Introduction: Total meniscus resection usually leads to osteoarthritis of the knee joint. Preservation and refixation of the injured menisci are therefore of great clinical importance. Materials and Methods: The present study examines 40 meniscal injuries in 37 patients that were arthroscopically treated with Clearfix meniscal screws (Mitek, Norderstedt, Germany) in the period from August 1999 to December 2002. The mean patient age was 27.7 (range 16-62) years. Nine patients were female. A total of 24 patients (27 meniscal tears) also had an anterior cruciate ligament (ACL) lesion, and 5 patients had cartilage injuries. Twenty-two patients were treated within the first 2 weeks following the trauma, 10 patients within 8 weeks, and 5 patients after 8 weeks. The lesions were a bucket-handle tear of the medial meniscus in 11 cases and a bucket-handle tear of the lateral meniscus in 2 cases. The posterior horn of the medial meniscus was torn in 13 cases, and the pars intermedia in 3 cases. The posterior horn of the lateral meniscus was torn in 8 cases, and the pars intermedia in 3 cases. The mean tear length was 2.9 (+/- 1.5) cm and was fixed with an average of 1.8 (+/- 0.7) screws. In 7 cases, the anterior horn was treated with an additional meniscal suture. A total of 35 patients were examined after an average of 18 (range 7-45) months. Results: In the event of a moderate outcome, MRI was performed as part of the follow-up investigation. The average Lysholm score was 93 (+/- 7.4), the Tegner activity index was 6.3 (+/- 2.0) before the accident and 5.8 (+/- 2.0) at the follow-up, the Marshall knee score was 47 (+/- 3.8). The VAS pain assessment was 1.6 (+/- 1.3) and the VAS function assessment was 7.9 (+/- 1.6). Of the 7 patients with a moderate result, 2 patients without additional ligament lesions suffered re-ruptures after and 13 months, respectively. The other 5 patients with a moderate result each had multiple accompanying injuries or pre-existing damage to the affected knee joint. Conclusion: The Clearfix screws achieved a clinical success rate of 82% in isolated meniscal tears in stable knee joints and a clinical success rate of 100% with additional ACL reconstruction. We investigated the combinatorial effects of whole-body vertical vibration (WBVV) with the primarily osteoanabolic parathyroid hormone (PTH) and the mainly antiresorptive strontium ranelate (SR) in a rat model of osteoporosis. Ovariectomies were performed on 76 three-month-old Sprague-Dawley rats (OVX, n = 76; NON-OVX, n = 12). After 8 weeks, the ovariectomized rats were divided into 6 groups. One group (OVX + PTH) received daily injections of PTH (40 µg/kg body weight/day) for 6 weeks. Another group (OVX + SR) was fed SR-supplemented chow (600 mg/kg body weight/day). Three groups (OVX + VIB, OVX + PTH + VIB, and OVX + SR + VIB) were treated with WBVV twice a day at 70 Hz for 15 min. Two groups (OVX + PTH + VIB, OVX + SR + VIB) were treated additionally with PTH and SR, respectively. The rats were killed at 14 weeks post-ovariectomy. The lumbar vertebrae and femora were removed for biomechanical and morphological assessment. PTH produced statistically significant improvements in biomechanical and structural properties, including bone mineral density (BMD) and trabecular bone quality. In contrast, SR treatment exerted mild effects, with significant effects in cortical thickness only. SR produced no significant improvement in biomechanical properties. WBVV as a single or an adjunctive therapy produced no significant improvements. In conclusion, vibration therapy administered as a single or dual treatment had no significant impact on bones affected by osteoporosis. Five of 97 cruciate ligament prostheses (5.15 per cent) implanted for an average of 3 years (8-53 months) in 70 patients with acute chronic cruciate ligament instability were removed due to failure after a mean of 9 (3-15) months. Investigation of the explanted polyethylene terephthalate (PET)-grafts by scanning electron and light microscopy showed not bony but fibrous tissue ingrowth in all parts of the prostheses. The extent of local host response in the interface region differed with the intensity of mechanical strain at the three parts of the prostheses (intra-articular, bone tunnel, cortical). Inflammatory cells and foreign body giant cells were abundant close to the rupture sites at the intra-articular entries of the bone tunnels and scarce at the intra-articular part of the prostheses and at the cortical anchoring. Evaluation of the reasons for early prostheses failure indicates that (1) non-ideal placement of bone tunnels