Microneedles are mainly used for pain-free drug administration and in biosensing for wearable systems. They are also promising for fields such as agronomy for precision farming, but their fabrication is not straightforward, often requiring expensive equipment and cleanroom protocols, being unsuitable for mass production. Here, we report a new and simple method for the scalable fabrication of all-inkjet-printed conductive microneedles based on silver nanoparticles (extensible to any other metallic nanoparticle ink) and a simple example of their application for monitoring the electrochemical properties of plants. This study focuses on the Kadalundi estuary, Kerala's first community reserve, investigating the prevalence and impacts of microplastics on both the estuarine environment and selected fish species. This study presents the initial evidence indicating the consumption of microplastic particles by 12 commercially important edible fish species inhabiting the Kadalundi estuary. Analysis revealed significant accumulations of microplastic fibers within the surface water. In examining 12 fish species from demersal and pelagic habitats, microplastics were found in both the gastrointestinal tracts and gills. In the digestive tracts, microplastic fragments constituted the highest proportion (46%), while in the gills, microplastic fibers were dominant (52.4%). This study observed a prevalence of blue microplastics over other colors in both water and fish samples. Notably, demersal species showed a higher incidence of ingested microplastics. Polymer analysis identified Polypropylene (PP), Nylon, Low-Density Polyethylene (LDPE), Polyethylene (PE), Polypropylene isotactic (iPP), PE 1 Octene copolymer, and Rayon in water samples, while fish samples predominantly contained LDPE, PP, PE, and Nylon. Risk assessment utilizing the Polymer Hazard Index (PHI) categorized certain polymers as posing minor to moderate risks. Pollution Load Index (PLI) computations indicated moderate to high levels of microplastic contamination across various sampling sites in the estuary. Principal Component Analysis (PCA) revealed a lack of correlation between fish size and microplastic ingestion, underscoring environmental factors' influence on microplastic intake. Aim: Recent evidence has questioned the usefulness of anastomotic drain (AD) after low anterior resection (LAR). However, the implementation and adoption of a no-drain policy are still poor. This study aims to assess the clinical outcomes of the implementation of a no-drain policy for rectal cancer surgery into a real-life setting and the adherence of the surgeons to such policy. Method: A retrospective analysis was conducted on patients who underwent elective minimally invasive LAR between January 2015 and December 2019 at two tertiary referral centers. In 2017, both centers implemented a policy aimed at reducing the use of AD. Patients were retrospectively categorized into two groups: the drain policy (DP) group, comprising patients treated before 2017, and the no-drain policy (NDP) group, consisting of patients treated from 2017 onwards. The endpoints were the rate of anastomotic leak (AL) and of related interventions. Results: Among the 272 patients included, 188 (69.1%) were in the NDP group, and 84 (30.9%) were in the DP group. Baseline characteristics were similar between the two groups. AL rate was 11.2% in the NDP group compared to 10.7% in the DP group (p = 1.000), and the AL grade distribution (grade A, 19.1% (4/21) vs 28.6% (2/9); grade B, 28.6% (6/21) vs 11.1% (1/9); grade C, 52.4% (11/21) vs 66.7% (6/9), p = 0.759) did not significantly differ between the groups. All patients with symptomatic AL and AD underwent surgical treatment for the leak, while those with symptomatic AL in the NPD group were managed with surgery (66.7%), endoscopic (19.0%), or percutaneous (14.3%) interventions. Postoperative outcomes were similar between the groups. Three years after implementing the no-drain policy, AD was utilized in only 16.5% of cases, compared to 76.2% at the study's outset. Previous research conducted in central-east region of Argentina recorded potential orthohantavirus host rodents in diverse environments, but no research has focused particularly on islands, the environments that present the greatest risk to humans. For this reason, the aims of this research were to determine the orthohantavirus host in the rodent community focused on islands of Paraná River Delta, central-east region of Argentina, to identify temporal and spatial factors associated with orthohantavirus prevalence variations, to compare the functional traits of seropositive and seronegative rodents, and to explore the association between orthohantavirus prevalence and rodent community characteristics between August 2014 and May 2018. With a trapping effort of 14,600 trap-nights, a total of 348 sigmodontine rodent specimens belonging to seven species were captured 361 times. The overall antibody prevalence was 4.9%. Particularly, 14.9% of Oligoryzomys flavescens and 1.5% of Oxymycterus rufus, mainly reproductively