With a higher prevalence in schizophrenia, delusions of thought insertion (TI) are regarded as one of the most severe symptoms of psychosis. Patients suffering from TI report that external agents are able to place thoughts into their minds or skulls. A version of the doxastic approach characterizes delusions as abnormal beliefs rooted in anomalous experiences. Nonetheless, the exact role of these experiences in determining the content and the way in which delusional beliefs are fixated is still under debate. While endorsement models claim that the abnormal experience comprises the very content of the delusional beliefs, explanationist approaches claim that delusional beliefs emerge as explanations for abnormal experiences with less specific content. This paper combines conceptual analysis with phenomenological data to examine the merits of both endorsement and explanationist approaches to TI. I propose that potential solutions to the dispute could lay in finding a middle ground between the two approaches, and non-exhaustive ways in which hybrid doxastic approaches to TI could be formulated. In this paper I aim to illuminate the significance of thought insertion for debates about the first-person concept. My starting point is the often-voiced contention that thought insertion might challenge the thesis that introspection-based self-ascriptions of psychological properties are immune to error through misidentification relative to the first-person concept. In the first part of the paper I explain what a thought insertion-based counterexample to this immunity thesis should be like. I then argue that various thought insertion-involving scenarios do not give rise to successful counterexamples to the immunity of the target class of self-ascriptions. In the second part of the paper I turn to defend a Metasemantic Explanation of why the immunity thesis holds. The Metasemantic Explanation rests on a reference-fixing story about the mental 'I' whose key contention is that introspective impressions play an essential role in fixing its reference. It is part of my argument in favour of the proposed reference-fixing story, as well as of the Metasemantic Explanation, that they respect the paradigmatic features of self-ascriptions of inserted thoughts. Delusions of thought insertion involve subjects claiming that external agents of different nature had placed thoughts into their minds/heads. However, despite being regarded as one of the most severe and complex symptoms of psychotic disorders, a number of disagreements surround the description of its most fundamental phenomenology. This work has reviewed classic and current research on thought insertion in order to examine and clarify its main experiential features as reported by patients from a first-person perspective. The review shows that such features can be grouped into two categories: (i) experiential changes characterizing the period preceding the adoption of the delusion and, (ii) subjective features of full-blown delusional cases. While the discussion of the latter set of experiential features has received some attention within literature, the examination of the former set has been largely neglected. After this, the review offers a discussion of the most important conceptual disagreements surrounding the phenomenological descriptions of the symptom. Overcoming disagreements regarding the experiential structure of thought insertion is fundamental to elaborate phenomenologically and empirically coherent explanatory theories of the symptom and advance its clinical treatment. It has long been known that the popular account of egocentric thoughts developed by David Lewis is in conflict with a natural account of communication, according to which successful communication requires the transmission of a thought content from speaker to hearer. In this paper, I discuss a number of proposed attempts to reconcile these two accounts of egocentric thought and communication. Each of them postulates two kinds of mental content, where one is egocentric, and the other is transmitted from speaker to hearer in communication. I argue that Ninan’s account, which involves multi-centered contents, and Kölbel’s and Moss’s accounts, which postulate centered and uncentered proxy contents, respectively, are unsatisfactory. Therefore, I propose a two-dimensionalist account, according to which egocentric thoughts are represented by primary intensions, while the thoughts shared by speaker and hearer in cases of successful communication are represented by secondary intensions. Finally, I argue that aside from being able to reconcile Lewis’s account of egocentric thoughts with the natural account of communication, two-dimensionalism also provides the means for modeling agreement and disagreement. On a widely held view, episodes of inner speech provide at least one way in which we become conscious of our thoughts. However, it can be argued, on the one hand, that consciousness of thoughts in virtue of inner speech presupposes (unconscious) interpretation of the simulated speech. On the other hand, the need for such self-interpretation (even if unconscious) seems to clash with distinctive first-personal characteristics that we would normally ascribe to consciousness of one’s own thoughts: a special reliability; a lack of conscious ambiguity and incomprehensibility; and a sense of causal agency. I try to resolve this puzzle by proposing an account for the requisite self-interpretation of inner speech in terms of Bayesian probabilistic inference. Drawing on “perceptual loop” accounts of speech control, I argue that such interpretive probabilistic inferences are used for the control of inner speech, and that as a consequence of this function, they are biased toward the correct interpretations. I conclude by showing how this model can explain the first-personal characteristics of consciousness of one’s own thoughts. The aim of this study was to assess the mental representation of the insertion axis