Soils, often perceived as merely the surface layer of Earth, play a crucial role as the foundation for terrestrial life and are deeply integrated into the worldviews, livelihoods and cultural practices of communities in Mexico. Mexican soils and the ecosystem services that provide are essential for food security and ecological resilience, especially amidst environmental challenges like climate change, desertification and biodiversity loss. Their conservation, restoration and sustainable management are essential to maintain the country's natural environment and to ensure the integral wellbeing of its inhabitants. This study evaluates the ecosystem services provided by soils in a priority area for biodiversity conservation in Mexico, adopting an approach that merges ecological economics perspectives with local community views. By understanding how residents of this region value their soils, this research highlights the ecological and economical significance of soils and their intrinsic cultural and social value within communities that have coexisted with these ecosystems for generations.Highlights: Valuation of soil ecosystem services reveals critical roles in nutrient cycling, climate regulation and hydrological stability. Background: The neuromuscular activity has a critical role in the permeability of the upper airways. Objective: The present study aimed to conduct a detailed and comparative investigation of the orofacial musculature and motor skills of children with obstructive sleep apnea (OSA). Materials and methods: Children aged 7 to 12 years with OSA (OSA group, n = 12) and without OSA (Control group, n = 12) were compared. Orofacial appearance/posture and motor skills were assessed using the orofacial myofunctional evaluation protocol with scores. Bite force, tongue pressure measurements, and surface electromyography of swallowing were also performed. Results: Compared to the control group, the OSA group obtained lower scores in appearance/posture, mobility, and orofacial functions (P < 0.0001), and lower values of bite force (P = 0.019) and tongue strength in protrusion (P = 0.008) and deglutition (P = 0.004). The OSA group also displayed lower values of maximum speed (P = 0.003) and peak activity (P = 0.0005) during spontaneous swallowing and higher relative energy expenditure (integral) of the muscles in the water swallowing task (50 mL) (P ≤ 0.01). Notably, the effect size ranged from moderate to large for all groups differences. Conclusion: The children with OSA showed impairments in orofacial musculature and motor skills, as evidenced by reduced muscle recruitment capacity and coordination in applying pressure/force and performing orofacial movements and functions. Hanging drop cultures provide a favorable environment for the gentle, gel-free formation of highly uniform three-dimensional cell cultures often used in drug screening applications. Initial cell numbers can be limited, as with primary cells provided by minimally invasive biopsies. Therefore, it can be beneficial to divide cells into miniaturized arrays of hanging drops to supply a larger number of samples. Here, we present a framework for the miniaturization of hanging drop networks to nanoliter volumes. The principles of a single hanging drop are described and used to construct the fundamental equations for a microfluidic system composed of multiple connected drops. Constitutive equations for the hanging drop as a nonlinear capacitive element are derived for application in the electronic-hydraulic analogy, forming the basis for more complex, time-dependent numerical modeling of hanging drop networks. This is supplemented by traditional computational fluid dynamics simulation to provide further information about flow conditions within the wells. A fabrication protocol is presented and demonstrated for creating transparent, microscale arrays of pinned hanging drops. A custom interface, pressure-based fluidic system, and environmental chamber have been developed to support the device. Finally, fluid flow on the chip is demonstrated to align with expected behavior based on the principles derived for hanging drop networks. Challenges with the system and potential areas for improvement are discussed. Objective: The objective of this study is to elucidate the clinical and demographic profiles, as well as perioperative outcomes, of patients undergoing surgery for non-hiatal diaphragmatic hernias. Additionally, it aims to analyse these outcomes based on the surgical approach employed (transthoracic versus transabdominal). Methods: This retrospective, observational study was conducted at a single center and involved patients diagnosed with non-hiatal diaphragmatic hernia who underwent either emergency or elective surgery between July 2007 and March 2023. Clinical characteristics and perioperative outcomes of these patients were compared using appropriate statistical tests.The research protocol for this observational, retrospective, and comparative study followed the Declaration of Helsinki's ethical requirements. The need for Clinical Research Ethics Committee approval was waived according to our institutional law because the study was a retrospective cohort study based on anonymous data of patients. Informed consent was waived because this study involved the secondary analysis of patient medical records. Additionally, this study followed the STROBE guidelines for reporting observational studies. Results: The analysis included 22 patients being 59.1% men, with median age of 61 years. The predominant clinical presentation was restrictive lung disease (40.9%). The majority of cases (68%) had traumatic aetiology with a median defect size of 4 cm (range of 3–8 cm). Elective surgery was performed in 15 cases (68.1%) and transthoracic approach was employed in 13 patients (54.5%). Postoperative major morbidity reached 27.2% and mortality within 30 days was 9.1%. Emergency surgeries accounted for 44.4% of transabdominal interventions, compared to 23% in the transthoracic subgroup (p = 0.376). There were no statistically significant differences between the transabdominal and trasnthoracic approaches in terms of global postoperative complications (88.8% vs. 84.6%, p = 1), major complications (44.4% vs 15.4%, p = 0.734), mortality (11.1% v 7.6%, p = 1) and recurrence (11.1% vs 7.6%, p = 1). Postoperative stay was significantly shorter in the transthoracic subgroup (6 days vs. 14 days, p = 0.011). Conclusions: Non-hiatal diaphragmatic hernias are characterized by significant postoperative major morbidity and mortality rates, standing at 27.2% and 9.1%, respectively, accompanied by a recurrence rate of 9.1%. Low-grade gliomas, especially glioneuronal tumors, are a common cause of epilepsy in children. Seizures associated with low-grade pediatric tumors are medically refractory and present a significant burden to patients. Often, morbidity and patients´ quality of life are determined