We propose two novel methods for computing the superconformal index of 5d superconformal field theories that cannot be described by conventional Lagrangian descriptions under mass deformations. The first approach involves the use of Higgs branch flows from UV Lagrangian theories, guided by transitions in 5-brane webs in Type IIB string theory. The second method employs the relationship between O7+-plane and O7−-plane with eight D7-branes, which applies to particular non-Lagrangian theories realized by brane configurations involving an O7+-plane. As a concrete application of our method, we compute the superconformal indices for all known rank-1 non-Lagrangian theories, which we also use to identify flavor symmetries and their global forms at the conformal field theory (CFT) fixed points. We propose blowup equations for 6d little string theories which generalize Nakajima-Yoshioka's blowup equations for the 4d/5d instanton partition functions on Omega background. We find that unlike the blowup equations for standard SQFTs, we need to sum over auxiliary magnetic fluxes on the blown-up ℙ1 for a non-dynamical 2-form gauge field which plays a role in canceling the mixed anomalies of the gauge symmetries. We demonstrate with explicit examples that the blowup equations, when combined with the modular properties, can be solved in order to determine the elliptic genera of little strings. ℤn S-folds of 5d SCFTs, including TN, which lead to brane webs with E6,7,8 7-branes were discussed recently in [1, 2]. We generalize the construction to ‘fractional quotients’, which are based on ℤn actions linking multiple copies of the seed theory and lead to H0,1,2 7-branes. We provide the holographic duals for both classes. This expands the space of explicitly known Type IIB AdS6 solutions by incorporating F-theory 7-branes of type E6,7,8 and H0,1,2, extending previous constructions for O7-planes. We study the IR phases of 3D class R theories associated with closed non-hyperbolic 3-manifolds. Non-hyperbolic 3-manifolds can be obtained by performing Dehn fillings on 1-cusped hyperbolic 3-manifolds along exceptional slopes. In 3D-3D correspondence, the ‘exceptional’ Dehn filling corresponds to the gauging of an SU(2) flavor symmetry in a superconformal field theory associated with a 1-cusped 3-manifold with ‘small’ Chern-Simons levels. With several explicit examples, we analyze various interesting non-perturbative IR phenomena (such as spontaneous SUSY breaking, generation of mass gap and supersymmetry enhancement) from the ‘exceptional’ gaugings. Interestingly, distinguished features of the IR phases can be captured by simple topological properties of non-hyperbolic 3-manifolds. We also find that 3D class R theories associated with certain classes of atoroidal non-hyperbolic 3-manifolds always exhibit supersymmetry enhancement at low energy and actually flow to 3D rank-0 N = 4 SCFTs with trivial vacuum moduli space. Continuous wound infusion with local anesthesia is an effective method for reducing postoperative pain after laparoscopic colorectal surgery. However, most subcutaneous local anesthesia is delivered through continuous injection, which can be inconvenient for patients. This study compared the effectiveness of postoperative pain relief from the application of a local poloxamer 407-based ropivacaine hydrogel (Gel) to the incision site with continuous infusion-type ropivacaine administration (On-Q) in patients undergoing laparoscopic colorectal surgery. This prospective, randomized, non-inferiority study included 61 patients who underwent laparoscopic colorectal surgery with an incision length of 3–6 cm. All 61 patients were randomly assigned to the Gel group (poloxamer 407-based 0.75% ropivacaine, 22.5 mg) or the On-Q group (0.2% ropivacaine, 4 mg/hour for two days). Postoperative analgesia was induced in all patients with intravenous patient-controlled analgesia (IV-PCA). The outcome measures, which were assessed for 72 h after surgery, included the total amount of fentanyl consumed via IV-PCA (primary endpoint), and the amount of rescue analgesia (pethidine) and postoperative pain intensity assessed using a numeric rating scale (NRS) [secondary endpoints]. The Gel was administered to 31 patients and On-Q was used for 30 patients. There was no significant difference in the total usage of fentanyl between the two groups (Gel group, 1623.98 mcg; On-Q group, 1595.12 mcg; P = 0.806). There was also no significant difference in the frequency of analgesic rescue medication use (P = 0.213) or NRS scores (postoperative 6 h, P = 0.860; 24 h, P = 0.333; 48 h, P = 0.168; and 72 h, P = 0.655) between the two groups. The Gel, which continuously delivers a local anesthetic to operative sites, can thus be considered an effective device for analgesia and pain relief for midline incisions in laparoscopic colorectal surgery. Background: The aim of this study is to determine the optimal indications for preoperative pelvic radiotherapy (RT) in patients with metastatic rectal cancer who underwent curative-intent surgical resection and/or ablation. Methods: Between January 2000 and October 2019, 246 patients who met our inclusion criteria were enrolled. Preoperative RT was performed in 22 patients (8.9%). Lower margin below the peritoneal reflection (p < 0.001), mesorectal fascia (MRF) invasion (p = 0.02), and lateral pelvic lymph node (LPLN) involvement (p = 0.005) were more frequent in the preoperative RT group. Results: During the median follow-up period of 13.3 months (interquartile range [IQR]: 6.0–36.3 months), local recurrence (LR) was identified in 60 patients (24.4%). It was the first site of recurrence in 45 of them (18.3%). Among them, three patients were in the preoperative RT group. On multivariable analysis, lower margin below the peritoneal reflection, MRF invasion, LPLN involvement, carcinoembryonic antigen (CEA) level ≥ 10 ng/mL before treatment, and preoperative RT were significant prognostic factors for LR-free