active adult males, had antibodies against orthohantavirus. Even though O. flavescens inhabit all islands, our results suggest spatial heterogeneity in the viral distribution, with two months after periods of low temperature presenting increases in seroprevalence. This could be a response to the increased proportion of adults present in the rodent population. In addition, an association was found between the high seroprevalence and the diversity of the rodent assemblage. We also found 1.5% of O. rufus exposed to orthohantavirus, which shows us that further investigation of the ecology of the virus is needed to answer whether this species act as a spillover or a new competent host. The evidence does not support the routine use of abdominal drainage (AD) in colorectal surgery. However, there is no data on the usefulness of AD, specifically, after ileal pouch-anal anastomosis (IPAA). The aim of this study is to assess post-operative outcomes of patients undergoing IPAA with or without AD at a high volume referral center. A retrospective analysis of prospectively collected data of consecutive patients undergoing IPAA with AD (AD group) or without AD (NAD group) was performed. Baseline characteristics, operative, and postoperative data were analyzed and compared between the two groups. A total of 97 patients were included in the analysis, 46 were in AD group and 51 in NAD group. AD group had a higher BMI (23.9 ± 3.9 kg/m2 vs 21.9 ± 3.0 kg/m2; p = 0.007) and more commonly underwent two-stage proctocolectomy with IPAA compared to the NAD group (50.0% vs 3.9%; p < 0.001). There was no difference in anastomotic leak rate (6.5% AD vs 5.9% NAD group; p = 1.000), major post-operative complication (8.6% vs 7.9%; p = 0.893); median length of stay [IQR] (5 [5–7] days vs 5 [4–7] days; p = 0.305) and readmission < 90 days (8.7% vs 3.9%; p = 0.418). Background: Evidence on the efficacy of minimally invasive (MI) segmental resection of splenic flexure cancer (SFC) is not available, mostly due to the rarity of this tumor. This study aimed to determine the survival outcomes of MI and open treatment, and to investigate whether MI is noninferior to open procedure regarding short-term outcomes. Methods: This nationwide retrospective cohort study included all consecutive SFC segmental resections performed in 30 referral centers between 2006 and 2016. The primary endpoint assessing efficacy was the overall survival (OS). The secondary endpoints included cancer-specific mortality (CSM), recurrence rate (RR), short-term clinical outcomes (a composite of Clavien-Dindo > 2 complications and 30-day mortality), and pathological outcomes (a composite of lymph nodes removed ≧12, and proximal and distal free resection margins length ≧ 5 cm). For these composites, a 6% noninferiority margin was chosen based on clinical relevance estimate. Results : A total of 606 patients underwent either an open (208, 34.3%) or a MI (398, 65.7%) SFC segmental resection. At univariable analysis, OS and CSM were improved in the MI group (log-rank test p = 0.004 and Gray's tests p = 0.004, respectively), while recurrences were comparable (Gray's tests p = 0.434). Cox multivariable analysis did not support that OS and CSM were better in the MI group (p = 0.109 and p = 0.163, respectively). Successful pathological outcome, observed in 53.2% of open and 58.3% of MI resections, supported noninferiority (difference 5.1%; 1-sided 95%CI − 4.7% to ∞). Successful short-term clinical outcome was documented in 93.3% of Open and 93.0% of MI procedures, and supported noninferiority as well (difference − 0.3%; 1-sided 95%CI − 5.0% to ∞). Conclusions: Among patients with SFC, the minimally invasive approach met the criterion for noninferiority for postoperative complications and pathological outcomes, and was found to provide results of OS, CSM, and RR comparable to those of open resection. Purpose: The likelihood of a stoma following ileocolic resection (ICR) for Crohn's disease (CD) is an important consideration. This study aims to identify the factors associated with an increased likelihood of a stoma and develop a predictive scoring system (SS). Methods: Patient data were collected from St. Marks Hospital, London, UK and Humanitas Clinical and Research Center, Milan, Italy, on all patients who underwent an ICR for CD from 2005 to 2017. A logistic regression analysis was used for multivariate analysis. The SS was developed from the logistic regression model. The performance of the SS was evaluated using receiver operating characteristics area under the curve (AUROC). Results: A total of 628 surgeries were included in the analysis. Sixty-nine surgeries were excluded due to missing data. The remaining 559 were divided into two cohorts for the scoring system's development (n = 434) and validation (n = 125). The regression model was statistically significant (p < 0.0001). The statistically significant independent variables included sex, preoperative albumin and haemoglobin levels, surgical access and simultaneous colonic resection. The AUROC for the development and validation