Depressive symptoms display heterogeneous trajectories across adolescence, which can lead to different consequences. This study aimed to identify trajectories of depressive symptoms from adolescence to young adulthood, assessing the association with social and health outcomes at young adulthood. Adolescents born in 1990, enrolled in schools of Porto, Portugal, in 2003–2004 (EPITeen study) completed the Beck Depression Inventory II (BDI-II) at 13, 17 and 21 years. Mixed models and model-based clustering were used to describe the trajectories in the BDI-II score (n = 2010). Outcomes were assessed at age 21 years with self-administered questionnaires and face-to-face interviews (n = 1594). Odds ratios or regression coefficients, with 95% confidence intervals, were estimated using logistic and linear regression. Three trajectory classes of depressive symptoms were identified, similar in shape in both sexes: High (8.4%), Moderate (31.3%) and Low (60.2%). Participants in High or Moderate classes were more likely to describe lower scores of community involvement, more medical appointments during the last year, higher levels of pain and had higher probability of self-rating health as "good" or "fair or poor". Females in the High and Moderate classes were more likely to be current smokers, to describe ever using drugs and to report more sexual partners, emergency room visits and the use of antidepressants. The risk of depressive symptoms in adulthood is likely to be early determined in adolescence. Objective: To identify developmental trajectories of adiposity from birth until early adulthood, and to investigate how they relate with cardiometabolic risk factors at 21 years of age.Methods: Participants' weight and height measurements were obtained using the EPITeen cohort protocol at 13, 17 and 21 years of age, and extracted from child health books as recorded during health routine evaluations since birth. Blood pressure, triglycerides, cholesterol and insulin resistance (HOMA-IR) were assessed at 21 years. Trajectories were defined using 719 participants contributing 11 459 measurements. The individual growth curves were modelled using mixed-effects fractional polynomial, and the trajectories were estimated using normal mixture modelling for model-based clustering. Differences in cardiometabolic risk factors at 21 years according to adiposity trajectories were estimated through analysis of covariance (ANCOVA), and adjusted means are presented.Results: Two trajectories-'Average body mass index (BMI) growth' (80.7%) and 'Higher BMI growth' (19.3%)-were identified. Compared with those in 'Average BMI growth', 'Higher BMI growth' participants were more frequently delivered by caesarean section, mothers were younger and had higher BMI, and parental education was lower; and at 21 years showed higher adjusted mean systolic (111.6 vs 108.3 mm Hg, P<0.001) and diastolic blood pressure (71.9 vs 68.4 mm Hg, P<0.001), and lower high-density lipoprotein cholesterol (53.3 vs 57.0 mg dl(-1), P=0.001). As there was a significant interaction between trajectories and sex, triglycerides and HOMA-IR were stratified by sex and we found significantly higher triglycerides, in males, and higher HOMA-IR in both sexes in 'Higher BMI growth' trajectory. All the differences were attenuated after adjustment for BMI at 21 years.Conclusions: In this long-term follow-up, we were able to identify two adiposity trajectories, statistically related to the BMI and cardiometabolic profile in adulthood. Background:It is established that growth during early life is predictive of several health outcomes later in life, including body composition. The role of fetal vs postnatal growth remains controversial. We aimed to evaluate the effect of birth weight (BW) and newborn weight change (NWC) during the first 96 h of life on body composition during childhood, measured by: body mass (BMI), fat mass (FMI), and fat-free mass indexes (FFMI), waist circumference (WC) and waist-to-height ratio (WHtR).Methods:As part of the Generation XXI birth cohort, children were recruited in 2005/2006 at all public units providing obstetrical and neonatal care in Porto, Portugal. Information was collected by face-to-face interview and abstracted from clinical records. Newborn's anthropometrics were obtained by trained examiners and NWC was estimated as (weight−BW)/BW × 