survival (LRFS). In the patient group without any risk factors, the 2-year LRFS rate was 94.9% without preoperative RT. In the patient group with one or more risk factors, the 2-year LRFS was 64.4% without and 95.2% with preoperative RT. Conclusion: LR developed in about 25% of patients within 2 years. Preoperative RT should be considered, especially in patients with a risk factor for LR, including lower margin below the peritoneal reflection, MRF invasion, LPLN involvement, or CEA ≥ 10 ng/mL before treatment. Although stromal fibroblasts play a critical role in cancer progression, their identities remain unclear as they exhibit high heterogeneity and plasticity. Here, a master transcription factor (mTF) constructing core-regulatory circuitry, PRRX1, which determines the fibroblast lineage with a myofibroblastic phenotype, is identified for the fibroblast subgroup. PRRX1 orchestrates the functional drift of fibroblasts into myofibroblastic phenotype via TGF-β signaling by remodeling a super-enhancer landscape. Such reprogrammed fibroblasts have myofibroblastic functions resulting in markedly enhanced tumorigenicity and aggressiveness of cancer. PRRX1 expression in cancer-associated fibroblast (CAF) has an unfavorable prognosis in multiple cancer types. Fibroblast-specific PRRX1 depletion induces long-term and sustained complete remission of chemotherapy-resistant cancer in genetically engineered mice models. This study reveals CAF subpopulations based on super-enhancer profiles including PRRX1. Therefore, mTFs, including PRRX1, provide another opportunity for establishing a hierarchical classification system of fibroblasts and cancer treatment by targeting fibroblasts. Cancer associated fibroblasts are an important and highly heterogeneous component of the tumor microenvironment. Patients with pathologic complete response (pCR) achievement can consider local excision or "watch and wait" strategy instead of a radical surgery. This study analyzed the predictive factors of pCR in rectal cancer patients who underwent radical operation after neoadjuvant chemoradiotherapy (nCRT). This study also analyzed the recurrence patterns in patients who achieved pCR and the oncologic outcomes and prognostic factors by ypStage. Between 2000 and 2013, 1,089 consecutive rectal cancer patients who underwent radical resection after nCRT were analyzed. These patients were classified into two groups according to pCR. The clinicopathologic and oncologic outcomes were analyzed and compared between the two groups. Multivariate analysis was conducted on factors related to pCR. The proportion of patients achieving pCR was 18.2% (n = 198). The pCR group demonstrated earlier clinical T and N stages, smaller tumor size, better differentiation, and a lower percentage of circumferential resection margin (CRM) involvement than did the non-pCR group. The prognostic factors associated with poorer disease-free survival were high preoperative carcinoembryonic antigen levels, non-pCR, poor histology, lymphatic/perineural invasion, and involvement of CRM. Multivariate analysis revealed that clinical node negativity, tumor size < 4 cm, and well differentiation were significant independent clinical predictors for achieving pCR. Patients with pCR displayed better long-term outcomes than those with non-pCR. The pCR-prediction model, based on predictive factors, is potentially useful for prognosis and for prescribing a treatment strategy in patients with advanced rectal cancer who need nCRT. We explore the ℤ2,3,4,6 S-foldings of some 5d superconformal field theories from the (p, q) 5-brane web perspective. The S-folding involves both a spatial quotient and an SL(2, ℤ) transformation on 5-branes simultaneously. The ℤ2,3,4,6 S-foldings are achieved by the insertion of the D4, E6, E7, E8 7-branes, respectively. The deficit angles and monodromies of these 7-branes are exactly those necessary for the S-foldings. We explore the details of the S-folding process, especially the enhancement of global flavor symmetry in various simple cases. The characteristic of the S-folding depends sharply on whether the fixed point of the discrete symmetry is at the center of a compact face (or surface), at a 5-brane, or at a crossing point of 5 branes. Background: Sphincter-saving operation is the treatment of choice in patients with lower rectal cancer, although abdominoperineal resection (APR) is necessary in some cases for adequate oncological outcomes. This study compared the oncologic outcomes of patients with low rectal cancer undergoing APR and intersphincteric resection (ISR) after neoadjuvant chemoradiotherapy (nCRT). Methods: We compared 104 patients who underwent ISR for lower rectal cancer between 2008 and 2014 with 79 patients who underwent APR during the same period. Cases in which tumors involved the levator ani muscle or the external anal sphincter, as well as those in which surgeons were unable to obtain a negative distal margin via sphincter-saving operation were excluded. Fifty-two patients who underwent ISR were matched via propensity scoring with 52 patients who underwent APR. The analyzed variables included patient and tumor characteristics and long-term outcomes before and after 1:1 propensity score matching (PSM). Results: After matching, there were no significant differences between the two groups in clinical stage and tumor distance from anal verge. Short- and long-term outcomes were similar between the two groups. The 5-year disease-free survival rate was 76.3% in the ISR group and 57.8% in the APR group (p = 0.080). The 5-year cancer-specific survival rate was 89.5% in the ISR group and 85.4% in the APR group (p = 0.383). We discuss supersymmetric defects in 6d N = (1, 0) SCFTs with SO(Nc) gauge group and Nc− 8 fundamental flavors. The codimension 2 and 4 defects are engineered by coupling the 6d gauge fields to charged free fields in four and two dimensions, respectively. We find that the partition function in the presence of the