cohorts were 0.803 and 0.905, respectively (p < 0.0001). Youden's index suggested the cut-off score of − 95.9, with a sensitivity of 87.6% and a specificity of 62.9%. Conclusions: Male sex, low preoperative albumin, anaemia, laparoscopic conversion and simultaneous colonic resection were associated with an increased likelihood of requiring a stoma and were used to develop an SS. Background: Theoretical advantages of Turnbull-Cutait pull-through delayed coloanal anastomosis (DCAA) are a reduced risk of anastomotic leak and therefore avoidance of stoma. Gradually abandoned in favor of immediate coloanal anastomosis (ICAA) with diverting stoma, DCAA has regained popularity in recent years in reconstructive surgery for low RC, especially when combined with minimally invasive surgery (MIS). The aim of this study was to perform the first meta-analysis, exploring the safety and outcomes of DCAA compared to ICAA with protective stoma. Methods: A systematic search of MEDLINE, EMBASE, and CENTRAL and Google Scholar databases was performed for studies published from January 2000 until December 2020. The systematic review and meta-analysis were performed according to the Cochrane Handbook for Systematic Review on Interventions recommendations and Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) guidelines. Results: Out of 2626 studies screened, 9 were included in the systematic review and 4 studies in the meta-analysis. Outcomes included were postoperative complications, pelvic sepsis and risk of definitive stoma. Considering postoperative complications classified as Clavien–Dindo III, no significant difference existed in the rate of postoperative morbidity between DCAA and ICAA (13% versus 21%; OR 1.17; 95% CI 0.38–3.62; p = 0.78; I2 = 20%). Patients in the DCAA group experienced a lower rate of postoperative pelvic sepsis compared with patients undergoing ICAA with diverting stoma (7% versus 14%; OR 0.37; 95% CI 0.16–0.85; p = 0.02; I2 = 0%). The risk of definitive stoma was comparable between the two groups (2% versus 2% OR 0.77; 95% CI 0.15–3.85; p = 0.75; I2 = 0%). Conclusions: According to the limited current evidence, DCAA is associated with a significant decrease in pelvic sepsis. Hypertrophic cardiomyopathy (HCM) often leads to heart failure. Mutations in sarcomeric proteins are most frequently the cause of HCM but in many patients the gene defect is not known. Here we report on a young man who was diagnosed with HCM shortly after birth. Whole exome sequencing revealed a mutation in the FLNC gene (c.7289C > T; p.Ala2430Val) that was previously shown to cause aggregation of the mutant protein in transfected cells. Myocardial tissue from patients with this mutation has not been analyzed before and thus, the underlying etiology is not well understood. Myocardial tissue of our patient obtained during myectomy at the age of 23 years was analyzed in detail by histochemistry, immunofluorescence staining, electron microscopy and western blot analysis. Cardiac histology showed a pathology typical for myofibrillar myopathy with myofibril disarray and abnormal protein aggregates containing BAG3, desmin, HSPB5 and filamin C. Analysis of sarcomeric and intercalated disc proteins showed focally reduced expression of the gap junction protein connexin43 and Xin-positive sarcomeric lesions in the cardiomyocytes of our patient. In addition, autophagy pathways were altered with upregulation of LC3-II, WIPI1 and HSPB5, 6, 7 and 8. We conclude that the p.Ala2430Val mutation in FLNC most probably is associated with HCM characterized by abnormal intercalated discs, disarray of myofibrils and aggregates containing Z-disc proteins similar to myofibrillar myopathy, which supports the pathological effect of the mutation. Despite proven clinical benefits in the short term, technical difficulties limit utilization of laparoscopy in rectal cancer surgery (RCS). Transanal Total Mesorectal Excision (taTME) overcomes many technical limitations of laparoscopic RCS. However, the costs of this procedure have not been addressed yet. Our goal was to perform a comparative cost analysis of taTME and laparoscopic TME (lapTME). Consecutive patients undergoing curative TME between 1 February 2014 and 31 October 2018 were selected from a prospectively maintained database and stratified, according to the type of procedure, into taTME and lapTME groups. Patient demographics, tumour characteristics, operative parameters, and short-term outcomes were analyzed. The main outcome measure was intraoperative costs of the two procedures. Secondary outcomes were short-term outcome and the utilization of hospital resources to manage the postoperative course. Hundred and fifty-two patients with rectal cancer (66 lapTME, 86 taTME) were included in the study. Surgical supplies required for taTME procedure exceeded the cost of lapTME of 754,54 €. The duration of surgery was not significantly different between the two approaches (266 ± 92.85 vs 271 ± 83.63, p = 0.50). Short-term outcomes were comparable including postoperative complication rate (17 vs 20%, p = 0.68), reintervention rate, and length of stay. There was no difference in hospital resources utilization to manage postoperative course including blood test, diagnostics, consultations, and medications. TaTME has higher intraoperative costs in terms of supplies with respect to lapTME. Short-term outcomes and hospital resources to manage postoperative course are comparable. [ABSTRACT FROM AUTHOR]

