Considering high drug attrition rates in clinical studies and the overall complexity and challenging environment of drug development, it is increasingly important to understand the therapeutic molecule and target and how they intersect with disease biology as fully as possible. This requires one to use numerous tools and investigative approaches in combination. Genetically engineered mouse models are a critical component to the drug development toolbox as they can provide key insights across multiple steps of the drug development process. While knock-out and knock-in mice can inform questions of basic biology, genetically engineered mice can also be applied to model diseases for efficacy studies, to discriminate on-target and off-target effects of novel therapeutics, and to inform an array of biologic and pharmacologic questions, including pharmacodynamics, pharmacokinetics, and biomarker discovery. However, use of these models requires not only an understanding of their strengths and limitations but also a careful consideration of the context in which they are being used and the hypotheses being addressed by them. Additionally, they should not be used in isolation, but instead in combination with other biochemical, in vitro, and clinical data to create a broad understanding of the drug, target, and disease biology.

Day care is a potential alternative to inpatient care of cancer patients. Using day care reduces medical costs substantially compared to inpatient care, which is driving the transfer of many inpatient chemotherapy protocols to day care hospitals (DCHs). However, in contrast to inpatient management, day care provides limited observation time, which could increase the risk of renal toxicity when using these protocols. We present a case report of acute kidney injury following high-dose methotrexate administration in a DCH. Based on a review of the current literature on acid-base balance, we also discuss appropriate patient selection and judicious hydration and urine alkalinization so as to prevent toxicity in the day care setting.

Francesco Forte dedicated three books to the figure of Luigi Einaudi. The first was published in 1982, the second in 2009, and the third in 2016. Through these books he provides a comprehensive analysis of the liberal thought of Einaudi. This article focuses on Forte’s exploration and comments regarding Einaudi’s views and proposals for the European Federation (Einaudi 2011) and the European Central Bank’s unconventional monetary policy. Forte’s analysis of Einaudi has the merit of exalting one of the most prominent and lesser known features of his thought. Einaudi appears as the architect of what we call today the European Union. However no historian, economist, or politician has ever recognized the fundamental role of Einaudi. This lack of recognition should be extended to the Eurotower banker whose unconventional monetary policy has drawn so much from Einaudi’s theory of financial stability.

In his two pivotal works: For an European Economic Federation (1943) and The Economic Problems of the European Federation, (1944) written just before and during his Swiss exile, Luigi Einaudi listed the mandatory tasks of the European Federation. This would only be a start, for as he warned, only later experience would show whether or not the list should be extended. Einaudi does not mention any specific criteria to be used to select tasks for the European Federation. Using the principles of subsidiarity and fiscal federalism, this paper first analyses Einaudi’s allocation of competences to the European Federation. Comparisons are also drawn to his 1944 conclusions and other works on European Union (EU) competencies published nearly sixty years later. The second part of the paper deals with Einaudi’s recommendations on taxes to be levied by the Federation. He was skeptical of the system of transfers from member states to the Federation, but supported a system with its own autonomous resources. Today, this is a topical problem for financing the EU’s budget. The third part deals with Einaudi’s view on public deficits. Thanks to his work as member of the Italian Constituent Assembly, a principle advocating balance between spending and resources was introduced into the Constitution. As President, he interpreted this article as implying the equilibrium between income and expenses. Einaudi therefore anticipated the fiscal discipline introduced at the EU level with the Maastricht treaty in the nineties, and lately in several member state national constitutions. The fourth part assesses the results of EU’s fiscal discipline. Success in reducing the deficit and stabilizing the public debt before the crisis is undeniable. However, a balanced budget was achieved more by tax increases than through the reduction of expenditures. This occurred because of the “missing criterion” of Maastricht. In conclusion, the paper presents and discusses this criterion.