rather by the control of seizures than the oncological process itself and the resolution of epilepsy represents an important part in the treatment of LGGs. The pathogenesis of tumor-related seizures in focal LGG tumors is multifactorial, and mechanisms differ probably among patients and tumor types. Pediatric low-grade tumors associated with epilepsy include a series of neoplasms that have a pure astrocytic or glioneuronal lineage. They are usually benign tumors with a neocortical localization typically in the temporal lobes, but also in other supratentorial locations. Gangliogliomas and dysembryoplastic neuroepithelial tumors (DNET) are the most common entities together with astrocytic gliomas (pilocytic astrocytomas and pleomorphic xanthoastrocytoma) and angiocentric gliomas, and dual pathology is found in up to 40% of glioneuronal tumors. The treatment of low-grade gliomas and associated epilepsy is based mainly on resection and the extent of surgery is the main predictor of postoperative seizure control in patients with a LGG. Long-term epilepsy-associated tumors (LEATs) tend to be well-circumscribed, and therefore, the chances for a complete resection and epilepsy control with a safe approach are very high. New treatments have emerged as alternatives to open microsurgical approaches, including laser thermal ablation or the use of BRAF inhibitors. Future advances in identifying seizure-related biomarkers and molecular tumor pathways will facilitate targeted treatment strategies that will have a deep impact both in oncologic and epilepsy outcomes. Purpose: Herein, the extraction of lignin from rubber tree residues (Hevea brasiliensis Müll. Arg), particularly from the seed shell, is reported with the aim of providing added value to this plantation. Methods: The protocol employed consisted of an organosolv extraction using formic or acetic acids and p-TsOH as catalyst assisted by ultrasound, avoiding the use of high temperatures and mineral acids. Results: 22% yield of lignin was obtained from the dried rubber seed shell using the formic acid/p-TsOH combination in comparison with 13% yield obtained with acetic acid/p-TsOH. The extracted lignin was oxidized isolating typical fragments derived from syringyl, guaiacyl and p-hydroxyphenyl units. The lignin was characterized by IR, UV, NMR, TGA and DSC. Moreover, total phenol content and antioxidant activity were also evaluated. Conclusion: The results exhibited the presence of typical lignin aromatic composition. The lignin obtained by the seed shell could be effectively employed in numerous future applications and the method could be considered as a low-cost acid catalysed process. [ABSTRACT FROM AUTHOR]

Ash emission from volcanic eruptions affects the environment, society, and human health. This study shows the total concentration and lung bioaccessible fraction of eight potential toxic metal(loid)s in five Popocatépetl ashfall samples. Mineralogical phases and particle size distribution of the ashfall were analyzed by X-ray diffraction (XRD) and Scanning Electron Microscope (SEM) techniques, respectively. The bioaccessibility test of Gamble solution (GS) and Artificial Lysosomal Fluid (ALF) were conducted to simulate extracellular (pH 7) and intracellular (pH 4.5) conditions, respectively. The studied metal(loid)s showed the following total concentration (mg kg−1): 1.98 (As), 0.17 (Cd), 134.09 (Cr), 8.66 (Cu), 697.33 (Mn), 55.35 (Ni), 8.77 (Pb), and 104.10 (Zn). Geochemical indices suggested that some metal(loid)s are slightly enriched compared to the local soil background concentrations. Several mineralogical phases were identified in the collected ashfall deposits, such as plagioclase, pyroxene, and Fe–Ti oxide, among others. According to the risk assessment results, the non-carcinogenic risk related to ashfall exposure returns an HQ > 1 for children. In contrast, the estimation of carcinogenic risk was found to be within the tolerable limit. Metal(loid)s showed low bioaccessibility (< 30%) in GS and ALF, with the highest values found in ALF solution for As (12.18%) and Cu (7.57%). Despite their metal-bioaccessibility, our findings also showed that dominant ash particle size ranged between fine (< 2.5 μm) and extremely fine (< 1 μm), considered highly inhalable fractions. The results obtained in this work indicate that volcanic ashes are bioinsoluble and biodurable, and exhibit low bioaccessibility when in contact with lung human fluids. [ABSTRACT FROM AUTHOR]

Shortened telomere length (TL) has been associated with lower cognitive performance, different neurological diseases in adults, and certain neurodevelopmental disorders in children. However, the evidence about the association between TL and neuropsychological developmental outcomes in children from the general population is scarce. Therefore, this study aimed to explore the association between TL and neuropsychological function in children 4–5 years of age. We included 686 children from the INMA Project, a population-based birth cohort in Spain. Leucocyte TL was determined by quantitative PCR method, and neuropsychological outcomes were measured using the McCarthy Scales of Children's Abilities (MCSA). Multiple linear regression models were used to estimate associations adjusted for potential confounding variables. Main findings showed that a longer TL was associated with a higher mean working memory score (β = 4.55; 95% CI = 0.39, 8.71). In addition, longer TL was associated with a higher mean global quantitative score (β = 3.85; 95% CI = −0.19, 7.89), although the association was marginally significant. To our knowledge, this is the first study that shows a positive association between TL and better neuropsychological outcomes in children. Although further research is required to confirm these results, this study supports the hypothesis that TL is essential in protecting and maintaining a child's health, including cognitive functions such as working memory. Objectives: Analyse alternative methods of intrathecal antibody detection by comparing chemiluminescent immunoassay (CLIA) and enzyme-linked immunosorbent assay (ELISA) techniques to determine if CLIA can replace ELISA in the diagnosis of CNS infections. Methods: A panel of 280 paired samples—cerebrospinal fluid (CSF) and serum—with known antibody reactivities (Varicella, n = 60; Measles, n = 120) and negative samples (n = 100) were used to evaluate the performance of six serological test kits (Enzygnost, VirClia®, and Serion ELISA (Measles and Variella). Results: For Measles virus IgG, the VirClia® IgG monotest revealed 97% and 94% positive and negative agreement to the Enzygnost as reference test, respectively. In contrast, Serion ELISA kits yielded values of 18% and 90%. For the Varicella Zoster virus (VZV) IgG, the VirClia® IgG monotest showed 97% and 90% positive and negative agreement compared to Enzygnost. The Serion ELISA kits showed values of 55% and 86%, respectively. ROC analysis revealed that the areas under the curve for Measles and VZV IgGs were 0.7 and 0.852, respectively, using the Serion kit, and 0.963 and 0.955, for Vircell S.L CLIA technique. VirClia® monotest values were calculated using an antibody index cut-off of 1.3. Conclusion: The findings indicate that CLIA testing can improve antibody detection in CSF samples, aiding the diagnosis of infectious neurological impairments. [ABSTRACT FROM AUTHOR]