This study aims to identify issues plaguing rural India's water supply chain and present a general methodology for the implementation of an IoT-based water distribution and management system in a rural setting. Through the literature review, significant hurdles in the water supply chain of rural India, as well as the potential role of IoT in water supply chain management, were recognized. An implementation plan was proposed to design an IoT-based water distribution and management system with the help of flow meters, ultrasonic sensors, motors, etc. Also, the feasibility of such a system was explored with the help of a use case. This study highlighted that one of the most significant reasons for water scarcity in rural India is the lack of proper infrastructure leading to water resources mismanagement. In the context of our use case, despite adequate availability of physical water for consumption, by the time water is delivered to the consumers, a significant amount of it is wasted due to the lack of an efficient water management system. It was determined that the investment return for the proposed system would break even within three months of installment. This study's results are based on inputs from governmental data collected in the Census report of 2011. As such, the ground level scenario is bound to be comparatively different today. Postoperative C-reactive protein (CRP) level allows to rule out infectious complications ensuring safe hospital discharge in colorectal cancer surgery. Since its clinical significance in Crohn's disease (CD) has not been studied yet, we investigated whether CRP can guide decision-making on hospital discharge in this population. Only consecutive CD patients undergoing resections with primary anastomosis and without covering stoma (2013–2017) were analysed. Pre- and post-operative CRP values, measured daily until discharge, were correlated with postoperative complications including anastomotic leakage (AL), infectious and non-infectious complications. The diagnostic accuracy of CRP in predicting AL was evaluated according to the area under the curve (AUC), using the receiver-operating characteristic (ROC) methodology. Two-hundred and fifty-one consecutive patients undergoing elective surgery for CD were selected. AL was diagnosed in 10 patients (4%). High CRP level was associated with AL on postoperative day (POD) 3–5 (p = 0.002, AUC 0.825) with a positive predictive value of 60%. CRP linear difference of 140 between POD 1 and 3 (AUC 0.800) maximizes sensitivity and specificity with a NPV of 98.6%. CRP trend, measured with the linear difference between POD 1 and 3, is able to rule out anastomotic complications with a high NPV and may allow a safe early hospital discharge after surgery for CD. The controlling factors of spatio-temporal variation in δ18O and δD composition of rainwater and the rain–vapour interaction in the southern, middle and northern regions of Kerala, India, are presented. The isotopic distribution (δ18O) of rainfall ranged between 0 and −6‰ during January to September and between −6 to −12‰ during October to December. The rain during post-monsoon season has significant contribution to recycled vapour. Evaporation from falling raindrops is significant despite high relative humidity. The sudden isotopic depletion of ground level vapour from baseline isotopic composition is due to interaction and isotopic exchange with falling raindrops. [ABSTRACT FROM AUTHOR]

Abundance, distribution, movement patterns, and habitat selection of a reservoir species influence the dispersal of zoonotic pathogens, and hence, the risk for humans. Movements and microhabitat use of rodent species, and their potential role in the transmission of hantavirus were studied in Otamendi Natural Reserve, Buenos Aires, Argentina. Movement estimators and qualitative characteristics of rodent paths were determined by means of a spool and line device method. Sampling was conducted during November and December 2011, and March, April, June, October, and December 2012. Forty-six Oxymycterus rufus, 41 Akodon azarae, 10 Scapteromys aquaticus and 5 Oligoryzomys flavescens were captured. Movement patterns and distances varied according to sex, habitat type, reproductive season, and body size among species. O. flavescens, reservoir of the etiologic agent of hantavirus pulmonary syndrome in the region, moved short distances, had the most linear paths and did not share paths with other species. A. azarae had an intermediate linearity index, its movements were longer in the highland grassland than in the lowland marsh and the salty grassland, and larger individuals traveled longer distances. O. rufus had the most tortuous paths and the males moved more during the non-breeding season. S. aquaticus movements were associated with habitat type with longer distances traveled in the lowland marsh than in the salty grassland. Hantavirus antibodies were detected in 20% of A. azarae and were not detected in any other species. Seropositive individuals were captured during the breeding season and 85% of them were males. A. azarae moved randomly and shared paths with all the other species, which could promote hantavirus spillover events. [ABSTRACT FROM AUTHOR]

