Purpose: Quantifying the stability of individual plants or their contribution to soil reinforcement against erosion or landslides requires an understanding of the tensile properties of their roots. This work developed a new analytical model to understand the tensile stress–strain behaviour of a single root axis, which is the first to incorporating root anatomical features, in order to reduce the existing uncertainty in predictions. Methods: The root was modelled as a linear elastic stele connected to a surrounding linear elastic cortex by means of a linear elastic stele–cortex interface. By solving for force equilibrium, an analytical solution for the full tensile stress–strain behaviour — including any intermediate brittle failures of the stele, cortex and/or interface — was obtained. This model was compared to tensile tests and laser ablation tomography for maize roots. Results: The new modelling approach demonstrated that the root tensile strength is fully determined by the strength of the stele alone, which was an order of magnitude larger than that of the cortex while also 3–4 times stiffer. The reduction in root stiffness beyond the yield point was linked to continuing fracturing of the cortex and debonding along the stele–cortex interface. A larger proportion of the variation in experimentally measured biomechanical characteristics could be explained compared to root diameter power-law fitting methods typically applied in the literature. Conclusion: Stele and cortex biomechanical properties are substantially different, affecting the tensile behaviour of plant roots. This article reports the case of a patient who developed acute coronary syndrome while receiving postoperative pain treatment via an epidural catheter (EC). Platelet function testing was performed before removal of the EC to assess the bleeding risk under ongoing dual antiplatelet therapy. Platelet function testing showed low responsiveness to clopidogrel and acetylsalicylic acid (ASS). The EC was removed uneventfully and clopidogrel was subsequently replaced by prasugrel and platelet function showed improved depression of thrombocyte aggregation. Possible reasons for low responsiveness to clopidogrel and ASS, such as drug-drug interactions with proton pump inhibitors and genetic factors are discussed. [ABSTRACT FROM AUTHOR]

Objective: To study the effect of BMI on the prevalence, severity, and 36-month progression of early degenerative changes in the knee by using 3T MRI in middle-aged subjects without radiographic osteoarthritis (OA).Materials and Methods: We examined baseline and 36-month follow-up MR studies from 137 middle-aged individuals (45-55 years old) with risk factors for knee OA but no radiographic OA from the Osteoarthritis Initiative. Subjects were grouped into three categories: normal BMI (BMI < 25 kg/m(2), n = 38), overweight (BMI 25-29.9 kg/m(2), n = 37), and obese (BMI ≥ 30 kg/m(2), n = 62). Using 3T MRI, cartilage, meniscus, and bone marrow abnormalities were graded using the OA Whole-organ MR Imaging Score (WORMS). The statistical analysis was corrected as necessary for differences in age, sex, and OA risk factors other than BMI.Results: The overall prevalence of lesions was 64% for meniscus and 79% for cartilage (including low grade lesions). At baseline, the prevalence and severity of knee lesions was positively associated with BMI, with a nearly fourfold increase in meniscal tears and more than twofold increase in high-grade cartilage defects in obese individuals relative to normal-weight subjects. Over the 36-month follow-up period, the number of new or worsening cartilage lesions of any grade was significantly higher in obese subjects (p = 0.039), while there was no significant difference in meniscal lesion progression.Conclusion: Obesity was associated with both higher prevalence and severity of early degenerative changes in the knee in middle-aged individuals without radiographic OA and with significantly increased cartilage lesion progression (of any grade) over 36 months. We propose a method for determining the effective parameters of the upper marine sediment layer on extended tracks from the spectra of wideband signals in conditions of hydrodynamic variability. As an example, we consider the Shallow Water 2006 experiment on the Atlantic shelf of the United States, which used signals with a band of 300 ± 30 Hz received by a vertical array. The length of the track was ~20 km at a sea depth of ~80 m. Frequency-mode analysis of the received signals showed that spatiotemporal fluctuations of the wave medium lead to random changes in mode amplitudes while retaining the relative stability of the mode phase difference. This is the basis of the proposed method, which makes it possible to determine the track-averaged values of the sound velocity in the bottom and density of the bottom under conditions of hydrodynamic variability. High definition macroradiography was used to provide an image of the detailed structural organization of the cancellous bone in human lumbar vertebrae. The fractal signature analysis (FSA) method was used to quantify the horizontal and vertical trabecular organization recorded within the image. Comparison of the FSA of the postero-anterior and lateral macroradiographs in postmortem lumbar vertebrae showed that neither the superimposition of the neural arch nor the radiographic angle affected the trabecular measurement within the vertebral body. FSA analysis of the trabecular structure measured from the macroradiographs of lumbar vertebrae in two groups of postmenopausal women, with high and low bone mineral density (BMD), showed that the large vertical trabecular structures correlated with the women's body weight (P < 0.01-0.03) and body mass index (P < 0.005-0.05), the fine horizontal structures correlated with the women's age (P < 0.005-0.05), and fine vertical trabecular structures were significantly greater (P < 0.005-0.05) in the low compared with the high BMD group. The permeation of monovalent organic cations through adenosine 3',5'-cyclic monophosphate-(cAMP) activated channels was studied by recording macroscopic currents in excised inside-out membrane patches from the dendritic knobs of isolated mammalian olfactory receptor neurons (ORNs). Current-voltage relations were measured when bathing solution Na+ was replaced by monovalent organic cations. Permeability ratios relative to Na+ ions were calculated from changes in reversal potentials. Some of the small organic cations tested included ammonium (NH4+), hydroxylammonium and formamidinium, with relative permeability ratios of 1.41, 2.3 and 1.01 respectively. The larger methylated and ethylated ammonium ions studied included: DMA (dimethylammonium), TMA (tetramethylammonium) and TEA (tetraethylammonium) and they all had permeability ratios larger than 0.09. Even large cations such as choline, arginine and tris(hydroxymethyl)aminomethane (Tris) were appreciably permeant through the cAMP-activated channel with permeability ratios ranging from 0.19 to 0.7. The size of the permeating cations, as assessed by molecular weight, was a good predictor of the permeability. The permeability sequence of the cAMP-activated channel in our study was PNH4 > PNa > PDMA > PTMA > PCholine > PTEA. Higher permeability ratios of hydroxylammonium, arginine and tris(hydroxymethyl)aminomethane cannot be explained by ionic size alone. Our results indicate that: (i) cAMP-activated channels poorly select between monovalent cations; (ii) the pore dimension must be at least 6.5 x 6.5 A, in order to allow TEA and Tris to permeate and (iii) molecular sieving must be an important mechanism for the permeation of large organic ions through the channels with specific ion binding playing a smaller role than in other structurally similar channels. In addition, the results clearly indicate that cyclic nucleotide-gated (CNG) channels in different cells are not the same, the olfactory CNG channel being different from that of the photoreceptors, particularly with respect to the permeation of large organic cations, which the ORN channels allow to permeate readily. Through a targeted knockdown (KD) screen of chromatin regulatory genes, we identified the EP400 complex components EPC1 and EPC2 as critical oncogenic cofactors in acute myeloid leukemia (AML). EPC1 and EPC2 were required for the clonogenic potential of human AML cells of multiple molecular subtypes. Focusing on MLL-mutated AML as an exemplar, Epc1 or Epc2 KD-induced apoptosis of murine MLL-AF9 AML cells and abolished leukemia stem cell potential. By contrast, normal hematopoietic stem and progenitor cells (HSPC) were spared. Similar selectivity was observed for human primary AML cells versus normal CD34+ HSPC. In keeping with these distinct functional consequences, Epc1 or Epc2 KD-induced divergent transcriptional consequences in murine MLL-AF9 granulocyte-macrophage progenitor-like (GMP) cells versus normal GMP, with a signature of increased MYC activity in leukemic but not normal cells. This was caused by accumulation of MYC protein and was also observed following KD of other EP400 complex genes. Pharmacological inhibition of MYC:MAX dimerization, or concomitant MYC KD, reduced apoptosis following EPC1 KD, linking the accumulation of MYC to cell death. Therefore, EPC1 and EPC2 are components of a complex that directly or indirectly serves to prevent MYC accumulation and AML cell apoptosis, thus sustaining oncogenic potential. Laparoscopic and robotic partial nephrectomy have become the preferred option for surgical management of incidentally discovered small renal tumors. Currently there is no consensus on which aspects of the procedure should be performed laparoscopically versus robotically. We believe that combining a laparoscopic exposure and hilar dissection followed by tumor extirpation and renorrhaphy with robotic assistance provides improved perioperative outcomes compared to a pure robotic approach alone. We performed a comparison of perioperative outcomes between combined laparoscopic-robotic partial nephrectomy-or hybrid procedure-and pure robotic partial nephrectomy (RPN). A multi-center retrospective analysis of patients undergoing RPN and hybrid PN using the da Vinci S system was performed. Patient data were reviewed for demographic and perioperative variables. Statistical analysis was performed using the Welch t test and linear regression, and nonparametric tests with similar significance results. Thirty-one patients underwent RPN while 77 patients underwent hybrid PN between 2007 and 2011. Preoperative variables were comparable in both groups with the exception of lesion size and nephrometry score which were significantly higher in patients undergoing hybrid PN. Length of surgery, estimated blood loss and morphine used were significantly less in the hybrid group, while warm ischemia time was significantly longer. The difference in WIT was accounted for in this data by adjusting for nephrometry score. In our multi-center series, the hybrid approach was associated with a shorter operative time, reduced blood loss and lower narcotic usage. We believe this approach is a valid alternative to RPN. The paper presents an analysis of energy fluctuations of high-frequency (2-4.5 kHz) sound signals propagating in a shallow water in the presence of nonlinear (soliton-like) internal waves (2006 Shallow Water experiment, US Atlantic shelf). Signals were received by three single hydrophones in different directions at distances of ∼4, ∼12, and ∼5 km from the source. The angle between the first two acoustic tracks was ∼15°. The third track was almost an extension of the first and was on the other side of the source. A relatively short (one to two solitons) nonlinear internal wave packet first moved approximately along the first two tracks and then along the third track. It is demonstrated that in the presence of solitons on the track in the frequency spectrum of energy fluctuations, there is an isolated frequency that depends, in particular, on the angle between the soliton front and the acoustic track. The experimental results agree well with the theory previously proposed by the authors, where the occurrence mechanism of fluctuations is explained using the ray approach. [ABSTRACT FROM AUTHOR]

MicroRNAs (miRNAs) contribute to the pathogenesis of many forms of cancer, including the pediatric cancer neuroblastoma, but the underlying mechanisms leading to altered miRNA expression are often unknown. Here, a novel integrated approach for analyzing DNA methylation coupled with miRNA and mRNA expression data sets identified 67 epigenetically regulated miRNA in neuroblastoma. A large proportion (42%) of these miRNAs was associated with poor patient survival when underexpressed in tumors. Moreover, we demonstrate that this panel of epigenetically silenced miRNAs targets a large set of genes that are overexpressed in tumors from patients with poor survival in a highly redundant manner. The genes targeted by the epigenetically regulated miRNAs are enriched for a number of biological processes, including regulation of cell differentiation. Functional studies involving ectopic overexpression of several of the epigenetically silenced miRNAs had a negative impact on neuroblastoma cell viability, providing further support to the concept that inactivation of these miRNAs is important for neuroblastoma disease pathogenesis. One