Abstract Recently, crowdsourcing has been proposed as a tool for fighting misinformation online. Will internet users listen to crowdsourced fact-checking, and how? In this experiment we test how participants follow others’ opinions to evaluate the validity of a science-themed Facebook post and examine which factors mediate the use of this information. Participants observed a post presenting either scientific information or misinformation, along with a graphical summary of previous participants’ judgements. Even though most participants reported not having used information from previous raters, their responses were influenced by previous assessments. This happened regardless of whether prior judgements were accurate or misleading. Presenting crowdsourced fact-checking however did not translate into the blind copying of the majority response. Rather, participants tended to use this social information as a cue to guide their response, while also relying on individual evaluation and research for extra information. These results highlight the role of individual reasoning when evaluating online information, while pointing to the potential benefit of crowd-sourcing-based solutions in making online users more resilient to misinformation.

Erythrosine B (EB) is a dye widely used in the food and textile industries. Despite many studies that have been proposed in the literature about the electrochemical oxidation of dyes, few studies considered such recalcitrant xanthene compound, although it has been recognized as a threat to health and the environment. Then, this study investigates the oxidation of EB by a homogeneous electro-Fenton process using iron (II) sulfate heptahydrate as a catalyst, carbon felt cathode, and Ti/RuO2 anode. The treated synthetic wastewater contains 100 mg L−1 of EB and has a pH = 3. The effects of three independent variables have been considered for process optimization, such as applied current intensity (0.1–0.5 A), iron concentration (1–10 mM), and stirring rate (100–1000 rpm). Their interactions were investigated considering response surface methodology (RSM) based on Doehlert design as optimization method. EB removal efficiency and energy consumption were considered as model responses after 30 min of electrolysis. Analysis of variance (ANOVA) revealed that the quadratic model was adequately fitted to the experimental data with R2 (0.9819), adj-R2 (0.9276), and low Fisher probability (< 0.0181) for the EB removal model, and R2 (0.9968), adj-R2 (0.9872) and low Fisher probability (< 0.0014) relative to the energy consumption model, suggesting a robust statistical significance. The energy consumption model significantly depends on current density, as expected. The foregoing results obtained by RSM led to the following optimal conditions for EB degradation: current intensity of 0.2 A, iron concentration of 9.397 mM, and stirring rate of 500 rpm, which gave a maximum decolorization rate of 98.15% with a minimum energy consumption of 0.74 kWh m−3 after 30 min of electrolysis. The competitiveness of the electro-Fenton process has been confirmed by the literature analysis proposed as well as by the preliminary economic analysis proposed in the second section of the study. Anodic oxidation is a promising method for removing organic pollutants from water due to its high nonselectivity and effectiveness. Nevertheless, its widespread application is limited due to its low current efficiency, high energy consumption and low treatment rates. These problems may be overcome by the optimization of the process parameters, reactor design and electrode geometry, by coupling the experimental investigations with mathematical modeling. Here we review the modeling of anodic oxidation with focus on basics of this process, the competition phenomenon in real wastewater, flow cells and batch cells, historical aspects, general modeling equations, modeling with plate electrodes, modeling with porous 3-dimension electrodes and the density functional theory. Mathematical modeling can provide current, voltage and concentration distributions in the system. Mathematical modeling can also determine the effects on the performance of parameters such as diffusion layer thickness, flow velocity, applied current density, solution treatment time, initial concentration and diffusion coefficients of organic pollutants, electrode surface area, and oxidation reaction rate constant. Mathematical models allow to determine whether the limiting factor of the process is kinetics or diffusion, and to study the impact of competition of phenomena. This study aimed at evaluating the efficacy of different radiotherapy (RT) fractionation regimens in managing uncomplicated painful bone metastases (BM) and identifying predictive factors for pain control. Patients with 1 to 4 symptomatic BM from any primary solid tumors and a life expectancy exceeding 3 months were included in the study and received palliative RT, with SBRT restricted in the context of oligometastatic disease or in patients with good prognosis. Pain analysis using the Brief Pain Inventory (BPI) tool was conducted at baseline, 1 and 3 months after RT. Analgesic intake was recorded as morphine-equivalent doses (OME). Pain response was assessed using the International Consensus on Palliative Radiotherapy Endpoint (ICPRE). Multivariate logistic regression analyzed patient-related, tumor-related, and treatment-related factors predicting BM pain control at 3 months post-RT. From Feb 2022 to Feb 2023, 44 patients with 65 symptomatic BM were investigated. Breast (32%) and lung (24%) tumors were the most common primary tumors. Treatment plans included 3DCRT (60%) and VMAT (40%), with a median biological effective dose for tumors (BED) of 29 Gy [14–108]. All patients completed the 3-month follow-up. Pain response rates were 62% at 1 month and 60% at 3 months. Responders had better PS ECOG scores (67%; P = 0.008) and received active systemic therapies (67%: P = 0.036). Non-responders had lower pretreatment BPI (mean: 13.7 vs. 58.2; P = 0.032), with significantly higher values after 1 month (mean: 9.1 vs. 5.3, P = 0.033). Baseline BPI (OR: 1.17; 95% CI: 1.032–1.327; P = 0.014) and BPI at 1 month (OR: 0.83; 95% CI: 0.698–0.976; P = 0.025) were independent predictors of pain response at 3 months. Our findings show that palliative RT ensured short-term pain control in patients with BM, regardless of tumor type and dose-fractionation regimen. A larger sample size and a longer follow-up could potentially identify which patients are likely to benefit most from RT, and which fractionation might be indicated for achieving a durable pain relief. Background: To investigate the capability of a not-yet commercially available fully automated lexicographic optimization (LO) planning algorithm, called mCycle (Elekta AB, Stockholm, Sweden), to further improve the plan quality of an already-validated Wish List (WL) pushing on the organs-at-risk (OAR) sparing without compromising target coverage and plan delivery accuracy. Material and Methods: Twenty-four mono-institutional consecutive cervical cancer Volumetric-Modulated Arc Therapy (VMAT) plans delivered between November 2019 and April 2022 (50 