Objectives: Mental health problems are highly prevalent in people with Alzheimer's disease (AD) and their family caregivers. Therefore, this study aimed to examine the effects of the Mindfulness-Based Health Care Program (MBHC) on the psychological distress of family caregivers of people with AD and, indirectly, on the behavioral and psychological symptoms of dementia (BPSD) in people with AD. Method: A sample of 66 caregivers of people with AD was randomly assigned to either 8 weeks of MBHC or treatment as usual (TAU) groups. The psychological distress in family caregivers, measured by the Hospital Anxiety and Depression Scale (HADS), and BPSD in patients, measured by the Neuropsychiatric Inventory Questionnaire (NPI-Q), were evaluated and compared at baseline, post-intervention, and 3-month follow-up. Results: A total of 50 participants (75.6%) completed the MBHC post-intervention and 30 (50%) at the 3-month follow-up. At post-intervention, compared to TAU, MBHC was associated with significantly greater decreases in psychological distress (β = − 3.86; 95%CI, − 7.67, − 0.04; p = 0.047), specifically in anxiety symptoms (β = − 2.84; 95%CI, − 5.38, − 0.30; p = 0.029), but no significant changes were observed in depressive symptoms. MBHC did not produce a significant change in psychological distress at 3-month follow-up. MBHC did not yield a significant reduction in BPSD in people with AD, neither at post-intervention nor at 3-month follow-up. Conclusions: The results suggest that 8-week mindfulness training can effectively reduce psychological distress and anxiety symptoms in caregivers of people with Alzheimer's disease. To investigate the association between three selected pain polymorphisms and clinical, functional, sensory-related, psychophysical, psychological or cognitive variables in a sample of women with fibromyalgia (FMS). One hundred twenty-three (n = 123) women with FMS completed demographic (age, height, weight), clinical (years with pain, intensity of pain at rest and during daily living activities), functional (quality of life, physical function), sensory-related (sensitization-associated and neuropathic-associated symptoms), psychophysical (pressure pain thresholds), psychological (sleep quality, depressive and anxiety level) and cognitive (pain catastrophizing, kinesiophobia) variables. Those three genotypes of the OPRM1 rs1799971, HTR1B rs6296 and COMT rs4680 single nucleotide polymorphisms were obtained by polymerase chain reactions from no-stimulated whole saliva collection. No significant differences in demographic, clinical, functional, sensory-related, psychophysical, psychological and cognitive variables according to OPRM1 rs1799971, HTR1B rs6296 or COMT rs4680 genotype were identified in our sample of women with FMS. A multilevel analysis did not either reveal any significant gene-to-gene interaction between OPRM1 rs1799971 x HTR1B rs6296, OPRM1 rs1799971 x COMT rs4680 and HTR1B rs6296 x COMT rs4680 for any of the investigated outcomes. This study revealed that three single nucleotide polymorphisms, OPRM1 rs1799971, HTR1B rs6296 or COMT rs4680, mostly associated with chronic pain were not involved in phenotyping features of FMS. Potential gene-to-gene interaction and their association with clinical phenotype in women with FMS should be further investigated in future studies including large sample sizes. Ammonia is emerging as a viable alternative to fossil fuels in combustion systems, aiding in the reduction of carbon emissions. However, its use faces challenges, including NOx emissions and low flame speed. Innovative approaches and technologies have significantly advanced the development and implementation of ammonia as a zero-carbon fuel. This review explores current advancements in using ammonia as a fuel substitute, highlighting the complexities that various systems need to overcome before reaching full commercial maturity in support of practical decarbonising global strategies. Different from other reviews, this article incorporates insights of various industrial partners currently working towards green ammonia technologies. In this work, we obtain an expression for the total observational frequency shift of photons emitted by massive geodesic particles circularly orbiting a black hole in a general spherically symmetric background. Our general relations are presented in terms of the metric components and their derivatives that characterize the black hole parameters. As a concrete example of this general relativistic approach, a special case is studied by applying the formalism to a nonsingular black hole conformally related to the Schwarzchild solution that possesses a length scale parameter l and an integer parameter N in addition to the black hole mass. Besides, we express the nonsingular black hole mass in terms of the observational redshift/blueshift. Finally, we investigate the effects of the free parameters of the conformal gravity theory on the observational frequency shift and compare results with those of the standard Schwarzschild black hole. Objectives: The main objective of this paper was to examine the cost-utility of attachment-based compassion therapy (ABCT) compared to Mindfulness-Based Stress Reduction (MBSR) and treatment-as-usual (TAU) on patients with depressive and/or anxious disorder, or adjustment disorder with depressive and/or anxious symptomatology in terms of effects on quality-adjusted life years (QALYs) as well as healthcare costs from a public healthcare system perspective. Method: A 6-month randomized controlled trial was conducted. Ninety Spanish patients with mental disorders (depressive, anxious, or adjustment disorders) received 8 weekly group sessions of TAU + ABCT, TAU + MBSR, or TAU alone. Data collection took place at pre- and 6-month follow-up. Cost-utility of the two treatment groups (ABCT vs MBSR vs TAU) was compared by examining treatment outcomes in terms of QALYs (obtained with the EQ-5D-3L) and healthcare costs (data about service use obtained with the Client Service Receipt Inventory). Results: Both MBSR and ABCT were more efficient than TAU alone, although the results did not reach statistical significance. Compared to ABCT, MBSR produced an increase both in terms of costs (€53.69, 95% CI [− 571.27 to 513.14]) and effects (0.004 QALYs, 95% CI [− 0.031 to 0.049]); ICUR = €13,422.50/QALY). Both interventions significantly reduced the number of visits to general practice compared to TAU. Conclusions: This study has contributed to the evidence base of mindfulness- and compassion-based programs and provided promising information about the cost-utility of MBSR for patients with emotional disorders. This study aimed to understand the perspectives of correctional officers participating in Mental Health First Aid (MHFA) training. A total of 54 correctional officers from two State Department of Corrections participated in the study. Correctional officers completed two focus group meetings. The focus group meetings were analyzed using narrative analysis. The themes of the focus group meetings were (1) the emotional toll of working in the criminal justice system, (2) correctional officers managing poor mental health, (3) barriers to mental health-seeking behaviors, (4) benefits and facilitators to MHFA, and (5) recommendations for future MHFA training. In summary, correctional officers emphasized the need for additional mental health resources to cope with the unpredictable and challenging work environment. State Departments of Corrections should prioritize the mental health of correctional officers and work toward destigmatizing mental health across these settings by implementing MHFA for all correctional professionals. Background and objective: To revise and critically summarize the available scientific evidence regarding the effect of exercise on sleep quality in patients with chronic kidney disease (CKD). Methods: A systematic review of randomized controlled trials (RCTs) and comparative studies was conducted, searching MEDLINE/PubMed, SPORTDiscus, and Scopus using keywords "Exercise", "Physical Activity", "Chronic Kidney Disease," and "Sleep". The methodological quality of included studies was assessed using the PEDRo and MINORS scales. Results: A total of 8 RCTs and 3 comparative studies were included, showing a low (n = 1), fair (n = 7), and good (n = 3) methodological quality. Most of the studies included patients undergoing hemodialysis (HD) (n = 8). Self-reported sleep quality (n = 9), sleepiness (n = 2), and objective sleep status (n = 2) were the main outcomes analyzed. The most frequent exercise interventions included aerobic training (n = 7), resistance training (n = 2), or a combination of both (n = 4). Generally, exercise induced positive effects on the reported outcomes. Data synthesis indicated that participants who exercised obtained significant improvements on their self-reported sleep quality in comparison with those included in the control groups, with a mean difference in the Pittsburgh Sleep Quality Index of − 5.27 points (95% CI − 7.76, − 2.77; p < 0.001). Conclusion: Preliminary scientific evidence indicates that patients with CKD, especially those undergoing HD, report improvements in self-reported sleep quality after taking part in aerobic exercise programs, while combined training interventions yielded diverse findings. The genus Bison is one of the most common herbivorous mammals in the Late Pleistocene faunas of North America. It is also widely present throughout Mexico; however, published information on the bison species recorded in this country is very limited. We regard five species of Bison as valid in the Mexican Quaternary: B. alaskensis, B. priscus, B. latifrons, B. antiquus, and B. bison; also, given its biochronologic utility, we consider that B. antiquus is represented by two different chronomorphs with two different recognizable morphologies: a mainly Pleistocene "antiquus" morphology and a Holocene "occidentalis" morphology, both are part of the same species. Paleontological work carried out in the Mixteca Alta region of Oaxaca, Mexico, allowed the collection of additional cranial and postcranial specimens of Bison. Two different species were identified, B. antiquus and Bison cf. B. latifrons. Both species had a very wide geographic distribution in North America, with their records from Oaxaca as the most southern. Their combined records in the Mixteca Alta region allows to constrain the age of the Oaxacan localities to a range of 210–4.8 ka, most of them belonging to the Rancholabrean land mammal age. Colorectal cancer (CRC) is one of the most common tumours worldwide, and 70% of CRC patients are over 65 years of age. However, the scientific evidence available for these patients is poor, as they are underrepresented in clinical trials. Therefore, a group of experts from the Oncogeriatrics Section of the Spanish Society of Medical Oncology (SEOM), the Spanish Cooperative Group for the Treatment of Digestive Tumours, (TTD) and the Multidisciplinary Spanish Group of Digestive Cancer (GEMCAD) have reviewed the scientific evidence available in older patients with CRC. This group of experts recommends a multidisciplinary approach and geriatric assessment (GA) before making a therapeutic decision because GA predicts the risk of toxicity and survival and helps to individualize treatment. In addition, elderly patients with localized CRC should undergo standard cancer resection, preferably laparoscopically. The indication for adjuvant chemotherapy (CT) should be considered based on the potential benefit, the risk of recurrence, the life expectancy and patient comorbidities. When the disease is metastatic, the possibility of radical treatment with surgery, radiofrequency (RF) or stereotactic body radiation therapy (SBRT) should be considered. The efficacy of palliative CT is similar to that seen in younger patients, but elderly patients are at increased risk of toxicity. This work describes the synthesis of poly(lactic acid) by the ring-opening polymerization of L-lactide in the presence of oligo(butylene succinate) with two different molecular weights (Mn = 6100 and 16300 g/mol) as a macroinitiator during reactive processing. The macroinitiators were added in concentrations 1 wt%, 2.5 wt%, 5 wt%, 10 wt% and 15 wt% in respect to the L-lactide mass in the premix. The properties of the received copolymers were extensively studied with spectroscopic techniques, GPC, DSC, XRD, TGA as well as nanoindentation. Blocky copolymers were received with number average molecular weights ranging from 30 to 100 kg/mol, which decreased with increasing the PBSu content in the feed. The introduction of the flexible PBSu chains decreased the single glass transition detected, while DSC and XRD gave indications that both components crystallized in the copolymers with PBSu premix content > 5 wt%. Thermal stability was maintained and depended on the composition and molecular weight. Nanoindentation showed that despite the decreasing trend of hardness and elastic modulus with increasing PBSu content, the PLA-PBSu 2.5% copolymers had simultaneously higher elasticity modulus and strength compared to the other compositions, possibly because of a complementary effect of their high molecular weight and crystallinity. Objectives: Assess the efficacy of biomechanical preparation using a reciprocating system followed by final irrigation