locus, miR-340, induced either differentiation or apoptosis in a cell context dependent manner, indicating a tumor suppressive function for this miRNA. Intriguingly, it was determined that miR-340 is upregulated by demethylation of an upstream genomic region that occurs during the process of neuroblastoma cell differentiation induced by all-trans retinoic acid (ATRA). Further biological studies of miR-340 revealed that it directly represses the SOX2 transcription factor by targeting of its 3′-untranslated region, explaining the mechanism by which SOX2 is downregulated by ATRA. Although SOX2 contributes to the maintenance of stem cells in an undifferentiated state, we demonstrate that miR-340-mediated downregulation of SOX2 is not required for ATRA induced differentiation to occur. In summary, our results exemplify the dynamic nature of the miRNA epigenome and identify a remarkable network of miRNA/mRNA interactions that significantly contribute to neuroblastoma disease pathogenesis. Background and aims: Soil carbon storage is an important component of global carbon cycling. Andean Andisols have high carbon content and are vulnerable to erosion because of agricultural intensification and deforestation. This study examines the effects of land use on erosion and soil carbon storage in the Río Chimbo watershed of Ecuador. Methods: Soil carbon content, age, and erosion estimated from Cs inventories was measured along an elevational transect under annual cropping, natural forest, páramo, pasture, and tree plantations. Results: Land use, particularly annual cropping, affected Cs levels in the upper soil layers, but did not have an impact on total carbon storage to a depth of 1 m. Relative erosion rates estimated from Cs inventories at sites under annual cropping averaged 27 t ha y over the erosion rate of non-cultivated sites. A linear relationship was observed between soil carbon age (determined by C levels) and Cs levels, where pasture sites had lower Cs and older carbon compared to natural forest sites. Conclusions: The effects of land use on soil loss in the Río Chimbo watershed suggest a loss and/or removal of soil carbon, particularly under annual cropping. [ABSTRACT FROM AUTHOR]

Background:Neuroblastoma remains a major cause of cancer-linked mortality in children. miR-204 has been used in microRNA expression signatures predictive of neuroblastoma patient survival. The aim of this study was to explore the independent association of miR-204 with survival in a neuroblastoma cohort, and to investigate the phenotypic effects mediated by miR-204 expression in neuroblastoma.Methods:Neuroblastoma cell lines were transiently transfected with miR-204 mimics and assessed for cell viability using MTS assays. Apoptosis levels in cell lines were evaluated by FACS analysis of Annexin V-/propidium iodide-stained cells transfected with miR-204 mimics and treated with chemotherapy drug or vehicle control. Potential targets of miR-204 were validated using luciferase reporter assays.Results:miR-204 expression in primary neuroblastoma tumours was predictive of patient event-free and overall survival, independent of established known risk factors. Ectopic miR-204 expression significantly increased sensitivity to cisplatin and etoposide in vitro. miR-204 direct targeting of the 3′ UTR of BCL2 and NTRK2 (TrkB) was confirmed.Conclusion:miR-204 is a novel predictor of outcome in neuroblastoma, functioning, at least in part, through increasing sensitivity to cisplatin by direct targeting and downregulation of anti-apoptotic BCL2. miR-204 also targets full-length NTRK2, a potent oncogene involved with chemotherapy drug resistance in neuroblastoma. BACKGROUND/OBJECTIVES:Data from the Avon Longitudinal Study of Parents and Children (ALSPAC) have previously shown that dietary patterns are observable by 3 years. However, it is not clear when dietary patterns emerge. We aimed to describe dietary patterns in early life and their associations with maternal and infant sociodemographic characteristics.SUBJECTS/METHODS:Principal component analysis was applied to diet questionnaires of ALSPAC participants at 6 months (n=7052) and 15 months (n=5610) to extract dietary patterns. The sociodemographic factors associated with dietary patterns were investigated using regression analyses.RESULTS:Four dietary patterns were extracted at both 6 and 15 months. A traditional-style pattern characterized by home-prepared meats, vegetables and desserts, a second pattern characterized by ready-prepared baby foods and a third pattern characterized by discretionary foods such as biscuits, sweets and crisps were identified at both ages. At 6 months, the fourth pattern was characterized predominantly by breastfeeding and at 15 months, by contemporary-style foods including herbs, legumes, nuts, raw fruit and vegetables. Higher maternal age and education, number of siblings and lower body mass index (BMI) was associated with higher scores on the breastfeeding or meat, vegetables and desserts patterns, whereas higher discretionary food pattern scores were associated with younger maternal age, lower education, higher BMI and more siblings. Associations between sociodemographic factors and the ready-prepared baby food pattern scores were inconsistent across ages.CONCLUSIONS:Dietary patterns emerge from infancy and are associated with sociodemographic characteristics. To investigate the frequency of isocitrate dehydrogenase 1 (IDH1) and 2 (IDH2) mutations in pediatric acute myeloid leukemia (AML) and acute lymphoid leukemia (ALL), we sequenced these genes in diagnostic samples from 515 patients (227 AMLs and 288 ALLs). Somatic IDH1/IDH2 mutations were rare in ALL (N=1), but were more common in AML, occurring in 3.5% (IDH1 N=3 and IDH2 N=5), with the frequency higher in AMLs with a normal karyotype (9.8%). The identified IDH1 mutations occurred in codon 132 resulting in replacement of arginine with either cysteine (N=3) or histidine (N=1). By contrast, mutations in IDH2 did not affect the homologous residue but instead altered codon 140, resulting in replacement of arginine with either glutamine (N=4) or tryptophan (N=1). Structural modeling of IDH2 suggested that codon 140 mutations disrupt the enzyme's ability to bind its substrate isocitrate. Accordingly, recombinant IDH2 R140Q/W were unable to carry out the decarboxylation of isocitrate to α-ketoglutarate (α-KG), but instead gained the neomorphic activity to reduce α-KG to R(-)-2-hydroxyglutarete (2-HG). Analysis of primary leukemic blasts confirmed high levels of 2-HG in AMLs with IDH1/IDH2 mutations. Considerable shifts Δ f/ϕ ≈ 10 of the low-frequency sound field interference pattern in the frequency domain, associated