The composition of breeding pairs can deviate from random, resulting in a positive association between homologous traits between mates. Assortative mating can arise either as a by-product of constraints imposed on random pairing by variation in individual life histories or because of mutual sex preferences. Because individuals that prefer high-quality mates gain fitness advantages, mate choice should result in positive assortative mating for traits that are associated with individual quality. Telomeres are nucleoprotein complexes that function to maintain chromosome integrity. Telomeres shorten at each cell division, but the rate of shortening depends on exogenous and endogenous effects, including antioxidant defense mechanisms that reduce the negative impact of pro-oxidants on telomere length. Once a critical length is attained, cells enter either senescence or apoptosis, causing a decline in tissue renewal and ultimately in individual performance. Assortative mating is therefore expected both for telomere length and for antioxidant defense because both males and females may accrue benefits from choosing a mate with long telomeres and efficient antioxidant defense. Consistent with this prediction, in the barn swallow (Hirundo rustica), we found a positive correlation between telomere length (RTL) and total antioxidant capacity (TAC) of social mates which was not the spurious result of age or breeding date. Because parents with relatively long telomeres produce offspring which also have relatively long telomeres, assortative mating seems to be adaptive. The evidence of assortative mating for telomere length and antioxidant capacity suggests that epigamic signals exist that reliably reflect these traits and may mediate mutual mating preferences. Telomeres protect chromosome integrity but shorten at each cell division, impairing organismal functions. Because individuals vary in telomere length and rate of shortening, as well as in antioxidant defenses reducing the negative impact of pro-oxidants on telomeres, adaptive mate choice should result in assortative mating for telomere length and antioxidants. Consistent with this prediction, in barn swallows, telomere length and antioxidant capacity were positively correlated between social mates.

The Italian classical liberal tradition of Federalism and European integration is well founded. Starting with Luigi Einaudi, who took up the heritage of Carlo Cattaneo, it has continued with Giovanni Malagodi, Giovanni Demaria, and Bruno Leoni, not to mention the liberals of Einaudi’s times. The same reflection continues today. Alberto Alesina is a prominent personality among the scholars who can be considered as full heirs of Einaudi. A foresight, often impressive, is the thread linking Einaudi’s writings on European federation. Between the twenties and the early fifties of the 20th century, Einaudi traced with great precision the profile and many details of a politically and economically unified Europe. Today, we do not have the federation that he wished for, but the economic sovereignty among EU countries has been shared to a great extent. Other prominent Italian liberal thinkers and politicians have reflected on the idea of a federal Europe and on the incipient European construction: thinkers such as Bruno Leoni, Giovanni Malagodi and Giovanni Demaria. Giovanni Malagodi, for many years a member of the Italian Parliament and secretary general of the Italian Liberal party, in his speech for the ratification of the Treaty of Rome, advocated a European monetary union with a single currency substituting those of the Member States of the EEC. Both Giovanni Demaria and Bruno Leoni warned against the danger of Government interventionist policies in the European common market. Alesina, in various scholarly contributions on European integration and in the essay The Future of Europe, adopts a liberal perspective for his critical analysis of EU policies.

Even though platelet volume has been supposed to be indicator of platelet activation, contrasting results have been reported on its relationship with the extent of coronary artery disease (CAD). No data have been so far reported on Platelet-Large Cell Ratio (P-LCR). Thus, the aim of the current study was to investigate whether P-LCR is associated with CAD. We measured P-LCR in 1882 consecutive patients undergoing coronary angiography. Significant CAD was defined as stenosis >50% in at least 1 coronary vessel. We additionally measured Carotid Intima-Media Thickness (IMT) in 359 patients. The relationship between P-LCR and platelet aggregation was evaluated by PFA-100 and Multiplate. Patients with higher P-LCR were older (P = 0.038), with larger prevalence of diabetes (P < 0.0001), dilated cardiomyopathy or valvular heart disease (P = 0.004) and less often family history of CAD (P = 0.045), more often on statins (P = 0.002), and diuretics (P = 0.016). P-LCR was significantly associated with baseline glycaemia (P = 0.001) and RBC count (P < 0.001), but inversely related to platelet count (P < 0.0001). P-LCR was not associated with the prevalence of CAD (adjusted P = 0.3) or its severity. In addition, P-LCR was not related to Carotid IMT or platelet aggregation in patients with or without aspirin therapy. This study showed that P-LCR is not related to platelet aggregation, aspirin resistance, the extent of CAD and carotid IMT. Thus, P-LCR can not be considered as a marker of platelet reactivity or a risk factor for CAD.