Gy/25 fractions) have been retrospectively selected. In mCycle the LO planning algorithm was combined with the a-priori multi-criterial optimization (MCO). Two versions of WL have been defined to reproduce manual plans (WL01), and to improve the OAR sparing without affecting minimum target coverage and plan delivery accuracy (WL02). Robust WLs have been tuned using a subset of 4 randomly selected patients. The remaining plans have been automatically re-planned by using the designed WLs. Manual plans (MP) and mCycle plans (mCP01 and mCP02) were compared in terms of dose distributions, complexity, delivery accuracy, and clinical acceptability. Two senior physicians independently performed a blind clinical evaluation, ranking the three competing plans. Furthermore, a previous defined global quality index has been used to gather into a single score the plan quality evaluation. Results: The WL tweaking requests 5 and 3 working days for the WL01 and the WL02, respectively. The re-planning took in both cases 3 working days. mCP01 best performed in terms of target coverage (PTV V95% (%): MP 98.0 [95.6–99.3], mCP01 99.2 [89.7–99.9], mCP02 96.9 [89.4–99.5]), while mCP02 showed a large OAR sparing improvement, especially in the rectum parameters (e.g., Rectum D50% (Gy): MP 41.7 [30.2–47.0], mCP01 40.3 [31.4–45.8], mCP02 32.6 [26.9–42.6]). An increase in plan complexity has been registered in mCPs without affecting plan delivery accuracy. In the blind comparisons, all automated plans were considered clinically acceptable, and mCPs were preferred over MP in 90% of cases. Globally, automated plans registered a plan quality score at least comparable to MP. Conclusions: This study showed the flexibility of the Lexicographic approach in creating more demanding Wish Lists able to potentially minimize toxicities in RT plans. [ABSTRACT FROM AUTHOR]

Immune checkpoint inhibitor (ICI) therapy has had limited success (<30%) in treating metastatic recurrent Head and Neck Oropharyngeal Squamous Cell Carcinomas (OPSCCs). We postulate that spatial determinants in the tumor play a critical role in cancer therapy outcomes. Here, we describe the case of a male patient diagnosed with p16+ OPSCC and extensive lung metastatic disease who failed Nivolumab and Pembrolizumab/Lenvatinib therapies. Using advanced integrative spatial proteogenomic analysis on the patient's recurrent OPSCC tumors we demonstrate that: (i) unbiased tissue clustering based on spatial transcriptomics (ST) successfully detected tumor cells and enabled the investigation of phenotypic traits such as proliferation or drug-resistance genes in the tumor's leading-edge and core; (ii) spatial proteomic imagining used in conjunction with ST (SpiCi, Spatial Proteomics inferred Cell identification) can resolve the profiling of tumor infiltrating immune cells, (iii) ST data allows for the discovery and ranking of clinically relevant alternative medicines based on their interaction with their matching ligand-receptor. Importantly, when the spatial profiles of ICI pre- and post-failure OPSCC tumors were compared, they exhibited highly similar PD-1/PD-L1low and VEGFAhigh expression, suggesting that these new tumors were not the product of ICI resistance but rather of Lenvatinib dose reduction due to complications. Axillary lymphadenopathy is a common side effect of COVID-19 vaccination, leading to increased imaging-detected asymptomatic and symptomatic unilateral axillary lymphadenopathy. This has threatened to negatively impact the workflow of breast imaging services, leading to the release of ten recommendations by the European Society of Breast Imaging (EUSOBI) in August 2021. Considering the rapidly changing scenario and data scarcity, these initial recommendations kept a highly conservative approach. As of 2023, according to newly acquired evidence, EUSOBI proposes the following updates, in order to reduce unnecessary examinations and avoid delaying necessary examinations. First, recommendation n. 3 has been revised to state that breast examinations should not be delayed or rescheduled because of COVID-19 vaccination, as evidence from the first pandemic waves highlights how delayed or missed screening tests have a negative effect on breast cancer morbidity and mortality, and that there is a near-zero risk of subsequent malignant findings in asymptomatic patients who have unilateral lymphadenopathy and no suspicious breast findings. Second, recommendation n. 7 has been revised to simplify follow-up strategies: in patients without breast cancer history and no imaging findings suspicious for cancer, symptomatic and asymptomatic imaging-detected unilateral lymphadenopathy on the same side of recent COVID-19 vaccination (within 12 weeks) should be classified as a benign finding (BI-RADS 2) and no further work-up should be pursued. All other recommendations issued by EUSOBI in 2021 remain valid. Key points: Worldwide COVID-19 vaccination campaigns have become commonplace. Post-vaccination COVID-19 lymphadenopathy has been reported in up to 44% of cases in at least one follow-up examination. Breast examinations should not be delayed because of COVID-19 vaccination. In average-risk patients without suspicious breast findings, ipsilateral imaging-detected axillary lymphadenopathy within 12 weeks after vaccination should be classified as BI-RADS 2. We present a new experimental approach to further understand the injectivity impairment due to reinjection of produced water in an oilfield, containing residual oil and solids. A unique microfluidic setup with imposed flowrate is characterized by excellent reproducibility and allows one to determine the kinetics of external cake formation and the propagation of the damage inside the porous medium, similar to what happens at the injection wellbore. The growth rates of the external cake and that of the propagation of the internal damage exhibit discontinuities, likely related to a pressure buildup up to a threshold Laplace pressure above which the O/W Pickering droplets are pushed through, and which sets a limit to the cake growth. Finally, the external cake reaches a quasi-stationary thickness whose mechanisms are discussed below. Direct visualization readily achieved in microfluidic experiments, coupled with spatiotemporal image analysis, enables better spatial resolution than core flooding experiments and shows that the damage occurs in a small region close to the entry to the porous medium. These developments lead to the derivation of an analytical model of the damage formation. It appears that although very localized, this damage strongly decreases the global permeability of the whole porous medium. Finally, controlled temperature experiments permit to identify the variation of the viscosity of the oil droplets (or the viscosity ratio), as the primary mechanism by which temperature influences clogging. Clogging is slowed at high temperatures, but the final state is characterized by particle clogging and is thus irreversible. Notwithstanding announcements of progress, "international original sin" (the denomination of external debt in foreign currency) remains a persistent phenomenon in emerging markets. Although some middle-income