protocols, then intracanal medication, on reducing endotoxins and cultivable bacteria of infected teeth in irradiated patients. Materials and methods: Twenty-two infected single-rooted canals in patients submitted to head and neck radiotherapy were prepared by reciprocating motion and 2.5% NaOCl. Patients were randomly divided into two groups of 11 patients before the final irrigation protocol: apical positive pressure (APP) or passive ultrasonic activation (PUA). Both groups were treated in two sessions, using Ca(OH)2 as intracanal medication for 14 days. Root canal content sampling was performed after canal access (S1), after biomechanical preparation plus the irrigation protocol (S2), and after intracanal medication (S3). Chromogenic limulus amoebocyte lysate assay measured endotoxin levels (EU/mL), and bacterial load was determined by culture techniques (CFU/mL). Results: Treatment protocols reduced bacterial counts after S2 in both groups (p = 0.01). S3 differed from S1 (p = 0.01), but not from S2 (p = 0.4). Endotoxin levels were reduced in both groups after S2 (P = 0.03) and were lower in S3 than in S2, with significant differences in the APP group (p = 0.03). Conclusions: Biomechanical preparation using a reciprocating system and 2.5% NaOCl in irradiated teeth, followed by the irrigation protocol (APP or PUA), demonstrated efficacy in reducing endodontic contaminants. Ca(OH)2 as intracanal medication should be performed in irradiated patients with infected root canals. Background: The anterolateral thigh (ALT) free flap is commonly used for complex lower limb reconstruction. Recently, other thin perforator flaps—such as the thin-ALT flap and the superficial circumflex iliac artery perforator (SCIP) flap—have become increasingly utilized in this indication. There is limited evidence examining the performance of these thin suprafascial flaps to the standard ALT flap. We aim to systematically review the literature, assessing the survival and complication rates of the standard ALT, thin-ALT, and SCIP flaps in lower limb reconstruction. Methods: Following PRISMA guidelines, we systematically searched BIOSIS®, PubMed®, Cochrane Library®, Embase®, MEDLINE®, and Web of Science® on Jan. 27, 2021. Only primary English studies published in peer-reviewed scientific journals and that explored lower limb reconstruction were included. Results: We included 68 studies, comprising 1652 standard ALT, 187 thin-ALT, and 129 SCIP flaps. Thin-ALT flaps demonstrated significantly lower rates of full flap failure (0.6%, N = 1/187) compared to standard ALT flaps (3.9%, N = 64/1652). Thin-ALT and SCIP flaps have been more frequently used in foot reconstruction (47.6%, N = 50/105; 82.9%, N = 29/35 respectively), whereas standard ALT flaps have been more frequently used in leg (43.0%, N = 182/423) and trauma-related reconstructions (84.3%, N = 334/396). Thin-ALT and SCIP flaps have a smaller mean surface area, as well as significantly lower rates of thinning (6.2%, N = 5/80; 4.5%, N = 5/112, respectively) compared to standard ALT flaps (26.1%, N = 43/165). Conclusions: Thin-ALT and SCIP flaps demonstrate low rates of flap failure and secondary complications that are comparable to standard ALT flaps. This is the first report on the application of metagenomic analysis of the microbiome of Lake Baikal bottom sediments in order to assess the community structure and metabolic potential of the microorganisms inhabiting the upper sediment horizon at the site of a deep-water station. Compared to the metagenome of near-bottom water, relative abundance of the phyla Methylomirabilota, Chloroflexota, and Acidobacterota was found to increase, as well as that of the genes for the pathways of methane, sulfur, and thiosulfate oxidation and of nitrate reduction. Genomes of members of 12 phyla forming the basis of the studied microbiome were assembled. Reconstruction of the metabolic pathways indicated the possible coexistence of microorganisms capable of gaining carbon and energy due to oxidation of carbohydrates, methane, thiosulfate, and ammonia, as well as via anaerobic processes of fermentation, denitrification, and (to a lesser degree) sulfate reduction. Purpose: To compare corneal densitometry in a consecutive series of 52 endothelial keratoplasties (DMEK/DSAEK) using a Scheimpflug-based device after six months of follow-up. Methods: Corneal densitometry (CD) values of 102 eyes were divided into three main groups: 33 DMEKs, 19 DSAEKs, and 50 healthy eyes without previous ocular surgery. The CD values were then analyzed and compared between the groups. We measured three main layers in depth and four different concentric zones at 1, 3, and 6 months postoperatively. Results: In the DMEK group, total CD significantly decreased from 38.02 ± 10.21 grayscale units (GSU) to 31.13 ± 9.25 GSU (P < 0.01) between the first and the sixth month postoperative. In the DSAEK group, we found significant changes only between the first and three months after surgery (from 42.62 ± 9.31 GSU to 38.71 ± 10.53 GSU (P < 0.01). Regarding the concentric zones, CD in the DMEK group significantly decreased in the central zone from 33.55 ± 12.07 GSU to 30.63 ± 10.15 GSU (P < 0.01) and significantly increased in the periphery from 30.63 ± 10.15 GSU to 36.72 ± 9.37 GSU, (P < 0.01). The DSAEK group showed no changes in the central zone (from 36.91 ± 13.80 GSU to 36.14 ± 11.47 GSU, P = 0.52) and CD significantly increased in the periphery (41.91 ± 9.28 GSU, P < 0.01). Conclusion: When comparing CD values in DMEK versus DSAEK, we found no differences by layers or at central-paracentral concentric zones, although CD differences in the peripheral zones were statistically significant. Lymphedema is a chronic disorder of the lymphatic system characterized by the accumulation of interstitial fluid in tissues. It affects millions of individuals worldwide, resulting in significant disability and morbidity. Despite its prevalence, it is often an underappreciated clinical entity. In this narrative review, we critically examined and synthesized currently available peer-reviewed literature on the global epidemiology, pathophysiology, and societal burden of lymphedema. Most patients have a secondary cause of lymphedema, such as lymphatic filariasis, mostly prevalent in Asia, South America, and Africa. A smaller proportion of lymphedema is secondary to malignancy or post-oncological treatment. Breast cancer, prostate cancer, cervical cancer, and melanoma are most often associated with causing lymphedema secondary to nodal involvement. Several tools have been developed to assess the effect of lymphedema on quality of life which can cause personal and financial impacts on those affected. Lymphedema is a globally prevalent but underappreciated disease in the developed and developing world. Lymphedema causes a significant burden on quality of life and financial stability to patients. Background: ESG is a safe and effective