codimension 2 defect on ℝ4× 핋2 in the Nekrasov-Shatashvili limit satisfies an elliptic difference equation which quantizes the Seiberg-Witten curve of the 6d theory. The expectation value of the codimension 4 defect appearing in the difference equation is an even (under reflection) degree Nc section over the elliptic curve when Nc is even, and an odd section when Nc is odd. We also find that RG-flows of the defects and the associated difference equations in the 6d SO(2N + 1) gauge theories triggered by Higgs VEVs of KK-momentum states provide quantum Seiberg-Witten curves for ℤ2 twisted compactifications of the 6d SO(2N) gauge theories. Background: To improve the quality of surgery for rectal cancer, both transanal total mesorectal excision (taTME) and robotic total mesorectal excision (R-TME) can be performed. However, few studies have compared outcomes of taTME and R-TME, especially for patients with low rectal cancer after undergoing neoadjuvant chemoradiation (nCRT). Thus, the objective of this study was to compare outcomes of taTME and R-TME for patients with low rectal cancer after undergoing nCRT. Methods: A total of 306 consecutive patients with low rectal cancer who underwent taTME or R-TME after nCRT between 2008 and 2018 were analyzed retrospectively. Patients were classified into two groups: 1) taTME surgery group (n = 94); and 2) R-TME surgery group (n = 212). Results: Clinicopathologic variables were comparable between the two groups. There was no significant difference in circumference margin involvement (1.1% in taTME vs. 2.8% in R-TME, p = 0.680) or distal resection margin (2.3 cm in taTME vs. 2.4 cm in R-TME, p = 0.629). Total operation time (239 min in taTME vs. 243 min in R-TME, p = 0.675) and major complications (including anastomosis site leakage, surgical site infection, and voiding difficulty) showed no significant difference between the two groups either. Conclusions: Transanal and robotic TMEs have similar short-term outcomes for patients with rectal cancer after undergoing nCRT. Background: Tumor budding is associated with adverse histology. It is a predictor of poor oncologic outcomes in colorectal cancer. However, it remains unclear whether tumor budding is a predictor of poor prognosis for rectal cancer patients regardless of neoadjuvant chemoradiotherapy (nCRT). Patients and Methods: This study analyzed 2888 rectal cancer patients who underwent radical surgery from 2007 to 2014. Among these patients, 939 underwent nCRT while 1949 did not receive nCRT. Tumor budding was defined as positive if the number of isolated tumor cells or small clusters of up to five tumor cells at the invasive front of the tumor was five or more. If the number was less than five, it was defined as negative. Patients were categorized according to tumor budding status. We used 1:1 propensity score matching to adjust for potential baseline confounders between the two groups. Results: Among 2888 patients, 939 received nCRT while 1949 did not receive nCRT. A total of 418 patients who received nCRT were matched (209 in each group). A total of 1024 patients without nCRT were also matched (512 in each group). In matched patients, 5-year overall survival (OS) and 5-year disease-free survival (DFS) rates for the positive budding group were significantly lower than those in the negative budding group regardless of nCRT. On multivariate analysis of prognostic factors, positive budding was associated with poorer disease-free survival independent of nCRT. We generalize Nakajima-Yoshioka's blowup formula to calculate the partition functions counting the spectrum of bound states to half-BPS Wilson loop operators in 5d (and 6d) supersymmetric field theories. The partition function in the presence of a Wilson loop operator on the Ω-background is factorized when put on the blowup ℂ ̂ 2 into two Wilson loop partition functions under the localization. This structure provides a set of blowup equations for Wilson loop operators. We explain how to formulate the blowup equations and solve them to compute the partition functions of Wilson loop operators. We test this idea by explicitly calculating the Wilson loop partition functions in various 5d/6d field theories and comparing them against known results and expected dualities. We construct novel web diagrams with a trivalent or quadrivalent gluing for various 6d/5d theories from certain Higgsings of 6d conformal matter theories on a circle. The theories realized on the web diagrams include 5d Kaluza-Klein theories from circle compactifications of the 6d G2 gauge theory with 4 flavors, the 6d F4 gauge theory with 3 flavors, the 6d E6 gauge theory with 4 flavors and the 6d E7 gauge theory with 3 flavors. The Higgsings also give rise to 5d Kaluza-Klein theories from twisted compactifications of 6d theories including the 5d pure SU(3) gauge theory with the Chern-Simons level 9 and the 5d pure SU(4) gauge theory with the Chern-Simons level 8. Intersphincteric resection (ISR) is a surgical technique intended to avoid abdominoperineal resection (APR) in patients diagnosed with low-lying rectal cancer. However, the oncologic outcomes of minimally invasive ISR are still controversial. We analyzed the long-term oncologic outcomes of open and minimally invasive ISR. A total of 313 rectal cancer patients who underwent ISR between 2000 and 2014 were analyzed, including 147 in the open surgery group and 166 in the minimally invasive surgery (MIS) group. This study also analyzed 113 patients who received neoadjuvant chemoradiotherapy (nCRT) for advanced lower rectal cancer. Propensity score matching (PSM) was used to correct for differences between the two groups. 