countries have succeeded in developing markets in local-currency sovereign debt and attracting foreign investors, they continue to hedge their currency exposures through transactions with local pension funds and other resident investors. The result is to shift the locus of currency mismatches within emerging economies but not to eliminate them. Other countries have limited original sin by limiting external borrowing, passing up valuable investment opportunities in pursuit of stability. We document these trends, analyzing regional and global aggregates and national case studies. Our conclusion is that there remains a case for an international initiative to address currency risk in low- and middle-income economies so they can more fully exploit economic development opportunities. Psoriasis is a skin disorder which mostly affects adults, beginning in childhood in almost one-third of patients. In adults it is associated with increased risk for cardiovascular diseases (CVD), while this association is still debated at younger age. Our aim was to evaluate the association between psoriasis and metabolic markers and cardiovascular findings in this age group. Twenty consecutive patients previously diagnosed with psoriasis (group A) were enrolled and compared with healthy non- psoriatic age- and sex-matched subjects (group B). The severity of the disease, CV risk factors, including anthropometric data with adiposity and its distribution, blood pressure (BP), laboratory metabolic tests, echocardiography and vascular ultrasound (transcranial echo-Doppler and carotid artery echo-Doppler with carotid intima-media thickness, cIMT) were performed for each subject. Personal history for CV risk, BP, anthropometric data were similar between the two groups, while familiar history for psoriasis was more frequent in group A (p < 0.02). C-IMT was significantly higher in group A compared to B (right, p = 0.001; left, p = 0.002). In addition, c-IMT was positively correlated with disease duration, triglycerides and triglycerides/glucose. Cerebral flow velocities, cardiac measurements, systo-diastolic function, ventricle geometry and mass were normal and comparable between the two groups, and did not correlate with CV risk factors. In childhood psoriasis c-IMT could represent a marker of pre-clinical cardiovascular involvement and contribute to start a personalized management, while cardiac findings seem to be normal in the early stage of disease. Entanglement is assuming a central role in modern quantum many-body physics. Yet, for lattice gauge theories its certification remains extremely challenging. A key difficulty stems from the local gauge constraints underlying the gauge theory, which separate the full Hilbert space into a direct sum of subspaces characterized by different superselection rules. In this work, we develop the theoretical framework of entanglement witnessing for lattice gauge theories that takes this subtlety into account. We illustrate the concept at the example of a U(1) lattice gauge theory in 2+1 dimensions, without and with dynamical fermionic matter. As this framework circumvents costly state tomography, it opens the door to resource-efficient certification of entanglement in theoretical studies as well as in laboratory quantum simulations of gauge theories. We present an original anisotropic stress-dependent rock physics model for the organic rich shales of the Inoceramus formation, the main source rock and unconventional reservoir in the Austral Basin, Argentina. We implement a novel combination of anisotropic poroelastic theories which take into account organic matter content, lithologic description, fluid type, saturation and stress state. In this approach, we model the infill as a mixture of solid organic matter and interconnected pore fluids, using total organic carbon analysis and petrophysical data from two wells. The compliance of the matrix is considered to be stress-dependent following the porosity deformation approach (PDA). The elasticity and density of the multiminerallic saturated rock is obtained using the mineral fractions obtained from X-ray diffraction information, porosity analysis and fluid properties. This allows us to compute synthetic acoustic velocities. The calibration of the model also involved the inversion of several unknowns (the set of PDA parameters, clays and kerogen physical properties) by minimizing the misfit between modelled and ultrasonic measured velocities. Due to the lack of oblique velocity data, to complete the compliance tensor, a static-to-dynamic ratio was built for each sample, and constant anellipticity was assumed with increasing stress. The model calibrated with this innovative procedure demonstrated its usefulness to predict stiffness, compliance, and compressional and shear wave velocity variations under variable applied stress. It is also useful for the estimation of stress-related changes of porosity and Biot's effective stress coefficients, which can be difficult to measure in shales, and therefore there are few values reported in the literature. Breast density is an independent risk factor for the development of breast cancer and also decreases the sensitivity of mammography for screening. Consequently, women with extremely dense breasts face an increased risk of late diagnosis of breast cancer. These women are, therefore, underserved with current mammographic screening programs. The results of recent studies reporting on contrast-enhanced breast MRI as a screening method in women with extremely dense breasts provide compelling evidence that this approach can enable an important reduction in breast cancer mortality for these women and is cost-effective. Because there is now a valid option to improve breast cancer screening, the European Society of Breast Imaging (EUSOBI) recommends that women should be informed about their breast density. EUSOBI thus calls on all providers of mammography screening to share density information with the women being screened. In light of the available evidence, in women aged 50 to 70 years with extremely dense breasts, the EUSOBI now recommends offering screening breast MRI every 2 to 4 years. The EUSOBI acknowledges that it may currently not be possible to offer breast MRI immediately and everywhere and underscores that quality assurance procedures need to be established, but urges radiological societies and policymakers to act on this now. Since the wishes and values of individual women differ, in screening the principles of shared decision-making should be embraced. In particular, women should be counselled on the benefits and risks of mammography and MRI-based screening, so that they are capable of making an informed choice about their preferred screening method. Purpose: Our study investigated the association between treatment-related lymphopenia and overall survival (OS) in a series of glioblastoma (GBM) patients. We also explored clinical and dosimetric predictors of lymphocytes depletion. Methods: Between 2015 and 2019, 64 patients were treated at the same institution with postoperative chemoradiotherapy. Peripheral lymphocyte count (PLC) data and dose–volume histogram parameters were collected. Radiotherapy (RT) schedule consisted in standard total dose of 60 Gy in 30 daily fractions, with concomitant and adjuvant temozolomide (TMZ). Posttreatment acute absolute lymphopenia (nadir AAL) was calculated as a PLC lower than 1.0 × 103/mm3. Acute relative lymphopenia (ARL) was expressed by the nadir-PLC/baseline-PLC ratio < 0.5. Nadir-PLC was the lowest PLC registered between the end of RT and the first month of follow-up. Survival rates were estimated with Kaplan–Meier curves. Clinical and dosimetric variables related to AAL/ARL and OS were identified by univariate and multivariate analyses. Results: A total of 57 patients were eligible and included in the analyses. The median PLC was significantly decreased following chemoradiotherapy (2180/mm3 vs 900/mm3). Median OS was 16 months (range 5–55 months), with no significant difference between patients who developed nadir AAL and those who did not (16 months vs 16.5 months; p = 0.304). When considering ARL vs non-ARL, median OS was 14 months vs 26 months (p = 0.013), respectively. In multivariate Cox regression only age, sex, extent of surgery, access to adjuvant chemotherapy and brain D98% were independently associated with OS. Conclusion: Although iatrogenic immunosuppression could be associated with inferior clinical outcomes, our data show that treatment-related lymphopenia does not adversely affect GBM survival. The phenomenon of moral transformation, though important, has received little attention in virtue ethics. In this paper we propose a virtue-ethical model of moral transformation as character transformation by tracking the development of new identity-defining (‘core’) character traits, their expressions, and their priority structure, through the change in what appears as possible or impossible to the moral agent. We propose that character transformation culminates when what previously appeared as morally possible to the agent now appears impossible, i.e. unconceived and unthinkable, moving through stages of transformation where some possibilities gradually disappear while others open up. While we show an example of moral transformation towards virtue, we allow that such transformation can occur in the opposite direction, hence we make claims about ‘character traits’ rather than virtues of vices. Through the example of former slave-trader Rodrigo’s transformation in the film The Mission, we follow the parallel development of new objects of value and ways of valuing (with respect to a group of indigenous people of South America) with the closing down of the possibility of disrespecting and harming them, to the end-point of transformation, where allowing their capture is for Rodrigo both unconceived and, when conceived, unthinkable. Disinformation about science can impose enormous economic and public health burdens. A recently proposed strategy to help online users recognise false content is to follow the techniques of professional fact checkers, such as looking for information on other websites (lateral reading) and looking beyond the first results suggested by search engines (click restraint). In two preregistered online experiments (N = 5387), we simulated a social media environment and tested two interventions, one in the form of a pop-up meant to advise participants to follow such techniques, the other based on monetary incentives. We measured participants' ability to identify whether information was scientifically valid or invalid. Analysis of participants' search style reveals that both monetary incentives and pop-up increased the use of fact-checking strategies. Monetary incentives were overall effective in increasing accuracy, whereas the pop-up worked when the source of information was unknown. Pop-up and incentives, when used together, produced a cumulative effect on accuracy. We suggest that monetary incentives enhance content relevance, and could be combined with fact-checking techniques to counteract disinformation. [ABSTRACT FROM AUTHOR]

Introns of human transfer RNA precursors (pre-tRNAs) are excised by the tRNA splicing endonuclease TSEN in complex with the RNA kinase CLP1. Mutations in TSEN/CLP1 occur in patients with pontocerebellar hypoplasia (PCH), however, their role in the disease is unclear. Here, we show that intron excision is catalyzed by tetrameric TSEN assembled from inactive heterodimers independently of CLP1. Splice site recognition involves the mature domain and the anticodon-intron base pair of pre-tRNAs. The 2.1-Å resolution X-ray crystal structure of a TSEN15–34 heterodimer and differential scanning fluorimetry analyses show that PCH mutations cause thermal destabilization. While endonuclease activity in recombinant mutant TSEN is unaltered, we observe assembly defects and reduced pre-tRNA cleavage activity resulting in an imbalanced pre-tRNA pool in PCH patient-derived fibroblasts. Our work defines the molecular principles of intron excision in humans and provides evidence that modulation of TSEN stability may contribute to PCH phenotypes. Mutations within subunits of the tRNA splicing endonuclease complex (TSEN) are associated with pontocerebellar hypoplasia (PCH). Patients with borderline personality disorder (BPD) often display increased stress vulnerability, which may be linked to altered hypothalamus–pituitary–adrenal (HPA) axis functioning. Corresponding deviations of the cortisol awakening response (CAR) are presumed to mirror maladaptive neuroendocrine processes, which may explain why CARs are increased compared to healthy controls (HC). Prior research speculated that these alterations may be caused by early life stress and/or chronic stress related to the ongoing burden of the disorder. Yet, it remains to be investigated how BPD influences CAR in the course of development. Therefore, the current study examined CAR in female adolescents and adults with BPD compared to HC with a particular focus on associations with age. These potential associations were especially focused, as it was hypothesized that the CAR would be even more elevated (i.e., higher) in older individuals with BPD. CAR was assessed in 54 female individuals with BPD (aged 15–40 years) and 54 sex-, age-, and intelligence-matched HC (aged 15–48 years). Group differences were investigated and analyses of covariance using age as continuous predictor were performed to analyze potential developmental associations with CAR alongside BPD-specific effects. Pearson's correlations were calculated to examine associations between CAR and age. Analyses were repeated with potential confounders as control factors. Results not only demonstrated increased CARs in female individuals with BPD compared to HC but demonstrated elevated CARs with increasing age in BPD individuals exclusively. Effects remained stable after controlling for potential confounders. Thereby, findings suggest that endocrine alterations in BPD may reinforce with increasing age and BPD chronicity. Unilateral axillary lymphadenopathy is a frequent mild side effect of COVID-19 vaccination. European Society of Breast Imaging (EUSOBI) proposes ten recommendations to standardise its management and reduce unnecessary additional imaging and invasive procedures: (1) in patients with