100, adjusted for age in hours. At age 4 and 7, children were re-evaluated and anthropometric measurements were taken according to standard procedures. Life course data for 717 full-term singletons were presented. Path analysis was used to compute adjusted regression coefficients (β) and 95% confidence intervals.Results:BW had a direct effect on body composition at age 4: for each 100 g increase in BW, there was an increase of 0.043 (0.024; 0.062) on BMI, 0.037 (0.020; 0.055) on FMI, 0.024 (0.007; 0.042) on FFMI, 0.048 (0.031; 0.066) on WC, and 0.022 (0.004; 0.039) on WHtR z-scores. At age 7, BW was positively associated with body composition measures, but this effect was mediated by body composition at age 4. NWC had no effect on body composition at ages 4 or 7. Positive associations were found between body composition at ages 4 and 7.Conclusion:It appears that childhood body composition is programmed by fetal growth and this intra-uterine period seems more important to the development of body composition than immediate postnatal period. Background:Prostate cancer mortality has been decreasing in several high income countries and previous studies analysed the trends mostly according to geographical criteria. We aimed to identify patterns in the time trends of prostate cancer mortality across countries using a model-based approach.Methods:Model-based clustering was used to identify patterns of variation in prostate cancer mortality (1980-2010) across 37 European, five non-European high-income countries and four leading emerging economies. We characterised the patterns observed regarding the geographical distribution and gross national income of the countries, as well as the trends observed in mortality/incidence ratios.Results:We identified three clusters of countries with similar variation in prostate cancer mortality: pattern 1 ('no mortality decline'), characterised by a continued increase throughout the whole period; patterns 2 ('later mortality decline') and 3 ('earlier mortality decline') depict mortality declines, starting in the late and early 1990s, respectively. These clusters are also homogeneous regarding the variation in the prostate cancer mortality/incidence ratios, while are heterogeneous with reference to the geographical region of the countries and distribution of the gross national income.Conclusion:We provide a general model for the description and interpretation of the trends in prostate cancer mortality worldwide, based on three main patterns. Research on the importance of serum uric acid (SUA) as a contributing metabolic factor to cardiovascular diseases has conducted to conflicting results, with most studies assuming a cross-sectional design. The aim of this study was to evaluate the association of SUA and metabolic syndrome (MetS) and its features. A representative sample of 2,485 individuals aged ≥18 years was randomly selected from the non-institutionalized resident population of Porto, Portugal. A total of 1,054 eligible subjects were included for the longitudinal analyses. Hyperuricemia was defined as SUA ≥70 mg/L in men and ≥60 mg/L in women. MetS was defined according the Joint Interim (2009) criteria. Associations were estimated using Poison regression and binomial models. In the cross-sectional analysis, subjects with hyperuricemia had a 2.10-fold increased risk of MetS as compared with normouricemic subjects (PR = 2.10, 95% CI: 1.68-2.63). Among MetS features, high triglycerides presented the strongest association with hyperuricemia (PR = 2.32, 95% CI: 1.84-2.91). The MetS crude incidence rate was 4.5/100 person-year (95% CI: 3.9-5.2) in normal uricemic and 13.0/100 person-year (95% CI: 8.5-20.0) in hyperuricemic participants. Using a multivariate longitudinal approach, hyperuricemia was positively associated with MetS incidence rate ratios (IRR = 1.73, 95% CI: 1.08-2.76). One standard deviation increase of SUA concentration was associated with a 1.22-fold increase in MetS risk (IRR = 1.22, 95% CI: 1.05-1.42). Elevated SUA presented the strongest association with high-triglycerides concentration (IRR = 1.44, 95%: 1.22-1.71) and waist circumference (IRR = 1.25, 95%: 1.05-1.49). Background/Objectives:To evaluate longitudinally the role of physical activity (PA) (type and intensity) and diet (measured using DASH score, nutrients and food intake) in hypertension