5-year disease-free survival (DFS) rate was the primary end point. The length of hospital stay was significantly shorter in the MIS group (9.6 vs. 11.8 days, p < 0.001). Differences in overall postoperative morbidity rates between the groups were not significant; however, the rate of surgical site infection was significantly lower in the MIS group (1.2 vs. 10.9%, p < 0.001). The 5-year DFS associated with all stages combined in the matched patients were not significantly different: 75.2% in the open group vs. 64.2% in the MIS group (p = 0.214). Similar results were found in matched patients treated with nCRT, with 72.0% in the open group and 61.3% in the MIS group (p = 0.078) showing DFS. Both minimally-invasive and open ISR for rectal cancer yielded similar 5-year oncologic outcomes. MIS showed statistically significant advantages in some postoperative outcomes such as reduced surgical site infection and shorter hospital stay, and similar long-term outcomes compared with open ISR. We propose a systematic approach to computing the BPS spectrum of any 5d/6d supersymmetric quantum field theory in Coulomb phases, which admits either gauge theory descriptions or geometric descriptions, based on the Nakajima-Yoshioka's blowup equations. We provide a significant generalization of the blowup equation approach in terms of both properly quantized magnetic fluxes on the blowup ℂ ̂ 2 and the effective prepotential for 5d/6d field theories on the Omega background which is uniquely determined by the Chern-Simons couplings on their Coulomb branches. We employ our method to compute BPS spectra of all rank-1 and rank-2 5d Kaluza-Klein (KK) theories descending from 6d N = (1, 0) superconformal field theories (SCFTs) compactified on a circle with/without twist. We also discuss various 5d SCFTs and KK theories of higher ranks, which include a few exotic cases such as new rank-1 and rank-2 5d SCFTs engineered with frozen singularity as well as the 5d SU(3)8 gauge theory currently having neither a brane web nor a smooth shrinkable geometric description. The results serve as non-trivial checks for a large class of non-trivial dualities among 5d theories and also as independent evidences for the existence of certain exotic theories. We compute the partition function for 6d N = 1 SO(2N) gauge theories compactified on a circle with ℤ2 outer automorphism twist. We perform the computation based on 5-brane webs with two O5-planes using topological vertex with two O5-planes. As representative examples, we consider 6d SO(8) and SU(3) gauge theories with ℤ2 twist. Quantum curves arise from Seiberg-Witten curves associated to 4d N = 2 gauge theories by promoting coordinates to non-commutative operators. In this way the algebraic equation of the curve is interpreted as an operator equation where a Hamiltonian acts on a wave-function with zero eigenvalue. We find that this structure generalises when one considers torus-compactified 6d N = (1, 0) SCFTs. The corresponding quantum curves are elliptic in nature and hence the associated eigenvectors/eigenvalues can be expressed in terms of Jacobi forms. In this paper we focus on the class of 6d SCFTs arising from M5 branes transverse to a ℂ2/ℤk singularity. In the limit where the compactified 2-torus has zero size, the corresponding 4d N = 2 theories are known as class S k . We explicitly show that the eigenvectors associated to the quantum curve are expectation values of codimension 2 surface operators, while the corresponding eigenvalues are codimension 4 Wilson surface expectation values. We study 6d superconformal field theories (SCFTs) compactified on a circle with arbitrary twists. The theories obtained after compactification, often referred to as 5d Kaluza-Klein (KK) theories, can be viewed as starting points for RG flows to 5d SCFTs. According to a conjecture, all 5d SCFTs can be obtained in this fashion. We compute the Coulomb branch prepotential for all 5d KK theories obtainable in this manner and associate to these theories a smooth local genus one fibered Calabi-Yau threefold in which is encoded information about all possible RG flows to 5d SCFTs. These Calabi-Yau threefolds provide hitherto unknown M-theory duals of F-theory configurations compactified on a circle with twists. For certain exceptional KK theories that do not admit a standard geometric description we propose an algebraic description that appears to retain the properties of the local Calabi-Yau threefolds necessary to determine RG flows to 5d SCFTs, along with other relevant physical data. Purpose: This study aimed to identify the risk factors for locoregional recurrence (LR) and determine possible candidates for postoperative concurrent chemoradiotherapy (CCRT) in pathologic T3N0 (pT3N0) rectal cancer patients with a negative resection margin after total mesorectal excision (TME). Method: Data from 365 patients who had pT3N0 rectal cancer between 2003 and 2012 in the Samsung Medical Center were reviewed. All patients underwent upfront surgery without preoperative treatment. Postoperative management involved either no adjuvant therapy (n = 122), chemotherapy alone (n = 100), or CCRT (n = 143). Results: The median follow-up duration was 71 months. The 5-year overall survival, disease-free survival, and LR-free survival (LRFS) rates were 95.9%, 86.9%, and 96.3%, respectively. When comparing the three groups (surgery alone [n = 122], chemotherapy alone [n = 100], and CCRT [n = 143]), there was no significant difference in LRFS among them (94.0%, 93.4%, and 99.2%, respectively; p = 0.20). However, when patients were stratified by risk factors (distance from anal verge ≤ 5 cm and distal resection margin [DRM] ≤ 2 cm), the 5-year LRFS improved by more than 10% by adding CCRT (98.9% with CCRT vs. 87.4% without CCRT, p = 0.006) in those with more than one risk factor. Postoperative CCRT did not affect the 5-year LRFS (100% with CCRT vs. 99.0% without CCRT, p = 0.66) in patients with no risk factors. Conclusion: Postoperative CCRT significantly decreased LR in patients with pT3N0 rectal cancer with a negative resection margin but having a distance from the anal verge ≤ 5 cm or DRM ≤ 2 cm. Background: Laparoscopic liver resection for metastatic colorectal cancer (CRC) remains controversial. The objective of this study was to