previous history of breast cancer, vaccination should be performed in the contralateral arm or in the thigh; (2) collect vaccination data for all patients referred to breast imaging services, including patients undergoing breast cancer staging and follow-up imaging examinations; (3) perform breast imaging examinations preferentially before vaccination or at least 12 weeks after the last vaccine dose; (4) in patients with newly diagnosed breast cancer, apply standard imaging protocols regardless of vaccination status; (5) in any case of symptomatic or imaging-detected axillary lymphadenopathy before vaccination or at least 12 weeks after, examine with appropriate imaging the contralateral axilla and both breasts to exclude malignancy; (6) in case of axillary lymphadenopathy contralateral to the vaccination side, perform standard work-up; (7) in patients without breast cancer history and no suspicious breast imaging findings, lymphadenopathy only ipsilateral to the vaccination side within 12 weeks after vaccination can be considered benign or probably-benign, depending on clinical context; (8) in patients without breast cancer history, post-vaccination lymphadenopathy coupled with suspicious breast finding requires standard work-up, including biopsy when appropriate; (9) in patients with breast cancer history, interpret and manage post-vaccination lymphadenopathy considering the timeframe from vaccination and overall nodal metastatic risk; (10) complex or unclear cases should be managed by the multidisciplinary team. Nasopharyngeal carcinoma (NPC) is an Epstein–Barr virus (EBV)-associated heterogeneous disease and is characterized by peritumoral immune infiltrate. Adoptive T-cell therapy (ACT) has emerged as a potential therapeutic strategy for NPC. However, the tumor microenvironment remains a major roadblock for the successful implementation of ACT in clinical settings. Expression of checkpoint molecules by malignant cells can inhibit the effector function of adoptively transferred EBV-specific T cells. Here we present a novel case report of a patient with metastatic NPC who was successfully treated with a combination of EBV-specific ACT and programmed cell death-1 blockade therapy. Following combination immunotherapy, the patient showed complete resolution of metastatic disease with no evidence of disease relapse for 22 months. Follow-up immunological analysis revealed dramatic restructuring of the global T-cell repertoire that was coincident with the clinical response. Background: We conducted a phase 1 dose escalation study (ACTRN12618000140257 registered on 30/01/2018) to evaluate the safety, tolerability and immunogenicity of a therapeutic human papillomavirus (HPV) DNA vaccine (AMV002) in subjects previously treated for HPV-associated oropharyngeal squamous cell carcinoma (OPSCC). Methods: Eligible subjects had to have no evidence of recurrent and/or metastatic disease at least 12 weeks following the completion of treatment. Three dosing cohorts each consisted of four subjects: group 1: 0.25 mg/dose, group 2: 1 mg/dose, group 3: 4 mg/dose. AMV002 was delivered intradermally on days 0, 28 and 56. Incidence and severity of treatment-emergent adverse events (TEAE) including local reaction at the injection site, and vaccination compliance were recorded. T cell and antibody responses to HPV16 E6 and E7 were measured by interferon gamma (IFN-γ) enzyme-linked immunosorbent spot (ELISpot) assay and enzyme-linked immunosorbent assay (ELISA). Results: All subjects completed the vaccination programme and experienced mild discomfort at the injection site(s). Pre-immunisation, cell-mediated responses to HPV16 E6 and E7 were evident in all subjects, and E7-specific antibodies were detected in 11 (91.7%), reflecting previous exposure to HPV. Post-vaccination, 10 of 12 (83.3%) subjects responded to one or more of the E6 and/or E7 peptide pools, while 2 (16.7%) did not show additional vaccine-induced cell-mediated responses. Vaccination resulted in a ≥ 4-fold increase in anti-HPV16 E7 antibody titre in one subject in group 3. Conclusions: AMV002 was well tolerated at all dose levels and resulted in enhanced specific immunity to virus-derived tumour-associated antigens in subjects previously treated for HPV-associated OPSCC. Introduction: This article provides an overview of the natural history and management of adenoid cystic carcinoma infiltrating the skull base (SB ACC). Methods: A comprehensive review of the available literature and discussion regarding the current management of SB ACC. Results: This review describes the unique pathology, appropriate diagnostic work-up and contemporary management options in SB ACC. There is a specific focus on the role of surgical resection with post-operative radiotherapy, given the literature suggesting a superior locoregional control rate. The importance of surgical decision making and varying surgical approaches are detailed. In addition, the emerging role of biological agents is discussed. Conclusion: The gold standard for SB ACC is surgical resection and post-operative radiation, given the superior locoregional control. In patients not amendable to surgery, palliative radiation should be considered. The role of biological agents is still evolving. The Italian College of Breast Radiologists by the Italian Society of Medical Radiology (SIRM) provides recommendations for breast care provision and procedural prioritization during COVID-19 pandemic, being aware that medical decisions must be currently taken balancing patient's individual and community safety: (1) patients having a scheduled or to-be-scheduled appointment for in-depth diagnostic breast imaging or needle biopsy should confirm the appointment or obtain a new one; (2) patients who have suspicious symptoms of breast cancer (in particular: new onset palpable nodule; skin or nipple retraction; orange peel skin; unilateral secretion from the nipple) should request non-deferrable tests at radiology services; (3) asymptomatic women performing annual mammographic follow-up after breast cancer treatment should preferably schedule the appointment within 1 year and 3 months from the previous check, compatibly with the local organizational conditions; (4) asymptomatic women who have not responded to the invitation for screening mammography after the onset of the pandemic or have been informed of the suspension of the screening activity should schedule the check preferably within 3 months from the date of the not performed check, compatibly with local organizational conditions. On May 26, 2017, Harvard economist Ricardo Hausmann published an Op Ed titled "Hunger Bonds", urging investors to avoid Venezuelan sovereign bonds on the grounds that the country was prioritizing payments on the bonds over remedying a humanitarian crisis. Contemporaneously, news emerged regarding a suspicious looking bond issue by Venezuela's oil company that was purchased largely by Goldman Sachs. That bond got tagged with the label "Hunger Bond", and suffered a price hit. Using both quantitative data and interviews with investors, we examine the causes of the Hunger Bond penalty, its impact on the prices of other outstanding Venezuelan