with barotropic tide and internal tidal waves, were observed in the Shallow Water'06 experiment on the New Jersey shelf in the summer of 2006. The acoustic frequency shifts appear to be strongly dependent on the modes of the sound field. By examining different modal structure, it is possible to analyze the overall interference pattern and find which part is more sensitive either to the surface tide or the internal waves. This study investigated pointing movements in 3D asking two questions: (1) Is goal-directed reaching accompanied by self-motion, a component of the joint velocity vector that leaves the hand's movement unaffected? (2) Are differences in the terminal joint configurations among different speeds of reaching motor equivalent (i.e., terminal joint configurations differ more in directions of joint space that do not produce different pointer-tip positions than in directions that do) or non-motor equivalent (i.e., terminal joint configurations differ equally or more in directions of joint space that lead to different pointer-tip positions than in directions that do not affect the pointer-tip position). Subjects reached from an identical starting joint configuration and pointer-tip location to targets at slow, moderate, and fast speeds. Ten degrees of freedom of joint motion of the arm were recorded. The relationship between changes in the joint configuration and the three-dimensional pointer-tip position was expressed by a standard kinematic model, and the range- and null subspaces were computed from the associated Jacobian matrix. (1) The joint velocity vector and (2) the difference vector between terminal joint configurations from pairs of speed conditions were projected into the two subspaces. The relative length of the two components was used to quantify the amount of self-motion and the presence of motor equivalence, respectively. Results revealed that reaches were accompanied by a significant amount of self-motion at all reaching speeds. Self-motion scaled with movement speed. In addition, the difference in the terminal joint configuration between pairs of different reaching speeds revealed motor equivalence. The results are consistent with a control system that takes advantage of motor redundancy, allowing for flexibility in the face of perturbations, here induced by different movement speeds. Purpose: Patients with lung cancer frequently suffer psychological distress and guidelines in the United Kingdom recommend screening of all cancer patients for this problem. The audit investigated use of the Distress Thermometer in terms of staff adherence to locally developed guidelines, patient willingness to use the tool, its impact on referral rates to clinical psychology services and concordance between the tool and the clinical assessment.Method: Use of the Distress Thermometer was audited over a 3-month period in one lung cancer outpatient clinic. Referrals to clinical psychology services in response to clearly delineated referral indicators were assessed. Patient-reported outcomes were compared with practitioner assessment of need during clinical consultations to see whether the tool was measuring distress effectively.Results: Thirty three of 34 patients used the Distress Thermometer during the audit period. Ten reported distress levels above 4 in the emotional or family problems domains. On ten occasions, the clinical interview identified problems not elicited by the Distress Thermometer. Guidelines were adhered to by staff, and patients were offered information about local support services and referral to clinical psychology services where indicated. Whilst all patients were happy to receive written information about further sources of support, none wanted to be referred to psychological services at that time.Conclusions: The Distress Thermometer is acceptable to patients with lung cancer in outpatient settings but it did not increase referrals for psychological support. Staff found it to be a useful tool in opening up communication about patient issues although it should not replace a comprehensive clinical interview. [ABSTRACT FROM AUTHOR]

Objective:It is important to understand levels and social inequalities in childhood overweight within and between countries. This study examined prevalence and social inequality in adolescent overweight in 35 countries, and associations with macroeconomic factors.Design:International cross-sectional survey in national samples of schools.Subjects:A total of 11-, 13- and 15-year-olds from 35 countries in Europe and North America in 2001–2002 (N=162 305).Measurements:The main outcome measure was overweight based on self-reported height and weight (body mass index cut-points corresponding to body mass index of 25 kg/m2 at the age of 18 years). Measures included family and school affluence (within countries), and average country income and economic inequality (between countries).Results:There were large variations in adolescent overweight, from 3.5% in Lithuanian girls to 31.7% in boys from Malta. Prevalence of overweight was higher among children from less affluent families in 21 of 24 Western and 5 of 10 Central European countries. However, children from more affluent families were at higher risk of overweight in Croatia, Estonia and Latvia. In Poland, Lithuania, Macedonia and Finland, girls from less affluent families were more overweight whereas the opposite was found for boys. Average country income was associated with prevalence and inequality in overweight when considering all countries together. However, economic inequality as measured by the Gini coefficient was differentially associated with prevalence and socioeconomic inequality in overweight among the 23-high income and 10-middle income countries, with a positive relationship among the high income countries and a negative association among the middle income countries.Conclusion:The direction and magnitude of social inequality in adolescent overweight shows large international variation, with negative social gradients in most countries, but positive social gradients, especially for boys, in some Central European countries. The sound field produced by a low-frequency (100–1000 Hz) point source in a shallow water region in the presence of the temperature front is considered. It is shown that the latter anisotropic inhomogeinity leads to a substantial horizontal refraction and to a number of related effects: a redistribution of the sound field in the horizontal plane, changes in the spectral and mode contents of a pulse in the course of its propagation, and changes in the arrival times at the receiver. We investigated if the plant growth promoting fungus Trichoderma harzianum Rifai 1295-22 (also known