Experiments in semi-natural conditions were undertaken to assess hemp metal tolerance and its ability to accumulate cadmium, nickel and chromium. Cannabis sativa was grown in two soils, S1 and S2, containing 27, 74, 126 and 82, 115, 139 μg g−1 of Cd, Ni and Cr, respectively. After two months from germination and at ripeness, no significant alteration in plant growth or morphology was detected. On the contrary, a high hemp reactivity to heavy metal stress with an increase in phytochelatin and DNA content was observed during development, suggesting the Cannabis sativa ability to avoid cell damage by activating different molecular mechanisms. Metals were preferentially accumulated in the roots and only partially translocated to the above-ground tissues. The mean shoot Cd content was 14 and 66 μg g−1 for S1 and S2 soil, respectively. Although not negligible concentrations they were about 100 times lower than those calculated for the hyperaccumulator Thlaspi caerulescens. Similarly Ni uptake was limited if compared with that of the Ni-hyperaccumulator Alyssum murale. Chromium uptake was negligible. As expected on the base of the metal concentration detected in ripe plants, no statistically significant variation in soil metal content was detected after one crop of hemp. Nevertheless, a consistent amount (g) of Cd and Ni is expected to be extracted by 1 ha biomass of hemp (about 10 t) per year and along the time a slow restoration of deeper soil portions can be obtained by its wide root system (at least 0,5 m deep). In addition, the possibilities of growing hemp easily in different climates and using its biomass in non-food industries can make heavy metal contaminated soils productive. This means economical advantage along with a better quality of soil.

In this study the functional interaction of the antidepressant drugs amitriptyline, mianserin, maprotiline, imipramine, fluoxetine and the putative antidepressant drug flibanserin has been studied on 5-HT7-mediated responses to 5-carboxamidotryptamine (5-CT) in the guinea-pig ileum. 5-CT induced a concentration-dependent inhibition of the contractile response to substance P (100 nM). Except for fluoxetine and flibanserin, all the antidepressants antagonized by different degrees the 5-CT inhibitory response with the following rank affinity order: mianserinmaprotilineimipramineamitriptyline. Mianserin was the only antidepressant to show a profile of competitive antagonism at 5-HT7 receptors in a tenfold range of concentrations (0.1-1 microM), with an affinity (pA2) value of 8.1 +/- 0.6. The antagonism of the other antidepressants was not concentration-dependent (amitriptyline) or was associated with slight or moderate reduction of the maximal 5-CT response (imipramine or maprotiline). The apparent affinity (pKB) values were: amitriptyline, 7.0 +/- 0.2; maprotiline, 7.3 +/- 0.6; imipramine, 7.2 +/- 0.4. Our results show that various antidepressant drugs belonging to different chemical classes behave as antagonists at enteric 5-HT7 receptors through competitive or allosteric mechanisms. This evidence extends our previous findings demonstrating the interaction of antidepressants with other 5-HT receptors, namely 5-HT3 and 5-HT4 receptors.

The alpha-adrenoceptors that mediate contractions in strips of splenic artery from the pig were characterized by the use of selective agonists and subtype-selective antagonists. Noradrenaline, the alpha1-selective agonist phenylephrine and the alpha1-/alpha2-agonist oxymetazoline caused the preparations to contract with potency (pD2) values of 6.94, 6.14 and 7.27, respectively. Compared to noradrenaline, phenylephrine and oxymetazoline induced 93% and 78% of noradrenaline maximum effect. Conversely, the two alpha2-selective agonists clonidine and B-HT 920 induced only 31% and 13% of noradrenaline maximum effect. B-HT 920 only marginally contracted the tissue even when it was precontracted with phenylephrine. The alpha2-selective antagonist yohimbine antagonized oxymetazoline- and phenyleprine-induced contractions with affinity (pA2) values (6.80 and 6.74, respectively) consistent with alpha1-adrenoceptor interaction. This suggests that the pig splenic artery possesses only functional alpha1-adrenoceptors. The alpha1-adrenoceptor antagonists of varying subtype selectivities like WB-4101, 5-methylurapidil, benoxathian and BMY 7378 competitively antagonized phenylephrine-induced contractions with affinity values of 9.46, 8.26, 9.06 and 6.91, respectively. These values correlated highly with published affinity values for functional alpha1A-adrenoceptors (r=0.92) and alpha1a-clones (r=0.94) and less well with affinity values for functional alpha1B-adrenoceptors (r=0.84) and alpha1b-clones (r=0.87). Conversely, correlation with functional alpha1D-adrenoceptors (r=0.26) and alpha1d-clones (r=0.33) was poor. In addition the alpha1D-selective antagonist BMY 7378 had a low affinity value compared to that reported for alpha1D-adrenoceptors. Therefore, based on correlation studies, the plot that resembled the line of equal values most closely was that for the alpha1A-subtype. The alpha1A-selective antagonist RS-17053 antagonized phenylephrine-induced contractions in an apparently non-competitive manner and gave an apparent pA2 value of 7.06 which is similar to the "low" affinity values reported in other alpha1A-containing tissues. Exposure to the irreversible alpha1B/D-antagonist chloroethylclonidine slightly decreased maximum response to phenylephrine without significantly affecting its potency value, indicating that the phenylephrine response is substantially chloroethylclonidine-insensitive. It is concluded that splenic artery strips from the pig contract in response to phenylephrine through activation of alpha1-adrenoceptors which display the pharmacological profile of the alpha1A-subtype for which the recently reported alpha1A-selective antagonist RS-17053 shows low affinity. Evidence for contribution of the alpha1B-subtype in the overall contractile response is elusive while no evidence was obtained for the involvement of the alpha1D-subtype. The contribution of functional alpha2-adrenoceptors to the contractile response was ruled out.