compare the short-term and mid-term outcomes of patients with CRC undergoing laparoscopic versus open colorectal resection with simultaneous resection for liver metastases.Methods: A total of 126 patients underwent combined laparoscopic resection of CRC and synchronous colorectal liver metastases between 2008 and 2016. A total of 318 patients were treated by an open approach during the above period. By propensity score matching, 109 patients who underwent laparoscopic resection and 109 patients who had an open approach were compared. Analyzed variables included patient characteristics, tumor features, and short-term and mid-term outcomes.Results: Demographic features and pathologic outcomes were similar in both groups after propensity score matching. Three (2.8%) patients undergoing laparoscopic liver resection experienced conversion to open procedure. There was no difference in hospital stay (p = 0.078), transfusion rate (p = 0.686), or time of bowel function return (p = 0.570) between the two groups. The laparoscopic group and the open approach group also showed similar 3-year overall survival rate (74.4% vs. 79.1%; p = 0.792) and 3-year disease-free survival rate (58.5% vs. 55.2%; p = 0.391). However, postoperative morbidity rate was significantly lower in the laparoscopic group (20.2% vs. 33.0%; p = 0.032).Conclusions: Laparoscopic colorectal resection with simultaneous resection of liver metastases showed satisfactory oncologic outcomes with some short-term advantages compared to the open approach. We present a novel M-theoretic approach of constructing and classifying anyonic topological phases of matter, by establishing a correspondence between (2+1)d topological field theories and non-hyperbolic 3-manifolds. In this construction, the topological phases emerge as macroscopic world-volume theories of M5-branes wrapped around certain types of non-hyperbolic 3-manifolds. We devise a systematic algorithm for identifying the emergent topological phases from topological data of the internal wrapped 3-manifolds. Remarkably, our construction is not restricted to an unitary bosonic theory but it can also generate fermionic and/or non-unitary anyon models in an equivalent fashion. Background: The aim of the study was to determine factors predicting lymph node metastasis in patients with T1 or T2 colon cancer. Methods: A total of 906 patients with T1 or T2 colon cancer who underwent colon resection with regional lymphadenectomy in a tertiary hospital, from January 2008 to December 2013, were analyzed. The prognostic factors for LN metastasis and the risk factors for survival were analyzed. Results: There were 728 patients (80.4%) without lymph node metastasis (LN-negative group) and 178 patients (19.6%) with lymph node metastasis (LN-positive group). Tumor invasion depth (P < 0.001), lymphatic invasion (P < 0.001), and perineural invasion (P = 0.008) were significantly different between the two groups. During the median follow-up period of 69 months, the 5-year disease-free survival rate was 98.6% for the LN-negative group and 92.8% for the LN-positive group (P ≤ 0.001). In multivariate analysis, influencing factors associated with disease-free survival rate were LN metastasis (P = 0.001) and perineural invasion (P = 0.040). Female, depth of tumor invasion (P = 0.001), and lymphatic invasion (P < 0.001) were significant independent predictive factors for lymph node metastasis in multivariate analysis. Conclusion: Positive LN status predicted poor disease-free survival in patients with early cancer. This suggests that depth of tumor invasion ≥ sm2 and the presence of lymphatic invasion in early colon cancer provide useful information to determine which patients would benefit from radical surgery. Purpose: To date, there have been many studies on postoperative complications and oncologic outcomes, but the findings remain controversial. The purpose of the study was to determine the prognostic value of serum inflammatory markers as pretreatment in curatively resected colorectal cancer. The effects of serum inflammatory marker levels on postoperative morbidity were also examined. Methods: Prospectively collected data from 1590 patients with primary sporadic colorectal adenocarcinoma who underwent curative resection between September 2012 and December 2013 at our institution were retrospectively analyzed. White blood cell count (× 103/mcl), neutrophil/lymphocyte ratio, C-reactive protein, erythrocyte sedimentation rate (ESR), and Glasgow prognostic score (GPS) were evaluated. GPS was determined as follows: GPS of 2, elevated CRP level (> 1.0 mg/dl) and hypoalbuminemia (< 3.5 g/dl); GPS of 1, elevated CRP or hypoalbuminemia; and GPS of 0, neither elevated CRP nor hypoalbuminemia. Results: The median follow-up period for the 1590 patients was 59 months. The overall postoperative morbidity rate was 24.8%. In multivariate analysis, female sex (P = 0.006), rectal cancer other than colon cancer (P < 0.001), CEA level > 5 ng/ml (P < 0.001), ESR > 27 mm/h (P = 0.014), and a GPS of 1 or 2 (P < 0.001) were independent risk predictors for morbidity. A GPS of 1 or 2 (P < 0.001) and postoperative morbidity (P < 0.001) significantly affected both overall survival and disease-free survival in multivariate analyses. Background: The diagnostic impact of lymphovascular invasion (LVI), perineural invasion (PNI), and tumor budding in stage I colon cancer is currently unknown. This study was conducted to evaluate the prognostic impact of LVI, PNI, and tumor budding in stage I colon cancer. Methods: From January 2008 to December 2013, 720 patients who underwent curative surgery and were diagnosed with stage I colon cancer were reviewed retrospectively. These patients were categorized into two groups based on LVI, PNI, and tumor budding: the no risk group (n = 566) and risk group (n = 154). Results: Median follow-up period was 103.5 months, and the 5-year disease-free survival rate of the risk group was significantly lower than