Purpose: To determine the safety and efficacy of transurethral resection of the prostate (TUR-P) in patients 85 years or older. Methods: In this retrospective, multicentre study, patients equal or older than 85 years at the time of surgery (2015–2020) were included. Several pre-, peri- and postoperative parameters were collected. The main outcome criterion was spontaneous voiding with a post-void residual (PVR) volume < 100 ml at dismission and at 12 months after surgery. Results: One hundred sixty-eight patients (median age: 87 years, interquartile range [IQR]: 86–89) were recruited. The patients took on average 5.2 permanent medications (3–8), 107 (64%) were anticoagulated preoperatively and neurological co-morbidities were present in 29 (17%). The indication for surgery was recurrent urinary retention in 66.3% (n = 110) with a mean retention volume of 849 ml. The mean PVR volume of the remaining 35% was 146 ml. Surgery was successfully completed in all patients. A perioperative surgical revision had to be performed in 3% and 13 patients (7.7%) required blood transfusion. After catheter removal, 85% of patients were able to void spontaneously with a PVR < 100 ml, and 14.3% were dismissed with a catheter. Twelve months data were available for 93 patients (55%). Of this cohort, 78 (83.9%) were able to void spontaneously with a PVR < 100 ml, 12 (12.9%) were on permanent catheterization. One patient (0.6%) died perioperatively. The only significant factor associated with an unsuccessful outcome was the number of permanent medications (6.8 vs. 5.0, p = 0.005). The chronic inflammatory state at old age may contribute to the pathophysiology of or reflect chronic conditions resulting in loss of physical and mental functioning. Therefore, our objective was to examine the predictive value of a large battery of serum inflammatory markers as risk indicators for global functional decline and its specific physical and mental determinants in the oldest old. Global functional decline and specific aspects of physical and mental functional decline were assessed during an average of 1.66 years (±0.21) in a sample of 303 persons aged 80 years or older of the BELFRAIL study. Serum levels of 14 inflammatory proteins, including cytokines, growth factors, and acute phase proteins, were measured at baseline. Almost 20 % of the participants had a significant global functional decline over time. Interleukin (IL)-6 serum levels were uniquely positively associated with global functional decline, even after correcting for multiple confounders (odds ratio 1.51). Odds ratios for the individual aspects (physical dependency, physical performance, cognition, and depression) of functioning were lower, and composite scores of physical or mental decline were not significant. The proportion of global functional decline exhibited a dose–response curve with increasing levels of IL-6. Thus, IL-6 is an independent risk indicator for accelerated global functional decline in the oldest old. Shifting visual focus on the basis of the perceived gaze direction of another person is one form of joint attention. In the present study, we investigated whether this socially relevant form of orienting is reflexive and whether it is influenced by age. Green and Woldorff (Cognition 122:96-101, ) argued that rapid cueing effects (i.e., faster responses to validly than to invalidly cued targets) were limited to conditions in which a cue overlapped in time with a target. They attributed slower responses following invalid cues to the time needed to resolve the incongruent spatial information provided by the concurrently presented cue and target. In the present study, we examined the orienting responses of young (18-31 years), young-old (60-74 years), and old-old (75-91 years) adults following uninformative central gaze cues that overlapped in time with the target (Exp. 1) or that were removed prior to target presentation (Exp. 2). When the cue and target overlapped, all three groups localized validly cued targets more quickly than invalidly cued targets, and validity effects emerged earlier for the two younger groups (at 100 ms post-cue-onset) than for the old-old group (at 300 ms post-cue-onset). With a short-duration cue (Exp. 2), validity effects developed rapidly (by 100 ms) for all three groups, suggesting that validity effects resulted from reflexive orienting based on the gaze cue information rather than from cue-target conflict. Thus, although old-old adults may be slow to disengage from persistent gaze cues, attention continues to be reflexively guided by gaze cues late in life. [ABSTRACT FROM AUTHOR]

This study investigated age effects in prospective memory performance within older adults. The first aim was to explore this issue by examining event- and time-based prospective memory performance in two age groups: young-old (60–75 years) and old-old adults (76–90 years). Moreover, this study for the first time investigated whether forming implementation intentions could be used to improve prospective memory in young-old and old-old adults. Results showed a general effect of age in prospective memory performance for both task types. In addition, no general effect of implementation intentions in prospective memory performance across both task types and age groups was found. However, testing implementation intention effects separately for both age groups revealed that the formation of implementation intentions enhanced prospective memory only for the young-old adults, but did not substantially affect the performance in the time-based task and even impaired it in the event-based task for the old-old adults. Findings indicate that the formation of implementation intentions might be a powerful memory strategy for young-old adults, but not for the very old. [ABSTRACT FROM AUTHOR]

Objectives: The purpose of this study was to 1) investigate lifetime weight changes in community dwelling old-old, those age 80 or older; 2) explore reasons for weight changes from their personal perspective and 3) examine ways the old-old feel they can successfully lose weight. A cross sectional design was used for this qualitative study. Setting: One county of a southeastern U.S. state. Participants: Nine community dwelling old-old. Measurement: Content analysis of transcripts was used to examine the interviews. Results: All of the old-old interviewed had reached their maximum weight after the age of 65. Average lifetime weight gain was 73 pounds. All wanted to lose weight and found ways to incorporate small changes in their everyday lives. Although many studies have examined the issue of personality stability in early and middle adulthood, few have explored the limits of personality stability in the very old, who are often confronted with major changes in their health and life circumstances which can severely impact adaptive behavior. We conducted a longitudinal study of 74 elderly women with a mean age of 80. They were assessed on the personality traits of Neuroticism, Extroversion, and Openness. Although multiple regression analyses revealed moderate stability on all 3 traits, their stability was found to be influenced by negative changes in life circumstances. For example, decreased social support and increased unmet needs were associated with more Neuroticism. Less Extroversion was associated with poorer health and greater psychosocial needs. The trait of Openness was very stable and was the least affected by life events. These contradictory findings are reconciled by considering personality development within an interaction framework.