Fifty-nine depressed female inpatients were treated with 100 mg amitriptyline (AMT) IM for 4 weeks. Depression ratings and determinations of the parent drug and nortriptyline (NT) were performed weekly. No direct relationship between plasma AMT + NT concentrations and therapeutic response was apparent, but beneficial therapeutic responses and significantly lower side-effect scores were more frequently noted in subjects with concentrations in the 100-200 ng/ml range. AMT + NT concentrations were significantly correlated with age. No significant difference was found in the number of responders between younger and older subjects with two clinical improvement criteria; however, a significant difference emerged when a third more restrictive clinical outcome criterion was adopted. The implications of the present findings for patient treatment and for the interpretation of previous studies are discussed. The data collected point to a possible usefulness of monitoring AMT and NT plasma levels, even if further investigations are needed.

INTRODUCTION More personalized treatment strategies for bleedings in Mild Hemophilia A (MHA) are needed. In most MHA patients, FVIII: C levels increase after administration of DDAVP, a synthetic vasopressin analogue. However, great inter-individual variation in DDAVP response is observed. The aim of this study is to identify clinical and genetic determinants of the response to DDAVP. METHODS The RISE cohort is aimed to include at least 500 MHA patients (FVIII 2-40%) from 34 European centers. Data on potential determinants of DDAVP response are collected: age, ethnicity, BMI, FVIII genotype, blood group, response family members, other genetic markers, basal FVIII:C. Good Clinical Response (GCR) to DDAVP is defi ned as FVIII: C >50 IU/ ml one hour after administration. Data on the fi rst 244 patients are presented, analyzed by a univariate logistic regression. RESULTS A GCR is associated with age >18 years (OR 2.4; 95% CI 1.4 – 4.4), higher FVIII: C baseline level (OR 10.8; 95% CI 5.7 – 20.4), GCR in family members (OR 22.1; 95% CI 8.4 – 58.1), and FVIII genotype. A GCR was observed in 75% of the individuals with mutations in the A2 domain compared to only 40% with mutations in the C1 domain. Mutations at the binding site for von Willebrand factor are associated with a poor response (OR for GCR: 0.2, 95% CI 0.07 – 0.52). CONCLUSION A good clinical response to DDAVP in MHA is associated with a higher age, higher basal FVIII: C, a GCR in family members, and with the location of the FVIII missense mutation.

A comparative analysis of two primate and two rodent genomes suggests that telomerase was utilized, in some instances, for the repair of DNA double-strand breaks during mammalian evolution., Background In vertebrates, tandem arrays of TTAGGG hexamers are present at both telomeres and intrachromosomal sites (interstitial telomeric sequences (ITSs)). We previously showed that, in primates, ITSs were inserted during the repair of DNA double-strand breaks and proposed that they could arise from either the capture of telomeric fragments or the action of telomerase. Results An extensive comparative analysis of two primate (Homo sapiens and Pan troglodytes) and two rodent (Mus musculus and Rattus norvegicus) genomes allowed us to describe organization and insertion mechanisms of all the informative ITSs present in the four species. Two novel observations support the hypothesis of telomerase involvement in ITS insertion: in a highly significant fraction of informative loci, the ITSs were introduced at break sites where a few nucleotides homologous to the telomeric hexamer were exposed; in the rodent genomes, complex ITS loci are present in which a retrotranscribed fragment of the telomerase RNA, far away from the canonical template, was inserted together with the telomeric repeats. Moreover, mutational analysis of the TTAGGG arrays in the different species suggests that they were inserted as exact telomeric hexamers, further supporting the participation of telomerase in ITS formation. Conclusion These results strongly suggest that telomerase was utilized, in some instances, for the repair of DNA double-strand breaks occurring in the genomes of rodents and primates during evolution. The presence, in the rodent genomes, of sequences retrotranscribed from the telomerase RNA strengthens the hypothesis of the origin of telomerase from an ancient retrotransposon.
