspepsia, such as prokinetic and fundus-relaxing drugs for PDS and acid-suppressive drugs for EPS, whereas centrally active neuromodulators and herbal drugs play a minor part. Psychotherapy is effective only in a small subset of patients, whereas quality of life can be severely affected in nearly all patients. Future therapies might include novel compounds that attempt to treat the underlying gastric and duodenal inflammation.

Background Corticotropin-releasing factor overexpressing (CRF-OE) male mice showed an inhibited feeding response to a fast, and lower plasma acyl ghrelin and Fos expression in the arcuate nucleus compared to wild-type (WT) mice. We investigated whether hormones and hypothalamic feeding signals are impaired in CRF-OE mice and the influence of sex. Methods Male and female CRF-OE mice and WT littermates (4–6 months old) fed ad libitum or overnight fasted were assessed for body, adrenal glands and perigonadal fat weights, food intake, plasma hormones, blood glucose, and mRNA hypothalamic signals. Results Under fed conditions, compared to WT, CRF-OE mice have increased adrenal glands and perigonadal fat weight, plasma corticosterone, leptin and insulin, and hypothalamic leptin receptor and decreased plasma acyl ghrelin. Compared to male, female WT mice have lower body and perigonadal fat and plasma leptin but higher adrenal glands weights. CRF-OE mice lost these sex differences except for the adrenals. Male CRF-OE and WT mice did not differ in hypothalamic expression of neuropeptide Y (NPY) and proopiomelanocortin (POMC), while female CRF-OE compared to female WT and male CRF-OE had higher NPY mRNA levels. After fasting, female WT mice lost more body weight and ate more food than male WT, while CRF-OE mice had reduced body weight loss and inhibited food intake without sex difference. In male WT mice, fasting reduced plasma insulin and leptin and increased acyl ghrelin and corticosterone while female WT showed only a rise in corticosterone. In CRF-OE mice, fasting reduced insulin while leptin, acyl ghrelin and corticosterone were unchanged with no sex difference. Fasting blood glucose was higher in CRF-OE with female > male. In WT mice, fasting increased hypothalamic NPY expression in both sexes and decreased POMC only in males, while in CRF-OE mice, NPY did not change, and POMC decreased in males and increased in females. Conclusions These data indicate that CRF-OE mice have abnormal basal and fasting circulating hormones and hypothalamic feeding-related signals. CRF-OE also abolishes the sex difference in body weight, abdominal fat, and fasting-induced feeding and changes in plasma levels of leptin and acyl ghrelin. Chronische Schmerzstorungen mit somatischen und psychischen Faktoren (ICD-10: F45.41) gehoren zu den haufigen Krankheitsbildern in der psychosomatischen Praxis. Im vorliegenden Fall wurde aufgrund einer engen zeitlichen Korrelation zwischen einem Trauma und dem konsekutiven Auftreten von Symptomen einschlieslich depressiver Symptome der Verdacht auf eine chronifizierte Schmerzerkrankung mit relevanter somatoformer Komponente gestellt. Nach mehreren Monaten ohne richtungsweisenden somatischen Befund ergab jedoch eine neuerliche, zielgerichtete Diagnostik, dass ein primarer Hyperparathyreoidismus sowie ein papillares Schilddrusenkarzinom vorlagen. Die chirurgische Behandlung resultierte in einem nahezu vollstandigen Ruckgang der Beschwerden innerhalb kurzer Zeit.