bonds, and how long it was sustained. The primary determinants of the Hunger Bond penalty appear to have been a combination of negative press attention and crowd-sourced disapproval. For instance, we show that a large number of Google searches for "Hunger Bonds" are associated with a 200 basis point increase in the spread of the bond purchased by Goldman Sachs. Aim: Aortic stenosis is a frequent valvular disease, with transcatheter aortic valve implantation (TAVI) being performed when surgical replacement is at increased risk. However, TAVI-induced effects on myocardial efficiency are unknown. We aimed to investigate changes in LV mechano-energetic pre-/post-TAVI and their prognostic impact. Methods: A total of 46 patients (25 males) received transesophageal and simultaneous radial pressure plus transaortic gradient monitoring before/immediately after prosthesis deployment. Efficiency was computed as external work/potential energy, as derived from LV pressure–volume plots; myocardial oxygen consumption (MVO2) was estimated as PWImod, i.e. a noninvasively validated alternative for MVO2 estimation. Results: TAVI was successful in all patients, peak transaortic gradient decreasing − 40 ± 20 mmHg (p < 0.001). Efficiency improved post-TAVI (+ 0.6 ± 0.12; p = 0.004), with a concomitant PWImod reduction (− 16 ± 31%; p < 0.001). When contextualized to fixed PWImod value (5 ml/min/100 g), efficiency significantly affected survival (p = 0.029). Over 1026 ± 450-day follow-up, a change in efficiency pre-/post-TAVI ≤ 0.021 (median of the difference) predicted more deaths from any cause (30%) as compared with a change > 0.021 (17%), particularly in those patients with a pre-TAVI mean high-gradient (HG ≥ 40 mmHg) phenotype (p < 0.05). In particular, HG patients exhibited the lowest efficiency/PWImod ratio pre-/post-TAVI (p = 0.048), relative to the other aortic stenosis patients, suggestive of an unfavourable matching between cardiac function and metabolic demand, which foreshortens some intrinsic damaged muscle condition in these patients. Conclusion: LV mechanical efficiency improves immediately post-TAVI, notwithstanding an inhomogeneous mechano-energetic matching among the aortic stenosis patients, which can impact negatively on their long-term prognosis, particularly in those with the HG phenotype. The conventional paradigm about development banks is that these institutions exist to target well-identified market failures. However, market failures are not directly observable and can only be ascertained with a suitable learning process. Hence, the question is how do the policymakers know what activities should be promoted; how do they learn about the obstacles to the creation of new activities? Rather than assuming that the government has arrived at the right list of market failures and uses development banks to close some well-identified market gaps, we suggest that development banks can be in charge of identifying these market failures through their loan-screening and lending activities to guide their operations and provide critical inputs for the design of productive development policies. In fact, they can also identify government failures that stand in the way of development and call for needed public inputs. This intelligence role of development banks is similar to the role that modern theories of financial intermediation assign to banks as institutions with a comparative advantage in producing and processing information. Opinion Statement: Recent studies have evidenced the potential of combining anti-EGFR therapies with anti-PD-1/PD-L1 checkpoint therapies. Both anti-EGFR and anti-PD-1/PD-L1 have been separately tested in the treatment of cutaneous SCC (cSCC). Here, we review recent data on EGFR in the context of cancer progression, as a prognostic and as a therapeutic target in cSCC. In this study, galvanostatic electrolysis, through the use of the platinum supported on Ti (Ti/Pt) and Ti/TiO2-nanotubes/PbO2 anodes, was conducted in an electrochemical cell with 0.2 L of solution containing 100 mg dm−3 of the textile dye Acid Red 1 (AR1) using Na2SO4 as supporting electrolyte, applying 7.5 and 60 mA cm−2. From the voltammetric curves, it was possible to understand that Ti/TiO2-nanotubes/PbO2 electrode has high oxygen evolution overpotential than Ti/Pt anode. A direct electron transfer reaction is attained between the dye molecules and Ti/Pt surface, at lower currents. Conversely, the AR1 oxidation involves water decomposition intermediates, mainly ·OH radicals at Ti/TiO2-nanotubes/PbO2 anode. The electrolytic process was monitored by the UV-visible spectrometry and the chemical oxygen demand (COD). Results clearly show that Ti/TiO2-nanotubes/PbO2 anode performs better than Ti/TiO2 in removing ARI due to the electrosynthesis of strong oxidants on its surface (·OH and persulfates), achieving a higher oxidation rate, higher current efficiency, and less energy consumption than Ti/Pt electrode. The five-year survival rate for patients with head and neck squamous cell carcinoma (HNSCC) has remained at ~50% for the past 30 years despite advances in treatment. Tigilanol tiglate (TT, also known as EBC-46) is a novel diterpene ester that induces cell death in HNSCC in vitro and in mouse models, and has recently completed Phase I human clinical trials. The aim of this study was to optimise efficacy of TT treatment by altering different administration parameters. The tongue SCC cell line (SCC-15) was identified as the line with the lowest efficacy to treatment. Subcutaneous xenografts of SCC-15 cells were grown in BALB/c Foxn1nu and NOD/SCID mice and treated with intratumoral injection of 30 μg TT or a vehicle only control (40% propylene glycol (PG)). Greater efficacy of TT treatment was found in the BALB/c Foxn1nu mice compared to NOD/SCID mice. Immunohistochemical analysis indicated a potential role of the host's innate immune system in this difference, specifically neutrophil infiltration. Neither fractionated doses of TT nor the use of a different excipiant led to significantly increased efficacy. This study confirmed that TT in 40% PG given intratumorally as a single bolus dose was the most efficacious treatment for a tongue SCC mouse model. [ABSTRACT FROM AUTHOR]

