This paper presents a comprehensive geological and geotechnical study of the whole area affected by liquefaction following the 2012 Emilia earthquakes, including all the available information from the field reconnaissance surveys, in situ tests, and laboratory analyses. The compilation was performed at 120 liquefied sites to verify and validate the reliability of liquefaction charts in alluvial sediments, and to assess liquefaction induced by the 2012 seismic sequence in the Emilia plain. The results reveal a wide range of grain sizes (from clean sands to sandy silts) and compositional characteristics (quartz-rich to litharenitic) in the 2012 ejecta, and show a strong relationship between the liquefaction and stratigraphic architecture of the subsurface. The availability of in situ tests at the liquefied sites makes it possible to verify and validate the reliability of the liquefaction charts in alluvial sediments with respect to the real observations. For the analyzed Emilia case studies, the use of non-liquefiable crust provides better estimations of the liquefaction manifestations when coupled with the thickness of the liquefiable layer rather than with the liquefaction potential index. We provide a database of the surface ruptures produced by the 26 December 2018 Mw 4.9 earthquake that struck the eastern flank of Mt. Etna volcano in Sicily (southern Italy). Despite its relatively small magnitude, this shallow earthquake caused about 8 km of surface faulting, along the trace of the NNW-trending active Fiandaca Fault. Detailed field surveys have been performed in the epicentral area to map the ruptures and to characterize their kinematics. The surface ruptures show a dominant right-oblique sense of displacement with an average slip of about 0.09 m and a maximum value of 0.35 m. We have parsed and organized all observations in a concise database, with 932 homogeneous georeferenced records. The Fiandaca Fault is part of the complex active Timpe faults system affecting the eastern flank of Etna, and its seismic history indicates a prominent surface-faulting potential. Therefore, this database is essential for unravelling the seismotectonics of shallow earthquakes in volcanic areas, and contributes updating empirical scaling regressions that relate magnitude and extent of surface faulting. A 53-year-old Caucasian woman, with a history of LAGB, performed 18 years ago in another hospital, presented in May 2017 to our bariatric centre with a one-year history of vomiting and weight loss of 20 kg. Despite of the risk of complications due to the extent of surgery, we did not register any short or long-term surgical complications, and the patient is still in good health. Background: Adaptation by design consists in conservatively estimating the phase III sample size on the basis of phase II data; it is also called conservative sample size estimation (CSSE). The usual assumptions are that the effect size is the same in both phases and that phase II data are not used for phase III confirmatory analysis. CSSE has been introduced to increase the rate of successful trials, and it can be applied in most clinical areas. CSSE reduces the probability of underpowered experiments and can improve the overall success probability of phase II and III, but it also increases phase III sample size, increasing the time and cost of experiments. Thus, the balance between higher revenue and greater cost is the issue. Methods: A profit model was built assuming that CSSE was applied and considering income per patient, annual incidence, time on market, market share, phase III success probability, fixed cost of the 2 phases, and cost per patient under treatment. Results: Profit turns out to be a random variable depending on phase II sample size and conservativeness. Profit moments are obtained in a closed formula. Profit utility, which is a linear function of profit expectation and volatility, is evaluated in accordance with the modern theory of investment performances. Indications regarding phase II sample size and conservativeness can be derived on the basis of utility, for example, through utility optimization. Background: Laparoscopic gastrectomies are currently performed in many centers, but compliance with oncologic requirements still represents a subject open to debate. The aim of this work was to compare the short-term and oncologic outcomes after laparoscopic and open surgery in gastric adenocarcinoma.Methods: From June 2000 through June 2005, 147 patients in our institution underwent gastrectomy by open or mininvasive approach for adenocarcinoma. The laparoscopy group included 48 patients, 29 with early gastric cancer (EGC) and 19 with antral advanced gastric cancer (AGC). The short-term results and oncologic data were compared to those obtained in 99 patients who underwent open surgery. Survival in the laparoscopy group was analyzed.Results: In the laparoscopy group no intraoperative complications were observed, and conversion was needed in only one patient with a large advanced tumor. Overall, 32 lymph nodes were collected by D2 dissection, 30 for EGC, 34 for advanced cancers. The resection margin was 6.7 cm (range: 4-8 cm). The mean operating time was 240 min (range: 150-360 min), with a blood loss of 150 ml on average (range: 70-250 ml). Morbidity included two duodenal leaks that healed without reoperation; after enclosing or reinforcing the staple line, no further leaking was noted. There was one death from massive bleeding in a cirrhotic patient. Ambulation and oral feeding started significantly earlier than in open surgery. The mean hospital stay was 10 days (range: 7-24 days), significantly shorter than the stay of 18 days after open surgery (p < 0.05). All patients treated laparoscopically were alive without recurrence at the end of this study.Conclusions: Short-term results with laparoscopic gastrectomy were better than with open surgery in this study. Oncologic radicality was a major concern, but in the authors' experience the extent of lymphadenectomy was the same as in open surgery.