as “T22”) could be used to enhance the establishment and growth of crack willow ( Salix fragilis) in a soil containing no organic or metal pollutants and in a metal-contaminated soil by comparing this fungus with noninoculated controls and an ectomycorrhizal formulation commercially used to enhance the establishment of tree saplings. Crack willow saplings were grown in a temperature-controlled growth room over a period of 5 weeks’ in a garden center topsoil and over 12 weeks in a soil which had been used for disposal of building materials and sewage sludge containing elevated levels of heavy metals including cadmium (30 mg kg−1), lead (350 mg kg−1), manganese (210 mg kg−1), nickel (210 mg kg−1), and zinc (1,100 mg kg−1). After 5 weeks’ growth in clean soil, saplings grown with T. harzianum T22 produced shoots and roots that were 40% longer than those of the controls and shoots that were 20% longer than those of saplings grown with ectomycorrhiza (ECM). T. harzianum T22 saplings produced more than double the dry biomass of controls and more than 50% extra biomass than the ECM-treated saplings. After 12 weeks’ growth, saplings grown with T. harzianum T22 in the metal-contaminated soil produced 39% more dry weight biomass and were 16% taller than the noninoculated controls. This is the first report of tree growth stimulation by application of Trichoderma to roots, and is especially important as willow is a major source of wood fuel in the quest for renewable energy. These results also suggest willow trees inoculated with T. harzianum T22 could be used to increase the rate of revegetation and phytostabilization of metal-contaminated sites, a property of the fungus never previously demonstrated. Basic results of theoretical and experimental studies of acoustic effects caused by horizontal refraction in a shallow water are reviewed with special emphasis on the effect produced by intense internal waves on the propagation of broadband signals. The authors investigated the association between socioeconomic position and stage of breast cancer at the time of diagnosis in a nationwide Danish study. All 28 765 women with a primary invasive breast cancer diagnosed between 1983 and 1999 were identified in a nationwide clinical database and information on socioeconomic variables was obtained from Statistics Denmark. The risk of being diagnosed with a high-risk breast cancer, that is size >20 mm, lymph-node positive, ductal histology/high histologic grade and hormone receptor negative, was analysed by multivariate logistic regression. The adjusted odds ratio (OR) for high-risk breast cancer was reduced with longer education with a 12% reduced risk (95% confidence interval (CI), 0.80,0.96) in women with higher education and increased with reduced disposable income (low income group: OR, 1.22; 95% CI, 1.10,1.34). There was an urban-rural gradient, with higher risk among rural women (OR 1.10; 95 % CI, 1.02, 1.18) and lower risk among women in the capital suburbs (OR, 0.85; 95% CI, 0.78, 0.93) and capital area (OR, 0.93; 95% CI, 0.84-1.02). These factors were significant only for postmenopausal women, although similar patterns were observed among the premenopausal women, suggesting a subgroup of aggressive premenopausal breast cancers less influenced by socioeconomic factors. The antibiotic 2,4-diacetylphloroglucinol (Phl) is produced by a range of naturally occurring fluorescent pseudomonads. One isolate, Pseudomonas fluorescens F113, protects pea plants from the pathogenic fungus Pythiuni ultimum by reducing the number of pathogenic lesions on plant roots, but with a concurrent reduction in the emergence of plants such as pea. The genes responsible for Phl production have been shown to be functionally conserved between the wild-type (wt) P. fluorescens strains F113 and Q2-87. In this study the genes from F113 were isolated using an optimized long PCR method and a 6.7-kb gene cluster inserted into the chromosome of the non-Phl-producing P. fluorescens strain SBW25 EeZY6KX. This strain is a lacZY, kmR marked derivative of the wt SBW25 which effects biological control against the plant pathogen Pythium ultimum by competitive exclusion as a result of its strong rhizosphere-colonizing ability. We describe here the integration of the Phl antifungal and competitive exclusion mechanisms into a single strain, and the impact this has on survival and plant emergence in microcosms. The insertion of the Phl biosynthetic genes from the F113 into the SBW25 chromosome gave a Phl-producing transformant (strain Pa21) able to suppress P. ultimum through antibiotic production. The growth of Pa21 was not reduced in flask culture at 20°C compared with its parent strain. When inoculated on pea seedlings, the strain containing the Phl operon behaved similarly to the SBW25 EeZY6KX parent but did not show the tendency of the wt Phl producer F113 to cause lower pea seed emergence. Pea roots inoculated with SBW25 EeZY6KX have significantly lower indigenous populations than with F113 and the control. This is indicative of this strain's strong colonising presence. Summary:Relapse of hematologic malignancies after allogeneic stem cell transplantation remains a common problem, in particular for patients who have advanced disease at the time of transplantation. Thiotepa has excellent antileukemic and immunosuppressive activity, and could therefore be a useful drug in the conditioning regimen for patients with advanced hematologic neoplasms. We retrospectively analyzed toxicity, engraftment and survival data of 41 patients who received a conditioning regimen of thiotepa (600?mg/m2) and hyperfractionated TBI (10?Gy) prior to matched related (n=25) or matched unrelated (n=16) allogeneic stem cell transplantation. The mean age at transplantation was 37.8 years (range 20-59), all but five patients had advanced hematologic malignancies at the time of transplantation. GVHD prophylaxis was with standard cyclosporine and methotrexate. Engraftment was excellent, but the regimen was associated with a high incidence of grade III renal (41%) and hepatic (15%) toxicity, and high transplant-related mortality (44%at day+90). The 3-year event-free survival was 13%and overall survival 14%. Summary:The importance of the association between early lymphocyte recovery and outcome has not been well studied in autologous stem cell transplantation (ASCT). In this retrospective study, we analyzed 90 consecutive patients with non-Hodgkin's and Hodgkin's lymphoma who underwent ASCT. Patients were divided into two groups: group 1 with absolute lymphocyte count (ALC) on day +15 below the median of 667/mm3, and group 2 with ALC ?667/mm3. The median progression-free survival (PFS), but