The Dolomites (Italy) were included in the UNESCO World Heritage List because of their exceptional beauty and unique landscape, together with their scientific importance from the geological and geomorphological point of view. The present article is an in-depth study of previous investigations (Panizza in Geoheritage 1:33-42, 2009) some examples concerning all the components of geological heritage, not just the geomorphological one, are described and it discusses into perspective their intrinsic and extrinsic values in a UNESCO World Heritage context. They have been qualitatively assessed by attributing them an “intrinsic” value, mainly of local importance with reference to the territory of the Dolomites, or an “extrinsic” value, i.e. with reference to a wider context of regional or global importance. The examples concern stratigraphy, geomorphology, tectonics, petrography and palaeontology. Once the main characteristics and values of a specific site have been identified and evaluated as belonging to geological heritage (for example, from the stratigraphic viewpoint), other secondary geological values may be attributed (for example, also within the field of palaeontology). On the other hand, also other “non-geological” (cultural, scenic, archaeological, socioeconomic, ethical etc.) values can be added, although they can be emphasised in the description of a geosite or geomorphosite in order to accentuate their value. The examples illustrated can bring about a constructive contribution to the conceptual definition and methodological explanation of research on geological heritage. In the specific case of the Dolomites, comparative analyses have shown that they are unique in the world and witness a long interval of the Earth’s history in an extraordinary way. A giant cell tumor (GCT) is an intermediate grade, locally aggressive neoplasia. Despite advances in surgical and clinical treatments, cases located on the spine and pelvic bones remain a significant challenge. Failure of clinical treatment with denosumab and patient refusal of surgical procedures (hemipelvectomy) led to the use of cryoablation. We report the use of percutaneous CT-guided cryoablation as an alternative treatment, shown to be a minimally invasive, safe, and effective option for a GCT with extensive involvement of the pelvic bones and allowed structural and functional preservation of the involved bones. Purpose: The purpose of this study was to examine the relationship between physical impairments, quality of life and disability in patients following neck dissection, with consideration of patient and clinical characteristics.Methods: Cross-sectional study of patients < 5 years after neck dissection for head and neck cancer. Quality of life and self-reported disability were measured with the Neck Dissection Impairment Index, Quick Disabilities of the Arm, Shoulder and Hand, and Neck Disability Index. Active neck and shoulder range of motion and isometric muscle strength were also assessed. Generalised linear modelling was used to explore relationships between variables.Results: Eighty-four participants (68% male, median age 61 years) demonstrated reduced quality of life (median (interquartile range) score = 76 (49, 93) from 0 (worst) to 100 (best)), and mild levels of upper limb (14 (2, 32)) and neck disability (14 (6, 28)) (from 0 (best) to 100 (worst)). Bilateral neck dissection was associated with reduced quality of life (coeff (95% CI) = - 12.49 (- 24.69, - 0.29)). Post-operative chemoradiation therapy was associated with reduced quality of life (- 21.46 (- 37.57, - 5.35)) and neck disability (0.71 (0.10, 1.32)). Measures of shoulder flexibility or strength were associated with quality of life and self-reported disability.Conclusions: Quality of life and musculoskeletal disability after neck dissection are associated with factors from multiple domains including physical motor function and treatment modality.Implications For Cancer Survivors: Having reduced shoulder flexibility or strength is related to functional deficits and quality of life after neck dissection for head and neck cancer. Butterflies of the European Euphydryas aurinia complex include a series of taxa, showing morphological and eco-ethological differences. All of them are threatened by changes in land use affecting both the structural and functional connectivity of habitat patches, as well as habitat quality. In this framework, we analysed the use of resources by two of the three taxa occurring in Italy, E. (a.) glaciegenita (Alpine) and E. (a.) provincialis (Mediterranean), at ‘landscape’, ‘patch’ and ‘microhabitat’ scale. We collected data on spatial distribution of adults by Mark-Release-Recapture to understand which parameters affect adult distributions at landscape scale and to test the degree of adult aggregation at patch scale. We sampled first instar larvae within fixed plots and compared the microhabitat characteristics of occupied vs. unoccupied host plants. We found that both populations were affected by management at landscape scale, with contrasting patterns depending on both sites and sex. The food plants were not a limiting factor for adult distribution, probably because they were abundant in the study areas. Within patches, males of E. (a.) glaciegenita showed aggregate distributions in comparison to those of E. (a.) provincialis, suggesting the existence of a “lek strategy”. At microhabitat scale, we observed that females carefully chose their egg-laying sites according to host plant density and microclimatic cues, even though selection was driven by distinct factors in the two populations. Understanding the ecological requirements of all life stages is essential to develop appropriate conservation strategies to preserve butterflies of the E. aurinia complex in Italy. Abstract: This article summarises the information that should be provided to women and referring physicians about breast ultrasound (US). After explaining the physical principles, technical procedure and safety of US, information is given about its ability to make a correct diagnosis, depending on the setting in which it is applied. The following definite indications for breast US in female subjects are proposed: palpable lump; axillary adenopathy; first diagnostic approach for clinical abnormalities under 40 and in pregnant or lactating women; suspicious abnormalities at mammography or magnetic resonance imaging (MRI); suspicious nipple discharge; recent nipple inversion; skin retraction; breast inflammation; abnormalities in the area of the surgical scar after breast conserving surgery or mastectomy; abnormalities in the presence of breast implants; screening high-risk women, especially when MRI is not performed; loco-regional staging of a known breast cancer, when MRI is not performed; guidance for percutaneous interventions (needle biopsy, pre-surgical localisation, fluid collection drainage); monitoring patients with breast cancer receiving neo-adjuvant therapy, when MRI is not performed. Possible indications such as supplemental screening after mammography for women aged 40-74 with dense breasts are also listed. Moreover, inappropriate indications include screening for breast cancer as a stand-alone alternative to mammography. The structure and organisation of the breast US report and of classification systems such as the BI-RADS and consequent management recommendations are illustrated. Information about additional or new US technologies (colour-Doppler, elastography, and automated whole breast US) is also provided. Contrary to widespread presumption, a surprisingly large number of countries have been able to finance a significant fraction of their investment for extended periods using foreign finance. While many of these episodes are in countries where official finance is important, we also identify episodes where a substantial fraction of domestic investment is financed by private capital inflows. Although there is evidence of a positive growth effect of such inflows in the short run, that positive impact dissipates after 5 years and turns negative over longer horizons. Many such episodes end abruptly, with compression