incidence.Subjects/Methods:As part of the EPIPorto study, 549 participants (40 years), resident in Porto, Portugal, at risk of developing hypertension, were evaluated. Blood pressure (BP) measurements were obtained twice (1999-2003 and 2005-2008), with a median interval of 3.8 years. Hypertension was defined as systolic BP140 mm Hg and/or diastolic BP90 mm Hg, and/or if the subjects were under anti-hypertensive therapy. Validated questionnaires were used to assess usual PA and dietary intake during the previous year. Poisson regression was used to calculate the incident rate ratios (IRRs) and respective 95% confidence intervals (95% CIs).Results:In our population, the crude incidence rate (95% confidence interval (CI)) per 100 person-years of hypertension was 6.23 (5.26-7.20). After adjustment for several confounders, an inverse, though not significant, association was found across increasing tertiles of leisure-time PA and IRR (95% CI): 1 (reference); 0.77 (0.51-1.16); and 0.74 (0.48-1.11). No significant associations between the DASH score and hypertension incidence were observed. However, potassium intake (mg/1000 kcal) was shown to be inversely associated with hypertension development (upper tertile: >1863.0 for women and >1657.2 for men) vs first tertile (IRR=0.65 (0.44-0.96), P for trend=0.025). Additionally, in multivariate analysis, a significantly inverse association between the consumption of fruits/vegetables/pulses and hypertension incidence was found (upper vs first tertile: IRR=0.61 (0.40-0.93), P for trend=0.024).Conclusions:In Portuguese adults, after adjustment for several confounders, the development of hypertension was inversely associated with potassium and fruits/vegetables/pulses intake. Background:The majority of the studies have focused on the effect of general and central fat on coronary risk, neglecting the potential role of peripheral body fat.Objective:To assess the effect of surrogate measures for general, central and peripheral body fat on the occurrence of non-fatal acute myocardial infarction (AMI).Methods:Population-based case–control study; cases were patients aged 40 years consecutively hospitalized with an incident AMI (n=653), and controls were community participants without previous AMI, selected randomly from the hospitals’ catchment area population (n=1713). Body mass index (BMI), waist circumference (WC), hip circumference and a skinfolds composite index to estimate the proportion of peripheral subcutaneous fat in the arms were ascertained. Associations were summarized with odds ratios (OR) and 95% confidence intervals (95% CI), obtained from unconditional logistic regression with adjustment for the main confounders.Results:WC, and in particular waist-to-hip ratio (WHR), had strong direct associations with AMI risk. Peripheral subcutaneous fat was inversely associated with AMI in women, but directly in men. Using principal component analysis, three uncorrelated factors were identified representing different patterns of fat distribution: (1) generalized fat, with high BMI and high WC; (2) central fat, with high WC and WHR; and (3) peripheral subcutaneous fat. The first factor showed no significant association with AMI, but the second factor increased AMI risk in each sex (upper vs lower fourth: OR 12.2, 95% CI 5.34–27.9 in women; OR 25.0, 95% CI 14.0–44.7 in men). In contrast, the third factor was inversely associated with AMI in women (upper vs lower fourth: OR 0.59, 95% CI 0.36–0.96) and directly associated in men (OR 2.45, 95% CI 1.69–3.55; P-value for sex interaction<0.001).Conclusions:Central fat was associated with increased risk of AMI in women and men, while the peripheral subcutaneous fat index predicted a lower risk of AMI in women and a higher risk in men. The study aim was to develop a tool (software and ruler) to assess the dietary calcium and vitamin D intakes in Portugal, and evaluate the usefulness of non-dietary variables as intake predictors. Our findings indicated that is possible to estimate both using three and six food items, respectively, and non-dietary predictors. Trained interviewers collected information of 2,414 adults of Porto, Portugal, using a structured questionnaire and a validated semi-quantitative food frequency questionnaire (FFQ). Food items with the highest contribution to the total intake and non-dietary predictors (gender, age and body