not overall survival (OS), was significantly longer in group 2 when compared to group 1 (16 months vs not reached P=0.02). Group 2 patients also had significantly shorter hospital stay, received higher CD34+ cell dose, and had shorter time to neutrophil recovery. Multivariate analysis demonstrated day +15 ALC to be an independent prognostic indicator for PFS, but not OS, while CD34+ cell dose and the number of pretransplant treatments were better predictors for both PFS and OS. Objective: To compare fixed-flexion radiography of the knee with fluoroscopic semi-flexed radiography in terms of the reproducibility of measurements of minimum joint-space width (JSW) in the medial femorotibial joint.Methods: Posteroanterior radiographs of the right knees of 18 normal volunteers were acquired with the patients standing on an upright fluoroscopy table, the feet externally rotated 10 degrees and the toes touching the vertical table. Knees were positioned and radiographed with two different techniques: (1) semi-flexed positioning under fluoroscopic guidance using a horizontal X-ray beam; and (2) fixed-flexion positioning, with the knees and thighs touching the vertical table, using 10 degrees caudal beam angulation without fluoroscopy. Foot maps were drawn in each case. Subjects were repositioned and radiographed twice using each technique. The posteroanterior beam angle that optimally projected the medial tibia plateau with the patient in the fixed-flexion position was also determined for each subject in a separate examination using fluoroscopy. Ten patients with osteoarthritis were also examined with the fixed-flexion technique using a conventional radiographic unit. Minimum medial joint-space width (JSW) in the medial femorotibial joint was measured manually with a graduated lens and also with a semi-automated computer algorithm.Results: Reproducibility errors (root-mean-square SD) for manual and automated JSW measurement were 0.2 mm and 0.1 mm, respectively, for fluoroscopic semi-flexed positioning in volunteers; 0.3 mm and 0.1 mm, respectively, for fixed-flexion positioning in volunteers; and 0.2 mm and 0.1 mm, respectively, for fixed-flexion positioning in osteoarthritic patients. The anchoring and decomposition mechanisms of platinum(II) bis-acetylacetonate on alumina surfaces are studied by X-ray absorption spectroscopy at the platinum LIII edge. A distinction is made between highly reactive surfaces which are partially dehydroxylated and exhibit coordinatively unsaturated surface sites, and deactivated surfaces which are covered by a monolayer of OH groups. The samples are studied after three stages of a wet impregnation synthesis: after drying at room temperature, drying at 120 °C and calcination at 350 °C. The XANES signal and the filtered EXAFS signals of the first and second coordination shells around platinum were analyzed. Two different mechanisms are discussed for the two types of supports. Temperature-programmed reduction (TPR), X-ray photoelectron spectroscopy (XPS), X-ray diffraction (XRD) and in situ extended X-ray absorption fine structure (EXAFS) studies were performed to investigate Pt-Co/NaY and Pt-Co/Al2O3 bimetallic catalysts. The EXAFS experiments were carried out at the Pt LIII and Co K edges of the same sample. This particular approach allows a precise determination of the electronic and structural characteristics of the metallic part of the catalyst. For both systems in situ reduction under pure H2 results in the formation of nanometer-scale metallic clusters. For both Co and Pt, nearest neighbors are Co atoms. The complete set of parameters implies the presence of two families of nanometer-scale metallic clusters: monometallic Co nanosized particles and Pt-Co bimetallic clusters, in which only Pt-Co bonds exist (no Pt-Pt bonds). TPR and XPS results indicating a reduction of Co2+ ions in Pt-Co/NaY to a greater extent than in Pt-Co/Al2O3 give evidence of a facilitated reduction. XRD also shows the presence of nanometer-scale particles with only a very small fraction of larger bimetallic particles. In subsequent mild oxidation of the reduced systems the Co nanoparticles are still present inside the supercage of NaY zeolite in bimetallic form and the oxidation of the metallic particles is slowed down. Patients receiving high-dose preparation for stem cell transplantation are at risk for organ dysfunction (OD). Signs of early OD include hypoxia, mental status changes, and liver dysfunction. These early signs have not been correlated with potential cytokine mediators. We compared plasma concentrations of IL-6, TNF-α, and IL-10 in OD patients and controls. Cytokines were measured before preparation, 5 days before OD, day of OD, and 5 days after OD. TNF-α and IL-10 were not measurable prior to preparation. IL-10 was more likely to be measurable in OD patients than in controls 5 days prior to onset of OD (P = 0.039), on the day of OD (P = 0.023), and 5 days later (P < 0.0001). TNF-α was more likely to be measurable only on the day of OD (P = 0.0035). IL-6 was significantly elevated in OD patients at all time points. Patients who had measurable IL-6 on admission were 5.1 times more likely to develop OD (95% CI = 1.4–17.9; P = 0.011). Five days prior to OD for each 100 pg/ml increase in IL-6, patients were 2.75 times more likely to develop OD (95% CI = 1.3–5.8; P = 0.0087). Our purpose is to focus attention on the cancer family history, coupled with an understanding of the natural history and extracolonic tumor spectrum of familial adenomatous polyposis (FAP), through a family study. This family report provides an example of how colorectal cancer (CRC) can be prevented by knowledgeable gastroenterologists and colorectal surgeons who educate and compassionately counsel members of high-risk families so that their compliance with diagnostic screening and, ultimately, with protection through prophylactic colectomy, is achieved. A working pedigree of this extended family was constructed through interviews with the proband, followed by questionnaires sent to all primary and secondary relatives. Appropriately signed permission forms enabled us to secure pertinent medical and pathology records in order to ensure accuracy of historical information. Integral extracolonic tumors included medulloblastoma, papillary thyroid carcinoma, hepatoblastoma, and desmoid tumors. This study evaluated whether cytokine-induced blood stem cell mobilization also mobilized lymphoma cells and whether lymphoma cell mobilization affected outcome post autologous blood stem cell transplant. Blood stem cell collections from 26 non-Hodgkin’s lymphoma (NHL) patients harvested during steady-state (non-mobilized) and from 35 NHL patients