Congenital omphalo hepatico sigmoid colon fistula is yet not described in the literature. There are few case reports of hepatico colic fistula in adults but none is in the paediatric age group. Most of the adult hepatico colic fistulas are due to hepatic abscess, cyst, or foreign bodies. The primary hepatico colic fistula with umbilical connection is not described yet. We report a case of a 2-year-old male child who had an omphalo hepatico sigmoid fistula. The patient was managed with surgery and the fistula was divided, following which he had a biliary leak which was managed with ERC. The child developed extrahepatic portal vein occlusion and atrophy hypertrophy in the liver due to repeated cholangitis episodes. Currently, the patient is doing well at 4 years follow-up. Congenital omphalo hepatico sigmoid colon fistula is one of the rarest anomalies. Early recognition and treatment are essential to prevent complications. [ABSTRACT FROM AUTHOR]

Acute abdomen due to biliary peritonitis is mainly seen post laparoscopic cholecystectomy. Most common cause of primary biliary peritonitis is gall bladder perforation. Spontaneous bile duct perforation is a rare disorder, mostly seen in pediatric age group. Congenital anomalies, choledocholithiasis, and acute/chronic pancreatitis are some of the causes of spontaneous bile duct perforation. Early diagnosis and appropriate management is necessary to prevent morbidity and mortality. We report a case of a 9-year-old girl, with no past medical history, who presented with acute abdomen at a nearby hospital. She was referred to us after exploratory laparotomy, lavage, and drainage done outside. At our center, patient was evaluated clinically; operative findings from previous surgery were noted. After resuscitation, computed tomography of abdomen was done. Exploratory laparotomy was performed. Operative findings, management, and postoperative course are discussed. [ABSTRACT FROM AUTHOR]

Our study aims to review the literature on the management of pyogenic liver abscess, focusing on the choice of drainage. A case series of our experience with clinicopathological correlation is presented to highlight the indication and outcome of each modality of drainage. Intravenous antibiotic is the first line, and mainstay, of treatment. Drainage is necessary for large abscesses, equal to or larger than 5 cm in size, to facilitate resolution. While percutaneous drainage is appropriate as first-line surgical treatment in most cases, open surgical drainage is prudent in cases of rupture, multiloculation, associated biliary, or intra-abdominal pathology. Percutaneous drainage may help to optimize clinical condition prior to surgery. Nevertheless, in current good clinical practices, the choice of therapy needs to be individualized according to patient's clinical status and abscess factors. The article presents a case study of a 9-year-old male child who has a history of increasing swelling on his back since birth. It states that physical examinations have been conducted, such as X-ray chest and routine blood investigations, and confirmed a diagnosis of giant congenital lipoma. It also discusses the definition of lipoma, the common sites that lipoma targets, and some methods used to trigger lipoma.