that of the no risk group (p = 0.025). In multivariate analysis, only the risk group had prognostic factors for 5-year disease-free survival (p = 0.036). In addition, only differentiation was an independent predictor in the risk group (p = 0.009). Conclusion: LVI, PNI, and tumor budding are strong prognostic factors for stage I colon cancer. The phases of the non- supersymmetric theory can be characterized by Chern-Simons terms involving background U(1) gauge fields, allowing us to identify a phase transition at strong coupling. Purpose: The goal of this study was to evaluate the long-term oncologic outcomes after laparoscopic converted surgery for patients with colon cancer. Methods: Retrospective database of consecutive curative-intent laparoscopic-assisted surgery for primary stage I–III colon cancer was reviewed from 2000 to 2013. The patients were divided into non-conversion and conversion groups. The patient characters, operative features, perioperative parameters, pathologic features, and oncologic outcomes were compared. Results: A total of 4010 patients were included in the study: 3929 in the non-conversion group and 81 (2%) in the conversion group. The median follow-up period was 63.9 months. There were significant differences in age, preoperative clinical T-stage, and tumor size between the groups. In operative details between the two groups, there were also significant differences in access to surgery, tumor location, cancer obstruction, cancer perforation, and estimated blood loss (P < 0.001). The two most common reasons for conversion were adhesion (n = 37, 46%) and bleeding (n = 21, 26%). Multivariate analysis showed that conversion was an independent predictor of both overall survival (OS) (P < 0.001) and disease-free survival (P = 0.003). The 5-year OS rate of the conversion group was 79.6%, and that of the non-conversion group was 96.2% (P < 0.001). The multivariate predictors of conversion were age, type of surgery, cancer obstruction, cancer perforation, and clinical T-stage. Conclusion: Conversion to open surgery may affect patient survival and recurrence after laparoscopic-assisted surgery for colon cancer. Background: This study evaluated the oncological outcome of surgical site infection (SSI) after colorectal cancer surgery.Methods: A total of 3675 consecutive patients with colorectal cancer who underwent curative resection from January 2009 to December 2011 were analyzed. The prognostic significance of SSI was evaluated. Risk factors for SSI were also identified using multivariate regression analysis.Results: Overall SSI rate was 9.6%, in which 5.5% was superficial or deep SSI and 4.1% was organ/space SSI. Incidence of SSI varied significantly with tumor location (P < 0.001): 7.1% in colon cancer and 14.0% in rectal cancer. With a median follow-up period of 49.8 months, the 5-year disease-free survival rates of patients without and with SSI were 87% and 83%, respectively (P = 0.018). SSI predicted disease-free survival in univariate analysis. However, SSI was not an independent predictor of survival in multivariate analysis. Body mass index, ASA score, preoperative WBC count, rectal tumor, open surgery, operation time, and transfusion during surgery were independent predictors of SSI.Conclusion: SSI predicted disease-free survival in colorectal cancer patients following curative surgery. Purpose: This study aimed to evaluate the prognostic significance of lymphovascular (LVI), perineural invasion (PNI), and tumor budding positivity in patients with colorectal cancer.Methods: From January 2008 to December 2011, 3707 consecutive patients who underwent curative surgery for stage I-III colorectal cancer were assessed. These patients were then categorized into four groups based on LVI, PNI, and tumor budding (risk grouping): all negative (n = 1495), 1 + only (n = 1063), 2 + only (n = 861), and all positive (n = 288).Results: With a median follow-up period of 52 months, the 5-year disease-free survival rates of the risk groups were significantly different in terms of cancer staging (stage I, Stage II, and Stage III: P = 0.006, P < 0.001, and P < 0.001, respectively). Purpose: We compared the treatment outcome between surgery-first and neoadjuvant chemoradiation therapy (nCRT)-first strategies in patients with indistinguishable T2/T3-N0 rectal cancer on rectal magnetic resonance imaging (MRI).Methods: Our institutional review board approved this retrospective study, and informed consent was waived. Among 1910 patients who underwent rectal MRI between 2008 and 2012, 79 patients (mean age, 59.4 years, 49 men and 30 women) who had indistinguishable T2/T3-N0 rectal cancer on rectal MRI were included. Local recurrence-free survival (LRFS), recurrence-free survival (RFS), overall survival (OS), and disease-specific survival (DSS) were compared between the two groups. Treatment-related complications were evaluated.Results: Among 79 patients, 51 were treated by surgery first and 28 were treated by nCRT first. In comparison of survival of the surgery- and nCRT-first groups at 5 years, the LRFS rate was 95.6 and 96.3%, RFS rate was 91.0 and 92.4%, OS rate was 93.7 and 92.6%, and DSS rate was 98.0 and 92.6%, respectively. LRFS, RFS, OS, and DSS showed no significant difference between the two groups (p = 0.862, 0.677, 0.953, and 0.479). The complication rate was not significantly different between the groups (20.0% for surgery-first group vs. 10.7% for nCRT-first group, p = 0.357).Conclusion: Treatment outcomes were not significantly different between surgery-first and nCRT-first strategies for indistinguishable T2/T3-N0 rectal cancer on rectal MRI. Purpose: It remains unclear whether old age is a poor prognostic factor in colorectal cancer (CRC). We compared oncologic outcomes in CRC patients according to age, using 80 as the dividing point.Methods: CRC patients who underwent radical surgery from 2000 to 2011 were evaluated. We performed matched and adjusted analyses comparing oncologic