of surgeons with different degrees of experience, and reproducibility of the insertion axis in repeated measures. A mastoidectomy and a posterior tympanotomy were prepared in five different artificial temporal bones. A cone-beam CT was performed for each temporal bone and the data were registered on a magnetic navigation system. In these five temporal bones, 16 surgeons (3 experts; >50 cochlear implant surgery/year; 7 fellows with few cochlear implant experience, and 6 residents) were asked to determine the optimal insertion axis according to their mental representation. Compared to a planned ideal axis, the insertion axis was better determined by the experts with higher accuracy (axial: 7° ± 1.5°, coronal: 6° ± 1.5°) than fellows (axial: 14° ± 1.7°, coronal: 13° ± 1.7°; p < 0.05), or residents (axial: 15° ± 1.5°; p < 0.001, coronal: 17° ± 1.9°; p < 0.001). This study suggests that mental representation of the cochlea is experience-dependent. A high variation of the insertion axis to the scala tympani can be observed due to the complexity of the temporal bone anatomy and lack of landmarks to determine scala tympani orientation. Navigation systems can be used to evaluate and improve mental representation of the insertion axis to the scala tympani for cochlear implant surgery. [ABSTRACT FROM AUTHOR]

Stephens and Graham () maintain that in cases of thought insertion, the sense of ownership is preserved, but there is a defect in the sense of agency (i.e. the sense that one is the author or initiator of the thought). However, these theorists overlook the possibility that subjectivity might be preserved despite a defect in the sense of ownership. The claim that schizophrenia centers upon a loss of a sense of ownership is supported by an examination of some of the other notable disownership symptoms of the disorder, such as bodily alienation and experiences of 'unworlding.' Is there a way to make sense of the 'underlying characteristic modification' that ties together the various symptoms of schizophrenia and disrupts subjects' 'hold' on their own bodies and surroundings? I will argue that what accounts for subjects' usual sense of ownership are fully embodied processes of causal-contextual information integration, which are made possible by subjects' affective framing patterns. Attenuated affective framings lead to a loss of a sense of ownership and cause subjects to lose their 'grip' on bodily sensations and mental states, which ultimately can result in experiences of thought insertion. I will conclude with some brief remarks about implications for treatment, and point to several body-centered intervention methods that might help to restore subjects' sense of ownership. The standard approach to the core phenomenology of thought insertion characterizes it in terms of a normal sense of thought ownership coupled with an abnormal sense of thought agency. Recently, Fernández ( 2010 ) has argued that there are crucial problems with this approach and has proposed instead that what goes wrong fundamentally in such a phenomenology is a sense of thought commitment, characterized in terms of thought endorsement. Some of the most well-known arguments against epistemic externalism come in the form of thought experiments involving subjects who acquire beliefs through anomolous means such as clairvoyance. These thought experiments purport to provide counterexamples to the reliabilist conception of justification: their subjects are intuitively epistemically unjustified, yet meet reliabilist externalist criteria for justification. In this article, I address a recent defence of externalism due to Daniel Breyer, who argues that externalists need not consider such subjects justified, since they fail to own those beliefs in a way required for epistemic evaluability. I argue that the concept of belief ownership Breyer adopts leaves his account open to related counterexamples, and suggest a modification, drawing on analogies between these cases and cases of delusions, such as thought insertion. I will argue that a concept of authorship developed in the literature on delusions better grounds the sense of attribution required for epistemic evaluability. [ABSTRACT FROM AUTHOR]

Shaun Gallagher has actively looked into the possibility that psychopathologies involving 'thought insertion' might supply a counterexample to the Cartesian principle according to which one can always recognize one's own thoughts as one's own. Animated by a general distrust of a priori demonstrations, Gallagher is convinced that pitting clinical cases against philosophical arguments is a worthwhile endeavor. There is no doubt that, if true, a falsification of the immunity to error through misidentification would entail drastic revisions in how we conceive the boundary between self and other. However, I argue that (1) the idea of unearthing an exception to the Cartesian thesis is, on further reflection, not a realistic prospect and that (2) this casts doubt on the attempt to conjoin first-person phenomenology and third-person cognitive science in the service of philosophical debates. We challenge Gallagher’s distinction between the sense of ownership (SO) and the sense of agency (SA) as two separable modalities of experience of the minimal self and argue that a careful investigation of the examples provided to promote this distinction in fact reveals that SO and SA are intimately related and modulate each other. We propose a way to differentiate between the various notions of SO and SA that are currently used interchangeably in the debate, and suggest a more gradual reading of the two that allows for various blends of SO and SA. This detailed clinical report of a typical Capgras delusion (CD) in a demented patient is presented in order to foster future descriptions in neurological cases. In the framework of a recently developed model of familiar person processing, it is suggested that CD might be due to a dysfunction at the level of Person Identity Nodes.