harvested after cytokine administration (mobilized) were studied. The harvests were cultured and molecularly evaluated for clonal markers of the primary lymphoma. All patients underwent high-dose chemotherapy and autologous transplantation. Graft products from mobilized patients were more likely to contain lymphoma than graft products from non-mobilized patients (37% vs 19%) but this difference was not significant (P = 0.16). In a multivariate analysis, lymphoma contamination was not associated with patient age, gender, tumor grade, prior radiotherapy, duration of prior chemotherapy, mononuclear cell count, or the number of aphereses performed to obtain the product. Heavily pre-treated patients were less likely to have lymphoma-contaminated harvests (P = 0.064). Lymphoma contamination was positively associated with the number of progenitor cells collected (P = 0.047). In multivariate analyses, the only significant independent predictor of lymphoma contamination was the number of mononuclear cells collected (P = 0.031). Thrombocytopenia following myelotoxic therapy is a common problem and when severe (<20 000/μl) can lead to severe morbidity and mortality. Thrombopoietin (TPO) is a naturally occurring glycosylated peptide which stimulates the differentiation of bone marrow stem cells into megakaryocyte progenitor cells, induces the expression of megakaryocyte differentiation markers, promotes megakaryocyte proliferation, polyploidization and, ultimately, the formation of increased numbers of platelets in the circulation. TPO has now been produced by recombinant technology and has entered clinical trials. This open label phase I study was designed to determine the safety, tolerance and pharmacokinetics of recombinant thrombopoietin (rhTPO) when administered to patients after undergoing high-dose chemotherapy followed by autologous bone marrow transplantation. rhTPO was administered intravenously by bolus injection at doses ranging from 0.3 to 4.8 μg/kg/day every 3 days to 30 patients and 0.6 μg/kg daily to three patients. rhTPO was begun the day after marrow infusion and continued until platelet recovery to <20 000/μl. G-CSF was concomitantly administered to promote myeloid recovery. Serious adverse events or neutralizing antibodies to rhTPO were not observed during the study. Median platelet recovery after ABMT was 19 days (range, 11–41). Neither the dose nor the schedule of rhTPO appeared to have any impact upon the time course of platelet recovery. In this phase I study, rhTPO was found to be well tolerated without the development of neutralizing antibodies and without compromising neutrophil recovery. The influence of substrate concentration on plasmid transfer frequency in the rhizosphere was investigated using a physical model employing a hollow fiber membrane instead of a real root. The absolute number of transconjugants produced increased with increasing substrate (glucose) concentration, but the plasmid transfer frequency decreased exponentially with increasing substrate concentration from 4.4 × 10-3 at 90 μg ml-1 glucose to 1.35 × 10-5 at 3600 μg ml-1 glucose. These results were found to be heavily dependant on donor to recipient ratio and distribution of strains, but independent of initial donor and recipient inoculum density and regime. These observations also show that plasmid transfer frequency is reduced at high substrate concentrations, which suggests that plasmid transfer is either stimulated when growth limiting nutrient is scarce or inhibited when it is abundant. systemic fungal infections are a major problem in bone marrow transplant recipients who have prolonged neutropenia or who receive high-dose corticosteroids. prophylaxis with fluconazole or low-dose amphotericin b reduces, but does not eliminate these infections. to determine which prophylactic agent is better, we performed a prospective randomized study. patients undergoing allogeneic (related or unrelated) or autologous marrow or peripheral stem cell transplantation were randomized to receive fluconazole (400 mg/day p.o. or i.v.) or amphotericin b (0.2 mg/kg/day i.v.) beginning 1 day prior to stem cell transplantation and continuing until recovery of neutrophils to >500/μl. patients were removed from their study drug for drug-associated toxicity, invasive fungal infection or suspected fungal infection (defined as the presence of fever >38°C without positive culture while on broad-spectrum anti-bacterial antibiotics). Proven or suspected fungal infections were treated with high-dose amphotericin B (0.5–0.7 mg/kg/day). Patients were randomized at each institution and stratified for the type of transplant. The primary end-point of the study was prevention of documented fungal infection; secondary endpoints included fungal colonization, drug toxicity, duration of hospitalization, duration of fever, duration of neutropenia, duration and total dose of high-dose amphotericin B and overall survival to hospital discharge. From July 1992 to October 1994, a total of 355 patients entered into the trial with 159 patients randomized to amphotericin B and 196 to Fluconazole. Patient groups were comparable for diagnosis, age, sex, prior antibiotic or antifungal therapy, use of corticosteroids prior to transplantation and total duration of neutropenia. Development of CNS dysfunction in the setting of hematopoietic stem cell transplant (HSCT) has been previously shown to predict for subsequent second organ dysfunction and death. In this paper, we describe the characteristics of this isolated CNS dysfunction, and its relationship to multiple organ dysfunction syndrome (MODS) after HSCT. Twenty-one of 186 patients undergoing HSCT developed CNS dysfunction as their first organ dysfunction a mean of 22.8 ± 0.9 days after the start of the preparative regimen. Compared with 137 patients who developed no organ dysfunction, patients presenting with CNS dysfunction were more likely to have undergone allogeneic HSCT (P = 0.001) and to have received a total body irradiation-based regimen (P = 0.001), and were less likely to have been transplanted for lymphoma (P = 0.008). Patients who developed CNS dysfunction were more likely to die than those with no organ dysfunction (P < 0.001). of the 21 patients who developed cns dysfunction, 48% resolved their dysfunction by a mean of 4.6 days later without progression to second organ dysfunction, and 90% of these patients survived to day 100. fifty-two percent of patients with cns dysfunction progressed to second organ dysfunction (pulmonary or hepatic) a mean of 5.5 days later, and only 36% survived to day 100 (P = 0.02). In this retrospective study, we evaluated the predictability of PBSC dose for hematopoietic