and (2) inadequate strain were responsible for reoccurring instability and rupture after prosthetic cruciate ligament replacement. In a prospective clinical study of 54 patients with acute anterior cruciate ligament instability, 56 artificial ligaments made of polyethylene terephthalate (Trevira hochfest) were implanted to restore knee stability. The average follow-up of these artificial knee ligaments was 40.2 (12-79) months; five implants (10%) had to be explanted due to failure after an average of 17.8 (6-50) months. All explants were examined by histological and ultrastructural methods in a device retrieval analysis. With regard to short- and medium-term artificial ligament failure in the human knee joint, a non-isometric surgical implantation technique, inappropriate strain during rehabilitation and implant fatigue and wear were responsible for ligament failures. Abstract. Technetium-99m labelled antigranulocyte antibodies are ready to use and are sensitive and specific in the diagnosis of infectious diseases. [sup 99m]Tc-SSEA antigranulocyte antibodies have a very high affinity constant (K[sub d] 10[sup -12] M) for human neutrophils (PMNs), and excellent imaging qualities with high target/background ratios. The aim of this study was to compare the diagnostic accuracy of the [sup 99m]Tc-anti-SSEA-1 monoclonal antibody (Mab) with that of [99m]Tc-hexamethylpropylene amine oxime (HMPAO)-labelled white blood cells (WBCs). To this end, 17 patients with 23 proven infectious foci were examined with 555 MBq [sup 99m]Tc-anti-SSEA-1 MAb and with 370 MBq [sup 99m]Tc-HMPAO labelled autologous leucocytes within a period of 7 days. All the infections were confirmed by culture, biopsy, surgery and follow-up. Whole-body images and planar spot views with the antibody were performed at 1-h, 4-h and 24-h post injection; the biodistribution of the antibody was quantified, absorbed radiation doses were calculated and the diagnostic results were compared with the [sup 99m]Tc-HMPAO WBC images. Human anti-mouse antibody (HAMA) evaluation was performed in all patients before and 3 months after antibody injection. Blood was drawn at different times after [sup 99m]Tc-anti-SSEA-1 MAb injection to determine the amount of granulocyte-associated radioactivity and to calculate recovery. [sup 99m]Tc-anti-SSEA-1 MAb scintigraphy detected all 23 lesions, while 21 were detected with [sup 99m]Tc-HMPAO WBC scan. In this small group of patients, the sensitivity and specificity of [sup 99m]Tc-anti-SSEA-1 MAb scintigraphy were 95% and 96% respectively, as compared with 91% and 82% respectively for [sup 99m]Tc-HMPAO WBC scan. Classical stable plate osteosynthesis with its anatomical repositioning, absolute stability between fragments and medial support should only be applied to joint fractures and spongy bone. In cortical bone, the anatomical reposition connected with the plate promotes bone necrosis along the fracture and prevents callus formation. Direct cortical synthesis, a method also known as ``primary bone healing'', serves the bone's revascularisation and is not necessarily aimed at healing. Thus, this may also be termed ``necrosis healing''. Along the shaft of long bones, elastic plate osteosynthesis, a biological method, is safer than and superior to the rigid technique. This even applies to short oblique and transverse fractures insofar as nailing does not appear feasible. Elasticity is achieved by leaving a flexible stretch of at least 2 - 4 holes, i.e. as long as possible, without screws over the fracture and by employing a titanium plate. Thus, there is no punctate fatigue leading to plate breakage. The fitting of third fragments is deliberately left out. The same applies to all kinds of compression with lag screws, tension devices or DC-gliding holes - and this with the intention of allowing micromovements in the fracture's fissure. Periost and muscle are not removed and the fracture is not examined. Healing occurs spontaneously via a fixating callus forming within the first 3 - 6 weeks out of the periost-soft tissue combination. Histomorphological investigation dates the first woven- bone bridges between the fragments to 3 weeks subsequent to the accident. In Göttingen University trauma centre, 87 fractures have been attended to over 2 years using this technique. Despite considerable soft-tissue damage, no delayed bone healing, pseudoarthrosis or bone infection has been observed. The risks of elastic plate osteosynthesis lie in unbiological and exaggerated reposition methods, too short a flexible stretch, and insufficient anchorage of the screws. [ABSTRACT FROM AUTHOR]