outcomes between patients with ≥ 80 and < 80 years old.Results: Among 9562 patients, 222 were elderly. The median age was 82.0 years in elderly patients and 59.0 years in young patients. Elderly patients received less neoadjuvant or adjuvant therapy compared to young patients (p < 0.001). After recurrence, significantly fewer elderly patients received additional treatments (p < 0.001). Before matching, disease-free survival (DFS) and cancer-specific survival (CSS) were significantly lower for elderly patients compared to those for young patients (p < 0.001 and p < 0.001, respectively). After matching, DFS and CCS were not significantly different between the two groups (p = 0.400 and p = 0.267, respectively). In a multivariate analysis for prognostic factors, old age was not an independent poor prognostic factor of DFS and CCS (p = 0.619 and p = 0.137, respectively).Conclusions: Elderly patients aged ≥ 80 years with CRC had similar oncologic outcome to young patients, and age was not an independent prognostic factor. Background: The introduction of complete mesocolic excision (CME) with central vessel ligation (CVL) for right-sided colon cancer has improved oncologic outcomes. However, there is controversy over the oncologic safety of laparoscopic CME with CVL. This study compared short-term and long-term oncologic outcomes between laparoscopic and open modified CME (mCME) with CVL in patients with right-sided colon cancer.Methods: We enrolled 1239 patients who underwent open mCME with CVL and 1010 patients treated by a laparoscopic approach for right-side colon cancer between 2000 and 2013 and used 1:1 propensity score matching to adjust for potential baseline confounders between two groups.Results: After propensity score matching, 683 patients who underwent open mCME with CVL were compared with 683 patients treated with a laparoscopic approach. There were no significant differences between these groups in age, sex, ASA score, TNM stage, tumor size, lymphovascular invasion, and perineural invasion. Comparison of open and laparoscopic mCME groups showed no significant difference in postoperative morbidity (21.4 vs. 18.3%, p = 0.175) and mortality (0.1 vs. 0%, p = 1.000). The laparoscopic mCME group showed shorter length of hospital stay. The 5-year overall survival rate was 83.7% in the open group and 94.7% in the laparoscopic group (p < 0.001). The laparoscopic group also showed a significantly better 5-year disease-free survival rate (82.7 vs. 88.7%, p = 0.009) and 5-year disease-specific survival rate (83.7 vs. 94.7%, p < 0.001).Conclusion: Laparoscopic modified mesocolic excision with central vascular ligation is a safe and feasible approach with better short-term recovery profiles and potential oncologic benefits than the open approach for right-sided colon cancer. Background: Tumor budding is associated with adverse histology and is a predictor of lymph node metastasis. However, it remains unclear whether tumor budding is predictive of a poor prognosis for colon cancer patients. This study sought to investigate the prognostic significance of tumor budding in colon cancer. Methods: This study evaluated 4196 colon cancer patients who underwent radical surgery from 2007 to 2013 at a single institution. The patients were categorized according to tumor-budding status. Adjustment was made for using propensity score-matched analysis, and both disease-free survival (DFS) and overall survival (OS) were compared between the groups. Results: Among the 4196 patients, 2269 had low budding (< 5 buds), 1312 had intermediate budding (5-9 buds), and 615 had high budding (≥ 10 buds). High budding was associated with adverse histologic features such as elevated levels of preoperative carcinoembryonic antigen, advanced stage, poor histology, and the presence of lymphatic/vascular/perineural invasion. Before matching, DFS and OS decreased significantly with increasing tumor budding. After matching, the difference in survival between the low- and intermediate-budding groups disappeared. However, the OS and DFS rates for the high-budding group were significantly lower than for the other two groups. In the multivariate analysis of prognostic factors, high budding was an independent poor prognostic factor in DFS and OS, whereas tumor-budding positivity itself was not an independent prognostic factor. Objectives: To evaluate the clinical course of disappearing colorectal liver metastases (DLM) or residual tiny (≤5 mm) colorectal liver metastases (RTCLM) on gadoxetic acid-enhanced magnetic resonance imaging (MRI) and diffusion-weighted imaging (DWI) in patients who had colorectal liver metastases (CLM) and received chemotherapy.Methods: Among 137 patients who received chemotherapy for CLM and underwent gadoxetic acid-enhanced MRI and DWI between 2010 and 2012, 43 patients with 168 DLMs and 48 RTCLMs were included. The cumulative in situ recurrence rate of DLM and progression rate of RTCLM and their predictive factors were evaluated.Results: A total of 150 DLMs and 26 RTCLMs were followed up without additional treatment. At 1 and 2 years, respectively, the cumulative in situ recurrence rates for DLM were 10.9 % and 15.7 % and the cumulative progression rates for RTCLM were 27.2 % and 33.2 %. The in situ recurrence rate at 2 years was 4.9 % for the DLM group that did not show reticular hypointensity of liver parenchyma on hepatobiliary phase.Conclusions: DLM on gadoxetic acid-enhanced liver MRI and DWI indicates a high possibility of clinical complete response, especially in patients without chemotherapy-induced sinusoidal obstruction syndrome. A facile route for the synthesis of PHEMA hydrogel incorporating anatase nanoparticles is presented. The hydrogel features a refractive index of 1.527 and high transparency throughout the visual spectral range. Saturn rings have been machined in order to investigate the workability of the material and its potential applicability as intraocular lens implant. Purpose: The aim