engraftment comparing that calculated by ideal body weight (IBW) vs another calculated by actual body weight (ABW) for each patient. Sixty-three consecutive patients treated similarly using one transplant protocol were analyzed. While all patients had data available on CFU-GM and nucleated cells (NC), data on CD34+ enumeration was present only in 34 patients. We found that 49% of the patients were greater than 25% over their IBW. In addition, least-squares linear regression was used to assess the strength of the linear relationship between the inverse of cell dose/kg of ABW or IBW and time to AGC or platelet engraftment and showed no difference in r2 values for platelet engraftment, while using dose/kg of IBW greatly improved the ability of NC (r2 improved from 0.19 for ABW to 0.35 for IBW) and CFU-GM (r2 improved from 0.35 for ABW to 0.53 for IBW) to predict time to AGC engraftment, but did not change the CD34 r2. Hazard ratios were estimated using Cox proportional hazards regression and in all instances were found greater than 1.0 indicating that the probability of engraftment increased as cell dose/kg ABW or IBW increased. Finally, our data showed that 10 patients (16%) could have had one less apheresis procedure performed to obtain their set target stem cell dose calculated per kg IBW rather than ABW. The objective of this study was to evaluate the efficacy of transactional psychophysiological therapy (TPT) in a patient with recurrent vasovagal syncope (VVS) and to quantify the capacity of human dialogue to effect significant and consistent measurable therapeutic cardiovascular (CV) changes. A 31-year-old nurse with recurrent VVS and a reproducibly abnormal tilt-table test was refractory to pharmacological and conventional psychiatric treatments. She was treated with TPT. Her CV responses during psychotherapy were incorporated into the dialogue as an important source of communicative information, and she was taught psychophysiological techniques to correct exaggerated CV responses. These responses, during 16 weekly and 12 subsequent monthly sessions, were analysed using a one-way multiple analysis of variance. As TPT progressed, the magnitude and lability of CV responses as well as frequency of VVS were reduced. She has been relatively asymptomatic for 14 years post-therapy. In conclusion, (1) TPT may be an effective primary/adjunctive treatment for patients with VVS; (2) TPT may reduce syncopal episodes, perhaps by normalizing limbic input to the brainstem baroreflex system. Heart rates, respiratory rates, body temperatures, ad libitum 'day' and 'night' food consumption and body weight changes have been examined in 15 mature Merino wethers shorn in moderate environmental conditions. All sheep showed a depression in food consumption for two days after shearing. Sheep that gained weight during the next three weeks then increased their food consumption at night by approximately 30% although the average daily consumption was only increased by 5%. Sheep that lost weight showed a depressed food consumption throughout the three week period after shearing. Marked increases in the temperature difference between ear skin and air as well as thermal tachypnoea during the warmest period of the day were recorded in all sheep 14-16 days after shearing. This indicated that the critical temperature for all sheep had decreased by about 10°C. These signs of acclimatisation appeared at similar times in all sheep, suggesting that increased resistance to body cooling developed at similar rates in weight gain and weight loss sheep and independent of the origin of body heat production. The results of the investigations are discussed in relation to the concept that the initial response to cold stress includes a depression in food intake and that the duration of this depression is a function of the cold stimulus and the strain it induces in the sheep. Pseudomonas fluorescens F113, which produces the antimicrobial compound 2,4-diacetylphloroglucinol, is a prospective biocontrol agent. Soil enzyme activities were used to investigate the ecological impact of strain F113 in the rhizosphere of field-grown sugar beet. There were distinct trends in rhizosphere enzyme activities in relation to soil chemistry [determined by electro-ultrafiltration (EUF)]. The activities of enzymes from the P cycle (acid phosphatase, alkaline phosphatase and phosphodiesterase) and of arylsulphatase were negatively correlated with the amount of readily available P, whereas urease activity was positively correlated with the latter. Significant correlations between nutrient levels determined by EUF and enzyme activities in the rhizosphere were obtained, highlighting the usefulness of enzyme assays to document variations in soil nutrient cycling. Contrary to previous microcosm studies, which did not investigate plants grown to maturity, the biocontrol inoculant had no effect on enzyme activities or on soil chemistry in the rhizosphere. The results showed the importance of using homogenous soil microcosm systems, also employed in previous work, for risk assessment studies, whereby the effects of inherent soil variability were minimised, and effects of the pseudomonad on soil enzymology could be detected. Vine branches, rice husks and flax residues were composted. The dynamics of biomass carbon, C/N ratios and nitrification were studied. The highest quality level and the most stabilized composts with the highest values of total N (1.3-1.6%) and the smallest C/N ratios (8.0-9.0) were found with the vine branch composts. Compost application significantly increased the yield of tomatoes (24.0-61.1%) and the quality of fruits compared to soil treated with mineral fertilizers and manure. Inoculation of the vine branch compost with a Cephalosporium sp. had a positive effect on the yield and the quality of both ryegrass and tomatoes. The stabilization of the microbial biomass C level in the composts coincided with the beginning of intensive nitrification. Inoculation lowered the conductivity values and nitrate contents in all composts. It appeared that when nitrate N concentrations in the composts were more than 5% of the total N, NO accumulation in the fruits could result. Damping-off of lettuce ( Lactuca sativa) caused by Pythium ultimum was studied in pots containing a non-sterile potting mix in the glasshouse. Fifty P. ultimum sporangia g compost reduced the plant stand to 15% and shoot dry weight to 18%, but this reduction was totally prevented by applying Trichoderma harzianum at 2×10 viable propagules g potting mix. Gliocladium virens and Enterobacter cloacae also alleviated the damping-off. E. cloacae did not adversely affect the action of the fungal antagonists.