technique in the obesity management, usually indicated in class I and II obesity. It is also an acceptable treatment in patients with class III obesity who have high surgical risk or refuse surgery. This procedure results in a significant weight loss and important improvement in metabolic comorbidities. Nevertheless, there are several procedure-related complications. Few cases of gastric perforation following ESG have been reported. We present a case of septic shock after ESG with preoperative diagnostic uncertainties. Methods: We present the case of a 54-year-old male with a BMI of 43.6 kg/m2 who underwent ESG 7 days before in an external center. The patient came to the emergency department presenting abdominal pain, nausea, and vomiting since the day after the procedure. Physical examination revealed hemodynamic instability, altered level of consciousness, diffuse abdominal pain, and a painful umbilical lump due to a complicated umbilical hernia. Emergent surgery was decided after preoperative assessment. Results: Intraoperative gastroscopy was performed, viewing a gastric ischemic ulcer covered with fibrin and a mucosal defect and suspecting a covered gastric perforation. Firstly, we performed an open approach to the complicated umbilical hernia. Subsequently, an exploratory laparoscopy was performed through the hernial ring, where a fibrin-covered area was evidenced in the anterior face of the gastric body, adhered to the round ligament by a transmural suture of the ESG. Additionally, multiple transmural sutures were observed adhered to the greater omentum and lesser sac and an intramural hematoma in the greater gastric curvature. No intra-abdominal free fluid was evidenced. A laparoscopic barbed suture of the area covered with fibrin was performed, after its release from the round ligament. The adhesions of the sutures and metallic material from the ESG were released. Finally, two abdominal drains were placed in the anterior and posterior gastric face. The patient presented superficial incisional surgical site infection and was discharged 6 days after laparoscopic surgery. Conclusions: ESG is a novel procedure, which has proven to be an effective alternative in the treatment of obesity. Neolithic farming and animal husbandry were first developed in the Near East ~ 10,000 BCE and expanded westwards, reaching westernmost Iberia no later than 5500 BCE. It resulted in major social, cultural, economic and dietary changes. Yet, the impact of this change on human mandibular morphology in Iberia is yet to be assessed, which is regrettable because mandible form is impacted by population history and diet. In this study we used Mesolithic to Chalcolithic Iberian samples to examine the impact of this transition on mandibular morphology. We also compared these samples with a Southern Levantine Chalcolithic population to assess their relationship. Lastly, we assessed dental wear to determine if the morphological differences identified were related to the material properties of the diet. We found differences between samples in mandibular shape but not size, which we attribute to contrasting population histories between Mesolithic and later populations. Some differences in the severity of dental wear were also found between Mesolithic and later Iberian samples, and smaller between the Mesolithic Iberians and southern Levantines. Little relationship was found between wear magnitude and mandibular shape. Altogether, our results show that the Mesolithic–Neolithic Iberian transition resulted in a meaningful change in mandibular morphology, which was likely driven more by population history than by dietary change. The Head-on Quenching (HoQ) of laminar premixed ammonia–hydrogen-air flames under lean to stoichiometric condition is numerical investigated. Detailed chemistry including 34 reactive species and detailed multi-component transport model including thermal diffusion (Soret effect) are applied. The quenching distance is considered as a representative quantity for the HoQ process, and the influence of different system parameters on it has been investigated. These parameters involve fuel/air equivalence ratios, hydrogen content in gas mixture and pressure. It was found that an increase of quenching distance can be caused by a lower hydrogen addition and a leaner mixture condition. Furthermore, it was found that, regardless of the gas mixture, the quenching distance decreases monotonically with increasing pressure, obeying a power function with the exponent - 0.7. Moreover, numerical results show a relation between the quenching Peclet number and the dimensionless wall heat flux normalized by the flame power. Groups (Grp) 3 and 4 are aggressive molecular subgroups of medulloblastoma (MB), with high rates of leptomeningeal dissemination. To date, there is still a paucity of biomarkers for these subtypes of MBs. In this study, we investigated the clinical significance and biological functions of Musashi-1 (MSI1) in Grp3 and Grp4-MBs. First, we assessed the expression profile of MSI1 in 59 primary MB samples (15-WNT, 18-SHH, 9-Grp3, and 17-Grp4 subgroups) by qRT-PCR. MSI1 mRNA expression levels were also validated in an additional public dataset of MBs (GSE85217). The ROC curve was used to validate the diagnostic standards of MSI1 expression. Next, the potential correlated cell-cycle genes were measured by RNA-Seq. Cell cycle, cell viability, and apoptosis were evaluated in a Grp3/Grp4 MB cell line after knockdown of MSI1 and cisplatin treatment. We identified an overexpression of MSI1 with a high accuracy to discriminate Grp3/Grp4-MBs from non-Grp3/Grp4-MBs. We identified that MSI1 knockdown not only triggered transcriptional changes in the cell-cycle pathway, but also affected G2/M phase in vitro, supporting the role of knockdown of MSI1 in cell-cycle arrest. Finally, MSI1 knockdown decreased cell viability and sensitized D283-Med cells to cisplatin treatment by enhancing cell apoptosis. Based on these findings, we suggest that MSI1 modulates cell-cycle progression and may play a role as biomarker for Grp3/Grp4-MBs. Donato García was the mineralogy professor at the Royal Cabinet of Natural History and Central University of Madrid from 1816 to 1853. In addition to teaching mineralogy, he gathered geological and mineralogical material for didactic purposes, and for exhibitions in the hall of the Cabinet. He was a key figure in developing the subject matter of mineralogy in Spain during the first half of the nineteenth century. Donato Garcia likewise promoted the careers of very important future teachers at the Military Academy of Engineers, Natural Sciences Museums, and Universities, integrating oral lectures, mineral testing, visual recognition, and the use of crystallographic models. This paper investigates how the crystallographic system proposed by Haüy (in the mid-thirties) was assimilated through unpublished documents of Donato García's