The results of a 2-year survey on ticks infesting livestock on farms in Western Sicily, Italy, are reported. Several dogs living in the same area were also examined for ticks. Moreover, free-living ticks from one cattle farm were caught by CO2 traps and flagging. A total of 6208 specimens was collected belonging to 9 species: Rhipicephalus bursa (32.4%), Rhipicephalus turanicus (22.7%), Rhipicephalus sanguineus (19.3%), Hyalomma lusitanicum (12.0%), Haemaphysalis punctata (7.8%), Hyalomma marginatum (3.5%), Dermacentor marginatus (1.4%), Ixodes ricinus (0.8%) and Ixodes hexagonus (0.1%). The species belonging to the genus Rhipicephalus were present on all hosts and habitats monitored, R. bursa was found to be abundant on cattle (46.3%) and sheep (62.0%), R. sanguineus was prevalent on dogs (50.4%), and R. turanicus on donkeys (41.0%) and as free-living species (48.5%). A study to evaluate the natural rate of infection of Ixodes ricinus with Borrelia burgdorferi sensu lato was carried out in an endemic focus of Lyme disease in the Trieste area in northern Italy. Two-hundred and twenty-seven ticks collected in ten different stations were tested individually for the presence of the spirochetes using polymerase chain reaction techniques able to identify both Borrelia burgdorferi sensu lato and the four genospecies ( Borrelia burgdorferi sensu stricto, Borrelia garinii, Borrelia afzelii and group VS116). Multiple infection of individual ticks was found. The infection rate ranged from 0–70%. Infection of Ixodes ricinus with Borrelia burgdorferi group VS116 was found for the first time in Italy in both a high and a low endemic focus of Lyme disease. [ABSTRACT FROM AUTHOR]

It is shown how to deduce an asymptotic form of the Gordeev dispersion function of a Maxwellian plasma in a magnetic field, in the limit in which the Larmor radius parameter approaches infinity on the complex plane. This asymptotic form is necessary in the numerical computation of the elements of the plasma dielectric tensor in all physical circumstances characterized by large Larmor radius effects. A detailed analysis is made of the deduction of systems of differential equations describing the propagation of both ordinary and extraordinary waves in the electron cyclotron frequency range in a stratified plasma, perpendicularly to the magnetic field and across the electroncyclotron fundamental resonance layer. The equations are derived under conditions of not too large electron temperature (the so-called weakly relativistic condition) and of very weak nonuniformities of the confining magnetic field. The effects of the nonhomogeneities of the equilibrium plasma density and temperature are carefully examined. It is shown that the propagation equations derived previously in the literature can be extended with only a moderately larger effort in computations to take account also of very strong density and temperature gradients. The circadian rhythm of a tropical insectivorous bat, Taphozous melanopogon, free-runs in dim light and responds to dark breaks of a few hours' duration with 'advances' and 'delays' as a function of the phase experiencing the 'black out'. Similarly phase shifts also follow perturbations by light breaks. A system of nonlinear integro-differential equations is derived for the motion of the classical electron with a rigid and spherically symmetric 3D gaussian distribution of charge. The equations are analyzed for stability around the state of rest and of uniform rectilinear motion with velocity small with respect to the velocity of light. The extremely high-frequency and radiationless micro-oscillations that the electron executes when disturbed from the equilibrium states show the inconsistency of the Abraham-Lorentz equation and of all concepts associated with this equation, like the notion that the electron may have a mass of electromagnetic origin. This paper reports study of day-to-day instability in the locomotor activity rhythm of the nocturnal field mouse Mus booduga. The free-running period (τ) of this rhythm was estimated in constant darkness in n=347 adult male mice. The "onset" and "offset" of locomotor activity rhythm were used as phase markers of the circadian clock. The precision of the onset of locomotor activity was observed to be a non-linear function of τ, with maximal precision at τ close to 24 h. The precision of the offset of locomotor activity was found to increase with increasing τ. These results suggest that the homeostasis of τ is tighter when τ is close to 24 h. [ABSTRACT FROM AUTHOR]
