Background: Due to the shortage of midwives in Germany, it is not always possible to provide comprehensive basic postnatal care for young families. Therefore, it is quite likely that general practitioners (GP), who serve as the first contact person in all health matters in our health care system, will be increasingly consulted regarding questions on postnatal care in the future. Consequently, the aim of this qualitative study was to explore central competencies for the care of families in the postnatal phase. Methods: Midwives were questioned about their postnatal activities with the help of a semistructured questionnaire. Thereafter, the interviews were evaluated using the Mayring method. Results: Interviews were conducted with 25 midwives with an average age of 45 years. Knowledge about the (patho)physiological processes of the mother and child during the postnatal phase, a supportive basic attitude, and time are important elements in the work with young postnatal families. Additionally, it is important to obtain competences in psychosocial issues. Conclusion: The acquisition of competences for the postnatal phase appears to be useful for GPs. Background: A good 30 years ago general medicine fought for the status of a separate specialty against extensive resistance. Characteristic for the previous state of the "lessness syndrome" was, among other things, that there was no separate terminology, no defined territorial boundaries and no academic tradition in the old federal states of the Federal Republic of Germany (FRG). The aim of the present analysis was to present hypotheses for the development of the specialist in general medicine in Germany as an essential step out of the "lessness". Methods: General practitioners from the former German Democratic Republic (GDR) and the FRG were interviewed on the development of the specialist in general medicine using a partially standardized interview guide. The interviews were analyzed deductively and inductively according to Mayring. Results: In total, interviews were conducted with 28 participants. The average age of the participants was 75 years (minimum 50 years, maximum 91 years), 18% were female and 64% originally worked in the FRG. The participants saw, among other things, the blurred boundaries to other fields,which characterized the cooperation with specialists at that time and the lack of acceptance of the independence of the discipline as inhibiting factors in the development of a separate field. An accompanying symptom of this situation was the lack of and slow progress in establishing the discipline at universities. There was an important influence of the general practitioners qualified in the GDR for the introduction of an all-German specialist in general medicine. Conclusion: The specialist status for general medicine has been influenced by the self-image for the discipline in the former GDR. The development in the areas of optimization of further training and in research, which continues to this day, ensures that the field of general medicine can develop with increasing self-confidence. Background Due to the COVID-19 pandemic, the Objective Structured Clinical Examination (OSCE) examinations were cancelled in the short term. This project described how to offer medical students an alternative assessment at Miller's competence level of "shows how". Methods Students produced videos in which they can be seen performing various physical examinations related to family medicine consultations. Trained assessors rated the videos uploaded to the learning platform and gave verbal feedback if requested. Results The grading (65 "very good" and 38 "good") was very mild. Twelve per cent of the students requested verbal feedback. Their evaluation of the procedure was mixed. The evaluators consistently assessed their task and the process as positive. Technically, the video-based assessment could be implemented without any problems. Conclusions The short-term implementation of a video-based assessment in the context of medical education proved to be feasible. It could serve not only as an emergency solution under a pandemic, but also as an additional opportunity for formative feedback in the future. [ABSTRACT FROM AUTHOR]

Background From January 2022, physicians in Germany are obliged to issue prescriptions electronically. The so-called e-prescription is considered to play a key role to promote the digital connection within the healthcare sector. The aim of the study was to identify barriers and positive factors for the use of e-prescriptions for doctors and pharmacists, based on experience gathered from the "GERDA" project. Methods Doctors and pharmacists from Schleswig-Holstein and Baden- Württemberg with different experiences with e-prescriptions were interviewed using a semi-structured interview guide. The interviews were analyzed by using the qualitative content analysis by Mayring. Results A total of 26 participants were interviewed (mean = 54.3 years; SD = 8.9; min: 30 y., max: 71 y.), the proportion of female participants was 33 %. The interviewees considered the connection to the electronic patient file (ePA) and the associated improved patient safety as a major advantage of e-prescriptions. Among other things, a reimbursement of the pending acquisition costs for hardware and software was beneficial. However, participants considered the threat of losing free choice of pharmacies through the convenient transmission of prescriptions to the foreign mail order businesses as a barrier. To resolve existing legal gray areas, the elimination of faxed prescriptions was named, which could result in time advantages for the patients. Conclusions Despite many concerns, the respondents appraised advantages using e-prescriptions, especially with regard to a better network of doctors and pharmacists. At the 55th Congress for General Medicine and Family Medicine (DEGAM) in Lübeck, the German Armed Forces (Bundeswehr) medical service was represented for the first time with several contributions at a DEGAM congress. Family physicians form the largest group of specialists in the Bundeswehr. In speciality training as well as in provided care there are large overlaps between civilian family physicians and those in the armed forces. The aim of this article is to report on the workshop "Learning together and from one another - synergy potentials in speciality training and care between the medical services of the Bundeswehr and civilian family medicine". Brief lectures and presentations of the current postgraduate training and care models from both "worlds" on the subjects of postgraduate training to become a family physician in the armed forces, the competence-based general medicine curriculum and the German network of competence centers in postgraduate training formed starting points for a joint discussion. The first very specific plan that emerged was participation in and use of the competence centers for postgraduate training by trainers and trainees in the armed forces, in order to promote exchange at this level as well.