mineralogical lessons from 1824 to 1825, obtained from Spain's Archivo General de Palacio (Madrid). Hence, this manuscript can be considered the first scientific work reliably attributable to Donato and based on the notes of his pupil José Musso y Valiente, which stands as an early attempt to create a handbook of mineralogy in Spanish. Background: Periventricular nodular heterotopia (PVNH) is a congenital brain malformation often associated with seizures. We aimed to clarify the spectrum of epilepsy phenotypes in PVNH and the significance of specific brain malformation patterns. Methods: In this retrospective cohort study, we recruited people with PVNH and a history of seizures, and collected data via medical record review and a standardized questionnaire. Results: One hundred individuals were included, aged 1 month to 61 years. Mean seizure onset age was 7.9 years. Ten patients had a self-limited epilepsy course and 35 more were pharmacoresponsive. Fifty-five had ongoing seizures, of whom 23 met criteria for drug resistance. Patients were subdivided as follows: isolated PVNH ("PVNH-Only") single nodule (18) or multiple nodules (21) and PVNH with additional brain malformations ("PVNH-Plus") single nodule (8) or multiple nodules (53). Of PVNH-Only single nodule, none had drug-resistant seizures. Amongst PVNH-Plus, 55% with multiple unilateral nodules were pharmacoresponsive, compared to only 21% with bilateral nodules. PVNH-Plus with bilateral nodules demonstrated the highest proportion of drug resistance (39%). A review of genetic testing results revealed eight patients with pathogenic or likely pathogenic single-gene variants, two of which were FLNA. Five had copy number variants, two of which were pathogenic. Conclusions: The spectrum of epilepsy phenotypes in PVNH is broad, and seizure patterns are variable; however, epilepsy course may be predicted to an extent by the pattern of malformation. Objectives: Stroke treatment with intravenous tissue-type plasminogen activator (tPA) is effective and efficient, but as its benefits are highly time dependent, it is essential to treat the patient promptly after symptom onset. This study evaluates the cost-effectiveness of a blood biomarker test to differentiate ischemic and hemorrhagic stroke to guide pre-hospital treatment with tPA in patients with suspected stroke, compared with standard hospital management. The standard care for patients suffering stroke consists mainly in diagnosis, treatment, hospitalization and monitoring. Methods: A Markov model was built with four health states according to the modified Rankin scale, in adult patients with suspected moderate to severe stroke (NIHSS 4-22) within 4.5 hours after symptom onset. A Spanish Health System perspective was used. The time horizon was 15 years. Quality-adjusted life-years (QALYs) and life-years gained (LYGs) were used as a measure of effectiveness. Short- and long-term direct health costs were included. Costs were expressed in Euros (2022). A discount rate of 3% was used. Probabilistic sensitivity analysis and several one-way sensitivity analyses were conducted. Results: The use of a blood-test biomarker compared with standard care was associated with more QALYs (4.87 vs. 4.77), more LYGs (7.18 vs. 7.07), and greater costs (12,807€ vs. 12,713€). The ICER was 881€/QALY. Probabilistic sensitivity analysis showed that the biomarker test was cost-effective in 82% of iterations using a threshold of 24,000€/QALY. Expanding the fuel flexibility of continuous combustion systems to include multiphase fuel combustion offers additional support to combat the problem of energy security and, potentially, environmental pollution. In this study, apart from establishing stability limits and measuring post-combustion emissions, flames generated from simultaneous combustion of biodiesel and syngas were examined using C2* and CH* chemiluminescence imaging to capture changes in the reaction zone. The proportion of syngas in the fuel mix was varied from 0 to 30% content (by energy contribution) while maintaining a total power output of 15 kW. The overall equivalence ratio was held at 0.7 in cases other than for determining the flammability range. The results indicate a reduction of stability limits as gas proportion in fuel blend increases. Also, chemiluminescence imaging of the two targeted species suggest a general reduction in reaction rate as well as reaction zone area and length with increase in gas ratio in the dual phase tests. Furthermore, emissions performance in the context of NOx and CO was investigated as liquid-to-gas ratios were altered. Medulloblastoma is the most common type of pediatric malignant primary brain tumor, and about one-third of patients die due to disease recurrence and most survivors suffer from long-term side effects. MB is clinically, genetically, and epigenetically heterogeneous and subdivided into at least four molecular subgroups: WNT, SHH, Group 3, and Group 4. We evaluated common differentially expressed genes between a Brazilian RNA-seq GSE181293 dataset and microarray GSE85217 dataset cohort of pediatric MB samples using bioinformatics methodology in order to identify hub genes of the molecular subgroups based on PPI network construction, survival and functional analysis. The main finding was the identification of five hub genes from the WNT subgroup that are tumor suppressors, and whose lower expression is related to a worse prognosis for MB patients. Furthermore, the common genes correlated with the five tumor suppressors participate in important pathways and processes for tumor initiation and progression, as well as development and differentiation, and some of them control cell stemness and pluripotency. These genes have not yet been studied within the context of MB, representing new important elements for investigation in the search for therapeutic targets, prognostic markers or for understanding of MB biology. Punta de Gavilanes (Mazarrón Bay, Spain) was occupied from third millennium cal BC to the first century BC. Overall, the archaeobotanical remains suggest that agriculture and gathering coexisted in the site since the beginning of the occupation of the site. Depending on the sea level variation, the site passed from island in the middle Holocene to peninsula in the late Holocene. Seeds from this archaeological site associated with radiocarbon dates of ≈ 4200 and ≈ 1500 cal years BP include specimens of eight species of shrubs and trees, of which a winter flowering shrub (Coronilla talaverae Lahora and Sánchez-Gómez), is an endangered species that no longer occurs on Punta de Gavilanes area. The seeds of this endemic Coronilla species are associated with materials dated ≈ 3900 cal years BP in a Bronze Age cultural context. The vanished species was presumably locally exterminated by human alteration of its natural habitat or because of environmental changes. The species of Coronilla identified from Punta de Gavilanes is known from relatively distant seashore areas. This endangered species could be effectively preserved by reintroduction to areas that it occupied prior to human alteration, in particular Punta de los Gavilanes. [ABSTRACT FROM AUTHOR]

