Background: There is current controversy about the diagnostic overlap between personality disorders and trauma-related disorders. Patients and methods: Applying a multicenter study design, trauma-related disorders were investigated via interview assessment in 136 patients with borderline personality disorder (BPD) in 5 specialized treatment centers. Additionally a spectrum of psychological symptoms and prevalence of lifetime traumatic experiences were assessed by questionnaire measures. Results: Diagnostic overlap between BPD and PTSD was found to be high (79%) as well as the overlap of BPD with complex PTSD (55%) and severe dissociative disorders (41%). Including neglect and emotional violence as trauma categories, an extremely high prevalence of lifetime traumatic experiences was reported (96%). Experiences of sexual violence were reported by 48% of all female and 28% of all male patients. Severe forms of physical violence were reported by 65% of all patients. Conclusions: BPD patients with severe psychopathology show a high comorbidity with trauma-related disorders including dissociative disorders. Zusammenfassung.Patienten, Psychotherapeuten und Hausärzte können heute aus einem breiten Spektrum von psychotherapeutischen Methoden und Verfahren wählen, wenn die Indikation zu einer stationären oder ambulante Psychotherapie gestellt ist. Was ist dann aber “richtig”? Anhand einer fiktiven Anamnese werden in diesem Artikel verschiedene Therapieangebote vorgestellt, die richtig sein könnten (geordnet nach dem Anfangsbuchstaben des ersten Autors): Dialektisch-behaviorale Therapie (DBT), Paar- und Familientherapie, stationäre tiefenpsychologisch-fundierte Psychotherapie auf einer Borderline-Spezialstation, Gesprächspsychotherapie (GT), Katathym-imaginative Psychotherapie (KiP) und stationäre traumazentrierte Psychotherapie. Deutlich wird, dass es inzwischen für jede Patientin und jede Problematik mehrere geeignete Therapiestrategien gibt, die richtig sind, ohne dass eine vertreten könnte, die allein richtige zu sein. Vielmehr belegen alle Therapiemethoden, ihren Wirksamkeitsschwerpunkt, Wirksamkeitsgrenzen und klare Kontraindikationen zu haben.Abstract.Given an indication for out-patient or in-patient psychotherapy, patients, psychotherapists and general practitioners have nowadays the choice among a wide spectrum of psychotherapeutic methods and treatments. Or, which is the right one? In this essay, a fictitious anamnesis should exemplify the different therapies available for one and the same problem, every one being potentially adequate (listed in the order of the initials of the first author): Dialectic-behavioral therapy (DBT), couple and family therapy, in-patient psychodynamic psychotherapy at a special ward for borderline-patients, client-centered therapy (GT), imaginative therapy (Katathymes Bilderleben KIP) and in-patient trauma-centered psychotherapy.