of this study was to compare oncologic outcomes and perioperative variables following conventional laparoscopic surgery (LAP) versus hand-assisted laparoscopic surgery (HALS) for rectal cancer.Methods: Between January 2008 and December 2012, 2680 consecutive patients who underwent curative resection for rectal cancer were analyzed. We used 1:1 propensity score matching to adjust for potential baseline confounders between groups including age, sex, body mass index, American Society of Anesthesiologists score, tumor distance from the anal verge, clinical T and N categories, pathologic T and N categories, preoperative carcinoembryonic antigen level, and the status of preoperative concurrent chemoradiotherapy. After matching, we analyzed 278 patients in each group (n = 556).Results: The median follow-up period was 36.2 and 37.4 months in the HALS group and the conventional LAP group, respectively. Postoperative complications were not significantly different between the two groups (P = 0.531). The 5-year overall survival rate was 88.8 % in the HALS group and 91.2 % in the conventional LAP group (P = 0.329). The 5-year disease-free survival rate was 77.0 % in the HALS group and 79.7 % in the conventional LAP group (P = 0.591).Conclusions: HALS is considered a safe and feasible approach for rectal cancer treatment that enables the preservation of the advantages of conventional laparoscopic surgery. We evaluate the Nekrasov partition function of 5d gauge theories engineered by webs of 5-branes, using the refined topological vertex on the dual Calabi-Yau three-folds. The theories include certain non-Lagrangian theories such as the T theory. The refined topological vertex computation generically contains contributions from decoupled M2-branes which are not charged under the 5d gauge symmetry engineered. We argue that, after eliminating them, the refined topological string partition function agrees with the 5d Nekrasov partition function. We explicitly check this for the T theory as well as Sp(1) gauge theories with N = 2 , 3 , 4 flavors. In particular, our method leads to a new expression of the Sp(1) Nekrasov partition functions without any contour integrals. We also develop prescriptions to calculate the partition functions of theories obtained by Higgsing the T theory. We compute the partition function of the E theory via this prescription, and find the E global symmetry enhancement. Background: The clinical application of natural orifice transluminal endoscopic surgery (NOTES) for sigmoidectomy is associated with several difficulties that need to be overcome before wider clinical application of the procedure. The purpose of this study was to evaluate the technical feasibility and safety of transgastric sigmoidectomy in a survival animal model, as well as to evaluate the safety and usability of a custom paired magnetic intraluminal device, which we developed for the NOTES procedure.Methods: Survival experiments were conducted on 24-33-kg dogs. After anesthesia, a gastrotomy was created using double-channel endoscope, and peritoneoscopy was performed. The sigmoid colon was retracted laterally using paired magnetic intraluminal device, the mesocolon was dissected, and the branch vessel was sealed. The anvil was placed into the descending colon through anus. A proximal and distal colonic transection was then performed. The circular stapler was passed through the anus and performed end-to-end anastomosis. Afterwards the specimen was removed through gastrotomy, and the gastric incision was closed. Postoperatively, all dogs were recovered and monitored for well-being during convalescence. Reexploration was practiced under anesthesia 2 weeks after surgery for evaluation of intra-abdominal complications, and intra-peritoneal cultures for microorganism.Results: The mean operative time was 141 (range, 122-157) min. There were no complications or physical evidence of sepsis or bowel obstruction during the observation period. Only one dog exhibited decreased body weight, decreasing to 20.4 kg from 22.4 kg after surgery; all of the other dogs exhibited increased body weight. We observed no evidence of peritonitis, intra-abdominal abscess, bleeding, or organ injury on reexploration conducted on day 14 after surgery.Conclusions: Transgastric NOTES sigmoidectomy is a safe operation technique as evaluated in a dog model. Extramammary Paget’s disease (EMPD) most commonly occurs in the perineal and genital areas of elderly people. The current treatment of choice is adequate surgical excision. Between 1990 and 2007, 28 patients (27 men and one woman) with EMPD were treated at Asan Medical Center, Seoul, Korea, by wide local excision (WLE) with a 2- to 3-cm normal skin margin. “Carcinoma in situ” (CIS) was defined as confinement of Paget cells to the epithelium, whereas “invasive carcinoma” was defined as infiltration of Paget cells into more than dermal connective tissue. Of the 28 lesions, 21 (75%) were located in the penoscrotal area. Six (21.4%) patients had associated gastrointestinal neoplasms, three with associated gastrointestinal malignancies and four with synchronous colorectal adenomas, including one with both. Surgical procedures included WLE with primary repair in 13 patients (46.4%), WLE with skin graft in 12 (42.9%), WLE with skin graft and Gracilis muscle transposition in two (7.1%), and combined abdominoperineal resection and distal gastrectomy in one (3.6%). Four patients (14.3%) also underwent inguinal lymph node dissection, with three found to have lymph node metastases. Of 11 patients with invasive carcinoma, three (27.3%) had lymph node metastases, compared with none of 17 patients with CIS. Patients with lymph node metastases showed a significantly lower disease-specific survival rate ( P = 0.008). Patients with invasive carcinoma tended to have a lower disease-specific survival rate ( P = 0.087). EMPD in Korea showed an absolute male predominance and an association with gastrointestinal neoplasms.