mass index (BMI)) were selected for the tool. Different statistical approaches were used to predict the intake. A Bland–Altman plot compared the predictions from the tool and the full FFQ. The items selected to predict intake were milk (38%), cheese (12%), yogurt (10%) and gender for calcium and oily fish (39%), canned fish (9%), white fish (7%), eggs (5%), red meat (5%), age and BMI for vitamin D. The Bland–Altman plot showed that the mean differences were 0.0 (limits of agreement = [-220.67; 220.77]) mg/day and 0.0 (limits of agreement = [-1.03; 1.05]) µg/day, respectively for calcium and vitamin D. The equations estimated by the best statistical model to predict the calcium and vitamin D intake allowed for the design of a software and a circular ruler useful in clinical settings. The study of clinical features, diagnostic methods and prognostic factors of bacterial meningitis, in an urban area. All patients admitted between June 2001 and July 2004 in the emergency departments of a few hospitals, with the diagnosis of bacterial meningitis were included. CSF and blood cultures were performed in every case. Phenotypic characterization of strains of Streptococcus pneumoniae and Neisseria meningitidis identified by culture were performed. In order to detect the three most common agents it was done a PCR assay in culture negative CSF samples. Bacterial meningitis was diagnosed in 201 patients. Etiologic definition was based on culture in 142 patients (70.6%), done by CSF PCR assay in 33 (16.4%) other patients and exclusively by latex agglutination test results in two cases. Thus, an etiologic diagnosis was established in 177 (88%) cases. Antigenic characterization showed a slight prevalence of N. meningitidis phenotype C:2b:P1; the S. pneumoniae serotype characterization showed that 43.8% of identified serotypes are not included in any of the available vaccines. Eighteen patients died (8.9%). The statistic analysis found that factors associated with an adverse outcome were age older than 50 years (OR 7.07; IC 95% 1.1–27.4), the presence of comorbidities (OR 3.3; IC 95% 1.1–9.6) and the occurrence of systemic complications (OR 5.8; IC 95% 2.1–16.0). This epidemiologic pattern is similar to that found in other countries after the introduction of Haemophilus influenzae b conjugated vaccine. The association of culture and noncultural methods of diagnosis had a better performance in defining the etiology. Comparing to other series, in-patients mortality rate was lower (8.9%) than usually referred to, being considered unfavourable prognostic factors the age more than 50 years, the presence of comorbidities and of systemic complications. Pampulha Lake, Minas Gerais, Brazil, is being polluted via its tributaries, Sarandi and Ressaca. Instrumental neutron activation analysis and inductively coupled plasma mass spectrometry high resolution were applied to determine Al, As, B, Ba, Co, Cr, Cs, Cu, Fe, K, Mg, Mn, Mo, Na, P, Pb, Rb, Zn and Ti in Nile tilapia fish, Oreochromis niloticus. The organs analyzed were: intestine, spleen, heart, testicle, kidney, liver, gills and muscle. The results demonstrated relatively high concentrations of Al, Co, Cu, Fe, P and Ti in gills, Al and Cu in liver, Al in intestine and Fe in muscle and spleen. [ABSTRACT FROM AUTHOR]

Intense mining activities in Minas Gerais State – Brazil brings out tons of waste to the environment. Considerable concentrations of toxic elements penetrate the soil, ground waters and rivers. This endangers the environment quality not only in the surrounding areas but also in ichthyofauna and in more distant areas of cattle raising and agricultural activities. After seasonal floods, veterinary clinic studies have shown that most animals raised in this region are affected by symptomatologic nervous diseases, still not clearly diagnosed, which suggests intoxication. These pathologies are mostly noted after floods. Instrumental Neutron Activation Analysis was applied to determine Al, As, Au, Ba, Br, Ca, Cl, Co, Cr, Cs, Cu, Fe, Hg, K, La, Mg, Mn, Na, Nd, Rb, Sb, Sc, Sm, Th and Zn in environmental samples. The obtained results show that the water and sediment contaminated with heavy metals and toxic elements from the Das Velhas River upstream basin, the mining region, carry contamination to the ichthyofauna and farming region within a distance of approximately 400 km.