of the current account and sharp slowdowns in investment and growth. Summing over the inflow (current account deficit) episode and its aftermath, we find that growth is slower than when countries rely on domestic savings. Patients with head and neck cancer can report reduced health-related quality of life several years after treatment. The aim of this study was to identify risk factors for reduced quality of life in patients up to 5 years following neck dissection. This cross-sectional study was conducted at two hospitals in Brisbane, Australia. Patients completed two measures of quality of life: the Neck Dissection Impairment Index (NDII), a region- and disease-specific tool, and the Assessment of Quality of Life-4 Domains, a general tool. Generalised linear modelling was used to determine which demographic and clinical variables were associated with quality of life. The cohort included n = 129 patients (71% male, median age 61, median 3 years since surgery). Positive nodal disease was associated with better quality of life on the NDII [e.g. N2 vs N0 coeff (95% CI) = 22.84 (7.33, 38.37)]. Worse quality of life was associated with adjuvant treatment [e.g. Independent Living domain model: surgery with chemoradiation vs surgery only coeff (95% CI) = −0.11 (−0.22, −0.01)]. Positive nodal disease was associated with better quality of life, which may be a reflection of response shift. Multimodality treatment leads to worse quality of life compared with surgery only. Hyponatremia (plasma sodium concentration or P[Na+] <136 mEq/L) is the most common electrolyte unbalance in clinical practice. Although it constitutes a negative prognostic factor, it frequently remains underdiagnosed and undertreated. Tolvaptan is an oral V2-receptor antagonist which produces aquaresis. Given its emerging role in the treatment of dilutional hyponatremia, we aimed to compare the efficacy and safety of two different doses of this drug in an Emergency Department (ED) setting. Consecutive patients with moderate-severe euvolemic or hypervolemic hyponatremia were sequentially assigned to the 15 mg Group and to the 7.5 mg Group, and were revaluated at 6, 12 and 24 h. Further evaluations and administrations were scheduled daily until P[Na+] correction was achieved or the maximum period of 72 h was exceeded. A 1-month follow-up was performed. Twenty-three patients were enrolled: 12 were included in the 15 mg Group, 11 in the 7.5 mg Group. Both doses significantly elevated the P[Na+] over 24 h, although the 15 mg Group showed faster corrections than the 7.5 mg Group (12 vs 6 mEq/L/24 h; P = 0.025). An optimal correction rate (within 4-8 mEq/L/24 h) was observed in 45.4 % of the 7.5 mg Group against 25.0 % (P n.s.). The standard dose led to dangerous overcorrections (>12 mEq/L/24 h) in 41.7 % of the patients, while the low dose did not cause any (P = 0.037). No osmotic demyelination syndrome was observed. A 7.5 mg tolvaptan dose can be considered both effective and safe in treating hyponatremia in the ED, while a 15 mg dose implicates too high risk of overcorrection. [ABSTRACT FROM AUTHOR]

Background: Inflammatory breast cancer represents a rare condition that presents poor prognosis with high risk of recurrences and greatly affects patients' quality of life. The resective surgical treatment frequently requires challenging reconstructive solutions to restore a proper tissue layer for the chest wall. Reconstructive methods are nowadays still matter of debate. Methods: A retrospective review of all cases of massive inflammatory breast cancer that underwent mastectomy and reconstruction with fasciocutaneous flaps from October 2009 to September 2014 was performed at our department. Patient demographics, indications, flap specifics, complications, and number of operations were collected. Results: Our review identified five cases. A lateral thoracic flap was planned in two cases, a modified lateral thoraco-dorsal flap in one case, a combination of scapular and pubic flaps in other case, and a thoraco-epigastric and lateral thoracic flap association in the remaining case. The mean patient age was 60 years (range, 35 to 76 years). Flap dimensions ranged from 9cm to 14 cm in width and 20 to 24 cm in length. Partial necrosis of the flap distal part occurred in two cases requiring minor flap revision. Conclusions: Resective surgical treatment of inflammatory breast cancer might require a challenging reconstructive planning. In this scenario, fasciocutaneous flaps are an interesting option due to their versatility, easiness of harvesting, low morbidity, and complications rate. In this paper, the electrocatalytic properties of PbO and TiRuSnO anodes for direct and indirect electrochemical oxidation of a synthetic solution containing Indigo Carmine (IC) were compared. The electrolysis was performed using an electrolytic flow cell with parallel-plate electrodes and the progresses of oxidation were monitored by UV-vis and COD measurements. The effects of several operating parameters such as electrode material, current intensity, initial dye concentration, and pH on the degradation rate and current efficiency were determined. Some economic considerations were also taken into account. With both electrodes, IC removal was satisfactory described by a pseudo-first-order kinetic and the rate constant increased with applied current, chloride concentration and decreased with initial IC concentration. During direct electrolysis, PbO provided a faster oxidation rate, higher current efficiency, and lower energy consumption than TiRuSnO anode. On the contrary, in the presence of 825 ppm of NaCl, the TiRuSnO anode that has higher electrocatalytic activity for chlorine evolution was more efficient than PbO. [ABSTRACT FROM AUTHOR]

Chiral amines are essential precursors in the production of biologically active compounds, including several important drugs. Among the biocatalytic strategies that have been developed for their synthesis, the use of ω-transaminases (ω-TA) appears as an attractive alternative allowing the stereoselective amination of prochiral ketones. However, the problems associated with narrow substrate specificity, unfavourable reaction equilibrium and expensive amine donors still hamper its industrial application. The search for novel enzymes from nature can contribute to expand the catalytic repertoire of ω-TA and help to circumvent some of these problems. A genome mining approach, based on the work described by Höhne et al., was applied for selection of potential R-ω-TA. Additional criteria were used to select an enzyme that differs from previously described ones. A candidate R-ω-TA from Capronia semiimmersa was selected, cloned and expressed in Escherichia coli. Interestingly, alignment of this enzyme with previously reported TA sequences revealed the presence of two additional amino acid residues in a loop close to the active site. The impact of this change was analysed with a structural model based on crystallized R-ω-TAs. Analysis of the substrate specificity of R-ω-TA from C. semiimmersa indicates that it accepts a diversity of ketones as substrates yielding the corresponding amine with good yields and excellent enantioselectivity.