People with Alzheimer's disease (AD) and delusions have worse quality of life and prognosis. However, early markers of delusions have not been identified yet. The present study investigated whether there are any detectable differences in grey matter (GM) volume and cognitive changes in the year before symptom onset between patients with AD who did and did not develop delusions. Two matched samples of AD patients, 63 who did (PT-D) and 63 who did not develop delusions (PT-ND) over 1 year, were identified from the Alzheimer's Disease Neuroimaging Initiative database. The Neuropsychiatric Inventory (NPI) was used to assess the presence of delusions. Sixty-three additional matched healthy controls (HC) were selected. Repeated-measures ANCOVA models were used to investigate group-by-time effects on the volume of selected GM regions of interest and on cognitive performance. No neurocognitive differences were observed between patient groups prior to symptom onset. Greater episodic memory decline and GM loss in bilateral caudate nuclei, medio-temporal and midline cingulo-parietal regions were found in the PT-D compared with the PT-ND group. A pattern of faster GM loss in brain areas typically affected by AD and in cortical and subcortical targets of dopaminergic pathways, paralleled by worsening of episodic memory and behavioural symptoms, may explain the emergence of delusions in patients with AD. Objectives: This RCT investigated the impact of N-acetylcysteine (NAC) and calcium hydroxide [Ca(OH)2] intracanal medications (ICMs) in primary endodontic infection with apical periodontitis (PEIAP). Materials and methods: Thirty-six teeth with PEIAP were randomly divided into groups according to the ICM: NAC, Ca(OH)2 + saline solution (SSL), and Ca(OH)2 + 2% chlorhexidine-gel (2% CHX-gel) (all, n = 12). Root canal samples (RCSs) were collected before (s1) and after instrumentation (s2) and after 14 days of ICM (s3). Chemomechanical preparation (CMP) was performed with a Reciproc file and 2.5% NaOCl. Checkerboard DNA–DNA hybridization was used to assess 40 target bacteria species. Results: At s1, bacterial DNA was detected in 100% of RCSs (36/36). All 40 bacterial species were found in PEIAP. The mean number of species per RCS was 17.92 ± 13.18. The most frequent bacteria were S. mitis (65%), E. nodatum (63%), E. faecalis (63%), F. nucl sp vicentii (58%), T. forsythia (58%), and F. periodonticum (56%). CMP reduced the mean number of species per RCS to 6.8 ± 2.36 (p < 0.05). At s3, the intragroup analysis revealed a broader antimicrobial activity for Ca (OH)2 + 2% CHX-gel and NAC than Ca(OH)2 + SSL (p < 0.05). NAC eliminated 8/12 bacteria species resistant to both Ca (OH)2 ICMs, including P. micra, P. nigrescens, T. denticola, A. israelii, P. endodontalis, P. acnes, C. ochracea, and E. corrodens. Conclusions: Ca (OH)2 + 2% chlorhexidine gel (2% CHX gel) showed a greater bacterial elimination over the number of bacterial species; however, NAC eliminated 8/12 bacteria species resistant to both Ca (OH)2 ICMs (RBR-3xbnnn). Clinical relevance: The use of intracanal medication with a broad antimicrobial activity can optimize root canal disinfection. Ca(OH)2 + 2% CHX gel and NAC showed a broader antimicrobial activity than Ca(OH)2 + SSL against endodontic pathogens in primary root canal infection. The chestnut gall wasp Dryocosmus kuriphilus is a major insect pest affecting chestnut trees worldwide. Medium and long-term control of this pest can be improved by using resistant or less susceptible cultivars and hybrid clones. In addition, little is known about the local patterns of dispersal of the pest. We obtained data from trees of 3 chestnut species and 27 hybrid clones in a field trial in NW Spain with the aim of evaluating the susceptibility of the material to the gall wasp and identifying possible drivers of local spatial dispersal. In the first 3 years of the invasion by D. kuriphilus, the number of trees attacked and the number of galls on each tree were spatially clustered. Tree height significantly predicted both variables, suggesting that gall wasps may use visual cues to locate suitable host trees, at least in the early stage of invasion. Assessment of the susceptibility of hybrid clones/pure species must take concurrent indicators of infestation levels into account. We suggest the use of indices involving galls on shoots because these enable good assessment of the damage to chestnut trees. The study findings add to existing knowledge on the susceptibility of hybrid chestnut clones. We report, for the first time, two hybrid clones resistant to the pest and one hybrid clone which exhibited consistently low values for all of the indicators of infestation level. The results have important implications regarding selection of plant material for use in afforestation in Spain, where the current high rate of chestnut planting is expected to continue. Supernumerary robotic limbs are innovative devices in the field of wearable robotics which can provide humans with unprecedented sensorimotor abilities. However, scholars have raised awareness of the ethical issues that would arise from the large adoption of technologies for human augmentation in society. Most negative attitudes towards such technologies seem to rely on an allegedly clear distinction between therapy and enhancement in the use of technological devices. Based on such distinction, people tend to accept technologies when used for therapeutic purposes (e.g., prostheses), but tend to raise issues when similar devices are used for upgrading a physical or cognitive ability (e.g., supernumerary robotics limbs). However, as many scholars have pointed out, the distinction between therapy and enhancement might be theoretically flawed. In this paper, we present an alternative approach to the ethics of supernumerary limbs which is based on two related claims. First, we propose to conceive supernumerary limbs as tools that necessarily modify our psychological and bodily identity. At the same time, we stress that such a modification is not ethically bad in itself; on the contrary, it drives human interaction with the environment. Second, by comparing our view with the extended mind thesis, we claim that the mediation through tools is crucial for the formation of novel meanings and skills that constitute human interaction with the world. We will relate the latter claim to enactivism as a helpful theoretical perspective to frame issues related to artificial limbs and, more in general, to technologies for augmentation. Based on this approach, we finally sketch some suggestions for future directions in the ethics of supernumerary limbs. [ABSTRACT FROM AUTHOR]
